1.Mechanism of lung injury after cardiopulmonary resuscitation in rats

Objective:To investigate the mechanism of lung injury after cardiopulmonary resuscitation(CPR) in rats. Methods: Cardiac arrest was induced by asphyxiation(succinylcholine) and ice-cold 0.5 mol/L KCl in rats and CPR was carried out 5 min after arrest. Sixteen adult male Sprague-Dawley rats were randomly allocated into 2 groups:control group(n=8) and CPR group(n=8). The blood gas analysis,serum level of tumor necrosis factor ?(TNF-?),lung tissue wet mass/dry mass ratio(W/D),lung malondialde-hyde(MDA) content and myeloperoxidase(MPO) activities were measured 3 h after CPR. The histological appearances of the lungs were observed under light and electron microscope. Results: Compared to the control group,the lung W/D,serum TNF-?,pulmonary MDA and MPO activity increased dramatically in CPR group while PaO 2 decreased 3 h after resuscitation(P
2.Comparison of clinical efficacy of esomeprazole in treatment of gastric ulcer in young and elderly patients

Objective To compare the clinical efficacy and safety of esomeprazole in treatment of elderly and young patients with gastric ulcer.Methods 120 cases of elderly patients with gastric ulcer and 95 cases of young and middle-aged patients with gastric ulcer were selected this study.Two groups of patients were treated with amoxicillin and clavulanate potassium dispersible tablets 0.457 g,tinidazole tablets 0.5 g and esomeprazole magnesium enteric coated tablets 20mg orally,once daily treatment 1 week for a course of treatment,1 week after continue to every morn-ing fasting taking esomeprazole 20mg,1 times/D,treatment for 3weeks.The clinical curative effect,Hp clearance rate and incidence of adverse reactions between two groups of patients after treatment were compared.Results The total effective rate in the treatment of elderly group patients (87.50%)compared with young and middle -aged group (92.63%),there was no significant difference (χ2 =1.52,P>1.52).Elderly group of patients with helicobacter pylori clearance rate (71.67%)compared with young and middle-aged group (76.84%),there was no significant difference (χ2 =0.74,P>0.74).The incidence of adverse reactions (8.33%)of elderly group had obvious differ-ences by comparing with young and middle -aged group (2.1 1%),and the difference was statistically significant (χ2 =3.90,P<0.05).Conclusion Application of esomeprazole based triple therapy in the treatment of the elderly and young people in the gastric ulcer could heal ulcers and eradicate Hp,but young and middle -aged patients is superior to the elderly group in terms of adverse reactions.
3.Effect of Electroacupuncture on Regional Cerebral Blood Flow in Patients with Post-stroke Depression

Objective To explore the therapeutic effects of electroacupuncture on regional cerebral blood flow (rCBF) in patients with post-stroke depression. Methods 21 patients with post-stroke depression were randomly divided into electroacupuncture group (n=11) and drug group (n=10). The electroacupuncture group received electroacupuncture on double Hegu (LI4) and Taichong (LR3) once a day for 8 weeks, while the drug group took fluoxetine hydrochloride dispersible tablets 20 mg a day for 8 weeks. They were evaluated with Hamilton Self-rating Scale for Depression (HAMD) before, and 2 weeks, 4 weeks and 8 weeks after treatment respectively. The rCBF were measured with single positron emission computerized tomography (SPECT) before and 8 weeks after treatment. Results The score of HAMD decreased more in the electroacupuncture group than in the drug group 2 weeks after treatment (P<0.05), and there was no significant difference between the two groups 4 and 8 weeks after treatment (P>0.05). The rCBF was significantly greater in the electroacupuncture group than in the drug group. Conclusion That electroacupuncture is effective on post-stroke depression, which may be related with the increase of the rCBF levels.

