1.Effect of listening zhidiao tone in the five elements music together with language training on depressed mood and clinical curative effect of language rehabilitation of the stroke patients with aphasia

Objective To explore the effect of listening zhidiao tone in the five elements music together with language training on the rehabilitation of stroke patients with aphasia.Methods 57 subjects of stroke patients with aphasia were randomly divided into three groups,one was zhidiao music group,another was west music group,and the third was the control group.Four weeks later,using the scale called the Chinese Rehabilitation Research Center Standard Aphasia Examination (CRRCAE) and the Stroke Aphasic Depression Questionnaire Hospital Version(SADQ-H) to see the variance of scores of language function,and depression in the three groups before and after intervention.Results After the intervention,depressed mood scores of three groups decreased significantly,zhidiao music group experienced less depressed mood compared with the control group.According the Ridit analysis,the clinical curative effect was significantly better in the zhidiao music group than in the western music group or the control group.Conclusion Zhidiao music combined with language training can improve the patients’ depression status,and improve compliance and enthusiasm of language training,so as to promote the reha-bilitation of the language function.Listening to zhidiao music had a positive role in language rehabilitation in stroke patients with aphasia,and worthy of further promotion and application.
2.Expression of Nav1.6 in oral leukoplakia and oral squamous cell carcinoma

Objective:To study the expression and significance of sodium channel Nav1.6 in oral leukoplakia and squamous cell carcinoma.Methods:Immunohistochemistry and Western blot were used to detect the expression of Nav1.6 in 21 case of normal oral mucosa,32 of oral leukoplakia and 63 of oral squamous cell carcinoma(OSCC).Results:Immunohistochemical results showed that the positive rate of Nay1.6 in normal group,oral leukoplakia group and OSCC group was 6.25%,40% and 76.74% respectively.The expression of Nav1.6 was related to the differentiation of oral squamous cell carcinoma.Western blot results revealed that the content of Nav1.6 in normal oral mucosa,oral leukoplakia and OSCC was 0.469 ±0.015,0.545 ±0.016 and 0.848 ±0.020(P <0.05) respectively.Conclusion:Nay1.6 may play a role in the development of OSCC.
3.Study on the correlation of between infection, inflammation and coronary artery disease.

OBJECTIVERecently studies showed infections of Chlamydia pneumoniae (Cp), Helicobacter pylori (Hp) and cytomegalovirus (CMV) played roles in the development of atherosclerosis. The aim of this study was to study relationship between infection of Cp, Hp and CMV, systemic inflammation and coronary artery disease (CAD).
METHODSFourty-five patients with at least one coronary artery stenosis > 50% and 33 control subjects with negative coronary angiography were recruited for this case-control study from May 2000 to October 2001. Antibodies against Cp, Hp and CMV were measured and serum C-reactive protein (CRP) levels determined for each case. CRP level > 0.8 mg/dl was defined at elevated CRP level.
RESULTSThe prevalence of Cp IgG, Hp IgG or Hp IgA antibody was associated with CAD (P = 0.017, P < 0.001, P = 0.009). After adjustment for age, gender, smoking, hypertension, hyperlipidemia and diabetes, the association was still seen. Mean CRP value was significantly higher in patients with CAD, compared to those without CAD (P < 0.001). Multivariate analysis showed statistical significance (P = 0.03). Elevated levels of CRP were found to be an important parameter for CAD (P = 0.032). The prevalence of Cp IgG antibody, Hp IgG and CMV IgG antibody all showed no association with elevated levels of CRP (P = 0.391, P = 0.253, P = 0.724). The ratio of elevated levels of serum basic CRP in the group with IgG antibodies to 3 pathogens was 32.1% while in the group with IgG antibodies to CONCLUSIONSTwo chronic infections, Cp and Hp, might increase the risk of CAD. There was no association of CMV infection with CAD. C-reactive protein was an independent parameter of CAD, but the increased systemic inflammation in CAD did not seem to be related to aforesaid infection.

4.Decorin accelerates the liver regeneration after partial hepatectomy in fibrotic mice.

