1.The Guiding Sense of Umbilical Artery Blood pH in the Evaluation of Neonatal Asphyxia Diagnosis and Prognosis

Objective To explore the effect of umbilical artery blood pH on the diagnosis of neonatal asphyxia and the evaluation of its prognosis. Methods Blood samples were obtained from 108 normal newborns and 51 asphyxial newborns,who were randomly chosen among newborns delivered in our hospital from november 2007 to may 2008. Umbilical artery blood gas was measured and compared with apgur rating. Results With the degression of umbilical blood pH, the incidence rate of neonatal asphyxia increased and the normal apgar rating decreased(P < 0.01), which suggesting that there was tight relevant between umbilical blood pH and neonatal asphyxia. Conclusion Compared with apgur rating, umbilical artery blood pH could reflect the asphyxia degree of newborns more objectively andsensitively and it might play a guiding role in the prognosis of neonatal asphyxia.

3.STUDIES ON PATHOLOGIC CHANGES OF CEREBRAL CONCUSSION AND APOPTOSIS OF NERVOUS CELLS IN WISTAR RATS

Altogether 80 Wistar rats were used for an animal model of cerebral concussion, which were sacrificed on days 1,3,7,14 and 30 after injury and the brain tissue was collected. The pathologic changes of cerebral concussion and apoptosis of nervous cells were studied by means of light microscopy, electron microscopy and in situ terminal end labeling method. The results showed that the clinical situation for cerebral concussion occurred in rats struck by 100g standard weight from 1 meter high. The basic pathologic changes were the cerebral vascular dilatation, congestion, hemorrhage, and edema of cerebral tissue. Nervous cells underaent degeneration, apoptosis and necrosis, and the Nissl bodies obviously decreased, even disappeared. On days 1~3 after injury, dot or piece necrosis was seen in brain tissue, around which the tissue rarefied. Monocytes and foam cells increased, and lots of neurons underwent degeneration, apoptosis and necrosis. The edema of cerebral tissue reached its peak on day 7. Hippocampal pyramidal cells decreased in number and showed the changes of obvious ischemia. On days 14~30, blood vessels also showed dilatation, congestion and hemorrhage, whereas edema alleviated. The neurons in cerebral cortex and hippocampus also showed the changes of chronic ischemia. By in situ terminal end labeling the number of apoptotic neurons increased on day 1, reached its peak on day 3 and still existed on day 30. The results suggested that the main pathologic changes of cerebral concussion were blood circulatory disorder and nervous cell degeneration, apoptosis and necrosis. Apoptosis of nervous cells was one of the main changes in cerebral concussion.

