Objective To investigate the application of no-tumor touch isolation and killing tumor technique in gastric cancer radical cure. Methods 52 patients with gastric cancer underwent D 4 radical therapy from October 1997 to October 2003 in our hospital. No-tumor touch isolation and killing tumor technique was applied to all the cases. 44 patients with gastric cancer received D 1 radical therapy without the application of no-tumor touch isolation and killing tumor technique served as control. The 5-year survival rate of the two groups was compared. Results There were not obvious post-operative complications in the two groups. OBJECTIVE: To establish a future value creation-oriented sustainable development strategy model for Sichuan Province TCM counterparts.METHODS: A quantitative and qualitative analysis was performed based on the strategy management theory, empirical study, theoretical study, and model research. RESULTS: The Bian Que Company adopted specialization development strategy, variation competition strategy and the crosswise integrated strategy. CONCLUSION: The counterparts of Bian Que Pharmaceutical Company Limited in Sichuan Province may draw experience from this company to lay an emphasis on its core business, horizontally integrate and enlarge management scale, segment market in many goals to make it bigger and larger to achieve better performance.
4.Distribution of nesfatin-1/NUCB2 in the digestive system of humans and rodents

Objective To investigate the regional distribution and morphological features of nesfatin-1/NUCB2 in digestive system of the humans, Sprague-Dawley (SD) rats and the institute of cancer research (ICR) mice, so as to lay the foundation for further study of its functions in the digestive system. Methods The specimens were obtained from SD rats and ICR mice as well as 20 patients with digestive disease, who were admitted to the First hospital affiliated to Nanjing Medical University and receired surgercal treatment. The specimens from patients with malignant tumors were obtained 5 cm apart from cancerous tissues and from patients with benign tumors were obtained near the focus. The resected tissues included pancreas, stomach, duodenum, esophagus, liver, small intestine or colon. The distribution of nesfatin-1/NUCB2 was examined with immunohistochemical (IHC) staining and its protein level in each organ was measured using Western blotting. Results The immuinohistocemical study revealed the similar distribution pattern of nesfatin-1/NUCB2 in the digestive system of the patients, SD rats and ICR mice. Nesfatin-1/NUCB2 was found to localize in the center of the pancreatic islets, the lower 1/3 to 1/2 of the gastric mucosal glands, as well as the submucosa of the duodenum. Western blotting examination showed the expression of NUCB2 in all tissues from patients, SD rats and ICR mice, whereas the protein level of the nesfatin-1/NUCB2 was higher in pancreas (0.84±0.03, 0. 84±0.05 and 0. 84±0.04, respectively), stomach (0.86±0.06,0.81±0.02 and 0. 78±0.02, respectively) or duodenum (0.79±0.09,0. 79±0.04 and 0.78±0.05)than that in esophagus (0.43±0.04,0.44 ± 0.02 and 0.47 ± 0.06, respectively), liver (0.42±0.01,0.44±0.04 and 0.43 ± 0.01, objectively), small intestine (0.32±0.04,0. 32 ± 0. 04 and 0.34 ±0.04, respectively) or colon (0. 29±0.01,0.32±0.03 and 0. 28±0.03, respectively)(all P values=0. 000). Conclusion Nesfatin-1/NUCB2 is widely expressed in the pancreatic islets, gastric mucosal glands and duodenum of the patients, SD rats and ICR mice, which indicates that nesfatin-1/NUCB2 may be involved in the regulation of food intake, carbohydrate metabolism and gastrointestinal motility.
5.THE AGE OF APPEARANCE OF THE TAENIAE AND HAUSTRA OF THE HUMAN COLON AND CAUSE OF FORMATION

This paper is an investigation of 62 human fetuses of different age groups between the 10th week-38th week of fetal life. It aims at the assurance of the time of the first appearance of the taeniae and haustra and the discussion on the cause of the formation of the haustra. The haustra of the human colon first appeared at the 56 mm. CR stage (the end of 10 th week). The taenia mesocolica appeared much earlier than the other ones. We found that the taenia mesocolica could be seen at the end of 10th week of fetal life, while the other two taeniae at the 68 mm. CR stage, i. e. one week later than the first. The thickness of the circular muscle layer in the haustra cleft and of the longitudinal muscle layer of the taenia was increased along with the fetal age. The number and the depth of the haustra cleft were also in proportion with the increase of the fetal life. It is reasonable to suggest that the uneven distribution of the circular muscle layer of the haustra, especially the thickness of the circular muscle layer in the site of the cleft plays an important role in the formation of the haustra. The muscle fibres of the inner longitudinal muscle which interlace with the circular muscle are the morphological basis for the continuous presence of the haustra and constant in position. The fact that the haustra and taenia of the human colon appeared at about the same time leads us to doubt the truth of the traditional saying——the formation of the haustra is due to the uneven developmental rate of the longitudinal muscle and the length of the colon. The formation of the haustra has intimate relation with the kinds of food which one takes in.

