1.Influencing factors and occurrence status of metabolic syndrome in adult patients with hypopituitarism during hospitalization

Objective To investigate the prevalence status and influencing factors of metabolic syndrome (MS) in adult patients with hypopituitarism (HP) during hospitalization. Methods The data of adult HP patients who received treatment in the hospital were collected from March 2021 to March 2024. The prevalence status of MS in adult HP patients was counted, and logistic regression analysis was adopted to analyze the influencing factors of MS in adult HP patients. Results Among the 308 adult HP patients in this study, 121 cases developed MS and 187 cases did not develop MS, and they were included in the MS group (n=148) and the non-MS group (n=232). The incidence of MS in adult HP patients was 38.95% (148/380). Compared with the non-MS group, the levels of high-density lipoprotein cholesterol, serum total cholesterol and low-density lipoprotein cholesterol in the MS group were higher, the waist circumference was larger, and the growth hormone was lower (P<0.05). After logistic regression analysis, it was found that high-density lipoprotein cholesterol (OR=1.069, 95%CI: 1.010-1.132, P=0.021), total serum cholesterol (OR=1.065, 95%CI: 1.014-1.119, P=0.012), low-density lipoprotein cholesterol (OR=1.055, 95%CI: 1.005-1.108, P=0.031), waist circumference (OR=1.063, 95%CI: 1.006-1.123, P=0.030) and growth hormone (OR=1.077, 95%CI: 1.019-1.138, P=0.009) could independently affect the occurrence of MS in adult HP patients (P<0.05). Conclusion Adult HP patients during hospitalization are often complicated with MS. High-density lipoprotein cholesterol, serum total cholesterol, low-density lipoprotein cholesterol, waist circumference, and growth hormone are factors affecting the occurrence of MS in adult HP patients.
2.Analysis of the clinical characteristics of acute myeloid leukemia related to the treatment of hematological and solid tumors

Objective:To compare and analyze the clinical characteristics of acute myeloid leukemia (AML) related to the treatment of hematological tumors and solid tumors.Methods:The laboratory and clinical data of 41 patients with treatment-related AML (t-AML) in the Department of Hematology, Henan Cancer Hospital from January 2014 to December 2021 were retrospectively analyzed, and they were divided into hematological tumor group and solid tumor group. Survival analysis was performed using the Kaplan-Meier method and Log rank test.Results:The median interval from the first tumor diagnosis to t-AML in 41 patients was 21.0 (16.5-46.0) months; 24 (58.5%) had abnormal expression of lymphoid antigen, 28 (68.3%) had abnormal karyotype, 18 cases (43.9%) were positive for fusion gene, and 28 cases (68.3%) were positive for gene mutation; the median recurrence-free survival (RFS) was 11.0 months, and the median overall survival (OS) was 11.5 months. The proportion of acute promyelocytic leukemia ([APL], 0.0, 0/13), complete response ([CR],18.2%, 2/11), median OS (4.5 months) and median RFS (2.5 months) of t-AML patients in the hematological tumor group were significantly lower than those in the solid tumor group (35.7%, 10/28; 68.0%, 17/25; not reach; not reach), but the proportion of M4 /M5 (93.2%,12/13) was significantly higher than that in the solid tumor group (53.6%,15/18; all P values<0.05). Through subgroup analysis, the proportion of patients with positive PML-RARa and good prognosis karyotypes in the solid tumor group (35.7%, 10/28; 46.4%, 13/28) was significantly higher than that in the hematological tumor group (0.0, 0/13; 0.0, 0/13; P<0.05), while the proportion of patients with intermediate karyotypes (42.9%, 12/28) was significantly lower than that in the hematological tumor group (84.6%, 11/13; P<0.05), the difference was statistically significant. The CR rate (90.0%, 9/10), median OS (not reach) and median RFS (not reach) in the t-APL group were higher than those in the t-AML (without t-APL) group (38.5%, 10/26; 6 months; 8 months; P<0.05). After excluding the effect of t-APL patients, there was no significant difference in the CR rate, median OS and median RFS between the solid tumor group (8; 9 months; not reach) and the hematological tumor group (2; 4 months; 2 months; P>0.05). Univariate analysis showed that the primary tumor belongs to hematological tumor was a common risk factor for OS and RFS in t-AML patients ( P<0.10). Conclusions:Compared with patients with t-AML secondary to solid tumors, patients with t-AML secondary to hematological tumors have poorer treatment effects and poorer prognosis. After excluding the effect of t-APL patients, there are no significant differences in the treatment efficacy and prognosis between the two types of t-AML patients.
3.Methodological Evaluation of Advantages of Traditional Chinese Medicine Treatment of Sjögren's Syndrome

