1.Transesophageal Echocardiography during Radiofrequency Atheter Ablation of Atrioventricular Accessory Pathways

Objective: To evaluate the role of transesop hageal echocardiography (TEE) in monitoring patients during radiofrequency cathe ter ablation (RFCA). Methods: Thirty-one RFCA procedures were perfo rmed in 25 patients with left side accessory pathway and 4 patients with right s ide accessory pathway. Under local lidocain gel anesthesia, all patients were mo nitored during RFCA with TEE. Results: TEE was used to detect underl ying heart diseases in 3 patients, guide transseptal catheterization in 3, and f ind transient moderate mitral regrugitation in 2 during RFCA. It helped position the RFCA catheter's tip, and evaluate for possible complications in all patient s.Conclusion: This study suggestes that TEE might be useful in RFCA procedures under local anesthesia.
2.Central obesity and metabolic risk factors in middle-aged Chinese.

OBJECTIVECentral obesity is considered to be a central component of metabolic syndrome. Waist circumference (WC) has been widely used as a simple indicator of central obesity. This study is aimed to evaluate the sensitivity of WC cut-off values for predicting metabolic risk factors in middle-aged Chinese.
METHODSThe study involved 923 subjects aged 40-65 years. The metabolic risk factors were defined according to the Chinese Joint Committee for Developing Chinese Guidelines on Prevention and Treatment of Dyslipidemia in Adults. WC cut-off 85-90 cm and ⋝90 cm were used as cut-off values of central pre-obesity and central obesity in males, respectively, while WC 80-85 cm and ⋝85 cm were used as cut-off values of central pre-obesity and central obesity in females.
RESULTSFirst, WC values corresponding to body mass index (BMI) 24 kg/m2 and visceral fat area (VFA) 80 cm2 were 88.55 cm and 88.51 cm in males, and 81.46 cm and 82.51 cm in females respectively. Second, receiver operating characteristic curves showed that the optimal WC cut-off of value was 88.75 cm in males, higher than that in females (81.75 cm). Third, the subjects with higher WC values were more likely to have accumulating metabolic risk factors. The prevalence of metabolic risk factors increased linearly and significantly in relation to WC levels.
CONCLUSIONWC cut-off values of central pre-/central obesity are optimal to predict multiple metabolic risk factors.

5.Pollination biology of Fritillaria delavayi.

In this study, the processes of pollination ecology of Fritillaria delavayi were investigated to document its reproductive characteristics. Some individuals of F. delavayi could produce seeds under bagging without emasculation (11%), but the rate was significantly lower than that of the natural control (87%). It is suggesting that pollination of F. delavayi largely depends on pollen vectors. Bombus sushikini was the only effective pollinator of F. delavayi and the visitation frequency was 0.003 time xXflower(-1) x min(-1). Flowering of F. delavayi in whole population lasted for 35 d and single flower for 11 d. Pollen viability and stigma receptivity lasted for 9 d and were relatively long compared with other Fritillaria genus plants. Consequently, bumblebee pollination and long floral longevity seem to be important for reproductive assurance of F. delavayi in harsh alpine environments.

6.Study on the corrosion resistance of Cr ahoy in the artificial saliva with different pH value.

OBJECTIVETo study the effect on the corrosion resistance of Cr alloy in the artificial saliva with different pH value in vitro.
METHODSCompared to Ti alloy, electrochemical technique was used to measure the electric potential of corrosion (E(corr)), current density of corrosion (I(corr)) Cr alloy in the artificial saliva with different pH value. The corrosion and corrosion resistance mechanism on Cr alloy in the artificial saliva with different pH value had been investigated by the EIS curves.
RESULTSThe corrosion resistance of Co-Cr alloy was better than Ti alloy. The value of I(corr) was lower and the value of R(p) was larger than Ti alloy. The corrosion resistance of Ni-Cr alloy was worse than Ti alloy. The value of I(corr) was larger and the value of R(p) was lower than Ti alloy. The pH value in the artificial saliva was decreased, the I(corr) value of three types of alloy increased, the R(p) value of three types of alloy decreased. But the changes of Co-Cr alloy was smaller than that of Ti alloy or Ni-Cr alloy.
CONCLUSIONThe corrosion resistance of Co-Cr alloy was superior to that of Ni-Cr alloy and Ti alloy. In the acid artificial saliva, the corrosion resistance of three types of alloy descended evidently.

