1.Study on the correlation between trial time and postpartum infection

Objective To analysis the dependence factors of postpartum infection in our hospital trial cases in recent 5 years.To explore the reasonable parturition and improve the quality of obstetric measures.Methods A retrospective analysis of the incidence of infection after the trial was conducted, in which the clinical data of 106 patients were from June 2014 to June 1999,and compared with 212 cases without infection after parturition.Results In the trial of more than 48h,cesarean section,vaginal infection group were 85.8%,87.7% and 67.0%.They were higher than the control group 22.6%,36.8%and 35.4%.The difference between the two groups was statistically sig-nificant (P<0.05).Conclusion The trial parturition time is the leading cause of infection.In order to reduce the rate of cesarean section,to reduce maternal puerperal infection,subsequent complications such as uterine scar,strict control of indications for induction of spawning is needed.
2.A study of one step method for the treatment of mitral stenosis: report of 492 cases

Objective To summarize the experiences and long-term follow-up results in the treatment of severe mitral stenosis by balloon expansion in one step. Methods A new one-step method for direct mitral balloon expansion of the mitral valve was performed in 492 patients with mitral stenosis. Their mean age was 41.2?10.5y (15-68 y). 156 patients were male, and 336 were female. Results PBMV was effectively performed in 492 cases,and the success rate was 98.2% (483/492);mitral valve area was enlarged from 0.89?0.18cm 2 to 2.21?0.41cm 2 . 211 cases were followed-up from 8 months to 98 months. Restenosis rate for the dilated mitral valve was 11.4% (24/211),death rate 4.2%(9/211),and mitral valve replacement was done in 5.2% (11/211). Mean operation time was 36.5?12.1min. Conclusion Percutaneous mitral commissurotomy by balloon dilatation gave excellent immediate and late clinical results.

4.Distribution of nesfatin-1/NUCB2 in the digestive system of humans and rodents

Objective To investigate the regional distribution and morphological features of nesfatin-1/NUCB2 in digestive system of the humans, Sprague-Dawley (SD) rats and the institute of cancer research (ICR) mice, so as to lay the foundation for further study of its functions in the digestive system. Methods The specimens were obtained from SD rats and ICR mice as well as 20 patients with digestive disease, who were admitted to the First hospital affiliated to Nanjing Medical University and receired surgercal treatment. The specimens from patients with malignant tumors were obtained 5 cm apart from cancerous tissues and from patients with benign tumors were obtained near the focus. The resected tissues included pancreas, stomach, duodenum, esophagus, liver, small intestine or colon. The distribution of nesfatin-1/NUCB2 was examined with immunohistochemical (IHC) staining and its protein level in each organ was measured using Western blotting. Results The immuinohistocemical study revealed the similar distribution pattern of nesfatin-1/NUCB2 in the digestive system of the patients, SD rats and ICR mice. Nesfatin-1/NUCB2 was found to localize in the center of the pancreatic islets, the lower 1/3 to 1/2 of the gastric mucosal glands, as well as the submucosa of the duodenum. Western blotting examination showed the expression of NUCB2 in all tissues from patients, SD rats and ICR mice, whereas the protein level of the nesfatin-1/NUCB2 was higher in pancreas (0.84±0.03, 0. 84±0.05 and 0. 84±0.04, respectively), stomach (0.86±0.06,0.81±0.02 and 0. 78±0.02, respectively) or duodenum (0.79±0.09,0. 79±0.04 and 0.78±0.05)than that in esophagus (0.43±0.04,0.44 ± 0.02 and 0.47 ± 0.06, respectively), liver (0.42±0.01,0.44±0.04 and 0.43 ± 0.01, objectively), small intestine (0.32±0.04,0. 32 ± 0. 04 and 0.34 ±0.04, respectively) or colon (0. 29±0.01,0.32±0.03 and 0. 28±0.03, respectively)(all P values=0. 000). Conclusion Nesfatin-1/NUCB2 is widely expressed in the pancreatic islets, gastric mucosal glands and duodenum of the patients, SD rats and ICR mice, which indicates that nesfatin-1/NUCB2 may be involved in the regulation of food intake, carbohydrate metabolism and gastrointestinal motility.
5.CCL18 downregulates the expression of miR98 in breast cancer cells via N-Ras/c-myc/lin28 pathway

Objective To explore whether CCL18 is involved in regulating the expression of miRNAs in breast cancer.Methods The expression profile of miRNAs in the breast cancer cell following CCL18 treatment was determined by miRNAs microarray analysis.Then we performed QRT-PCR and Luciferase Reporter Assay to validate the results from the miRNAs microassay.We used transient transfection to change the expression of miR98 and c-myc in breast cancer cells.We then used QRT-PCR and Western blot to analyze the mechanism by which CCL18 downregulates the expression of miR98 in breast cancer cells.Results miRNAs microarray analysis showed that cells treated with CCL18 differentially expressed 20 miRNAs genes compared with those in the control group. Our QRT-PCR and Luciferase Reporter Assay confirmed the result.The mRNA and protein expressions of C-myc and lin28 were increased after CCL18 stimulation in breast cancer cells.Transfection with c-myc siRNAs rescued the increase of lin28 and loss of miR98 expression caused by CCL18 stimulation.Our results also showed that CCL18 could upregulate the expression of N-Ras at post-transcription level.Conclusion CCL18 downregulates the expression of miR98 via N-Ras/c-myc/lin28 pathway.The downregulated miR98 increases the expression of N-Ras after transfection,which further activates c-myc/lin28 pathway and forms a positive feedback loop.
6.Effects of High Intensity Focused Ultrasound on AFP mRNA in Peripheral Blood of Patients with Hepatocellular Carcinoma

