1. Therapeutic effect and apoptosis mechanism of lung-tonifying and expectorant decoction on lung cancer rats with Qi deficiency and blood stasis

Objective: To explore the effect and specific mechanism of lung-tonifying and expectorant decoction on lung cancer rats with Qi deficiency and blood stasis, and aim to provide a new idea on treating the disease with traditional Chinese medicine based on syndrome differentiation. Methods: A total of 60 C57BL/6J male rats were included in the study. The model of Qi deficiency and blood stasis was established in 60 rats by using multiple-factor stimulation. About 10 rats were randomly taken to verify whether the model establishment was successful and the rest of 50 rats were divided into 5 groups with 10 rats each: blank control group, cisplatin group, low dose group, medium dose group and high dose group. The blank control group was treated with normal saline, and cisplatin group was treated with cisplatin while the other three groups were treated with lung-tonifying and expectorant decoction at different doses. The volume change in transplanted tumor, tumor inhibition rate, apoptosis rate, and expression of Bcl-2, Bax, cleaved caspase-3 and cleaved caspase-9 in 5 groups were compared. Results: The rapidest growth rate of transplanted tumor volume was observed in blank control group and the slowest in cisplatin group. The growth rate was gradually decreased with the increasing dose of lung-tonifying and expectorant decoction, and the difference in growth of tumor volume among groups was statistically significant (P < 0.05). The cisplatin group showed the highest tumor inhibition rate, with dose-dependent increase (P < 0.05). The apoptosis rate in low dose group was higher than blank control group but lower than high dose group (P < 0.05). The apoptosis rate in medium dose group was significantly higher than blank control group (P < 0.05). The apoptosis rate in high dose group was significantly higher than control group (P < 0.05). The positive expression rates of Bcl-2 and Bax in all groups showed statistically significant difference (P < 0.05), while expression of cleaved caspase-3 and cleaved caspase-9 in 5 groups was significantly different, with dose-dependent increase (P < 0.05). Conclusions: The lung-tonifying and expectorant decoction inhibits the proliferation of tumor cells by inducing and activating the cell apoptosis in treatment of lung cancer with Qi deficiency and blood stasis, probably with good clinical therapeutic effect.
2.Mechanism of Cordycepin Inhibiting Proliferation and Promoting Apoptosis of Hepatoma Cells by Regulating Gli1

ObjectiveTo investigate the molecular mechanism of cordycepin inhibiting proliferation and promoting apoptosis of human hepatoma cells (HCCs). MethodGlioma-associated oncogene homolog 1 (Gli1) gene was silenced by small interfering RNA (siRNA) and transfected into SMMC-7721 cells, and then cell proliferation was detected by cell counting kit-8 (CCK-8) assay and cell cloning assay. SMMC-7721 cells were treated with different concentration of cordycepin, and the cell proliferation and apoptosis were examined. The expression of Gli1 and the downstream related genes was determined by Real-time polymerase chain reaction(PCR) and Western blot. ResultThe mRNA and protein expression of Gli1 in SMMC-7721 cells was higher than that in normal liver cells (P<0.01). The proliferation rate of SMMC-7721 with silenced Gli1 decreased at 72 and 96 h (P<0.05, P<0.01), and the colony-forming capacity lowered (P<0.01) compared with those in the blank group. Compared with the control, 80 μmol·L-1 and 120 μmol·L-1 cordycepin significantly inhibited the proliferation of SMMC-7721 cells at 72 and 96 h (P<0.05, P<0.01), and promoted the apoptosis of them (P<0.01). Moreover, 80 and 120 μmol·L-1 cordycepin restrained the mRNA and protein expression of Gli1 in SMMC7721 cells (P<0.05, P<0.01). At 120 μmol·L-1, cordycepin led to the decrease in the mRNA and protein levels of B-cell lymphoma-2 (Bcl-2) and c-Myc (P<0.05, P<0.01), and the increase in the mRNA and protein expression of cysteine-aspartic acid protease-3 (Caspase-3) (P<0.05). ConclusionGli1 is highly expressed in HCCs, and cordycepin can suppress the proliferation and enhance the apoptosis of HCCs by regulating Gli1 and the downstream apoptosis-related factors.
3.Efficacy and Safety of Kuanxiong Aerosol in the Treatment of Coronary Heart Disease Angina :A Meta-analysis