Screening and evaluating the diseases responding specifically to traditional Chinese medicine (TCM) will help to highlight the advantages of TCM treatment, and the evaluation method should be standardized with consideration to the unique characteristics of the diseases. The incidence of Sjögren's Syndrome (SS) is increasing year by year, while the pathogenesis of this disease remains unclear. Modern therapies for this disease include biological agents and immunosuppressants, which generally have unsatisfactory efficacy. The TCM treatment of SS focuses on the harmony of the physical and mental health. The Rheumatology Branch of the China Association of Chinese Medicine organizes experts in TCM, Western medicine, and evidence-based medicine to form working groups. Delphi method and bibliometric method were used for analysis, and SS was selected as a disease responding specifically to TCM. Furthermore, the evaluation system was established for this disease, and the consensus regarding this disease was reached after seminar discussion. This paper summarized the whole process of the evaluation of the advantages of TCM treatment of SS. First, because TCM atomization is widely used in clinical practice and enriches TCM administration methods, this therapy is included after other non-drug therapies were taken as characteristic therapies. Second, the evaluation indicators of therapeutic effect should be determined with consideration to international acceptance and the current research status. Third, the expression method should be accurate, standardized, and objective, highlight the natural advantages of TCM, and avoid arbitrary extension. This paper provides a reference for clinicians to explore other diseases responding specifically to TCM.
4.Needs for curriculum and specialty development of public health nursing: an investigation of medical students during the COVID-19 pandemic

ObjectiveThe purpose of this study is to investigate and analyze the needs of public health nursing courses and specialty settings under the COVID-19 pandemic. MethodsA total of 576 medical students majored in clinical medicine, preventive medicine, pharmacy, and nursing from a Shanghai university participated in this survey. The Questionnaire Star App was used to collect students' knowledge, attitude and behavior about public health safety and emergency preparedness, public health related curriculum and public health nursing specialty, through self-administered approach. ResultsThere were significant differences in the knowledge of emergency nursing among students with different majors and grades. Over 80% of the participants agreed that "public health popularization should be strengthened". Over 60% were willing to "invest more time and energy in learning and understanding public health-related disease prevention and control knowledge". The school has carried out corresponding training or practice activities in public health education. Compared with nursing major, students with other majors performed better in paying attention to and participating in the training or practical activities of public health education. More than 70% agreed that it was necessary to set up public health nursing courses, especially nursing specialty, with the approval rate as high as 92.3%. Students believed that "2 class hours per week", and "1/10 of the total class hours" was appropriate. In terms of course teaching, nursing students put forward more expectations on teaching objectives, methods, and characteristic courses. ConclusionUndergraduates lack knowledge of public health nursing, but most of them, especially nursing undergraduates, have a strong desire to learn and have a positive attitude towards opening public health nursing courses. However, there are still problems such as insufficient attention to and insufficient understanding of the courses. More efforts should be made to enhance the public health nursing education on public health emergencies, optimize the relevant settings of courses, so as to train nursing personnel suitable for the current health service needs in China.
5.Expression of CDK5 and its correlation with peripheral inflammation-related cells and prognosis in colon cancer patients