Objective To evaluate the effects of high intensity focused ultrasound (HIFU) on hepatic cancer cells spreading in blood.Methods AFP mRNA in peripheral blood of 19 patients with hepatocellular carcinoma was detected before and after HIFU therapy by RT PCR.Results ①Before HIFU therapy, 11 of 19 cases were AFP mRNA positive (57.9%), while the control group were all negative. AFP mRNA was correlated with some clinical parameters such as serum AFP level, tumor size, portal vein embolism and extrahepatic metastasis ( P 0.05). ③After one week of HIFU therapy, the AFP mRNA positive rate of the group with tumor size less than 8 cm was much lower than that of tumor size larger than 8 cm ( P
7.Effects of different radiofrequency ablation methods on atrioventricular block in patients with atrioventricular nodal reentrant tachycardia

Objective:Atrioventricular nodal reentrant tachycardia(AVNRT) is a common type of arrhythmia,for which radiofrequency ablation(RFCA) is the first therapeutic option.This study is to investigate the causes of 16 cases of temporary atrioventricular block(AVB) during RFCA and 2 cases of permanent AVB after RFCA in common and refractory AVNRT.Methods: We performed RFCA for 77 AVNRT patients by gradually increasing the ablation time,energy and frequency.Generally we chose the lower zone as the first position of ablation and,if invalid,moved the catheter gradually up to the middle or upper zone.Then we analyzed the correlation of AVB with A/V and H-wave in different positions.Results: The AVB risk was increased with the upward movement of the ablation zone and increase of A/V or H-wave(P 0.05). Conclusion: The method of gradually increasing the ablation time,energy and frequency can prevent temporary from permanent AVB.In the common AVNRT group,the middle and lower zone ablation,the multiple-peak A wave,small A wave and big V wave(A/V ≤ 0.45?0.37) with no H wave or H≤0.003?0.01 mV could significantly decrease the risk of RFCA related AVB,but the risk of AVB remains among the refractory AVNRT patients with CSO abnormality or expansion,even if with ablation in the lower zone.

Objective: To extract and determine the flavonoids of hawthorn fruits and their antioxidative effect. Method: The ultrasonic method was used for extracting flavonoids. The content of flavonoids was determined by spectrophotometry, and the antioxidation effect was determined by flow-injection chemiluminescence.. Results and Conclution: The recovery rate was 96%~105% and the coefficient of variation was 0.14 % by spectrophotometry. Hawthorn fruits had high antioxidative effect, showing dose-response relation.
9.Age patients laparoscopic cholecystectomy clinical curative effect and complications influencing factors analysis

Objective To explore the more than 80-year-old age patients laparoscopic cholecystectomy clinical curative effects and complications.Methods More than 260 cases of 80-year-old require cholecystectomy older patients,patients with detailed records related history,select one of the 200 patients with laparoscopic cholecystectomy to treatment,the rest of the 60 patients to do open cholecystectomy treatment,to observe the clinical curative effect of the two groups ; For patients with laparoscopic group in postoperative patients according to whether there was the occurrence of complications were divided into two groups,using multiple factors regression analysis the factors caused by complications.Results Laparoscopic surgery group blood loss,operative time,length of hospital stay,drainage time,lead flow,the surgical incision length were (62.7 ± 60.3) ml,(62.7 ± 21.4) ml,(5.6 ± 1.3) d,(3.6 ± 0.7) d,(174.4 ± 121.4)ml,(4.9 ± 1.2)cm,open group were (210.4 ± 120.4)ml,(115.7 ±30.4)ml,(8.8 ±3.4)d,(6.2 ± 2.5) d,(318.2 ± 132.5) ml,(12.2 ± 1.4) cm,two groups of various observation indexes were statistically significant differences (t =3.16,4.16,2.18,3.16,2,56,5.18,P < 0.05).Laparoscopic cholecystectomy patients in 200 cases of 6 patients with complications (3.0%),single factor analysis results showed that:age,diabetes history,operation time and blood loss,drainage laparoecopic cholecystectomy complications had a correlation (P < 0.05) ;Multiple factors analysis results showed that:with the age increasing,diabetes history was laparoscopic cholecystectomy complications independent risk factor (P < 0.05).Conclusion For age more than 80 years patients,laparoscopic cholecystectomy down than open surgery has good curative effect,its complications are the main factors for increased age and always have diabetes history.
10.The apoptotic activity of hydroxycamptothecin against human gastric carcinoma and its mechanism of action

Objective To study the apoptotic activity of hydroxycamptothecin (HCFT) against human gastric carcinoma and its mechanism of action.Methods The apoptotic activity of HCPT against gastric carcinoma SGC-7901 cells was detected by MTT assay.The apoptosis occurrence was determined by Annexin V/PI staining and flow cytometry analysis.The mRNA expression of Bmi-1 was detected by real-time PCR analysis.Western blot analysis was used to detect the protein expression of Bmi-1.Results The IC50 values of HCPT against gastric carcinoma SGC-7901 cells was (4.87 ± 0.35) μ mol/L.After treatment with 0,5,10,20 μ mol/L HCPT for 48 h,the percent of apoptosis was elevated from (1.62 ± 0.37)% to (21.45 ±4.54)%,(36.67 ±5.38)%,(54.26 ±7.14)% and the 2-△Ct values of Bmi-1 mRNA was decreased from 0.614 ±0.022 to 0.445 ±0.018,0.376 ±0.012,0.215 ±0.010,there had significant difference among the different treatment (P < 0.01).Treated with 0,5,10,20 μ mol/L HCPT for 48 h,the protein expression of Bmi-1 was down regulated by HCPT treatment in a dose-dependent manner by Western blot analysis.Conclusion HCPT shows effective activity to induce apoptosis in gastric carcinoma cells,which is mainly related to the down regulation of Bmi-1.
