1.Determination of triterpenes in Melastoma dodecandrum Lour. from different producing areas by HPLC

Objective To develop a HPLC method for the simultaneous content determination of three terpenoids inMelastoma dodecandrum Lour. (asiatic acid, betulinic acid, oleanolic acid).Methods The chromatographic column was set with waters SunFire C18 column (4.6 mm×150 mm, 5μm); the moving phase was acetonitrile -0.1%H3PO4; the column temperature was 30℃; the detection wavelength was 200 nm; the flow rate was 0.6 ml/min; and the sample volume was 25μl.Results A good linear relationship was observed in the range of 0.310-6.200μg (r=0.9999), 0.405-8.100μg (r=0.9999), 0.169-3.375μg (r=0.9998) for asiatic acid, betulinic acid, oleanolic acid respectively, with the average recovery rates of 102.08%, 101.81%, 102.22%. Conclusions The established method is convenient and sensitive, repeatability and stability, quality evaluation for Melastoma dodecandrum Lour.
2.Macroscopic and microscopic characteristics of Chinese caterpillar fungus.

Chinese Caterpillar Fungus (CCF) is one of the rare Chinese traditional drugs. As the resource is reducing sharply, the price is rising higher and higher, and there have been much more adulterants in the markets, but until now we don't have a scientific and accurate research on the identification study for this drug. On the basis of resource investigation, during the study of the samples collected by ourselves and the specimens stored in the museum, using the macroscopic and microscopic methods, referring to the literatures of entomology, emphasizing on the characteristics of polypide part, we have studied this species in detail of the macroscopic characters such as the insertion position of the stroma part, the annulations and segments of the caterpillar, the abdominal leg, the pinaculum, and the microscopic characters of the body wall; firstly added the microscopic character of the crotchets on the planta of abdominal leg. The result turned out that the characters which we have studied are regular and stable, and it have laid the foundation for the powder products and patent medicines which have used the crude drug of CCF.
3.Relationship between ultrasonographic characteristics of thyroid papillary microcarcinoma and metastasis of the cervical lateral lymph node

ObjectiveTo probe into the ultrasonic(US) predictive value for metastasis of the cervical lateral lymph node(LNM) with the preoperative ultrasonic examination in thyroid papillary microcarcinoma (PTMC),then,to further evaluate the significance of the preoperative ultrasonic score (US) for predicting metastasis of the cervical lateral lymph node.Methods From May 2004 to May 2011,165 patients diagnosed as PTMC (the ultrasonic imaging of lymph node showed no lateral LNM) were retrospectively analyzed and the US and final pathological data from the cases were collected to determine the relevant factors and different scores.ResultsThere were significant differences(P < 0.05 )among US characteristics of microcalcification,no well-defined margin,tumor located at the upper pole,the perimeter of the tumor contacted with the adjacent capsule of thyroid being >25 % and the presence of the cervical lateral lymph nodes metastasis in PTMC between the metastatic group and the non-metastatic group.The total scores of the ultrasonic ≥2.5 was the delimitation value,its sensitivity was 86.2% and its specificity was 89.7%.ConclusionsThe ultrasonic score has definite value in the differential diagnosis for the metastasis of the cervical lateral node in the patients of no lateral LNM,when the ultrasonic score is larger than 2.5,the possibility of the cervical lateral lymph node metastasis should be noticed.
4.Predictive factors for lateral lymph node metastasis in papillary thyroid microcarcinoma

Objective To find the clinical and histopathological predictive factors for lateral lymph node ( LN ) metastasis in thyroid papillary microcarcinoma( PTMC ).Methods From January 2007 to December 2010,141 patients with PTMC underwent central and lateral LN dissections were enrolled in the study.The data from the cases were analyzed retrospectively to determine the predictive factors for lateral LN metastasis.Results Among the aforementioned cases,37 cases ( 26.2% ) were PTMC with the presence of multifocality,24 ( 16.9% ) Hashimoto thyroiditis,84 ( 59.6% ) central LN metastasis,56 ( 39.7% ) lateral LN metastasis.Multifocality,central LN metastasis,underlying Hashimoto thyroiditis,and upper pole location were significantly related to lateral LN metastasis in univariate analysis( P<0.05 ).These four factors were also found to be independent predictive factors for lateral LN metastasis in multivariate analysis( P<0.05 ).9(6.4% ) of there patients were found to show skip metastasis in which there was lateral LN metastasis but no central LN metastasis.The upper pole location was statistically significantly associated with skip metastasis.Conclusions Patients with multifocality,central LN metastasis,underlying Hashimoto thyroiditis,and upper pole location should be paid more attention to the status of lateral LN.Skip metastasis occurs in a minority of patients with PTMC.Even if there is no central LN metastasis,patients with upper pole lesion should be searched carefully for the lateral cervical LN metastasis.
5.Chemical constituents of Helicia nilagirica seeds (Ⅰ)