Objective:To identify the clinical significance of CDK5 in colon cancer tissues.Methods:Two hundred colon cancer tissues were tested for CDK5 expression by immunohistochemistry on tissue microarrays. The correlation between CDK5 expression and clinicopathological features, prognosis and peripheral inflammation-related cells was analyzed.Results:CDK5 was low expressed in 100 cases (50.0%), and high in another 100 cases (50.0%). Longer time to tumor progression ( P=0.026) and overall survival ( P=0.035) were observed in patients with high CDK5 expression. By multivariate analysis , the expression of CDK5 was an independent risk factor for poor prognosis ( HR=0.45,95% CI: 0.21-0.99, P=0.049). The expression of CDK5 was not related to the counts of white blood cells and neutrophils ( P>0.05). Prognosis of patients with a positive lymph node ratio less than 0.15 was significantly better than that of patients with a higher lymph node ratio ( P<0.001). Conclusions:Patients with low CDK5 expression have poor prognosis, and CDK5 expression is not related to the counts of peripheral white blood cells and neutrophils.

7.Analysis of the chloroplast genome characteristics of Rhus chinensis by de novo sequencing.

Rhus chinensis is an important economic species, which could provide raw materials for pharmaceutical and industrial dyes. Rhus chinensis is famous for its resistance to drought, cold, and salt. It grows in temperate, warm temperate, and subtropical regions. We report here Rhus chinensis chloroplast genomes by de novo sequencing. The results show that the length of Rhus chinensis was 159 082 bp, exhibiting a typical four-part structure with two single-copy regions (long single copy [LSC] and short single copy [SSC] sections) separated by a pair of inverted repeats (IRs). The length of LSC and SSC was 85 394 bp and 18 663 bp, respectively. The genomes contained 126 genes, including 88 protein encoding genes, 8 rRNA and 30 tRNA genes. In the chloroplast genome, 61.97% of the sequence were gene coding region. In the sequence of gene encoding region, the vast majority of sequences were protein encoding region, accounting for 86.65%, followed by rRNA (10 620 bp, 10.77%) and tRNA (2 540 bp, 2.58%). In Rhus chinensis chloroplast genome, only 8 genes contain introns, all containing 1 intron except ycf3 gene (2 introns). The Rhus chinensis chloroplast genome contains 755 SSR locies. SSR mainly consists of dinucleotide and mononucleotide, accounting for 60% (453) and 28.74% (217) respectively. The clustering results show that Anacardiaceae were closest to Rhus chinensis, followed by Aceraceae and Sapindaceae. This study provides a molecular basis for the classification of Rhus chinensis.

This report presents an 84 year-old patient admitted into our hospital for dizziness with a history of Hashimoto's thyroiditis,chronic gastritis and vitiligo.Physical examinations showed skin depigmentation.Laboratory tests revealed anemia,positive intrinsic factor antibodies and normal adrenal function.This patient was finally diagnosed with autoimmune polyglandular syndromes(APS) type Ⅲ B +C.Recurrent anemia was mainly attributed to malabsorption caused by autoimmune gastritis.This article reviews the common clinical manifestations of APS,in order to call for geriatricians' attention to APS.APS Ⅲ B+C should be considered if vitiligo combined with anemia occurs in elderly patients.
10. Using the lateral second toe flap for reconstruction of great toe defect

Objective:
To explore the application and effect of using the lateral second toe flap transfer for reconstruction of great toe defect.
Methods:
The flap on tibial of second toe was designed according to the size of defect. The pedicle of the flap is the tibial proper plantar digital artery of the toe, and the rotation point is located between the toe web.13 cases of the great toe′s defects were repaired by this method, with the donor sites covered by full thickness skin from the leg.
Results:
13 flaps survived postoperatively. All cases were subject to postoperative follow-up, follow-up time was 6-48 months, an average of 18 months. Skin color and texture was close to that of the surrounding tissues, appearance is not bloated.
Conclusions
The second toe lateral skin flap is easy to operate, is flexible for rotating. As local material, it is an ideal skin flap to repair the great toe defect.
