1.Expression and signification of cell cycle regulation protein Cyclin D1-CDK4-p21 in scar cancer

Objective Dysfunction of cell cycle regulation is one of the key factors for cellular carcinogenesis .This paper aimed to study the expression and significance of cell cycle regulation protein Cyclin D 1-CDK4-p21 in scar cancer . Methods The expressions of Cyclin D1, CDK4 and p21 protains were detected in scar cancer group , pathological scar group and normal skin group respectively by using immunohistochemical staining (SP).The mRNA expression levels of Cyclin D1, CDK4 and p21 were detected by the use of nucleic acid-mediated in-situ hybridization .Correlation analysis was made on the indexes , and the average optical density and positive area were analyzed using image analysis . Results The expressions of Cyclin D1, CDK4 and p21 protains and the mRNA ex-pression levels of cyclin D1, CDK4 and p21 were high in scar cancer group, low in pathological scar group , and negative in normal skin group.The mean optical density and positive area in scar cancer group were significantly different from pathological scar group and normal skin group (P<0.01).But no significant difference was found be-tween pathological scar group and normal skin group (P>0.05).In terms of correlation analysis , the expressions of Cyclin D 1 and CDK4 as well as p21 and CDK4 in scar cancer tissue were both in posi-tive correlations. Conclusion The occurrence of scar cancer is related to the abnormal expression of Cyclin D 1 and CDK4.The complex formed by Cyclin D1 and CDK4 may promote the G1/S transition, proliferation and tumorigenesis of scar cancer .In scar canc-er, the inhibition of Cyclin D1-CDK4 complex might be caused by other members of CKI family or even inbibitors of other families apart from CDK family.
2.Treatment of corneal perforation with lamellar keratoplasty combined with autogenous corneal stromal padding

Objective:To study the clinical outcome of lamellar keratoplasty combined with autogenous corneal stromal padding for treatment of corneal perforation.Methods:An observational case series study was conducted.A total of 14 patients (14 eyes) with corneal perforation underwent lamellar keratoplasty in Henan Provincial People's Hospital from January 2017 to August 2018 were collected.During the operation, the corneal stromal autograft was taken to pad the corneal perforation.Visual acuity, intraocular pressure, corneal epithelial healing time, infection recurrence, and the status of corneal graft and anterior chamber were recorded on the 1st, 7th, 14th day, 1st, 3rd, 6th, 9th and 12th month after operation.OCT examination of anterior segment was performed at 1 month, 6 and 12 months postoperatively to observe the interlamellar structure of corneal implantation bed and stromal graft, anterior chamber and iris.The safety and effectiveness of the operation were evaluated according to the 1-year follow-up.The study protocol adhered to the Declaration of Helsinki and was approved by the Ethics Committee of Henan Eye Hospital (No.HNEECKY-2019[18]). Written informed consent was obtained from each patient prior to entering the study cohort.Results:The healing time of corneal epithelium was 2-4 days after operation, (3.01±0.59) days on average.At one day after surgery, all patients had normal anterior chamber depth without aqueous leak and no double chamber was observed.The corneal grafts attached to the implantation bed well and showed mild to moderate edema.There was no recurrence of corneal infection ulcer two weeks after operation, and the corneal ulcer was healed and anterior chamber was well formed in 14 patients.The corneal graft was transparent, and the anterior segment OCT image showed corneal implant bed and graft were well attached, and the anterior chamber depth was normal three months after operation.The best corrected visual acuity of 9 eyes was more than 0.3 one year after operation, and the vision acuity of the 14 patients improved obviously, and no refractory glaucoma occurred after operation.Conclusions:Lamellar keratoplasty combined with autogenous corneal stromal padding is a safe and effective surgical procedure for treating corneal perforation.
3.Fever,poor response, convulsions, and hepatomegaly