Objective To study the chemical constituents of Helicia nilagirica. Methods The ethanol extract was seperated by petroleum ether, dichloromethane, and n-butanol in sequence, then isolated by silica gel column chromatography. The structures were identified and elucidated by physicochemical pro-(perties) and spectral analysis. Results Four compounds were isolated from the petroleum ether extract and identified as n-hexadecane acid (Ⅰ), ?-sitosterol (Ⅱ), ?-sitosterol-3-O-?-D-glucoside-6′-acetate (Ⅲ), and daucosterol (Ⅳ). Conclusion All the compounds are isolated from the plant for the first time. Compounds Ⅰ, Ⅲ, Ⅳ are isolated from the plants of Helicia Lour. for the first time, and compound Ⅲ is a new natural product.
6.Quality standard of Radix Isatidis Preparations

AIM:To improve the quality control standard based on 3 kinds of Radix Isatidis preparations-Radix Isatidis syrup,chewable tablet and granules. METHODS: A TLC method was established for iden tification of with Isatidis indigotica RS,Vleucine CRS and Arginine CRS as the references.A HPLC method was developed for the determination of adenosine and igoitrin in Radix Isatidis Preparations,with DIKMA Diamonsil C_(18) column(5 ?m,200 mm?4.6 mm id),a mixure of acetonitrile-water-triethylamine(10∶90∶0.2,using glacial acetic acid to adjust to pH 5.2)-water(85∶15) as the mobile phase.The detection wavelength was set at 245 nm. RESULTS: Isatidis indigotica,Vleucine and Arginine were detected in all the preparations.The contents of adenosine and igoitrin varied greatly in tested samples from different pharmaceutical enterprises. CONCLUSION: The above methods are simple,accurate,and suitable for the quality control of Radix Isatidis Preparations.
7.Determination of Cinobufagin and Resibufogenin in Qianglijiuxin Dripping Pills by HPLC

Objective: To determine the contents of cinobufagin and resibufogenin in Qianglijiuxin Dripping Pills. Methods: The HPLC method was used at Kromasil C 18 column of 250mm?4.6mm, 5?m, 0.5% Dipotassium Hydrogen Phosphate Acetonitrile (50∶50) as the mobile phase, 1.0mL/min as the flow rate at detection wavelength of 296nm. Results: For cinobufagin, the linear range was 0.67?g~4.7?g and the average recovery rate was 97.78% with RSD=1.2%. For resibufogenin, the linear range was 0.33?g~2.3?g and the average recovery was rate 99.61% with RSD=2.0%.Conclusion: The method can be applied to content determination of cinobafagin and Resibufogenin in Qianglijiuxin Dripping pills.
8.Research progress on genotype and phenotype of primary hyperoxaluria

Primary hyperoxaluria (PH) are important causes of kidney stone and chronic kidney stone disease in children. Recurrent kidney stone disease and nephrocalcinosis should alter the physician to the possibility of an inborn error of metabolism as the underlying cause. Unfortunately, the lack of recognition of genotype and phenotype of PH resulted in an unacceptable delay in diagnosis and treatment, sometimes with grave consequences. This paper review the characteristics of genotype and phenotype, genotype-phenotype correlation, current treatment and future gene therapy of PH.

10.Highly active antiretroviral therapy for patients with HIV/HCV co-infection

ObjectiveTo investigate the efficacy of highly active antiretroviral therapy (HAART) for HIV/HCV co-infection patients. MethodsA randomized and double blinded trial was conducted in sixty-three HIV/HCV co-infected patients ( group A) and 62 HIV infected patients ( group B). The group A (study group) was further divided into A1, A2, A3 subgroups randomly by Spw-Pb network data system, and were given three different HAART regimens based on nevirapine (NVP), efavirenz (EFV) and lopinavir/ritonavir(LPV/r), respectively. CD4+ T lymphocyte counts, HIV virus load, glutamate-pyruvate transaminase (ALT) were detected at baseline and at the endpoint of study (48 weeks). SPSS 13.0 was used for statistical analysis. One-way ANOVA and LSD-t tests were performed. ResultsAfter 48 weeks treatment, HIV RNA became negative in 59 patients of group A (59/63, 93.7% ), while that in group B was 61 (61/62, 98.4% ) (x2 =0. 159, P > 0.05 ). CD4+ T lymphocyte count in group A was (208 ± 77 )/μL, which was significantly lower than that in group B (263 ± 78)/μL (t =-2. 759, P = 0. 008 ).ALT level in group A was (57 ±49)U/L, which was significantly higher than in group B (31 ± 14) U/L (t = 2. 027, P = 0.047). CD4 + T lymphocyte count in group A3 was significantly higher than that in A1 (t=-2. 191, P =0.045), while ALT level in A1 was much higher than that in subgroups A2 and A3 ( t = 2.568 and 2.478, P < 0. 05 ). The incurrence of drug-induced hepatitis in HIV/HCV co-infected group was much higher than that in HIV infected group (55.5% vs. 27.4%, x2 = 10. 182, P = 0.001 ).ConclusionsHCV co-infection in HIV patients shows no impact on virological response to HAART, but the immunological response is poorer.Hepatotoxicity is common among patients receiving HAART, especially those who are receiving NVP containing regimens. LPV/r based regimens are recommend for HIV/HCV coinfected patients.