BACKGROUNDConsidering the existence of a large number of liver cell degeneration and necrosis in fibrotic liver, liver function was damaged severely and could not effectively regenerate after partial hepatectomy (PHx). The aim of this study was to investigate whether decorin (DCN) could promote the liver regeneration after PHx in fibrotic mice.
METHODSForty mice (5-week-old, Balb/c) were injected with CCl4 intraperitoneally and liver fibrosis model was established after 5 weeks. The survival mice were randomly divided into two groups: control group and DCN group. Then, we performed 70% PHx on all these mice and injected DCN or phosphate-buffered saline plus normal saline (NS) to each group, respectively, after surgery. Liver body weight ratio (LBR), quantitative real-time polymerase chain reaction, and immunohistochemistry were used to analyze liver regeneration and fibrosis degree in both groups, and to find out whether exogenous protein DCN could promote the regeneration of fibrosis liver after PHx.
RESULTSExpressions of a-smooth muscle actin (SMA) mRNA and LBR had significant increases in the DCN group at postoperative Day 3 (POD 3, P < 0.05). The protein expressions of CD31, a-SMA, and tumor necrosis factor (TNF)-a were higher in the DCN group than those in the control group by immunohistochemistry at POD 3 (P < 0.05).
CONCLUSIONExogenous protein DCN could promote liver regeneration after PHx in fibrotic mice.

5.Deletion of P16 gene exon 2 in bronchofibroscopic brush-off cells of human lung carcinoma

OBJECTIVE: To analyze the relationship between deletion of P16 gene exon 2 and lung cancer and to evaluate the possibility of detecting P16 gene exon 2 deletion in brush-off cell instead of resected lung cancer mass. METHODS: P16 gene exon 2 deletion in bronchofibroscopic brush-off cells of lesion side and corresponding normal side was detected through PCR-electrophoresis-Image. Image value of P16 Vs beta-actin <0.6 was considered as P16 gene exon 2 deletion. RESULTS: The rate of P16 gene exon 2 deletion in normal side was 0 (0/19), whereas in lesion side was 35.5%(11/31), there is a significant statistical difference (P<0.01). In SCLC (small -cell lung carcinoma) samples, the rate of P16 gene exon 2 deletion was 0(0/7). whereas in NSCLC (non-small cell lung carcinoma) samples, that was 50%(11/12)(P<0.05). CONCLUSION: P16 gene exon 2 deletion might be related to the oncogenesis and development of lung carcinoma, especially NSCLC. Brush-off cell specimens may replace surgical specimens in Detecting P16 gene exon 2 deletion.
6.Significance of serum CD62p and CD63 levels in patients with head injury.

OBJECTIVETo determine the serum levels of CD62p (alpha-granular membrane protein) and CD63 (lysosome intact membrane protein) in patients with head injury and to observe its relation to injury severity.
METHODSFifty-three patients with head injury were divided into 3 groups; Group A patients with mild head injury; Group B with moderate head injury; and Group C with severe head injury. The serum levels of CD62p, CD63 were measured on 12 h, d 1, 3, 5 and 7 after injury.
RESULTThe serum levels of CD62p and CD63 in Group B and Group C were higher than those in Group A and control (P<0.05). The serum level of CD62p in Group C was higher than that in Group B (P<0.05). The serum levels of CD62p in Group C on d 1, 3, 5 after injury were higher than those on 12 h (P<0.05). The serum level of CD63 in Group B on d 3 after injury were higher than that on 12 h (P<0.05). The serum levels of CD63 in Group C on d 1, 3, 5 after injury were higher than those on 12 h (P<0.05).
CONCLUSIONThe serum levels of CD62p and CD63 in patients with head injury may be helpful for identifying the severity of injury, and CD62p seems to be more sensitive.

7. Current status and controversy of laparoscopic technique in the diagnosis and treatment of gallbladder cancer

This article introduces the current status and controversy of laparoscopic technique in the treatment of gallbladder carcinoma. Combined with the characteristics of incidental gallbladder carcinoma, the feasibility of laparoscopic techniques for the treatment of early gallbladder carcinoma is analyzed.In the era of minimally invasive medical, laparoscopic techniques should play a more important role in the management of gallbladder cancer, but the long-term prognosis of laparoscopic radical surgery for gallbladder cancer needs strict prospective and high-volume clinical research to validate.
8. Botulinum toxin A poisoning by local aesthetic injection: clinical and electrophysiological analyses of 20 cases

