1.Nursing and Guidance after Radiofrequency Thermocoagulation Ambulatory Surgery for Cervical Disc Herniation

Objective To explore a new modality of nursing care and convalescence guide for ambulatory (day) surgery of cervical disc herniation with CT guided multi-point radiofrequency (RF) thermocoagulation and target ablation. Methods 16 cases were reviewed. Results All the cases recovered satisfactorily without any complications. Conclusion The new modality of nursing care and convalescence guide is effective on RF for cervical disc herniation.
2.Analysis of the clinical data from 56 misdiagnosis patients with Tsutsugamushi

Objective To improve the understanding of the epidemiology,clinical features,diagnosis and treatment of Tsutsugamushi disease,and reduce the misdiagnosis rate.Methods The clinical data of 56 cases of Tsutsugamushi disease misdiagnosed were analyzed retrospectively.Results 28 cases were misdiagnosed as acute upper respiratory tract infection(50.0%),3 cases were malaria(5.4%),1 case was cholecystitis(1.8%),24 cases were infectious fever(42.8%).The average misdiagnosed time:10.3days.55 patients were found Eschar or ulcer (98.2%),54 cases were found Eschar or ulcer on one part of body,1 case was found two eschars.54 cases had been effectively treated,50 cases were treated with chloramphenicol,4 cases with azithromycin.The average time of temper-ature that dropped to normal were respectively:1.3d,1d.Conclusion awareness of Tsutsugamushi disease should be raised,history seriously should be inquired and specific signs should be identified,which can degrade the misdiagnosis rate and reduce the mortality rate.
3.Effects of Skill-training on Social Function in Patients with Schizophrenia Recession

Objective To explore the effects of skill-training on social function in patients with schizophrenia recession. Methods 60 patientswith schizophrenia recession were divided into 2 equal groups (n=30) for 1-year treatment. The control group was treated only withdrugs, and the treatment group was treated with skill-training in addition. The patients were assessed with the Scale of Social Skill of ChronicSchizophrenic Inpatients (SSSI) and the Positive and Negative Symptom Scale (PANSS) before, 6 months and 12 months after treatment.Results The total scores of PANSS decreased significantly in the treatment group compared with the control group (P<0.01). The scores ofSSSI decreased significantly in the treatment group after treatment. However, there was no significant differences in the control group (P>0.05) 6 months after treatment, but was 12 months after treatment (P<0.05). Conclusion Based on the medicine treatment, skill-training mayimprove the flinched mental symptom to remain the social function.
4.Selective blockage of abnormal Wnt pathway inhibits the growth and invasion of hepatocarcinoma cells

Objective To construct the recombinant expression vector encoding antisense Tcf fragment for the blockage of abnormal Wnt pathway, and to investigate its effect on the biological behaviors of human hepatocarcinoma cells. Methods Antisense expression vector was transfected into hepatocarcinoma cells SMMC-7721 with GeneJammer. RT-PCR and Western blot were used to detect Tcf expression. Cell proliferation and motility were compared by growth curves and Transwell plate assay. Cell apoptosis was determined by Annexin V and cell cycle was examined by fluorescent staining. Results The stable transfection of antisense Tcf in SMMC-7721 cells significantly reduced Tcf expression at both mRNA and protein levels. Compared with parental and mock-transfected 7721 (7721-vector) cells, antisense Tcf RNA transfected cells 7721-pTas showed much decreased activities of proliferation, migration and invasion in vitro. Furthermore, the apoptosis rate of 7721-pTas cells [(26.34±2.07)%] was significantly higher than that of 7721-vector cells [(6.53±1.02)%] and parental SMMC-7721 cells [(4.33±0.68)%] (P<0.001). The percentages of G0-G1 phase antisense transfected cells were 20.24% and 20.95%, higher than parental SMMC-7721 and 7721-vector cells, and percentages of S phase antisense transfected cells were 11.8% and 11.38%, lower than parental SMMC-7721 and 7721-vector cells, respectively. Conclusions Antisense RNA suppress the growth ability of liver cancer cells by inducing cell apoptosis and impeding the progress of cell cycle, which suggests that selective blockage of abnormal Wnt signal pathway by antisense Tcf RNA may be a potential new gene therapy for liver cancer.
5.Application of Lipoprotein Apheresis in Patients With Familial Hypercholesterolemia

