Objective To investigate the effects of the combination of tripterygium wilfordii Hook F (TwHF) and irbesartan on urinary podocyte in diabetic kidney disease (DKD) patients, and to discuss the mechanism of protective effect of TwHF on DKD.Methods A total of 45 type 2 diabetic kidney disease patients were enrolled into this prospective study, and were randomly divided into 3 groups:TwHF treatment group (DT,n =15),irbesartan treatment group (DI,n =15),and TwHF combined with irbesartan treatment group (DTI,n =15).After 6 weeks washout, the 3 groups were given TwHF (1-2 mg · kg-1 ·d-1),irbesartan (150-300 mg/d),and TwHF (1-2 mg · kg-1 · d-1) combined with irbesartan (150-300 mg/d) for 12 weeks respectively.Fifteen healthy volunteers served as controls.Urinary podocytes were identified and quantitated by immunofluorescence staining of urinary sediments labeled by monoclonal antibody podocalyxin. In addition,we studied urinary connective tissue growth factor (CTGF),osteopontin (OPN) and transforming growth factor β1 (TGFβ1) concentrations in DKD patients by enzyme-linked immunosorbent assay. Results Urinary detached podocytes were obviously higher in the urine of DKD patients than in healthy controls (P <0.01).Podocyte detection rate was 86.6% in the urine of DKD patients. The protein expressions of CTGF,OPN and TGFβ1 in patients with urinary podocyte were significantly increased than those without urinary podocyte (P < 0.05 or 0.01).Correlation analysis showed that there was positive correlation between urinary protein excretion and urinary podocytes (r =0.79,P < 0.01) and there were positive correlations between the number of urinary podocytes and urinary protein expressions of CTGF,OPN and TGFβ1 (r =0.56,0.41,0.44,respectively,all P values < 0.01).Urinary albumin excretion and urinary podocytes were significantly decreased in all treatment groups (P < 0.01),simultaneously,urinary concentrations of CTGF,OPN and TGFβ1 were reduced in all groups at week 12 after intervention of TwHF,irbesartan and TwHF combined with irbesartan (P < 0.01),and these changes were more distinguished in combined treatment group (P < 0.05).Conclusion Urinary podocyte in the urine may be suggested to be an early effective marker of disease activity in DKD.TwHF may be effective to prevent podocyte injury in DKD,which may be mediated,at least partly,by down-regulating the expression of CTGF,OPN and TGFβ1.There is a synergistic protective effect of TwHF combined with irbesartan on podocyte injury in DKD patients.

Objective To investigate the effects of Irbesartan on endothelial function and autonomic nervous system activity of patients with essential hypertension. Methods The endothelium dependent flow mediated dilatation (F-MD) and the endothelium independent nitroglycerin mediated dilatation (NTG-MD) were evaluated by the brachial ultrasound examination. Time-domain indices of heart rate variability (HRV) were measured by 24 h ambulatory electrocardiogram. The changes of blood lipoproteins, blood glucose, insulin levels, endothelial function and heart rate variability in 60 patients with mild-moderate essential hypertension and 20 healthy controls were compared, and the effects of different antihypertensive agents (AT1 receptor blocker and diuretic) for 8-week treatment were observed. Results Endothelial dysfunction and abnormal autonomic nervous activity were observed in patients with mild-moderate essential hypertension as follow: mildly decreased total HRV and mildly increased 24-hour average heart rate, significantly decreased parasympathetic tone-rMSSD and PNN50, obviously lowered endothelium dependent F-MD. The postprandial insulin level and insulin/glucose ratio of patients with hypertension was significantly higher than that of healthy controls. The decreased blood pressure level caused by AT1 receptor blocker (Irbesartan) and diuretic (Indapamide) was similar. Irbesartan for 8-week therapy resulted in a significant improvement of the endothelium dependent F-MD and insulin resistence, the increase of vagal nervous activity and the decrease of 24-hour average heart rate, but Indapamide presented no such effects. Conclusion This study suggested that patients with mild-moderate essential hypertension have significantly abnormal endothelial function and autonomic nervous activity, and the beneficial effect of Irbesartan is associated with an improvement of insulin resistence and heart rate variability, not with the decrease of blood pressure level.

OBJECTIVE To investigate the distribution of familial hepatitis B virus (HBV) genotypes and analyze the relationship between genotypes and clinical outcome. METHODS A total of 136 cases of familial hepatitis B were involved in this study, including 25 cases of asymptomatiec HBV carriers (ASC),85 cases of chronic hepatits B (CHB),24 cases of liver cirrhosis (LC) and 4 cases of hepatocellular carcinoma (HCC). Nested polymerase chain reaction-restriction fragment length polymorphism (ntPCR-RFLP)was used to identify the genotypes of HBV. RESULTS There were 124 cases with hepatitis genotype C,7 cases with genotypes B+C mixed,3 cases with genotype B and 2 cases that did not conformed to be any of genotypes from A to H. Genotype C in the HCC was predominant genotype (100%),next in the LC(96%),CHB (95.3%) and ASC(66.7%).Compared with genotype C,genotypes B and B+C only existed in ASC and CHB,the proportion was 9.5% vs 23.8% and 1.2% vs 2.3%,respectively. There were no other HBV genotypes from A to H seen in familial hepatitis B in Shanxi province. CONCLUSIONS HBV genotype C is the major one in familial hepatitis B in Shanxi. There are a few genotypes B and B+C existed. Genotype C induces a relatively bad prognosis.