Objective:
To observe the clinical and electrophysiological characteristics of botulinum toxin A poisoning induced by local cosmetic injection, and to explore the possible pathogenesis.
Methods:
Clinical data of 20 patients with botulinum toxin type A local cosmetic injection poisoning admitted to our hospital from January 2016 to December 2018 were collected and electrophysiological tests were carried out.
Results:
The clinical manifestations of 20 patients were mainly 15/20 (75.0%) with limb weakness, and the proximal end was heavier than the distal 20/20 (100%), and the upper limb was heavier than the lower limb 26/20 (80%), followed by dysphagia 12/20 (60%), dysarthria 5/20 (25%), cervical muscle weakness 5/20 (25%), extraocular muscle paralysis 4/20 (20%), masseter muscle weakness 3/20 (15%) and respiratory muscle weakness 2/20 (5.0%) with respiratory failure 1/20 (5.0%). The results of neurophysiological examination showed the highest rate of SFEMG (90.0%), followed by RNS (80.0%), EMG (65.0%) and NCV (40.0%), suggesting that neuromuscular junction dysfunction was the main presentation. Patients who actively gave botulinum antitoxin injection therapy and symptomatic support treatment had good prognosis.
Conclusions
Local cosmetic injection of botulinum toxin type A may lead to poisoning. The symptoms of poisoning are mainly caused by limb weakness. Neuroelectrophysiological examination can assist in the diagnosis of botulinum toxin poisoning.
9.PFNA and InterTAN intramedullary nailing in elderly patients with femoral intertrochanteric fractures: a Meta analysis.

OBJECTIVE:
To evaluate the efficacy and safety of proximal femoral nail antirotation(PFNA) vs InterTAN nail in treating the elderly intertrochanteric femoral fractures.
METHODS:
Data of the randomized controlled trials(RCTs) about PFNA vs InterTAN for the treatment of the elderly intertrochanteric femoral fractures were searched in as the Cochrane Library, PubMed, EMbase, Wanfang, CNKI, CBM and VIP from their establishment to January 2018 for collecting. After study selection, assessment and data extraction conducted by two reviewers independently, meta-analyses were performed by using the RevMan 5.3 sofware. The level of evidence was assessed by using the GRADEpro system.
RESULTS:
Twelve studies involving 1 015 patients were included. The results of meta, analyses showed that: (1)safety indicator: compared with the InterTAN, PFNA had shorter operation time, and less intraoperative bleeding. But InterTAN had less total postoperative complications and internal fixation failure, but there was no significant difference in the operative incision lengths, fracture healing time and other postoperative complications. (2)efficacy indicator: compared with the InterTAN, the Harris hip score was lower after 3 months, but Harris hip score had no significant difference between the two groups after 6, 12 months. Based on GRADEpro system, all the evidence was at level C and weak recommendation(2C).
CONCLUSIONS
The current evidence indicates that PFNA had a similar effect compared with the InterTAN. But InterTAN could provide better stability against rotation and axial pressure effect, can allow patients do functional exercise early such as ambulation to recovery the hip function. It also had less total postoperative complications and internal fixation failure for the poor quality of the original studies and the limited number of studies, a prudent choice is suggested and more high-quality, large-sample studies are need.

10.Relationship between sleep architecture and severity of obstructive sleep apnea.

OBJECTIVE:
To investigate the effect of obstructive sleep apnea (OSA) on different sleep stages, and the relationship between N3 stage of non-rapid eye movement sleep and respiratory abnormal events.
METHODS:
A total of 188 adult patients who underwent overnight polysomnography(PSG)monitoring in Sir Run Run shaw Hospital of Zhejiang University from June 24th to December 26th 2019 were enrolled in the study. OSA patients were classified into 3 groups (mild, moderate and severe) according to the apnea-hypopnea index (AHI). PSG data, AHI and the lowest SPO in each stage of sleep were compared among three groups.
RESULTS:
There was no significant difference in total sleep time and sleep efficiency among patients with different severity of OSA (all >0.05). The proportion of N3 stage in moderate and severe OSA groups were significantly smaller than that in mild OSA group (all <0.05). The proportion of N3 stage in severe OSA group was also smaller than that in moderate OSA group (<0.05). In addition, severe OSA group had a longer latency of N3 stage than mild and moderate OSA groups (all <0.05). The latency of N3 stage in moderate OSA group was longer than that in mild OSA group (<0.05). The AHI in N3 stage was markedly lower than that in other sleep stages (all <0.01), regardless of the severity of OSA. Supine AHI in N3 stage in mild and moderate groups was significantly lower than that in N1, N2 and rapid eye movement (REM) stages (all <0.01). Supine AHI in N3 stage in severe group was also lower than that in N2 and REM stages (<0.05 or <0.01). The lowest SPO in N3 stage was significantly higher than that in N1, N2 and REM stages (<0.05 or <0.01), regardless of the severity of OSA.
CONCLUSIONS
s The proportion of N3 stage is lower in OSA patients, and N3 stage has less sleep respiratory events than non-N3 stages. The results suggest that the increased N3 stage proportion may indicate less severity of OSA.
