1.Prognostic factors of relapse and metastasis after radical resection of lung adenocarcinoma

Objective To explore the risk factors of local relapse and distant metastasis after radical resection of lung adenocarcinoma.Methods A total of 102 patients with lung adenocarcinoma operated in First Affiliated Hospital of Xinjiang Medical University from January 2005 to January 2010 were collected.The correlation between clinicopathological characteristics and prognosis was evaluated by single-factor and multi-factor analyses.The survival curves were plotted using Kaplan-Meier.Single-factor analysis of statistical difference was tested using Log-rank test.Multi-factor analysis of prognostic factors were produced by COX regression proportional hazards model.Results In the whole group,1,2,3 and 5 year disease-free survival rates were 74.30%,58.00%,51.50% and 44.90% respectively,and the median disease-free survival was 30 months.Single-factor analysis showed that tumor size (x2 =9.951,P =0.002),clinical type (x2 =8.460,P =0.004),differentiated degree (x2 =4.807,P =0.028),lymph node metastasis (x2 =40.516,P =0.000),pathological stage (x2 =38.769,P =0.000) were prognostic factors for local relapse and distant metastasis in postoperative patients with lung adenocarcinoma.Muti-factor analysis showed that tumor size (OR =1.943,95 % CI:1.091-3.463,x2 =5.082,P =0.024),differentiated degree (OR =2.570,95 % CI:1.451-4.552,x2 =10.467,P =0.001),lymph node metastasis (OR =3.196,95% CI:1.037-9.849,x2 =4.096,P =0.043) were independent prognostic factors for local relapse and distant metastasis in postoperative patients with lung adenocarcinoma.Conclusion Tumor size,differentiated degree and lymph node metastasis are independent prognostic factors for local relapse and distant metastasis in postoperative patients with lung adenocarcinoma.

AIM:TostudythefunctionofmicroRNA(miR)-19aandmiR-92abyseed-targetinginhibitionin multiple myeloma cells and their signal pathways .METHODS:The experiments were divided into t-antimiR-19a group, t-antimiR-92a group, scramble control group and blank control group .The growth-inhibitory potencies were measured by MTT assay.The ability of cell colony formation was measured by cell colony formation assay .The ability of cell invasion was measured by Transwell experiment .The miR-19a and miR-92a target gene signal pathways were integrated by miRFo-cus software.RESULTS:MTT assay showed that t-antimiR-19a and t-antimiR-92a significantly inhibited the viability of multiple myeloma cells , and the best concentration and time were 0.5μmol/L and 48 h, respectively .The colony number in t-antimiR-19a/92a group was less than that in scramble control group .The transfection with t-antimiR-19a or t-antimiR-92a effectively decreased the cell invasion , as the relative invasion cell number was significantly decreased compared with scramble control group.miR-19a and miR-92a were involved in mTOR signaling, cell cycle and other cancer pathways . CONCLUSION:miR-19a and miR-92a cluster might be a potential target for therapeutic intervention in multiple myelo-ma.
3.Value of bedside index for severity in acute pancreatitis scoring system in diagnosing severe acute pancreatitis

Objective To evaluate the value of the Bedside Index for Severity in Acute Pancreatitis (BISAP) in diagnosing severe acute pancreatitis. Methods Sixty-eight patients with suspected diagnosis of severe acute pancreatitis were collected and were scored by BISAP, APACHE Ⅱ , Ranson and CTSI scoring systems, respectively. BISAP scoring system included the blood urea nitrogen, impaired mental status,systemic inflammatory response syndrome, age, and pleural effusion. The diagnosis criteria of severe acute pancreatitis was BISAP ≥ 3 points or APACHE IⅡ ≥ 8 points, Ranson ≥ 3 points, CTSI ≥ 3 points. The diagnostic accuracy of SAP of these scoring systems was calculated. Results Among these 68 cases, 63.2%(43/68) were graded ≥ 3 points in BISAP scoring system;60.3% (41/68) were marked ≥8 points in APACHE Ⅱ scoring system; 60.3% (41/68) were scored ≥ 3 points in Ranson scoring system; and 67.6%(46/68) were scored ≥3 points in CTSI scoring system. There was no statistical difference between BISAP scoring system and other three scoring systems in diagnosing severe acute pancreatitis. Conclusions As a new and simple scoring system, BISAP scoring system can be widely used in the diagnosis of severe acute pancreatitis.
4.Research Progress of Osimertinib Supported by Nanodrug Delivery System Against Non-small Cell Lung Cancer