OBJECTIVE:To systematically evaluate the efficacy and safety of Kuanxiong aerosol in the treatment of coronary heart disease angina ,and to provide evidence-based reference for climical drug use . METHODS :Retrieved from Cochrane Library,PubMed,Embase,CKNI,Wanfang data ,VIP,and CBM ,randomized controlled trials (RCTs)about Kuanxiong aerosol (trial group )versus nitroglycerin (control group )in the treatment of coronary heart disease angina were collected during the inception to Mar. 20th,2020. After literature screening and data extraction ,quality assessment was performed using the bias risk assessment tool recommended by the Cochrane System Evaluator Manual 5.1.0. Meta-analysis was performed by using Rev Man 5.3 statistical software. Sensitivity analysis was conducted for the stability of the result ,and trial sequential analysis (TSA)was performed by using TSA 0.9 software. RESULTS :A total of 11 RCTs were included ,with a total of 1 847 cases. Meta-analysis showed that improvement rate of angina pectoris (within 3 min)[RR=1.11,95%CI(1.02,1.22),P=0.02] and total response rate of angina pectoris (within 5 min)[RR=1.04,95% CI(1.01,1.07),P=0.01] in trial group were significantly higher than control group;the incidence of ADR [RR=0.44,95%CI(0.35,0.57),P<0.000 01] in trial group was significantly lower than control group. There was no statistical significance in total response rate of ECG [RR=1.02,95%CI(0.97,1.09),P=0.42] or the level of NO after treatment [SMD =-0.08,95%CI(-0.61,0.45),P=0.76] between 2 groups. The results of sensitiv ity analysis and TSA showed that the efficacy evidence of Kuanxiong aerosol in the treatment of coronary heart disease pectoris was not accurate ,but the evidence of safety was accurate. CONCLUSIONS :Kuanxiong aerosol can improve the efficacy in patients with coronary heart disease angina ,and the safety is better ,but the conclusions of efficacy needs to be further confirmed by enlarging sample size.
4.Study on the regional features of Chinese medicine etiology and pathogenesis of AIDS.

OBJECTIVETo analyze the differences in the Chinese medicine (CM) etiology and pathogenesis of AIDS.
METHODSA cross-sectional investigation study of AIDS patients and non-infected people was carried out in Henan, Guangdong and Xinjiang provinces respectively by questionnaire of clinical epidemiology and analyzed with frequencies of symptoms and signs combined with syndrome factors. The distribution differences of syndrome factors in the 3 provinces were compared.
RESULTSTotally 321 cases were investigated in the 3 provinces. As for the syndrome factors of AIDS, qi deficiency, blood deficiency, yin deficiency, yang deficiency, dampness evil, phlegm, qi stagnation, essence deficiency, and so on were dominated in the 3 provinces, but with their own features. For example, the scores of dampness evil were higher in Guangdong Province. Yin deficiency was dominated in Xinjiang Province. The scores of Henan Province were generally higher. As for the location of syndrome factors, Shen, Fei, Pi, and Gan were dominated in the 3 provinces. The score of Pi was the highest in Guangdong Province, while the score of Shen was the highest in Henan and Xinjiang provinces.
CONCLUSIONSAIDS has regional features in the nature and the syndrome factors of location in different provinces. It is necessary to take into consideration the regional factors when analyzing the pathogenesis of AIDS.

5.Effect of acupuncture on pain and cerebral hemodynamics in patients with migraine: a randomized controlled trial.

OBJECTIVE:
To observe the therapeutic effect between acupuncture combined with medication and simple medication on migraine and cerebral hemodynamics.
METHODS:
A total of 120 patients with migraine were randomized into an acupuncture plus medication group (60 cases, 3 cases dropped off) and a medication group (60 cases, 6 cases dropped off). In the medication group, flunarizine hydrochloride capsule was given orally before sleep, 10 mg a day. On the basis of the treatment in the medication group, acupuncture was applied at Sizhukong (TE 23), Shuaigu (GB 8), Taiyang (EX-HN 5), Fengchi (GB 20) and etc. in the acupuncture plus medication group, 30 min each time, once a day. Treatment for 4 weeks was required in both groups. Before and after treatment, the visual analogue scale (VAS) score, indexes of cerebral hemodynamic [blood flow velocity of anterior cerebral artery (ACA), middle cerebral artery (MCA), posterior cerebral artery (PCA), vertebral artery (VA) and basilar artery (BA)] and total TCM syndrome score were observed, and the clinical therapeutic effect and the incidence of the adverse events were evaluated in both groups.
RESULTS:
Compared before treatment, the VAS scores, the blood flow velocity of ACA, MCA, PCA, VA, BA and the total TCM syndrome scores were decreased in both groups (
CONCLUSION
Acupuncture combined with flunarizine hydrochloride capsule can effectively relieve the pain in patients with migraine, reduce the cerebral blood flow velocity, the efficacy is superior to simple flunarizine hydrochloride capsule.

6.Moxibustion combined with highly active antiretroviral therapy for CDand γ chain cytokines of HIV infected patients.

