AIM: To estalish a HPLC method for the determination of cinobufagin and resibufogenin in Niuhuangxiaoyan Tablet(Calculus Bovls, Radix et Rhizoma Rhei, Concha Margaritifera Usta, Venenum Bufonis, etc.). METHODS: Waters Symmetry Shield RP18(3.9mm?250mm,5?m) column at 30℃ was used with a mobile phase consisted of acetonitrile -0.5mol?L -1 potassium dihydrogen phosphate(solution was adjusted by phosphoric acid to pH=3.2) (42∶58) and a UV detector at 296nm. The flow rate was 1.0mL?min -1 . RESULTS: The linear range of cinobufagin was 0.0696～0.8352?g. The RSD of measurement precision test was 0.53%; The average recovery was 100.06% ( RSD =1.25%, n =5). The linear range of resibufogenin was 0.1576～0.9456?g. The RSD of measurement precision test was 0.15%. The average recovery was 100.54%( RSD =1.49%, n =5). CONCLUSION: The method is simple and accurate, and can be used for the quality control of Niuhuangxiaoyan Tablet.

Objective To compare the clinical efficacy between Jian Huan Tong Tiao needling (acupuncture at both healthy and affected sides) and acupuncture at the affected side alone in treating peripheral facial paralysis (FP) in acute stage, and to provide evidences for the acupuncture treatment of peripheral FP in acute stage.Method Fifty patients were randomized into a Jian Huan Tong Tiao needling group (25 cases) and an affected side needling group (25 cases). By selecting the same acupoints, the Jian Huan Tong Tiao needling group received acupuncture at both sides, while the affected side needling group received acupuncture only at the affected side, once every other day, successively for 4 weeks. House-Brackmann (H-B) facial nerve grading system was used to score the facial nerve function before and after the treatment, and the therapeutic efficacies were compared between the two groups.Result The facial nerve function was improved in both groups, while the therapeutic efficacy was superior in the Jian Huan Tong Tiao needling group to that in the affected side needling group (P<0.05); after 2-week treatment, the H-B score was higher in the Jian Huan Tong Tiao needling group than in the affected side needling group (P<0.05); of the recovered cases, the relapse rate was lower in theJian Huan Tong Tiao needling group than in the affected side needling group (P<0.05). Conclusion Jian Huan Tong Tiao needling can boost the recovery of the facial nerve function in peripheral FP, reduce the relapse rate, and produce a higher therapeutic efficacy than needling the affected side alone.

Objective To study the chemical constituents in the roots of Phlomis medicinalis.Methods The compound was isolated and repeatedly purified by macroporous resin,silica gel column chromatography,TLC,and Prep-HPLC.Its stracture was elucidated by physical and chemical properties and NMR spectra.Results One iridoid glucoside was obtained and elucidated as 8,10-dehydropulchelloside.Conclusion The compound is a new one.

AIM: To study the application of near infrared spectroscopy for quality control of in-process Wuji Baifeng Pill(Gallus Domesticus,Colla Cornus cervi,Carapax Trionycis,Otheca Mantidis,Radix et Rhizoma Ginseng,etc.). METHODS: Qualitative and quantitative methods for analysis of in-process product was developed by diffuse reflectance near infrared spectroscopy combined with chemometrics.Spectra of 95 samples of Tongren Wuji Baifeng Pill in-process and 19 raw material negative in-process products were collected.Similarity match model was built on the basis of spectra of 75 batches of qualified in-process products.Using this model,the similarity match values of other samples were calculated.Multivariate calibration models for paeonin and water content in in-process product were developed with partial least square algorithm. RESULTS: The results showed that the proposed similarity match model could reflect the quality characteristic of Tongren Wuji Baifeng Pills in-process and reveal the absence and presence of raw materials.The correlation coefficients of the calibration model were within 0.965 7 and 0.992 1 while the values of root mean square error of prediction were 0.005 5% and 0.45%,respectively. CONCLUSION: The experiment shows that the established method is accurate,fast and non-destructive,and can be applied to on-line detection.

ObjectiveTo probe into the ultrasonic(US) predictive value for metastasis of the cervical lateral lymph node(LNM) with the preoperative ultrasonic examination in thyroid papillary microcarcinoma (PTMC),then,to further evaluate the significance of the preoperative ultrasonic score (US) for predicting metastasis of the cervical lateral lymph node.Methods From May 2004 to May 2011,165 patients diagnosed as PTMC (the ultrasonic imaging of lymph node showed no lateral LNM) were retrospectively analyzed and the US and final pathological data from the cases were collected to determine the relevant factors and different scores.ResultsThere were significant differences(P ＜ 0.05 )among US characteristics of microcalcification,no well-defined margin,tumor located at the upper pole,the perimeter of the tumor contacted with the adjacent capsule of thyroid being ＞25 ％ and the presence of the cervical lateral lymph nodes metastasis in PTMC between the metastatic group and the non-metastatic group.The total scores of the ultrasonic ≥2.5 was the delimitation value,its sensitivity was 86.2％ and its specificity was 89.7％.ConclusionsThe ultrasonic score has definite value in the differential diagnosis for the metastasis of the cervical lateral node in the patients of no lateral LNM,when the ultrasonic score is larger than 2.5,the possibility of the cervical lateral lymph node metastasis should be noticed.