5.The risk factor for low bone mineral density in patients with inflammatory bowel disease

Objective To evaluate the prevalence of low bone mineral density in patients with inflammatory bowel disease(IBD) and to identify its risk factors. Methods A cross-sectional survey was carried out in IBD patients. Anthropemetric measures, biochemical markers of nutrition and bone mineral density measurement were completed for these patients as well as healthy control subjects. Results Seventy-seven Crohn's disease (CD) and 43 ulcerative colitis(UC) patients were enrolled, and 37 healthy volunteers were recruited as healthy controls(HC). The T value of CD patients, UC patients and HC was -1.72±1.20,-1.26±1.12 and-0.62±0.87 respectively and the T value of CD patients was significantly lower than that of HC (P=0.000). The prevelance of osteoporosis in CD, UC and HC was 23.3%, 14.0% and 0 respectively. The prevelance of osteoporosis in CD was higher than that in HC (P=0.003). Logistic regression analysis indicated that low BMI(BMI≤18.4 kg/m~2) was an independent risk factor for osteoporosis both in CD (OR=11.25,95% CI 3.198-39.580, P=0.000) and in UC (OR= 14. 50,95% CI 1.058-88.200, P=0.045) patients. Age, disease duration, clinical activity active index (CDAI), oral steroid therapy, immunosuppressant treatment and serum vitamin D concentration were not found to be correlated with osteoperosis in IBD patients. Conclusions Low bone mineral density is common in both CD and UC patients and low BMI is an independent risk factor for osteoporosis in IBD patients.
6.A fluorescence imaging tool targeting burn wounds: research on the application of pH low insertion peptide

Objective:pH low insertion peptide (pHLIP)-variant 7 (var7)-fluorescein isothiocyanate (FITC) was used to explore an accurate imaging tool that targeted burn wounds to better perform burn debridement.Methods:Twelve rat models of burn wound were established and pHLIP-var7-FITC with different concentrations (0.5, 1.5 and 2.0 mg/ml) were injected from the rat tail vein for in vivo fluorescence imaging. By determining the concentration of fluorescent conjugates to the burn wound, the scope of wound injury necrosis was judged by combining pathological sections, and its residue and toxicity in important organs such as heart, liver, kidneys, and brain were detected. The Kruskal-Wallis rank sum test, Bonferroni correction method and one-way analysis of variance were used for data analysis. Results:Within 24 h, the fluorescence photons per unit area of the burn wound in the group of 0.5 mg/ml, 1.5 mg/ml and 2.0 mg/ml were 1.49(1.31, 1.65), 2.46(1.88, 2.68), 2.77 (1.94, 3.10)×10 7 p·s -1·cm -2·Sr -1, with significant differences in the overall distribution of fluorescence photons ( H=73.55, P<0.001). The fluorescence intensity was stronger in the group with higher concentration, but with no significant difference in the number of fluorescence photons between the group of 1.5 mg/ml and 2.0 mg/ml ( P=0.263, Bonferroni correction method). At 14 time points (0.5, 1.0, 1.5, 2.0, 2.5, 3.0, 3.5, 4.0, 5.0, 6.0, 7.0, 8.0, 12, 24 h), there was no significant difference in the overall mean of fluorescence photons ( F=1.04, P=0.419), and the tissue with burn necrosis seen in tissue sections was highly consistent with the fluorescence imaging region. There was no obvious fluorescence residue in the heart, liver, kidney and brain sections. Conclusion:In superficial second-degree burn tissue, pHLIP-var7-FITC can accurately target and gather on the burn wound within 24 h, showing a clear boundary between burn tissue and normal tissue, which can assist clinical surgical debridement to determine the extent of injury.
7.Risk factors analysis and nursing strategies of coronary heart disease among young people

Objective To explore the risk factors of young patients with coronary heart disease ( CHD) , and the differences between the young patients and the elderly patients to provide a reference for the prevention of early CHD. Methods We retrospectively analyzed 437 cases of patients who came our hospital and had coronary artery bypass surgery from May 2009 to May 2012. The age of young people was less than 45 years wold patients (51 cases), and elderly patients (age greater than or equal to 65 years of age) were 386 cases. We compared and analyzed the indicators of the family history, lipid profile, blood glucose, blood pressure, heart function, postoperative infections, liver and kidney function, postoperative complications, between two groups of patients. Results Compared with the older patients, there were differences on the positive family history, smoking, obesity, dyslipidemia, cardiac function, angina classification, blood sugar, complications in young group (P<0. 05). Acute myocardial infarction was 33. 3% in young people, but it was 18. 1% in elderly patients (χ2 =2. 15,P<0. 05), old myocardial infarction was 7. 8% in young people, but it was 20. 7% in elderly patients (χ2 =2. 16,P<0. 05). Conclusions The major risk factors for CHD of young men are family history, smoking, being overweight, low HDL, high blood pressure, and the young patients would recovery faster than older patients after surgery. Early surgical treatment could prevent the occurrence of CHD and delay the progress.
8.Application of automatic and intelligent micro pump in changing vasoactive agents

