3.Design of dynamic skin impedance detector for the back electrode of high frequency surgical equipment.

For the conventional high frequency(HF) surgical equipment, the contact impedance between the two back electrodes when they are gluing with skin is 50-200 Omega. If the contact impedance between the two electrodes is either too large or too small, an alarm is needed. This detector is designed not only to detect the contact condition between skin and the back electrodes, but also to monitor and alarm the dynamic change of the contact impedance. It effectively prevents the potential damage to medical staff when the back electrodes of the double-loop HF surgical equipment is moved during operation. Thus, this design enhances the safety of HF surgical equipment in clinical use.

4.The facture and application of a new type of bedpan

Objective Our objective was to design a new type of bedpan (inflatable, bed urinal) and compare its effect with common bedpan. Methods We divided 144 patients with bone fracture and lying in bed into the test group and the control group with 72 cases in each group from May 2006 to February 2007. Inflatable bed urinals were used in the test group and common bedpans were used in the control group. The pain alleviation, comfort degree, staining of bed sheet and skin injury were observed and evaluated in each group. Results Application of inflatable bed urinal was superior to common bedpan in the following aspects: alleviation of pain, comfort degree, staining of bed sheets and skin injury. Conclusions Adoption of inflatable bed urinal could alleviate pain, prevent the incidence of complication and reduce the workload of nurses.
5.Hypoxia training attenuates left ventricular remodeling in rabbit with myocardial infarction

Objective Previous studies showed that hypoxia preconditioning could protect cardiac function against subsequent myo-cardial infarction injury. However, the effect of hypoxia on left ventricular after myocardial infarction is still unclear. This study therefore aims to investigate the effects of hypoxia training on left ventricular remodeling in rabbits post myocardial infarction. Methods Adult male rabbits were randomly divided into three groups: group SO (sham operated), group MI (myocardial infarc-tion only) and group MI-HT (myocardial infarction plus hypoxia training). Myocardial infarction was induced by left ventricular branch ligation. Hypoxia training was performed in a hypobaric chamber (having equivalent condition at an altitude of 4000 m, FiO214.9%) for 1 h/day, 5 days/week for four weeks. At the endpoints, vascular endothelial growth factor (VEGF) in the plasma was measured. Infarct size and capillary density were detected by histology. Left ventricular remodeling and function were as-sessed by echocardiography.Results After the 4-week experiment, compared with the group SO, plasma VEGF levels in groups MI (130.27 ± 18.58 pg/mL,P< 0.01) and MI-HT (181.93 ± 20.29 pg/mL,P< 0.01) were significantly increased. Infarct size in Group MI-HT (29.67% ± 7.73%) was deceased remarkably, while its capillary density (816.0 ± 122.2/mm2) was significantly increased. For both groups MI and MI-HT, left ventricular end-diastolic and end-systolic dimensions were increased whereas left ventricular ejection fraction was decreased. However, compared with group MI, group MI-HT diminished left ventricular end-diastolic (15.86 ± 1.09 mm,P< 0.05) and end-systolic dimensions (12.10 ± 1.20 mm,P< 0.01) significantly and im-proved left ventricular ejection fraction (54.39 ± 12.74 mm,P< 0.05).ConclusionHypoxia training may improve left ven-tricular function and reduce remodeling via angiogenesis in rabbits with MI.
6.Biliary complications and recovery of liver function after liver transplantation from citizen's deceased donors versus standard criteria donors

Objective To investigate the biliary complications and recovery of liver function after liver transplantation from citizen's deceased donors (DCD) versus standard criteria donors (SCD).Method The clinical data of 269 patients who underwent orthotopic liver transplantation from January 2009 to December 2016 at the Fuzhou General Hospital were collected.197 livers were from SCD and 72 from DCD.Propensity score matching (PSM) was used to compare the biliary complications and recovery of liver function after liver transplantation in the two groups.Results PSM matched 61 pairs of patients.There were 10 (16.4%) and 8 (13.1%) biliary complications in the DCD and the SCD groups,respectively,with no significant difference between them (P > 0.05).The recovery of liver function was significantly delayed in the DCD group when compared with the SCD group.The levels of ALT,AST,GGT and AKP in the DCD group were significant different on the postoperative first,third,fifth,seventh and fourteenth day (P < 0.05).At 30 days after surgery,there was no significant difference in liver function between the two groups.Conclusions Liver grafts from DCD had a significant impact on the recovery of liver function.When compared with the SCD group,the DCD group recovered significantly slower in liver function.There was no significant increase in the incidence of biliary complications.