OBJECTIVETo compare the effects of moxibustion combined with highly active antiretroviral therapy (HAART) and simple HAART for human immunodeficiency virus (HIV) infected patients.
METHODSA total of 100 patients with HIV receiving HAART were randomized into an observation group and a control group, 50 cases in each one. In the observation group, moxibustion was used at Zusanli (ST 36), Guanyuan (CV 4) and Sanyinjiao (SP 6), etc. combined with HAART of zidovudine, lamivudine, nevirapine and efavirenzone, etc. Simple HAART was used in the control group. The patients were observed for 18 months. The indexes were CD, CD/CD, interleukin 2 (IL-2), interleukin 7 (IL-7), the incidence of side effects and the score of quality of life.
RESULTSAfter treatment, CD, CD/CD, serum IL-2 and the scores of quality of life (physiological, psychological, social relation fields and comprehensive score) increased and serum IL-7 decreased compared with those before treatment in the two groups (<0.01,<0.05), with better results except CDin the observation group (<0.01,<0.05). The incidences of gastrointestinal side effects and total side effects in the observation group were lower than those in the control group (14% (7/50) vs 32% (16/50), 58% (29/50) vs 80% (40/50), both<0.05).
CONCLUSIONMoxibustion combined with HAART for HIV infected patients could reduce the incidence of side effects, improve medication compliance, CD/CD, IL-2, IL-7 and the quality of life.
7.Survey of clinical epidemiology of urinary calculi patients in Hubei province

Objective A retrospective study was conducted to investigate the clinical epidemiology of ufinary calculus in Hubei province to provide the population characteristics,stone distribution characteristics,treatment methods,surgical costs and other clinical epidemiological data of patients with trinary calculus,and to provide clinical evidence for the development of prevention and treatment measures for urinary calculi.Methods According to the geographical position,local tertiary hospitals were selected as survey objects and medical records such as age,sex,occupation,stone position,type of operation,and cost of surgical treatment of urinary stone disease were retrospectively investigated between January,2015 and December,2017.Result A total of 193 784 patients were enrolled in this study from 27 hospitals in Hubei province.In which,76 209 were admitted to the hospital for urinary calculus,accounting for 39.33%.Among them,49 460 patients were males (64.90%) and 26 749 were females(35.10%),which the ratio was 1.85∶ 1.All age groups can suffer from urinary calculus,with the peak age range from 41 to 70 years old with 58 985 cases,accounting for 77.40%.The main proportion of patients with urinary calculus were farmers with 21 818 cases (28.63%),comparing with workers (9.74%),busioess man (5.58%),government employees (5.14%),retirees (11.25%) and unemployed (12.58%).A total of 71 439 patients with upper urinary calculus and 4 770 patients with lower urinary calculus and the ratio of upper and lower urinary calculus was 14.98∶ 1.From 2015 to 2017,23 567 patients with urinary calculi underwent ureteroscopic]ithotripsy,and 14 681 patients underwent percutaneous nephrolithotomy.The average operation cost of patients with urolithiasis was 18 395.52 RMB,18 566.31 RMB,19 080.91 RMB in 2015,2016,2017 respectively.Conclusion Hubei province is an area with high incidence of urinary calculus,accounting for 39.33%.The high incidence age is 41-70 years.The occupation composition is mainly farmers.In the past three years,the cost of surgery gradually increased.
8.The influence of military medical university students' proactive personality on achievement motivation: a conditional process analysis

Objective:To explore the relationship between proactive personality, resilience and achievement motivation of military cadets, and to explore the mediating role of resilience between proactive personality and achievement motivation, as well as the moderating effect of cadets’leading ability on this mediating role.Methods:In this study, 109 military cadets were measured with proactive personality scale, Connor-Davidson resilience scale (CD-RISC) and achievement motivation scale. SPSS 26.0 was used for descriptive statistical analysis and correlation analysis.Results:①There was a significant correlation among the total scores of proactive personality, resilience and achievement motivation ( Ps<0.01). ②Proactive personality could significantly predict the level of achievement motivation ( a=0.454, P<0.001), and resilience could significantly predict the level of achievement motivation ( b=0.231, P=0.019). ③Resilience significantly mediated the relationship between proactive personality and achievement motivation ( c′=0.3, P=0.003). ④Cadets' leading ability moderated the effect of resilience on achievement motivation (index=0.338, 95% CI: 0.057 to 0.881). The mediating effect of resilience between proactive personality and achievement motivation was significant for cadets' leaders (Effect=0.381, 95% CI: 0.085 to 1.005), but not significant for other students (Effect=0.043, 95% CI: -0.069 to 0.252). Conclusion:The proactive personality of military cadets can affect the level of achievement motivation through psychological resilience, which is also moderated by cadre identity, suggesting that military education should improve the level of achievement motivation in many ways.
9.Correlation between electromagnetic perturbative index and intracranial pressure and their predictive values in implementation of decompressive craniectomy in acute ischemic stroke patients

