[Objective] To investigate the therapeutic effect of Honghua injection plus prednisone and cyclophosphamine on refractory nephrotic syndrom.[Methods] 38 patients with refractory nephrotic syndrom were given intravenous Honghua and cyclophosphamine plus oral prednisone.Serum total cholesterol,triglyceride,liver and renal function,blood routine test,24h protennuria were observed.[Results] Total effective ratio was 92.1%.Cure ratio was 73.68%.Partly cure ratio was 18.42% and ineffective ratio was 7.89%.[Conclusions] Patients with refractory nephrotic syndrom were treated by Honghua injection plus prednisone and cyclophosphamine,which could reduce the application of prednisone,its side effects and enforce its therapeutic effect.What is more,this treatment could lessen therapeutic process,prevent relapse and recoil,protect effectively the retreat syndrom of prednisone as well.

Objective To investigate the effects of the combination of tripterygium wilfordii Hook F (TwHF) and irbesartan on urinary podocyte in diabetic kidney disease (DKD) patients,and to discuss the mechanism of protective effect of TwHF on DKD.Methods A total of 45 type 2 diabetic kidney disease patients were enrolled into this prospective study,and were randomly divided into 3 groups:TwHF treatment group (DT,n =15),irbesartan treatment group (DI,n =15),and TwHF combined with irbesartan treatment group (DTI,n =15).After 6 weeks washout,the 3 groups were given TwHF (1-2 mg · kg-1 ·d-1),irbesartan (150-300 mg/d),and TwHF (1-2 mg · kg-1 · d-1) combined with irbesartan (150-300 mg/d) for 12 weeks respectively.Fifteen healthy volunteers served as controls.Urinary podocytes were identified and quantitated by immunofluorescence staining of urinary sediments labeled by monoclonal antibody podocalyxin.In addition,we studied urinary connective tissue growth factor (CTGF),osteopontin (OPN) and transforming growth factor β1 (TGFβ1) concentrations in DKD patients by enzyme-linked immunosorbent assay.Results Urinary detached podocytes were obviously higher in the urine of DKD patients than in healthy controls (P ＜0.01).Podocyte detection rate was 86.6％ in the urine of DKD patients.The protein expressions of CTGF,OPN and TGFβ1 in patients with urinary podocyte were significantly increased than those without urinary podocyte (P ＜ 0.05 or 0.01).Correlation analysis showed that there was positive correlation between urinary protein excretion and urinary podocytes (r =0.79,P ＜ 0.01) and there were positive correlations between the number of urinary podocytes and urinary protein expressions of CTGF,OPN and TGFβ1 (r =0.56,0.41,0.44,respectively,all P values ＜ 0.01).Urinary albumin excretion and urinary podocytes were significantly decreased in all treatment groups (P ＜ 0.01),simultaneously,urinary concentrations of CTGF,OPN and TGFβ1 were reduced in all groups at week 12 after intervention of TwHF,irbesartan and TwHF combined with irbesartan (P ＜ 0.01),and these changes were more distinguished in combined treatment group (P ＜ 0.05).Conclusion Urinary podocyte in the urine may be suggested to be an early effective marker of disease activity in DKD.TwHF may be effective to prevent podocyte injury in DKD,which may be mediated,at least partly,by down-regulating the expression of CTGF,OPN and TGFβ1.There is a synergistic protective effect of TwHF combined with irbesartan on podocyte injury in DKD patients.

To evaluate the value of Doppler tissue imaging(DTI) to assess left ventricular systolic and diastolic function by determining motion frequency of the mitral annulus in acute myocardial ischemia and myocardial infarction. The left anterior descending coronary artery(LAD) was ligated in 10 pigs and systolic motion velocity(Sa), early diastolic velocity(Ea), late diastolic velocity(Aa) and Ea/Aa of lateral mitral annulus in apical 4 chamber view using DTI velocity model before and after ligation of LAD were determined.The results were compared with routine Doppler. In basal state, Ea/Aa was positively correlated with VE/VA (the ratio of early and late diastolic velocity of mitral flow) with Doppler imaging (?=0.82). After the ligation of LAD, Sa, Ea and Ea/Aa significantly decreased, while there was no difference in Aa. DTI is simple and easy for determining mitral annulus motion. Objective To evaluate the relationship between the expressions CyclinD1 and nm23H1 and the response to neoadjuvant chemotherapy (NACT) in cervical carcinoma. Methods 100 patients with cervical cancer were enrolled in this study. These patient were randomly divided into two groups. The observed group were treated by neoadjuvant chemotherapy followed by radical hyterectomy and lymphonectomy. The other 50 cases were treated by operation as control group. The expression of CyclinD1 and nm23H1 were examined by immunohistochemistry using post-operation specimens. Results The total response rate of neoadjuvant chemotherapy was 75 %( CR+PR). The expression rate of CyclinD1 was 76 % in experimental group, and 54 % in control group(P 0.05). Conclusion The neoadjuvant chemotherapy was helpful for the patients with local advanced stage of cervical carcinoma. CyclinD1 may be one of the path of NACT, but nm23H1 may not related to NACT.