Objective To investigate the differential diagnosis of Reye syndrome and the characteristics of primary carnitine deficiency,and to provide diagnostic strategy for similar cases.Methods There was a case presented with fever,poor response,convulsions and hepatomegaly hospitalized in Wuhan Children's Hospital,and the clinical manifestations were described,the physical examination was comprehensively conducted,the auxiliary examination results were recorded,some pediatric specialists from ICU,neurology department,genetic metabolic department,digestive system department were invited to discuss the case.The treatment was adjusted according to the suggested opinions;the treatment effects and the final diagnosis were tracked.Results The primary diagnosis of the case was central nervous system infection or toxic encephalopathy at the time of admission,but Reye syndrome could not be excluded.Although the cerebrospinal fluid test and brain MRI examination detected nothing abnormal,liver function suggested alanine aminotrans ferase ALT increase,blood sugar decrease,the liver volume increase,which was detected by liver ultrasound.Blood amino acids examination revealed the carnitine level decreased,and it was confirmed as primary carnitine deficiency in the end.L-carnitine was used to treat the disease,and its effect was good.Conclusions Great importance should be attached to children with onset age,physical check-up,and multidisciplinary cooperation.Use monism to explain the illness and the auxiliary inspection as far as possible,so that it can get early diagnosis and treatment,and the outcome is good.
4.Experimental study on phase timing of rabbit liver VX2 tumor by contrast-enhanced ultrasonography : comparison with multidetector spiral CT

Objective To investigate the imaging characteristics of rabbit liver VX2 tumor with contrast-enhanced ultrasonography(CEUS) and multidetector spiral CT(MDCT), to explore the criterion of phase timing apllicable to the model.Methods CEUS and MDCT were acquired in thirty-three New Zealand rabbits with hepatic VX2 tumors.The initial time of hepatic arterial phase was verified as the time the hepatic artery beginning to enhance,the initial time of portal phase as the time the VX2 tumor reaching peak enhancement and the initial time of hepatic parenchymal phase as the time the hepatic parenchyma reaching peak enhancement.The enhancing characteristics were observed.The time-intensity curve of VX2 tumor and peripheral parenchyma in CEUS was analysed and compared with the time-desity curve of abdominal aorta, portal vein, hepatic parenchyma and tumor in MDCT.Results The initial time of heptic arterial phase,portal phase and parenchymal phase in CEUS and MDCT was (6.82±1.36)s and (9.43±2.23)s,(11.64±2.03)s and (13.77±2.01)s,(20.24±4.17)s and (22.71± 4.58)s,respectively.The initial time of each phase in CEUS was earlier than that in MDCT (P<0.01).The contrast enhancing characteristics in CEUS and MDCT were consistent as fast wash-in and fast wash-out.Conclusions The status of hemodynamic perfusion in rabbit hepatic VX2 tumor could be well displayed by CEUS and the phase timing criterion concluded in this experimental study could offer fundmental theory basis for relevant imaging evaluation.
5.Design, synthesis of quinolinone acid-containing compounds with anti-HIV integrase activity.

A series of novel quinolinone acid-containing compounds were designed and synthesized. Their structures were confirmed with 1H NMR and MS. The target compounds were tested for anti-HIV-1 integrase activities in vitro with enzyme linked immunosorbent assay (ELISA). The result showed that D-2, D-4 and D-7 have anti-integrase activity with IC50 < 100 micromol L(-1).
6.Stem cell and repair of injury in central nervous system.

All of neural stem cell within the central nervous system and derived from transplantation or embryonic stem cell have the ability of differentiating into various kinds of neural cells. Regeneration of neural cells plays a critical role on function recovery damaged central nervous system (CNS). 7.Analysis on bacterial culture of bronchoalveolar lavage fluid from 1 693 children with refractory pneumonia

Objective To evaluate pathogens and antimicrobial resistance of pathogens causing refractory pneumonia in children.Methods Children with refractory pneumonia who admitted to a hospital between May 2008 and December 2014 were performed bronchoscopy,and bronchoalveolar lavage fluid (BALF)were performed bacterial culture and antimicrobial resistance testing.Results 1 693 patients were recruited in the study,273 bacterial isolates were isolated from BALF speci-mens of 226 children,gram-positive bacteria accounted for 38.10% (104/273 ),the main gram-positive bacteria were Streptococcus pneumoniae (n=71)and Staphylococcus aureus (n=23);gram-negative bacteria accounted for 58.24%(159/273),including 44 isolates of Haemophilus parainfluenzae ,28 Klebsiella pneumoniae ,19 Escherichia coli ,and 17 Pseud-omonas aeruginosa ;10 isolates of fungi were also detected,8 of which were Candida albicans .The sensitivity of Streptococ-cus pneumoniae to quinolones,ceftriaxone and cefotaxime were high.Methicillin-resistant Staphylococcus aureus (MRSA) positive rate was 26.32%.ESBLs-producing rate of Haemophilus parainfluenzae and Klebsiella pneumoniae was 32.72% and 62.96% respectively.Conclusion The major pathogens causing refractory pneumonia were Streptococcus pneumoniae and Haemophilus parainfluenzae ,empirical treatment should be conducted accordingly,antimicrobial resist-ance should be considered if therapeutic effect is poor,and targeted therapy should be performed according to cultured re-sults and antimicrobial susceptibility testing result.
8.Endoscopic variceal ligation combined with Hassab's procedure in preventing the recurrence of esophageal varices