8.Therapeutic roles of parenteral and enteral nutrition in patients with disphagia complicating stroke

Objective To evaluate therapeutic roles of parenteral and enteral nutrition in patients with dysphagia complicating stroke. Method Seventy-two patients with dysphagia after stroke were randomly divided into group A (parenteral and enteral nutrition in sequence) and group B (parenteral nutrition) who were respectively given nutritional support 48 hours after the illness,measured hospital nutrition parameters,and observed the incidence rate of complications including tympanites,diarrhea and stress-induced ulcer,aspiration pneumonia.Death rate were compared between two groups at the end of the third week after admitted in hospital. Results Nutrition parameters in group A were higher than that in group B.There was significant difference (P0.05). Conclusion Parenteral and enteral nutrition in sequence for patients with dysphagia after stroke are safe and effective.
9.Expression of TAK1 and p38 genes in patients with acute myeloid leukemia and their clinical significance

Objective To investigate the expression levels of TAK1 and p38 genes among different subtypes of acute myeloid leukemia (AML) patients,and to analyze the clinical characteristics of patients with different expression levels of TAK1 and p38 genes.Methods GAPDH was made as an internal reference,14 healthy people as control group.The quantitative real-time PCR was used to detect the expression of TAK1 and p38 in bone marrow samples of 87 AML patients,and the results were analyzed statistically.Results The expression levels of TAK1 and p38 in experiment group were higher than those in control group (0.194± 0.125 vs 0.015±0.008,0.233±0.140 vs 0.010±0.005,P < 0.001).TAK1 expression in M4 was higher than that in M2,M3 and M5 (P =0.005,0.000,0.002),TAK1 expression in M3 was lower than that in M2 (P =0.022).p38 expression in M4 was higher than that in M1,M2,M3 and M5 (P =0.013,0.035,0.000,0.045),as it was higher in M2 and M5 than that in M3 (P =0.001,0.012).The CD56 positive rate cells and the number of peripheral blood leukocytes of the TAK1 high expression group were higher than those of the TAK1 low expression group,the CD19 positive rate of the p38 low expression group was higher than that of the p38 high expression group.Conclusion The expression levels of TAK1 and p38 genes are elevated in AML patients,and the up-regulation may play an important role in the pathogenesis of AML.
10.A case report of toxic epidermal necrolysis associated with geotrichosis due to Geotrichum silvicola

Objective To report a case of toxic epidermal necrolysis associated with geotrichosis due to Geotrichum silvicola. Methods The exudates from the body surface, blood and urine of the patient were examined by microscopy and simultaneously inoculated onto the Sabouraud dextrose agar (SDA) medium. The isolate was examined by microscopy, PCR which amplified the D1/D2 domain of 26S rDNA, and gene sequencing. Homologous sequences were searched in the GenBank/EMBL/DDBJ/PD nucleotide sequence library, and the genetic relationship was analyzed with the genealogical tree. Results Microscopy of pus from the abscess on the dorsa of left hand revealed a lot of spores and a few hyphae, which were not observed in the blood or urine specimens. Meanwhile, whitish colonies were grown in all the three successive cultures of blood and urine specimens, rather than the exudates on the body surface. After itraconazole and garlicin were administered for one week, both microscopic exam and fungus culture were negative. Microscopic exam of the isolate showed arthrospores arranged in chains, budding spores and a few of hyphae. It was found that there was a one-base difference between our isolate (Hebei-1) and the isolate from kerion -like eruptions (Changzheng-1), and a four-base difference between our isolate and the reference Geotrichum silvicola strain as well, in the D1/D2 domain of 26S rDNA. This isolate was identified to be most close to Changzheng-1 in the phylogenetic tree. Conclusion The patient with toxic epidermal necrolysis is associated with geotrichosis due to Geotrichum silvicola.
