Chinese Journal of Rehabilitation Theory and Practice 2016;22(10):1196-1199
Objective To investigate the effect of cerebroprotein hydrolysate on the serum high sensitivity C reactive protein (hsCRP), insulin-like growth factor-1 (IGF-1) and interleukin-18 (IL-18) in patients with hypoxic ischemic encephalopathy (HIE) in plateau region. Methods From January, 2013 to May, 2015, 104 cases with neonatal HIE in our department were randomly divided into conventional treat-ment group (n=52) and cerebroprotein hydrolysate group (n=52). Besides, 35 cases of healthy newborn were chosen as the control group. The total effective rate and the serum hsCRP, IGF-1, IL-18 levels were compared. Results The IGF-1 level was lower, and the levels of hsCRP and IL-18 were higher in moderate HIE group and severe HIE group than in the control group and the mild HIE group, respectively (P<0.05). The efficiency rate was significantly higher in the cerebroprotein hydrolysate group than in the conventional treatment group (χ2=8.922, P=0.012). Compared with the conventional treatment group, the IGF-1 level increased, and the levels of hsCRP and IL-18 decreased in the cerebroprotein hydrolysate group (P<0.05). Conclusion Cerebroprotein hydrolysate is effective on patients with HIE in plateau re-gion. The changes of serum hsCRP, IGF-1, and IL-18 levels can be used as auxiliary indexes for early diagnosis of HIE.

Objectives To explore the characteristics and value of magnetic resonance imaging (MRI) in predicting prognosis of hypoxic ischemic encephalopathy (HIE) in neonatal period, at 4 months and 4 years old. Methods Twenty-four patients with HIE were examined by MRI. Their MRI results in the neonatal period, at month 4 and year 4 and neurological functions at year 4 were compared. Results Periventricular signal alterations and deep gray matter involvement were common in HIE neonates. The neonates with deep gray matter involvement usually had neurological malfunction and poor prognosis. The patients with encephalomalacia and periventricular leukomalacia at 4 months and 4 years old also had poor prognosis. The abnormal MRI ifndings in HIE children at 4 month and 4 year old predicted the occurrence of neurological malfunction. Con-clusions The MRI of infant at 4 months old is important in prediction of neurological malfunction and provides guidance of clinical intervention for children with HIE.

Objective To observe the change of coagulation function in chronic stress depression model rats and the effects of elec-troacupuncture (EA) on it. Methods A total of 32 Sprague-Dawley rats were randomly divided in control group (n=8), model group (n=8), EA group (n=8) and fluoxetine group (n=8) after a week of acclimation. The 21-day unpredictable mild stress combined with solitarily feed-ing was used to make the depression model in the latter three groups, meanwhile EA group and the fluoxetine group accepted EA and fluox-etine. They were measured the prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT) and fibrinogen (FIB). Serum levels of 6-keto-prostaglandin F1α(6-keto-PGF1α), P-selectin, thromboxane B2 (TXB2) were determined with enzyme-linked im-munoabsorbent assay. Results The cross and rear scores were significantly lower in the model group than in the control group (P<0.01). The scores increased in EA group and the fluoxetine group (P<0.01) compared with the model group. Compared with the control group, PT, APTT and TT decreased (P<0.01), FIB increased (P<0.05), the level of 6-keto-PGF1αdecreased, the level of P-selectin and TXB2 increased (P<0.01) in the model group. Compared with the model group, PT, APTT and TT increased (P<0.05), the level of 6-keto-PGF1αincreased (P<0.01), and the level of P-selectin and TXB2 decreased (P<0.05) in EA group and the fluoxetine group, and FIB decreased in EA group (P<0.05). Conclusion The chronic stress depression rats are impaired in the coagulation function and platelet activation, and can be allayed with electroacupuncture and fluoxetine.

Objective To discuss the risk factors for susceptibility of subcutaneous tophi with an aim to provide clinical evidence for the prevention and treatment of tophi. Methods A total of 5 321 cases of gout patients whose course of disease was less than 10 years were selected and divided into two groups according to whether a subcutaneous tophus was present. The clinical information was collected and relevant biochemical indices were detected. Results There were significant differences in the ratios of regular exercise, involvement of upper limb joints, combining renal insufficiency, kidney stone and coronary heart disease between the tophus group and the non-tophus group(P<0.05 or P<0.01). There were significant statistical differences in age, disease duration, body mass index, systolic blood pressure, diastolic blood pressure, serum uric acid, serum creatinine, creatinine clearance rate, the number of joints involved between the two groups[(51.80±14.58 vs 48.45±14.45)year,(5.61±3.20 vs 3.28±3.06)year,(26.50±3.39 vs 27.05±3.45)kg/m2,(138±20 vs 133±18)mmHg, 1 mmHg=0.133 kPa,(89±13 vs 87±12)mmHg,(486.66±121.96 vs 460.02±119.10) μmol/L,(112.11±62.97 vs 104.43±46.28) μmol/L, (86.46±37.99 vs 93.00±40.11 )ml/min,(2.89±1.86 vs 1.76±1.22) joints, all P<0.01]. There were no statistical differences in family history, gender, history of smoking and drinking, the means of waistline, fasting blood glucose, triglyceride, and total cholesterol between the two groups(all P>0.05). Further logistic regression analysis indicated that disease duration, a large number of joints involved, the involvement of upper limb joints, combining kidney stones, higher serum uric acid level, and higher diastolic pressure were risk factors of the early onset of subcutaneous tophi, while regular exercise and body mass index were protective factors. Conclusion Patients with long duration, high serum level of uric acid, a large number of joints involved, upper limb joint involved, kidney stones, and hypertension were more likely to develop subcutaneous tophi. These risk factors should be intervened actively in clinic.

The study intended to evaluate the effect of high-dese atorvastafin on serum high sensitive C-reactive protein (hs-CRP) and renal function in patients with acute myocardial infarction undergoing elective pereutancous coronary intervention ( PCI ). One hundred and sixty seven patients were randomly divided into two groups: in test group (n =84) patients received oral atorvastatin 80 mg/d and in control group (n = 83) patients received atorvastatin 20 mg/d, the medication in both groups was lasted for 7 days before PCL Compared to levels at 24 h before PCI, serum hs-CRP and creatinine levels at 48 h after PCI were increased in both groups ( both P < 0. 05), and glomerular filtration rate was decreased ( P < 0. 05 ). Compared to control group, serum hs-CRP and creatinine levels 24 h before PCI and 48 h after PCI in test group were significantly lower, and glomerular filtration rate was significantly higher (P <0. 05, respectively). The incidence of contrast-induced nephropathy was lower in test group than that in control group[7% (6/84) vs.18% (15/83), P <0.05]. The results indicate that high-dose atorvastatin might be effective in protecting patients with acute myocardial infarction undergoing elective PCI from contrast-induced nephropathy via inflammatory response inhibition.