Objective To assess the efficacy of endoscopic variceal ligation (EVL) combined with Hassab 's procedure in the prevention of variceal recurrence. Methods One hundred and thirty-five patients with esophageal varices were randomized to receive EVL alone, Hassab's procedure alone or a combination of EVL and Hassab's procedure for variceal eradication. Ultrasonographic venous network images were recorded by an esophageal microprobe before and after the EVL or Hassab 's procedure. The clinical outcome and vascular network images of the 3 groups were analyzed. Results Esophageal varices were obliterated immediately after EVL alone, while both perforating veins and periesophageal collaterals remained patent, and 83% had recurrence of esophageal varices during an initial 3-year follow-up. Esophageal varices were reduced in size, periesophageal collaterals were obliterated after Hassab's procedure alone, and 30% experienced rebleeding and 95% with variceal recurrence. EVL combined with Hassab 's procedure obliterated all esophageal varices, perforating veins and periesophageal collaterals, and only 3 patients (8% ) recurred. Conclusion The existence of patent perforating veins and periesophageal collaterals is the reason of esophageal variceal recurrence after EVL alone. EVL combined with Hassab 's procedure can effectively prevent the recurrence, even if the cirrhotic portal hypertension persisted.
9.Value of contrast-enhanced ultrasonography on grading extracranial carotid stenosis

Objective To assess value of contrast enhanced ultrasonography(CEUS)in diagnosis of extracranial carotid stenosis.Methods Baseline ultrasonography and CEUS were performed in 108 patients with extracranial carotid stenosis.The location and degree of stenosis were judged and compared with the results of digital subtraction angiography(DSA).Results Of 108 patients,DSA diagnosed 81 vessels in 62 cases of mildintermediate stenosis which consisted of 33 common carotid arteries(CCAs),43 internal carotid arteries(ICAs)and 5 external carotid arteries(ECAS),51 vessels in 39 cases Of severe stenosis(46 ICAs and 5 CCAs)and 7vessels in 7 cases of complete occlusion(7 ICAs).The surface of intima and the interface of media-adventitia were depicted clearly by CEUS.The consistent rates in diagnosis on mild-intermediate stenosis,severe stenosis and complete obstruction by baseline ultrasonography and CEUS were 76.5%and 9 1.4%(P<0.05).60.8%and94.1%(P<0.001),28.6%and 100%(P<0.05)respectively when judged by DSA as the golden standard.CEUS and DSA were correlated significantly on calculating the stenosis rate(r=0.975,P<0.001).Conclusions CEUS can assess the degree of extracranial carotid stenosis accurately and non-invasively which is superior to baseline ultrasonography and correlated significantly with DSA.
10.Agrobacterium-mediated transformation of Cymbidium sinensis.

Genetic transformation is an effective method to improve breeding objective traits of orchids. However, there is little information about genetic transformation of Cymbidium sinensis. Rhizomes from shoot-tip culture of C. sinensis cv. 'Qijianbaimo' were used to establish a practical transformation protocol of C. sinensis. Pre-culture time, concentration and treating methods of acetosyringone, concentration of infection bacteria fluid (OD600), infection time, and co-culture time had significant effects on β-glucuronidase (GUS) transient expression rate of C. sinensis cv. 'Qijianbaimo' rhizome. The GUS transient expression rate of rhizome was the highest (11.67%) when rhizomes pre-cultured for 39 d were soaked in bacterium suspension (OD600 = 0.9) supplemented with 200 μmol/L acetosyringone for 35 min, followed by culturing on co-culture medium supplemented with 200 μmol/L acetosyringone for 7 d. Under this transformation conditions, 3 transgenic plantlets, confirmed by GUS histochemical assay and PCR, were obtained from 400 regenerated plantlets, and the genetic transformation rate was 0.75%. This proved that it was feasible to create new cultivars by the use of Agrobacterium-mediated genetic transformation in C. sinense.