Objective:To analyze the correlation between electromagnetic perturbation index and intracranial pressure (ICP) in patients with acute ischemic stroke (AIS) after mechanical thrombectomy, and to explore their values in early warning of decompressive craniectomy (DC).Methods:Forty-three patients with AIS after mechanical thrombectomy admitted to our hospital from January 1, 2018 to December 31, 2019 were enrolled in our prospective cohort study. Electromagnetic perturbation index and invasive ICP were continually monitored for 1-5 d in all patients 24 h after mechanical thrombectomy. According to the mean ICP on the first d, all patients were divided into normal ICP group (ICP<15 mmHg), mild increased ICP group (15≤ICP≤22 mmHg) and moderate to severe increased ICP group (ICP>22 mmHg). According to the implementation of DC, these patients were divided into decompressive craniectomy group and non-decompressive craniectomy group. The differences in clinical data of patients in different ICP groups were compared. The correlation between electromagnetic perturbation index and ICP was analyzed by Pearson correlation method. Receiver operating characteristic (ROC) curve was drawn to analyze the diagnostic value of electromagnetic perturbation index in increased ICP (ICP>22 mmHg). Multivariate Logistic regression analysis was used to determine the independent influencing factors for DC after mechanical thrombectomy in AIS patients. ROC curve was used to evaluate the diagnostic values of electromagnetic perturbation index and ICP in DC implementation after mechanical thrombectomy in AIS patients.Results:(1) In these 43 patients, 8 had normal ICP, 13 had mild increased ICP, and 22 had moderate to severe increased ICP. There were significant differences in baseline Glasgow Coma Scale (GCS) scores, baseline National Institutes of Health Stroke Scale (NIHSS) scores, baseline Alberta stroke program early CT scale (ASPECTS) scores, percentage of patients accepted DC, and electromagnetic perturbation index among the 3 groups ( P<0.05). Correlation analysis showed that electromagnetic perturbation index was negatively correlated with ICP ( r=-0.699, P=0.000). ROC curve showed that the area under curve (AUC) of electromagnetic perturbation index in diagnosing ICP>22 mmHg was 0.850 ( 95%CI: 0.690-1.000, P=0.000), enjoying the optimal cutoff value of 126. (2) Among the 43 patients, 27 were in the decompressive craniectomy group and 16 were in the non-decompressive craniectomy group. Multivariable Logistic regression analysis showed that baseline NIHSS scores, baseline ASPECTS scores, electromagnetic perturbation index, and ICP were independent risk factors for DC implementation after mechanical thrombectomy in AIS patients ( P<0.05). ROC curve showed that the AUC of ICP in predicting DC implementation after mechanical thrombectomy was 0.851 ( 95%CI: 0.728-0.973, P=0.000), enjoying the optimal cutoff value of 18.5 mmHg; the AUC of electromagnetic perturbation index in predicting DC implementation after mechanical thrombectomy was 0.764 ( 95%CI: 0.609-0.919, P=0.004), enjoying the optimal cutoff value of 137.5. Conclusion:There is a good correlation between electromagnetic perturbation index and ICP, which can be used as reference indexes for early warning of DC after mechanical thrombectomy in AIS patients.
10.Research Progress on Mechanism of Traditional Chinese Medicines in Treating Gout

Gout is the second largest metabolic disease in China, which can cause joint tissue damage and a variety of chronic diseases, and seriously affect human life and health. The increase in uric acid caused by disorder of purine metabolism or abnormal uric acid excretion is the biochemical basis of its pathogenesis. Western medical treatment mainly uses anti-inflammatory drugs such as colchicine, non-steroidal anti-inflammatory drugs, and uric acid lowering drugs such as febuxostat and benzbromarone, which have obvious effects, but there are problems such as easy to recurrence after drug withdrawal and more adverse reactions. Traditional Chinese medicine (TCM) has a long history in the treatment of gout, and has the advantages of multi-channel, multi-target, and multi-level symptomatic treatment. It exerts therapeutic effects through lowering uric acid, anti-inflammatory, anti-oxidation, and protecting the kidneys. Its curative effect is obvious and the adverse reaction rate is low. In recent years, there have been many studies on the mechanism of TCM for gout animal models. Based on the review of relevant literature in recent years, this article has systematically sorted out the pathogenesis of gout, the mechanism of TCM for gout and related experimental design. The paper summarized and analyzed the mechanism of TCM in the treatment of gout from the aspects of regulating the level of inflammatory factors, inhibiting oxidation reaction, reducing uric acid and regulating signaling pathway, so as to provide reference for the research and development of drugs for gout.