Objective: To explore the safety and efficacy of lipoprotein apheresis (LA) in treating the patients with familial hypercholesterolemia (FH).
Methods: A total of 12 FH patients treated in our hospital from 2015-02 to 2016-10 were retrospectively studied. Based on intensive cholesterol lowering therapy with rosuvastatin (10-20) mg Qd and Ezetimibe 10 mg Qd, the patients received LA by double ifltration plasma pheresis (DFPP) via bilateral elbow central vein or femoral vein. The changes of lipid level were compared at before and after LA treatment.
Results: For pre- and immediately after LA treatment, the average total cholesterol (TC) was (9.42±3.65) mmol/L vs (2.84±0.83) mmol/L, low density lipoprotein cholesterol (LDL-C) was (7.31±3.46) mmol/L vs (1.95±0.82) mmol/L; at 1, 3, 7 and 30 days after treatment, TC and LDL-C levels showed increasing trend, while they were still lower than they were before treatment, allP<0.01. For pre- and immediately, 1 day, 3 days after treatment, the average HDL-C level was (0.96±0.31) mmol/L, (0.63±0.17) mmol/L, (0.56±0.15) mmol/L and (0.68±0.22) mmol/L respectively,P<0.05-0.01. For pre- and immediately after LA treatment, the average TG level was (1.90±0.86) mmol/L vs (0.88±0.38) mmol/L,P<0.05. Only 1 patient had the symptoms of hypotension, nausea and sweat, the patient was relieved by expectant treatment.
Conclusion: LA therapy may decrease blood levels of TC and LDL-C at short term in FH patients with good tolerance;even TC and LDL-C could slowly increase after treatment, while combining with lipid lowering therapy, it has been a safe and effective method for treating relevant patients.
6.Clinical features of coronary artery ectasia in the elderly

Objective To investigate the incidence, imaging and clinical characteristics in elderly patients with coronary artery ectasia (CAE). Methods A retrospective analysis was conducted on patients with CAE who underwent coronary angiography between January 2006 and December 2012. According to age, the enrolled patients were divided into two groups (elderly group, age≥ 65 years; non-elderly group, age < 65 years). The clinical feature, imaging characteristics and the 5-year survival rate of the two groups were compared.Results The preva-lence of CAE in elderly patients was 0.33%. Patients in elderly group were found to have significantly higher proportion of female (30.1%vs. 10.1%,P< 0.001), three-vessel disease (60.5%vs. 45.2%,P = 0.003) and localized ectasia (55.0%vs. 40.2%,P = 0.003). In addition, body mass index (20.90 ± 2.71 kg/m2vs. 22.31 ± 2.98 kg/m2,P < 0.001) and percentage of current smokers (45.0%vs. 64.6%,P < 0.001) were significantly lower in elderly group. Cumulative survival curves demonstrated reduced 5-year cumulative survival at the follow-up in the elderly group compared with the non-elderly group (88.0%vs. 96.0%,P = 0.002). But the 5-year event free survival rate failed to show a significant difference between the two groups (31.0%vs. 35.0%,P= 0.311).ConclusionThe prevalence of CAE in elderly patients was 0.33%, which was about 1/3 of the entire numbers of CAE patients. There were significant differences between the elderly and the non-elderly patients with CAE in terms of coronary artery disease risk factors and coronary artery ectatic characteristics. CAE might be asso-ciated with increased mortality risk in the elderly.
7.Effect of gynostemma pentaphyllum saponins on pituitary⁃adrenal axis induced by hypoxia in rats

Objective :
To observe the effect of hypoxia on pituitary adrenal axis in rats and the therapeutic effect of
Gynostemma pentaphyllum saponins(GPs)intervention.
Methods :
50 male SD rats were randomly divided into normoxia control group , hypoxia control group , and hypoxia + GPs low , medium , and high dose intervention groups , with 10 rats in each group. Rats in the normoxia group were kept in a normoxia animal room for 28 days , while rats in the hypoxia group and drug intervention group were kept in a low⁃pressure oxygen chamber for 28 days. Among
them , hypoxia + GPs low , medium and high dose groups of rats were gavaged daily with GPs of 40 mg/kg , 80 mg/kg , and 160 mg/kg , respectively. After 28 days , rats were killed in batches , their body weight and pituitary weight were measured , and pituitary index was calculated ; HE staining method was used to observe the morphology and structure of rat pituitary gland ; immunohistochemical and Western blot methods were used to observe the expression level of pituitary adrenocorticotropin ( ACTH) protein in rats ; ELISA method was used to observe serum ACTH and corticosterone ( CORT) concentrations in rats.
Results :
Hypoxia had no significant effect on pituitary gland morphology and quality in rats. Hypoxia might induce the expression of rat pituitary ACTH protein and an increase in serum ACTH and CORT concentrations , while GPs might reduce the expression of rat pituitary ACTH protein and serum ACTH and CORT concentrations induced by hypoxia.
Conclusion
GPs have a significant effect on the pituitary adrenal axis induced by hypoxia in rats , and there is a dose⁃effect relationship between GPs and effect.
8.Analysis on cases of fetus limb deformity using systematic continuous sequence approach during early and middle pregnancy

