1.Preliminary Study on the Processing Mechanism of Cacumen Platycladi

To establish an HPLC method for the determination of myricetrin, quercitrin, myricetin, quercetin, kaempferol, amentoflavone and hinokiflavone in Cacumen Platycladi and processed Cacumen Platycladi, and compare the difference before and after the processing. Methods:The method described in Chinese pharmacopoeia was used in the processing. The chromato-graphic column was Welch Ultimate LP-C18 (250 mm × 4. 6 mm,5 μm),the mobile phase was methanol and 0. 2% phosphoric acid with gradient elution, the detection wavelength was 330 nm, and the flow rate was 1 ml·min-1 . The column temperatuhe was 35℃, the injection volume was 20μl. Results:The linear relationship respectively of myricetrin, quercitrin, myricetin, quercetin, kaempfer-ol, amentoflavone and hinokiflavone was good(r≥0. 999 0). The average recovery was within the range of 97. 6%-101. 1% (RSD≤1. 14%, n=6). The content of myricetrin, quercitrin, amentoflavone and hinokiflavone in Cacumen Platycladi was reduced after the processing, and the content of quercetin and kaempferol was increased after the processing. Conclusion: The method is simple,and can be used in the determination of Cacumen Platycladi and its processing product. The results of the content show that the flavones in Cacumen Platycladi can be changed by the processing.
2.CLINICAL ANALYSIS AND SURGERY MANAGEMENT OF 65 CASE OF INFECTIVE ENDOCARDITIS

This paper is to analyze the clinical features of infective endocarditis (IE) and summarize the experience of surgical treatment. The retrospective review was based on 65 consecutive patients, who fulfilled the Duck criteria of the diagnosis of endocarditis, admitted from Jan 1997 to Jun 2002. There were 44 males and 21 females, with age ranged from 9 to 68 years. Pre operative blood cultures were positive in 20 out of 55 patients(45%), with Streptococcus in 8 (45%). Staphylococcus in 9 (40%), and other bacteria in 3 (15%). Echo cardiography revealed vegetations in 62 cases, involving the mitral valve in 19 cases, aortic valve in 31 case, pulmonary valve in 5 cases, tricuspid valve in 5 cases, and valsalva sinus in 2 cases. Among 65 patients, aortic valve replacement was performed in 22, mitral valve replacement in 13, combined aortic and mitral valve replacements in 5, mitral valve plasty in 3, repair of atrial septal defect in 2, ventricular septal defects in 14, rupture of Valsalva sinus in 3, correction of tetralogy of Fallot in 1, patent ductus arteriosus in 2. Four patients died in hospital. 56 patients (91%) were followed up for 6 months to 69 months. Two patients with replacement of aortic valve died 1 and 3 years later due to congestive heart failure and sepsis. In the other 54 patient, no recurrent infection was found. One year, 3 year, and 5 year survival rates were 96 6%, 90 1%,90 1%, respectively. The results suggest that urgent early diagnosis and optimal surgical intervention play key role in successful treatment of infective endocarditis.
3.Content Comparison of Flavonoids in Tribulus Terrestris from Different Habitats

Objective:To establish an HPLC method for the determination of rutin, luteolin, quercetin, kaempferol, isorhamnetin and chrysin in Tribulus terrestris L. Methods:The chromatographic column was Welch Ultimate LP-C18(250 mm ×4.6 mm, 5 μm), the mobile phase was acetonitrile and 0. 05% phosphoric acid with gradient elution,the detection wavelength was 360 nm,and the flow rate was 1. 0 ml·min-1 . Results: The linear relationship of standard curve of rutin, luteolin, quercetin, kaempferol, isorhamnetin and chrysin was good (r≥0. 999 4). The average recovery was 97. 5%-99. 7% with RSD≤1. 24% (n=6). Conclusion:The meth-od is simple, and the precision,recovery and repeatability all meet the requirements,which can be used for the content determination of Tribulus terrestris L.
4.Optimization of the Extraction and Preparation Process of Total Terpene Constituents in Platycladi Seed