Objective To study the injury of rat cardiomyocytes exposed to nonionizing radiation and its mechanism. Methods Primarily cultured cardiomyocytes were irradiated by 9 GHz 950 mW/cm2 microwave pulse (MWP)and 0~100 MHz 6?104 V/m electromagnetic pulse (EMP)for 72 h respectively ,then a series of apparatus including atom force microscope,laser scanning confocal microscope and flow cytometer were used to detect the changes of cell membrane conformation,structure and function. Objective: To survey the content of serum total cholesterol, LDL and HDL in pa- tients with gastric cancer and to investigate the correlation between serum total cholesterol, LDL, HDL and the differentiation of gastric cancer. Methods: 143 patients with gastric cancer were selected and assigned into 3 groups according to pathology grouping after operation: 1) the well-differentiated group, consisting of 16 patients with papillary adenocarcinoma, and 43 patients with tubi-form well-differentiated adenocarcinoma; 2) the moderately differentiated group, including 39 patients with tubiform moderately differentiated adenocarcinoma, 3) the poorly differentiated group, including 20 patients with poorly differentiated adenocarcinoma, and 14 patients with mucinous adenocarcinoma, and 11 patients with signetring cell carcinoma. In addition, 30 patients with gastric ulcer and 30 health adults were selected as control. Veinous blood of every member was collected, and then the content of serum total cholesterol was surveyed by enzymic method, and the contents of serum LDL and HDL were measured by precipitation method. Results: The level of serum total cholesterol, LDL and HDL before operation in patients with gastric cancer decreased significantly than those with gastric ulcer and normal people (P0.05). There was statistically significance among different differentiation in patients with gastric cancer. The differences of levels of serum total cholesterol, LDL and HDL were statistically significant in gastric cancer of different differentiated groups. Objective To explore the changes of gene and protein expressions of nuclear factor-κB(NF-κB) and tumor necrosis factor-α (TNF-α) in immature rats with pilocarpine-induced epilepsy treated with ghrelin. Methods The pllocarpine-induced epilepsy model in immatured rats were built, then the rats were divided into three groups: Ghrelin-treated group, saline-treated group and model group, meanwhile the normal control group was set. The NF-κB and TNF-α levels of gene and protein in the cerebral cortex of immature rats were detected. Results The expression levels of gene and protein of NF-κB and TNF-α were increased in model group,but decreased in the normal control group;NF-κB and TNF-α levels in Ghrelin treated group were obviously lower than those of saline-treated group and model group(P < 0. 05). Conclusion The protective mechanism of Ghrelin for nerve cell is cutting down the expressions of NF-κB and TNF-α in the cerebral cortex of immature rats with epilepsy and lessening inflammatory reaction in neurocytes.
7.Investigation of qulity of sexual life and its impact factors in young females patients who have hysterectomy

Objective To investigate sexual life associated with hysterectomy of young women and its related factors.Methods Data from 199 patients with hysterectomy were collected.Statistical analysis was performed between the postoperative recovery condition among them,including daily life and sex life.Results There was significant difference on sexual life quality of the patients by hysterectomy among patient age,occupation,education,postoperative time,disease and complication (P < 0.05).The age of 41-50 years and 51-60 years old patients' quality of life satisfaction was significantly higher than that of age ≤40 years old patients (OR =3.8,95 % CI 1.36-10.66; OR =3.6,95 % CI 1.24-10.43).The patients of the occupation of service sectors,workers,and patients with free occupation have a decreased satisfactions compared with that of farmer patients (OR =0.46,95 % CI 0.12-1.75; OR =0.75,95 % CI 0.27-2.12; OR =0.94 95 % CI 0.45-1.98).There was a increasing satisfactions in administrative and the intellectuals (OR =1.11,95 % CI 0.34-3.62; OR =1.33,95 % CI 0.35-5.10).Patients with primary school educations as a reference for comparison,the quality of life of patients with associated college and above degree were increased than patients with primary school (OR =1.15,95 % CI 0.43-3.04),and were decreased in the patients of the high and middle school (OR =0.98,95 % CI 0.46-2.11; OR =0.63,95 % CI 0.26-1.54; OR =0.15,95 % CI 0.02-1.27).The quality of life satisfaction of long period patients (7-12 months,13-24 months,24 months after the operation)was significantly higher than that of short period patients with 6 months(OR =1.94,95 % CI 0.65-5.83; OR =6.32,95 % CI 2.17-18.38; OR =8.47,95 % CI 2.92-24.56,P < 0.001).Conclusion The doctors need focus on relative knowledge education of perioperation.It is important to inform patients and their spouses to have the knowledge of female physiology,fully understanding the impact of the extent of surgery on sexual life of patients to enhance the confidence and improve the life quality of patients.
8.Effects of Xuebijing injection combined with dexamethasone in prevention and treatment of rats' pulmonary injury induced by Paraquat