7.Rehabilitation nursing care of double endo-button technique under total arthroscope for acute acromioclavicular joint dislocation

Objective To evaluate the rehabilitation nursing care of double endo-button technique under total arthroscope for acute acromioclavicular joint dislocation.Methods From August 2007 to March 2009,8 cases (8 shoulders,6 males and 2 females,22 - 53 years old,mean age 34.4 ) with acute acromioclavicular joint dislocation ( Rockwood type Ⅲ) were treated in author' s hospital.After surgery we made systemic rehabilitation care plan,phased into the body functional rehabilitation exercises,under the premise of ensuring safety of patients as soon as possible to urge the active and passive functional exercise,regular follow-up.Radiographic analysis and the Constant scores and VAS ( Visual Analogue Scale) were used to provide a final evaluation of shoulder function for 3 months,6 months and 12 months after operation.Results All of 8 patients were followed up for more than 3 months( average 7.2 months).The Constant score averaged 93.9,with a mean VAS score of 1.75.There were no patients with A-C joint re-dislocation during this period after analyzing X-ray,but VAS score of 1 patient was 4 by 3 months.The operation for 8 cases was satisfactory.Most patients returned to their pre-injury activity level.Conclusions Total sxthrosocopic double endo-button technique for reconstruction of coracoclavicular ligament is an effective approach to treatment of fresh Rockwood Ⅲ acromioclavicular dislocation. This approach enjoys earlier recovery and less complication. Postoperative rehabilitation nursing is as important as surgery,not only for good functional recovery,but also for patient satisfaction.
8.Case-control study on effects of two approaches on multifidus muscle injuries during lumbar interbody fusion of lumbar vertebrae.

OBJECTIVETo study differences of multifidus muscle injury between Wiltse intermuscular approach and intramuscular stripping approach in one-level small incision transforaminal lumbar interbody fusion (TLIF) surgery.
METHODSA total of 46 patients with unilateral lumbar degenerative disease underwent small incision TLIF from August 2009 to February 2011 by one group of surgeons at a single institution. The decompression side of all patients adopted intra-muscular stripping approach; for the non-decompression side, 22 patients adopted Wiltse intermuscular approach (group A),and 24 patients adopted intra-muscular striping approach (group B). In group A, there were 13 males and 9 females, ranging in age from 36 to 74 years old,averaged 52.7 +/- 9.2; 1 patient had disease in L3, 4 12 in L4,5 and 9 in L5S1. In group B,there were 11 males and 13 females,ranging in age from 32 tio 72 years old, averaged 51.8 +/- 8.7; two patients had disease in L3,4, 14 in L4,5, and 8 in L5S1. The following data were compared between the 2 groups: surgical time from skin incision to completion of pedicle screw placement, suturation time, blood loss. Clinical effects were evaluated by VAS score pre-operatively, as well as 1, 6 and 12 months post-operatively. At the latest follow-up, all the patients were evaluated by MRI. This enabled the cross-sectional area (CSA) of lean multifidus muscle, and the T2 signal intensity ratio of multifidus to psoas muscle, to be compared at the operative level.
RESULTSThere was no obvious difference in suturation time, but less surgical time from skin incision to completion of pedicle screw placement, less blood loss, less postoperative back pain in Wiltse intermuscular approach group. For the comparison between the two groups or paired comparison between sides in the Wiltse group, the reduction of lean CSA and increase in the multifidus:psoas T2 signal intensity ratio were all significant lower in Wiltse intermuscular approach group or side.
CONCLUSIONThe Wiltse intermuscular approach is an easy way for pedicle screw placement, and caused less paraspinal muscle damage than intra-muscular stripping approach, and had positive effects on less back pain.

10. Study on the prediction of cervical lymph node metastasis risk in preoperative thyroid papillary carcinoma by ultrasonic elemental observation of thyroid nodules

Objective:
To evaluate the correlation between ultrasound features of papillary thyroid carcinoma (PTC) and lymph node metastasis by preoperative ultrasound elemental observation of thyroid nodules.
Methods:
Three hundred and seventy-six patients who underwent primary thyroid surgery and confirmed by ultrasound and pathological data as single-focal PTC from Jannary to December 2017 in Sir Run Run Shaw Hospital of Zhejiang Univbersity College of Medicine were retrospectively analyzed. According to the presence or absence of lymph node metastasis, they were divided into central and lateral lymph node metastasis group and non-metastasis group. Independent risk factors for central lymph node metastasis (CLNM) and lateral lymph node metastasis (LLNM) were analyzed by χ2 test and multivariate Logistic regression.
Results:
Multivariate analysis showed that the posterior margin of the cancer was <0.25 cm from the posterior wall of the thyroid gland as an independent risk factor for CLNM (