Objective To study the mechanism of anti-aging effect of dimethylaminoethanol (DMAE) and compound amino acid (AA) injection by mesotheray in D-galactose-induced skin aging rat.Methods Eighty rats were randomly divided into aging treatment group (60 cases),aging control group (10 cases) and normal control group (10 cases).The skin aging models were established by subcutaneous infectim of D-galactose.From the 18th day,the aging treatment group were injected intradermally in the rats' both sides hip skin with 0.2％ DMAE+ AA,0.1％ DMAE+AA,0.2％DMAE,0.1％ DMAE,AA,and saline,once a week.After four weeks,HE,water content,superoxide dismutase (SOD) activity,hydroxyproline (HYP) and malondialdehyde (MDA) content were measured.Results Compared with the aging control group,the epidermal and the dermal thickness and the collagen area of rats skin' increased significantly in 0.2 ％ DMAE+ AA and 0.1％ DMAE+ AA treatment groups (P＜0.05).0.2％ DMAE+AA and 0.1 ％ DMAE+AA treatment groups also had higher SOD activity,HYP content and lower MDA content than other groups (P＜0.05),but no difference was noted among normal control group,0.2％ DMAE+AA and 0.1％ DMAE+AA group (P＞0.05).There were no differences in water content among groups (P＞0.05).Conclusions Intradermal injection with 0.1％ DMAE+ AA and 0.2 ％ DMAE+AA in aging rats may increase the epidermal and the dermal thickness and the collagen of rats skin' improve SOD activity,HYP content and decreased MDA content,indicating that it has ability to clear skin free radicals,enhance antioxidant capacity and skin collagen metabolism,and thus prevent skin aging.

ObjectiveTo study the expression of telomerase of fibroblasts in the formation and development of scar,and to investgate the relationship between the telomerase activity and the formation and development of scar.MethodsExpression of telomerase was detected by immunohistochemical techniques in 18 specimens from granulation tissue,17 keloid,16 specimens from hypertrophic scar,28 specimens from mature scar,32 specimens from normal skin,and SPSS16.0 statistics software was used to analyze the relationship between telomerase and scar.Results It was shown that the positive expression rate of telomerase in the granulation tissue group was 94.4 ％,that in the keloid group was 58.8 ％,that in the hypertrophic scar group was 18.8 ％, and that in the mature scar and in the normal skin was zero. comparisons between groups,in addition to the normal scar group and normal skin group,other groups were statistically significant differences.ConclnsionsScar formation is a process with multi-factor participation,while telomerase activation is an important factor. Detecting telomerase activity in the development process of scar formation may determine the prognosis.Through the control of telomerase activity in the pathogenesis of scar may become a new approach of scar treatment.

Objective To explore ultrasound performances and misdiagnostic causes of aggressive fibromatosis (AF).Methods Ultrasound performance and misdiagnostic causes of 45 patients (47 tumors) with AF confirmed by histopathology were analyzed retrospectively.Results Of 45 patients,ultrasound showed lesions of varying size,and the maximum diameter of 36 masses (36/47,76.60％) were more than 3 cm;37 masses (37/47,78.72％) were irregular pale leaf;45 masses (45/47,95.74％) had no complete capsule;45 masses (45/47,95.74％) were shown as interogeneous internal echo mixed with hyperechoic area in the hypoechoic internal;29 cases were diagnosed correctly,16 cases were misdiagnosed,the diagnostic accordance rate was 64.44％ (29/46).Conclusion AF has certain ultrasonic characteristics.Combined with the patient's medical history and physical sign,ultrasound can significantly improve the preoperative detection rate and diagnostic accuracy of AF.

目的探讨轮椅技能训练的简单化、实用化和规范化。方法将 216例脊髓损伤住院患者按性别、年龄和损伤平面分为4组进行轮椅技能训练,项目包括绕障碍物行走计时、100m竞速、400m竞速、2柱×10折返跑、上下台和上下坡道,将各项目按成绩分为10个级别,制定出不同组别患者应该达到的水平,评定患者的训练效果。结果训练后,患者的100m、400m竞速、2柱×10跑折返、绕障碍计时成绩和上台(坡)的能力明显提高(P<0.01);训练效果明显75人、有效141人。结论轮椅技能训练可使脊髓损伤患者真正达到生活自理、自立,但应根据患者的损伤平面制定规范化的训练方案。