Liver fibrosis is a wound healing response that occurs in the setting of chronic liver injury and is caused by imbalance in the synthesis and degradation of extracellular matrix (ECM). If left untreated, it can progress to liver cirrhosis and hepatocellular carcinoma. The activation of hepatic stellate cell (HSC) is now well established as a central driver of liver fibrosis. The activated HSC will transform into myofibroblasts that produce ECM protein. Transforming growth factor-β1 (TGF-β1) can induce the activation of hepatic stellate cell (HSC), and TGF-β1/Smads signaling pathway is one of the important pathways to promote liver fibrosis. Non-coding RNA (ncRNA) does not encode proteins during the transcription but plays an important regulatory role in the post-transcriptional process of genes. Accumulating evidence shows that the occurrence of liver fibrosis is closely related to the abnormal expression of ncRNA which participates in the activation of HSC by regulating TGF-β1 signal transduction and then affects the process of liver fibrosis. MiRNA-mediated TGF-β1/Smads signaling pathway can not only promote liver fibrosis but also play a role in anti-fibrosis. Long non-coding RNA (lncRNA) not only promotes the development of liver fibrosis by binding to target genes but also enhances TGF-β1 signal transduction by acting as competitive endogenous RNA. circular RNA (circRNA) acts as a ''sponge'' to regulate TGF-β1/Smads pathway, thereby inhibiting HSC activation and exerting the anti-liver fibrosis effect. Chinese medicinal plays an essential part in the prevention and treatment of liver fibrosis, and the active components can inhibit TGF-β1/Smads pathway by regulating the expression of miRNA, thus alleviating liver fibrosis. This article reviews the role and mechanism of miRNA-, lncRNA- and circRNA-mediated TGF-β1/Smads signaling pathway in liver fibrosis and summarizes the anti-liver fibrosis effect of active components of Chinese medicinals by regulating miRNA-mediated TGF-β1/Smads signaling pathway, which can serve as a reference for clinical treatment of liver fibrosis and the development of new drugs.
8.Effects of Isodon ternifolius extract on TLR 4/NF-κB/NLRP3 pathway in hepatic stellate cells and hepatocytes

OBJECTIVE To investigate the effects of Isodon ternifolius extract on Toll -like receptor 4(TLR4)/nuclear factor - κB(NF-κB)/NOD-like receptor protein 3(NLRP3)pathway in hepatic stellate cells and hepatocytes . METHODS Using primary hepatic stellate cells and primary hepatocytes of rats as subjects ,inflammatory injury model was induced by lipopolysaccharide (LPS,100 ng/mL). Using colchicine (1 μmol/L)as positive control and TLR 4 inhibitor(TAK-242,1 μmol/L)as reference ,the contents of transforming growth factor -β1(TGF-β1),α-smooth muscle actin (α-SMA),type Ⅰ collagen(COL-Ⅰ),COL-Ⅲ in the supernatant of hepatic stellate cells ,and the contents of alanine aminotransferase (ALT),aspartate aminotransferase (AST),interleukin-1β (IL-1β) and IL -18 in the supernatant of hepatocytes were detected after intervention with I. ternifolius extract(1 mg/mL). The mRNA expressions of TLR 4,inhibitor α of NF -κB(IκBα),NF-κB p 65,NLRP3,Gasdermin D (GSDMD),apoptosis-associated speck -like protein containing CARD (ASC) and protein expressions of TLR 4, phosphorylated I κBα(p-IκBα),NF-κB p 65,NLRP3,GSDMD,ASC in the above cells were determined . RESULTS Compared with LPS model group ,the contents of TGF -β1,α-SMA, COL-Ⅰ and COL -Ⅲ in the supernatant of hepatic stellate cells and the contents of ALT ,AST,IL-1β and IL -18 in the supernatant of hepatocytes were significantly decreased ;the mRNA expressions of TLR 4,NF-κB p 65,NLRP3,GSDMD,ASC and the protein expressions of TLR 4,p-IκBα,NF-κB p 65,NLRP3,GSDMD,ASC in the two kinds of cells were significantly down - regulated,and the mRNA expression of I κBα was significantly up -regulated in all administration groups (P<0.05). Compared with TLR 4 inhibitor group ,the improvement of most of above indexes was more significant in the TLR 4 inhibitor+I. ternifolius extract group (P<0.05). CONCLUTIONS I. ternifolius extract could inhibit the activation of TLR 4/NF-κB/NLRP3 pathway, reduce the release of fibrosis factors and inflammatory factors ,alleviate the inflammatory injury of liver cells ,and inhibit the activation of hepatic stellate cells ,so as to protect liver cells and resist liver fibrosis .