Objective To explore the prevention and treatment effect and its mechanism of Xuebijing injec-tion combined with dexamethasone on rats' paraqnat-induced acute and chronic pulmonary injury.Method One hundred and twenty of male Wistar rats were randomly divided into six groups:nomud group(A),administrated with saline;model group(B)and treatment groups(group C,D,E,F)were given 20%PQ(100 mg/kg.ip),and 2 hours later the normal and model groups were administrated with the same volume of saline for treatment,rats in group C and group D received 1.25 g/kg and 2.5 g/kg Xuebijing injection respectively.rdts in group E received 25,ng/kg dexamethasone,rats in group F receired 2.5 g/kg Xuebijing injection combined with 2.5 g/kg dexamethasone,one time per day till to be killed,while rats killed at 28 d were treated for 7 days.At 2 d,3 d,4 d after poisoned,five rats in each group were killed,serum SOD,MDA level and arterial gas(at 3 d)were measured.At 28 d,the rest of rats were killed,and serum TGF-β1,lung tissure HYP were measured.The pathology of the lung tissue was ob-served at 3 d and 28 d in guoup A,B,F.Results Compared with group B,poisoning symptoms in the treatment groups were milder and serum.SOD,MDA,TGV-β1,lung tissure HYP level were better,arterial oxygen content were higer.Among treatment groups,the treatment effects in group F were the best,SOD and MDA of 3 d,HYP and TGF-β1 of 28 d in group B and F were respectively(37.47±13.00,91.86±21.35)nmol/mL;(11.34±3.07,5.63±1.58)nmoL/mL;(2.54±0.63,1.32±0.07)mg/g;(484.13±63.79,202.22±49.83)pg/mL.The difference was significant(P<0.05).The pathology of the lung tissue showed that acute lung hemorrhage,edema or chronic pulmonary fibrosis in group F were milder than that of group B.Conclusions In early stage,Xuebijing injection combined with dexamethasone has a stronger ability to clear out oxidized free radical and inhibit lipid super oxidized reaction.This may ameliorate acute pulmonary blooding and edema.In later stage,they could ameliorate chronic pulmonary fibrosis by inhibiting TGF-β1 secretion and HYP generation.
9.Therapeutic effect of rhIL-11 and rhG-CSF on mouse bone marrow injury induced by neutron irradiation

Objective To explore the therapeutic effect of rhIL-11 and rhG-CSF on mouse bone marrow injury induced by neutron irradiation.Methods 130 male BALB/c mice were irradiated by 3.0 Gy neutron and mice peripheral blood cells,bone marrow pathological changes,bone marrow nucleated cell counts,AgNOR content,apoptosis and necrosis rates and Bax protein content were observed by means of blood cells automatic analyzer,HE staining,AgNOR staining,flow cytometry,immunohistochemistry staining and image analysis.Results In the irradiation group and the rhIL-11 group,the mice peripheral blood white blood cells,bone marrow nucleated cell counts and AgNOR content was decreased progressively.The Bax protein was positively or strongly positively expressed in the cytoplasm of the hematopoietic cells and the Bax protein content was increased progressively at 6 h,1 d,3 d after irradiation.In the irradiation group,the rates of apoptosis and necrosis in the mice hematopoietic cells were greatly increased and that of necrosis was significant at 6 h after irradiation.In the rhIL-11 + rhG-CSF group,the counts of bone marrow nucleated cell and AgNOR were increased and the Bax protein content was decreased at 3 d after irradiation,while in the rhIL-11 group,the indexes mentioned above were not obviously different compared with those of the irradiation group.Conclusions The mice bone marrow hematopoietic function is seriously damaged by 3.0 Gy neutron irradiation,rhIL-11 and rhG-CSF could improve the mice hernatopoietic function after neutron irradiation,and combination of them is more effective to stimulate the hematopoitic function than either of them alone.

The improvement of women and children health,along with the growth in health needs,brings forth new challenges to maternal and children health care institutions.The dual demands of quantity and quality in maternal and children health services compel the institutions to improve capabilities under the current conditions,and more importantly,to further clarify the functional orientation which is the key issue for its development.This article is based on the analysis of the historical development and current situation of the maternal and children health care institutions,which sets up the functional orientation according with the rules and characteristics of maternal and children health services.The paper proposed a series of suggestions on its development,such as the health service admission system,appropriate staffing standards and the institution-construction standards.