Objective:To investigate the optimal extraction and dripping pills preparation of total terpene constituents in platycladi seed. Methods:The extraction processes of ultrasound, reflux, microwave and homogenization were investigated by single factor tests. The effects of extraction time, temperature, solid-liquid ratio and extraction times on the extraction of total terpene constituents in platy-cladi seed were investigated by orthogonal tests. Orthogonal tests were used to investigate the effects of the ratio of polyethylene glycol 6000 to poloxamer 188, the ratio of drug to matrix, the temperature of liquid and the dropping process on the quality of dropping pills. Results:The homogeneous extraction was used, and the optimum extraction conditions were as follows: the extraction time was 10 min, the temperature was 80℃, the solid-liquid ratio was 1 :10, and extraction time was once. The best preparation process of drop-ping pills was as follows:the ratio of polyethylene glycol 6000 to poloxamer 188 was 5 :1, the ratio of drug to matrix was 1 :3, the temperature of liquid was 80℃ and the dropping distance was 10 cm. Conclusion: The method of homogeneous extraction is simple, which can break the materials and extract the active ingredients simultaneously. The preparation process of dropping pills is reasonable and feasible, which is easy for the production and application.
5.Purification Method of Ultrasonic Emulsification Combined with Ultrafiltration Membrane Separation for Flavonoids in Cacumen Platycladi

Objective:To obtain flavonoids of Cacumen Platycladi with high purity by using the method of ultrasonic emulsification combined with ultrafiltration membrane separation.Methods:Seven compounds including myricetrin as the reference substances,an HPLC method was used for the content determination.Using the single factor experiments,ultrasound,microwave,reflux and ultrasonic emulsification were compared,and using the orthogonal experiments,the ultrasonic emulsification time,ethanol concentration,solid-liquid ratio and times were studied.Biomax-5 membrane was adopted to improve the membrane separation.Results:The single factor experiments showed the homogeneous extraction method with the highest contents.The orthogonal experiments showed the optimal extracting conditions as follows:the ultrasonic emulsification time was 15 min,the ethanol concentration was 50%,the material-liquid ratio was 1∶10,and the extraction times was 3.The conditions of ultrafiltration membrane separation were as follows:the flow rate was 1.5 L·min-1,and the membrane separation pressure was 0.8 kg.Conclusion:The combination of ultrasonic emulsification and ultrafiltration membrane separation is feasible in the extraction and purification of flavonoids in Cacumen Platycladi,and the product is with high purity,suggesting the method has good application prospect.
6.Double-index Method for Quality Control of Tribulus Terrestris L

Objective:To establish the quality control method for Tribulus terrestris L. by colorimetry and HPLC. Methods:The HPLC method was with a Welch Ultimate LP-C18 column(250 mm × 4. 6 mm,5μm),the mobile phase was methanol-water(80:20) and the detection wavelength was 203 nm. The colorimetry was with a perchloric acid method. The saponins of Tribulus terrestrist as the index,the determination method for total saponins and saponins of Tribulus terrestris L. was established. Results:The results of the HPLC and colorimetry methods showed saponins of Tribulus terrestris had good linear relationship within the range of 0. 820-7. 380 μg and 24. 600-86. 100 μg with the average recovery of 99. 3% and 99. 5%,respectively. Total saponins and saponins of Tribulus terres-tris in Tribulus terrestris from 18 habitats were measured by the methods. Conclusion:The methods are sensitive,accurate and repro-ducible,and can be used as the quality control methods for Tribulus terrestris.
7.Field evaluation of BinaxNOW~ rapid diagnostic devices for malaria