Osimertinib is an irreversible third representative epidermal growth factor receptor tyrosine kinase inhibitor (EGFR-TKI) for the treatment of non-small cell lung cancer (NSCLC) with T790M resistance and classical EGFR mutations. However, the therapeutic effectiveness of osimertinib is limited by acquired drug-resistance, poor water solubility and low tumor accumulation rates. Nanodrug delivery systems can increase the solubility and stability of drugs, prolong the blood circulation time of drugs, improve the uptake rate of cells, promote drug accumulation in tumor tissues, and improve drug resistance. Thus, they are effective in overcoming the limitations of traditional targeted drugs. In this study, we reviewed the mechanism of action of the third-generation EGFR-TKI osimertinib, focused on research advances in osimertinib nanodrug delivery systems against NSCLC, and explored the challenges and future development direction in this field.
5.On the preparation of an amphiphilic cholesterol adsorbent and its adsorption properties.

An amphiphilic LDL adsorbent was synthesized with dietary fiber as the carrier, laurylamine as hydrophobic group and 3-Chloro-2-hydroxylpropysodium sulfonate as sulphonation reagent. The effects of reaction time, reaction temperature and amount of 3-Chloro-2-hydroxylpropysodium sulfonate on SO4(2-) content were studied, and the required preparations were made. The condition for adsorption was obtained by investigating the influence of adsorbent amount and adsorption time. The results show that the adsorption percentages for the removal of total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C) and high-density lipoprotein cholesterol (HDL-C) are 49.6%, 60.0% and 18.2%, respectively. This amphiphilic adsorbent possesses a better selectivity in removing cholesterol.

The preparation of a novel amphiphilic dietary fiber LDL adsorbent with sulfonic group and laurylamine group was studied. The effects of reaction time and reaction temperature on the adsorption rate were studied. The results show that the adsorption rates for the removal of Total cholesterol (TC), Low-density lipoprotein cholesterol (LDL-C) and High-density lipoprotein cholesterol (HDL-C) are 40.8%, 50.8% and 23.6%, respectively. The amphiphilic adsorbent has better selectivity in removing LDL-C.

8.The features under volume management behavior and its relationship with health belief in peritoneal dialysis patients

Objective:To explore the status of volume management behavior in patients with peritoneal dialysis, and to explore the relationship between health belief and volume management behavior.Methods:Convenient sampling was used to select 129 patients who underwent regular dialysis in the peritoneal dialysis center of the General Hospital of Ningxia Medical University from January to December 2019. The general condition questionnaire, Health Belief Scale, and Capacity Management Behavior Scale for patients with peritoneal dialysis were used.Results:The total score of the Capacity Management Behavior Scale of peritoneal dialysis patients was 20.23±3.54. Among all the entries: "Weigh and record the infusion volume and drainage volume" and "Regular monitoring of renal function and electrolytes and other related examinations as directed by the doctor" scored higher; while the item "Eat less high-salt and high-sodium food and adjust fluid intake according to the amount of ultrafiltration, edema and urine output" item scored lower; single factor analysis found that different educational levels, different employment conditions, whether had diabetes mellius and different over hydration had statistical significance ( F value was 3.911, t values were 2.409, 4.990, 6.070, P<0.05). The dimension of the perception maintenance capacity balance disorder was negatively correlated with the total score and each dimension of the capacity management behavior( r values were -0.243, -0.260, -0.299, P<0.05) , and the liquid intake self-efficacy dimension is positively correlated with the total score and each dimension of the capacity management behavior ( r values were 0.329, 0.397, 0.393, P<0.05). Conclusions:The level of capacity management behavior of peritoneal dialysis patients needs to be improved; employment status, and whether he has diabetes or not are the influencing factors of the patients' capacity management behavior; in health beliefs, perception of maintenance of volume balance disorders and self-efficacy and peritoneal dialysis are correlated with patients' capacity management behaviors.
9.Risk factors of extramedullary relapse after allogeneic hematopoietic stem cell transplantation in patients with myeloid leukemia