Objective To summarize prenatal two-dimensional ultrasound examination results of different types of fetal limb deformity,and emphasise on some rare deformity disease such as phocomelia and sirenomelia.To investigate the value of systematic continuous sequence approach in the examination of fetal limb during pregnancy periods.Methods Eighteen thousand seven hundred and eight cases of fetus were selected from September 2014 to August 2016 in the 105th Hospital of Chinese People's Liberation Army,which were screened by routine and systematic ultrasonography.The systematic continuous sequence approach was used to focus on examination of the fetal limb.All fetuses with abnormal limbs were followed up.The ultrasonographic images were compared with the results of pathological examination and X-ray examination to summarize the ultrasonographic features of fetal limb deformities.Results Seventy-six cases of fetal limb deformity were detected by prenatal ultrasotmd system in 18 708 cases.Three cases were detected at 11-13 + 6 weeks of gestation,18 at 14-19 + 6 weeks,38 at 20-27 + 6 weeks,17 at 28-35 + 6 weeks.Types of limb deformities are as fllowing,11 cases of abnormal number of fingers (adactylism,polydactyly,syndactyly),2 cases of cleft hand deformity,45 cases of foot varus,1 case of sirenomelia and 4 cases of short limbs.Thirty-seven cases of fetal limb deformity combined with other system malformations and 12 cases were with multiple system malformations.Karyotypes were abnormal in 11 cases,including 3 cases of trisomy 21,6 cases of trisomy 18 and 3 cases of trisomy 13.One case of prenatal ultrasound diagnosis of twins with sirenomelia.An autopsy of twin fetus indicated that there were epactal toes at the left foot.Prenatal ultrasonography.Conclusions The systematic continuous sequence approach in early and middle pregnancy play the advantages of each pregnancy examination.The fetal limb deformity can be detected early by combining application of systematic continuous sequence approach.
9.The significance of protein SP70 detection for differentiating benign and malignant pleural effusion

Objective To investigate the diagnostic value of detection of protein SP70 in differentiating benign and malignant pleural effusion.Methods A case-control study was conducted from July 2011 to February 2012.108 cases of pleural effusion from patients with clinically proven lung cancers and 122 cases of benign pleural effusion were collected.SP70 was detected by Sandwich ELISA,while CEA,CYFRA21-1,NSE were measured by electrochemiluminescence immunoassay for comparison.Meanwhile,protein SP70 on exfoliated cells in pleural effusion was detected by direct immunofluorescence,and was compared with the results of HE staining.The differences between the groups were evaluated by the chisquare test Fisher' s exact test.Results Positive rates of SP70,CEA,CYFRA21-1,NSE were 72.2%,58.3%,52.8% and 30.6% in malignant pleural effusion,obviously higher than benign pleural effusio (9.8%,13.1%,23.0% and 19.7%).The specificity of SP70,CEA,CYFRA21-1,NSE were 90.2%,86.9%,77.0% and 80.3%,NSCLC had significantly higher positive rate than SCLC(74.3% >0.0%,P =0.02 < 0.05),detection of protein SP70 in malignant pleural effusion had significantly higher coincidence rate than HE staining(72.2% vs 47.2%,x2 =14.03,P < 0.05).Conclusion Determination of the protein SP70 in pleural effusion and in exfoliated cells,can improve the sensitivity and specificity of the diagnosis of malignant pleural effusion.
10.Relationship Between ABO Blood Type and Spontaneous Re-canalization in Patients With Acute Myocardial Infarction

Objective: To study the relationship between ABO blood type and spontaneous re-canalization (SR) in patients with acute myocardial infarction (AMI). Methods: A total of 1209 consecutive AMI patients were enrolled. Based on TIMI grade, the patients were divided into 2 groups: Non-SR group, the patients with TIMI grade 0-1,n=442 and SR group, the patients with TIMI grade 2-3,n=767. The relationship between ABO blood type and SR was investigated. Results: Compared with Non-SR group, SR group had more patients with blood type O (32.3% vs 24.7%) and less blood type A (31.7% vs 24.9%). Meanwhile, we found that a lower cholesterol level was related to patients with O blood type and SR occurrence, bothP<0.05. Multi regression analysis indicated that with adjusted age, gender, BMI, hypertension, diabetes, smoking, LDL-C and C-reactive protein, ESR, fibrinogen, D-dimmer, endothelial cardiac function, blood type O may independently predict SR occurrence in AMI patients (OR=1.49, 95% CI 1.10-2.05), while blood type A may have disadvantage for SR (OR=0.65, 95% CI 0.48-0.80). Conclusion: ABO blood type has been related to SR in AMI patients, blood type O is in favor of SR, while blood type A has disadvantage for SR occurrence.