Objective To evaluate the effect of BinaxNOW rapid diagnostic devices for malaria in the field. Methods The symptomatic patients were detected by using the BinaxNOW devices and the results were compared with the microscopic examination of Giemsa-stained blood smears(as a golden standard). The sensitivity and specificity were calculated. The double-blind method was used in this study. Results In 443 symptomatic patients,151 cases were positive for malaria. The sensitivity was 98.69% and the specificity was 100%,and the coincidence rate of BinaxNOW with the microscopic examination was 99.55%. The sensitivity and specificity of Plasmodium falciparum were 100% and 99.66%,respectively,and the coincidence rate was 99.76%. The sensitivity and specificity of non-falciparum Plasmodium were 90.91% and 100%,respectively,and the coincidence rate was 99.36%. Conclusion The BinaxNOW rapid diagnostic devices for malaria are sensitive,specific,and stable for malaria diagnosis in the field.
8.Clinical follow-up study of jaundice type of human cytomegalovirus hepatitis in infants

Objective To explore the recovery time of hepatic function in infants with jaundice type of human cytomega-lovirus (HCMV) hepatitis and its influencing factors. Methods The clinical data of 73 infants with jaundice type of HCMV hepa-titis admitted to hospital from February 2005 to October 2012 were retrospectively analyzed. The effects on hepatic function of nine factors including age, sex, liver size, total bilirubin (TBil), direct bilirubin (DBil), alanine aminotransferase (ALT), aspar-tate aminotransferase (AST), total bile acid (TBA) and the loads of HCMV DNA were assessed by Kaplan Meier method, and further analyzed by Cox proportional hazards regression model. Results The results of Cox proportional hazards regression showed that age≤3 month (RR=0.27, 95%CI:0.10-0.70), DBil≤90μmol/L (RR=0.16, 95%CI:0.08-0.32), male (RR=0.49, 95%CI:0.26-0.94) and enlarged liver size<3 cm (RR=0.50, 95%CI:0.27-0.93) were independent factors that shorten the time for TBil back to be normal. Furthermore, AST≤120 U/L (RR=0.16, 95%CI:0.08-0.33) and enlarged liver size<3 cm (RR=0.28, 95%CI:0.15-0.49) were independent factors that shorten the time for AST back to be normal. The time for TBil back to be normal was (2.23 ± 1.54) months, which was significantly shorter than that [(3.63 ± 1.93) months] of AST (t=10.37, P<0.001). Conclusions Jaundice type HCMV hepatitis had good outcome and varied in disease course. The recovery of hepatic function was significantly adversely affected by the degree of cholestasis and hepatomegaly as well as AST level, and early treatment was conducive to the recovery.
9.Study on Processing Technology of Alum by Central Composite Design and Response Surface Method

Objective:To optimize the processing technology of alum by using oven processing combined with central composite design and response surface method to provide basis for processing standardization production of alum. Methods:Alum as the raw ma-terial and on the basis of single factor experiment, the processing temperature, processing time and alum amount were used as the three independent variables factors. A titration method was applied in the content determination of aluminium potassium sulfate in withered alum, and a central composite design and response surface method was used to optimize the processing technology of alum. Results:The optimal processing technology of alum was as follows:the processing temperature was 250℃ with 40-minute processing time and 8 g alum. Conclusion:The optimized alum processing technology by central composite design and response surface method is simple, stable and predictable, which can provide basis for the processing standardization production of alum.
10.Quantile regression analysis of health-related quality of life of rural residents in Shaanxi and its associated factors

Objective This study aimed to apply quantile regression to study Hanzhong rural residents health survey data,explore the local distribution characteristics of health-related quality of life (HRQOL) and influencing factors and present the value of quantile regression applying in analysis of HRQOL.Methods In this cross-sectional population-based study,we evaluated the HRQOL of 2 737 subjects through filling Short-Form Health Survey (SF-36).Quantile regression model was used to compare MCS and PCS scores and evaluate the associated factors.Results With different quantiles MCS and PCS score,the associated factors and influence degree were different.In general,the influences of marital status,educational level,physical activity,history of disease and HRQOL in the part of the percentile scores were significant.Conclusion Analysis of the distribution of HRQOL of rural residents in Hanzhong and influencing factors would benefit the improvement of HRQOL of local residents.