Objective:To evaluate risk factors and available treatments of extramedullary relapse (EMR) after allogeneic hematopoietic stem cell transplantation (allo-HSCT) in patients with myeloid leukemia.Methods:A total of 280 patients were retrospectively analyzed from January 2008 to December 2018 in Affiliated Cancer Hospital of Zhengzhou University. Clinical data were collected including disease patterns, pre-transplantation status, chromosome karyotype, conditioning regimen, types of donor, extramedullary disease before transplantation and graft-versus-host disease (GVHD). The log-rank test and Cox proportional hazard model were uesd for univariate analysis and multivariate analysis, respectively.Results:Twenty patients developed EMR (7.14%). The median time of EMR was 7.5 (1-123) months after allo-HSCT. The mortality of EMR was 80% (16/20). Univariate analysis identified disease patterns, second complete remission (CR2) or progressive disease before transplantation, extramedullary disease, abnormal karyotype and conditioning regimen without total body radiation as significant factors correlated to EMR ( P<0.05). Multi-variable analysis revealed that CR2 or progressive disease ( RR=3.468,95% CI 2.189-7.786), abnormal karyotype ( RR=1.494,95% CI 1.020-2.189) and extramedullary disease before transplantation ( RR=8.627,95% CI 3.921-18.452) were independent risk factors of EMR. Conclusions:The clinical outcome of EMR after allo-HSCT is poor.It is crucial to comprehensively assess and identify EMR as early as possible.
10.Serum vitamin D status in 668 elderly men in Beijing and variations in related factors

Objective To evaluate the levels of serum 25 hydroxyvitamin D (25(OH)D), parathyroid hormone(PTH)and osteocalcin,as well as bone mineral density(BMD),in aged men in Beijing, and the relationship among these factors.Methods A total of 668 elderly male patients who were treated at the Beijing Friendship Hospital were evaluated, and their medical data were collected. The serum concentrations of 25(OH)D,PTH,and osteocalcin were measured.Additionally,bone mineral density at the lumbar spine1- 4(L1- 4) and left femoral neck were measured using dual-energy X-ray absorptiometry. Participants were divided into 4 groups by age,3 groups according to BMD,and 4 groups according to serum 25(OH)D levels. The relationship among these factors was investigated. Results The mean serum concentrations of 25(OH)D and PTH were (40.2 ± 17.3) nmol/L and (45.19 ± 15.54) ng/L, respectively. The serum 25(OH)D levels in the group aged≥90 years[(27.1±16.8)nmol/L]was significantly lower compared to that in the other groups. Among different BMD groups, serum 25(OH)D levels reduced gradually, but differences were not significant(P>0.05).Age and PTH levels were not significantly associated with 25(OH) D groups (P>0.05). However, BMD of the femoral neck in the groups with severe [(0.782±0.121)g/m2] and mild[(0.775±0.247)g/m2]vitamin D deficiency were significantly lower than that in the group with normal vitamin D levels [(0.837 ± 0.192) g/m2] (P<0.05). With the increase in vitamin D levels, osteocalcin levels decreased significantly(P<0.05).Conclusion Vitamin D deficiency is highly prevalent among aged men in Beijing and the 25(OH)D levels have some correlation with BMD and osteocalcin.