Objective To explore the effects of automatic and intelligent micro pump in changing vasoactive agents. Methods This research selected 60 patients with a great number of vasoactive agents (dopamine or adrenalin hydrochloride) after cardiac surgery of Cardiovascular Surgery in Chinese PLA General Hospital from April to November 2016. Those patients were divided into experimental group (n=30, changing agents with the automatic and intelligent micro pump) and control group (n=30, changing agents with the traditional method of dual pump). The fluctuation of blood pressure (BP) of patients between two groups was compared before and after changing agents. Results There was no significant difference in BP of both group patients before changing vasoactive agents (P> 0.05). The mean BP of patients in the control group was significantly higher than that in the experimental group 30 seconds after changing vasoactive agents (P=0.011). The degree of fluctuation of BP of patients in the control group was greater than that in the experimental group 30 seconds and one minute after changing agents with a significant difference (P<0.05). Conclusions The automatic and intelligent micro pump has a big advantage in maintaining a stable BP during changing agents by pump. It is worth to be used widely in clinical application.
9.Interactions of purinergic receptors in treating neuropathic pain using electroacupuncture

Objective To elucidate the mechanism underlying eletroacupuncture's (EA's) amply-documented analgesic effect.To observe its effect on the pain threshold and on the expression of P2X4 receptor and microglia activation in the spinal cords of rats with neuropathic pain.To demonstrate whether or not interfering with A1 receptors and P2X4 receptors at the same time could enhance the analgesic effect.Methods A total of 40 SpragueDawley rats weighing 150 to 180 g were randomly divided into a sham group,a CCI group,an EA group,a 2-chloroN(6)-cyclopentyladenosine (CCPA) group and a CCPA+EA group,each of 8.Chronic constriction injury (CCI)was induced successfully in the rats of all groups except the sham group.Five days later,EA and 20 μL injections of 0.1 mm/L CCPA were applied to the rat analogues of the Zusanli (ST36) and Yanglingquan (GB34) acupoints once a day for 15 days for the rats in the appropriate groups.The mechanical withdrawal threshold (MWT) and the thermal withdrawal latency (TWL) were measured before the CCI operation and 20 days afterward.L4-L6 spinal cord tissue was then resected and the fluorescence intensity of P2X4 and OX42 receptors was detected using double label immunohistochemical staining.The correlation between the mean fluorescence intensity and the pain threshold gap was analyzed.Results The average MWT and TWL of the CCI group were significantly lower than in the other four groups.The expression of P2X4 receptor and OX42 in the spinal cord increased significantly in the CCI group compared to the other four groups.There was significant correlation between the mean fluorescence intensity of P2X4 and OX42 receptors and the gap in pain threshold,with correlation coefficients of 0.907 and 0.717 respectively.Conclusion P2X4 receptor and microglia activation might be involved in the development of neuropathic pain.CCPA and EA can inhibit the activation of microglia and reduce the activity of P2X4 receptors.The interaction between A1 receptors and P2X4 receptors can strengthen the analgesic effect of EA.
10.Congenital leukemia in 10 neonates

Objective:To have a better understanding of congenital leukemia by summarizing its clinical features and prognosis.Methods:This study retrospectively recruited 10 neonates with congenital leukemia treated in Beijing Children's Hospital, Capital Medical University from January 2006 to December 2018. Clinical data including clinical manifestations, laboratory examinations, treatment and follow-up were described.Results:The 10 patients consisted of six boys and four girls. In all cases, symptoms presented within 11 d after birth. The admission complaints were jaundice ( n=4), polypnea ( n=3), fever ( n=2) and rash ( n=2). Physical examinations of the 10 patients showed eight with splenomegaly, seven with hepatomegaly and seven with petechia/skin rash. All patients had significantly increased white blood cell count (from 45.8×10 9/L to 553.0×10 9/L), complicated by different degrees of anemia and thrombocytopenia. By bone marrow biopsy, two cases were diagnosed as acute lymphoblastic leukemia and the other eight cases as acute myeloid leukemia. All cases refused chemotherapy on the preliminary diagnosis. Three cases lost follow-up and six died within two months after discharge requested by their parents. One baby had spontaneous remission, but relapsed two years later. Complete remission was achieved after strict management and no relapse was reported until ten years old. Conclusions:Congenital leukemia is a severe condition with high mortality. Some cases may achieve spontaneous remission, but long-term follow-up is needed.