Objective To find the clinical and histopathological predictive factors for lateral lymph node ( LN ) metastasis in thyroid papillary microcarcinoma( PTMC ).Methods From January 2007 to December 2010,141 patients with PTMC underwent central and lateral LN dissections were enrolled in the study.The data from the cases were analyzed retrospectively to determine the predictive factors for lateral LN metastasis.Results Among the aforementioned cases,37 cases ( 26.2％ ) were PTMC with the presence of multifocality,24 ( 16.9％ ) Hashimoto thyroiditis,84 ( 59.6％ ) central LN metastasis,56 ( 39.7％ ) lateral LN metastasis.Multifocality,central LN metastasis,underlying Hashimoto thyroiditis,and upper pole location were significantly related to lateral LN metastasis in univariate analysis( P＜0.05 ).These four factors were also found to be independent predictive factors for lateral LN metastasis in multivariate analysis( P＜0.05 ).9(6.4％ ) of there patients were found to show skip metastasis in which there was lateral LN metastasis but no central LN metastasis.The upper pole location was statistically significantly associated with skip metastasis.Conclusions Patients with multifocality,central LN metastasis,underlying Hashimoto thyroiditis,and upper pole location should be paid more attention to the status of lateral LN.Skip metastasis occurs in a minority of patients with PTMC.Even if there is no central LN metastasis,patients with upper pole lesion should be searched carefully for the lateral cervical LN metastasis.

Objective To explore the epidemiology and risk factors of toenail onychomycosis among traffic police-men in Tianjin, and to prevent and control its prevalence. Methods Epidemiological surveys were performed to a total of 1 270 traffic policemen during physical examination in Public Security Hospital in Tianjin using cross-section study. Fungi in feet and the prevalence of toenail onychomycosis were examined simultaneously during questionnaires. Logistic regression of single factor and multiple factors were applied to analyze the risk factors. Results The prevalence of toenail onychomyco-sis in Tianjin traffic policemen is 32%. Age (OR=1.455), police boots (OR=1.047), history of tinea pedis (OR=7.521) and his-tory of diabetes (OR=1.820) are the main risk factors. Conclusion The prevalence of onychomycosis among Tianjin traffic policemen is high. Synthetic and effective interventions should be employed to deal with the risk factors.

Zebrafish toxicity assessment system is one of the important vertebrate model systems. Zebrafish is playing an increas-ingly important role in the field of toxicology studies because of its small size, short generation cycle, the transparent embryo and high reproductive rate. Now it is widely used in the embryonic derelopmental toxicology, pathological toxicology, environmental toxicology and other areas of toxicology studies with its unique advantages.

Objective:To determine residual sulfite in traditional Chinese medicinal materials or pieces processed by sulfur fumi-gation respectively by iodine titration, acid-base titration and ion chromatography and compare the results. Methods:The three meth-ods were used to determine four kinds of Chinese herbal medicines including Codonopsis radix, Dioscoreae rhizoma, Achyranthis bident-atae Radix and Atractulodis Macrocephalae Rhizoma, the recovery tests were also performed and the results were analyzed and compared to summarize the characteristics and quality control requirements of each method. Results:Iodine titration and acid-base titration had the advantages of simple operation process and low cost. However, there were many interference factors in the two methods, and due to different principles, they were suitable for the determination of different varieties of herbal medicines. Ion chromatography method had the advantages of high sensitivity and strong specificity, while the cost was high. Conclusion: It is suggested that proper methods should be chosen for the determination of sulfur dioxide residues according to actual situations.

ObjectiveTo investigate the efficacy of highly active antiretroviral therapy (HAART) for HIV/HCV co-infection patients. MethodsA randomized and double blinded trial was conducted in sixty-three HIV/HCV co-infected patients ( group A) and 62 HIV infected patients ( group B). The group A (study group) was further divided into A1, A2, A3 subgroups randomly by Spw-Pb network data system, and were given three different HAART regimens based on nevirapine (NVP), efavirenz (EFV) and lopinavir/ritonavir(LPV/r), respectively. CD4+ T lymphocyte counts, HIV virus load, glutamate-pyruvate transaminase (ALT) were detected at baseline and at the endpoint of study (48 weeks). SPSS 13.0 was used for statistical analysis. One-way ANOVA and LSD-t tests were performed. ResultsAfter 48 weeks treatment, HIV RNA became negative in 59 patients of group A (59/63, 93.7％ ), while that in group B was 61 (61/62, 98.4％ ) (x2 =0. 159, P ＞ 0.05 ). CD4+ T lymphocyte count in group A was (208 ± 77 )/μL, which was significantly lower than that in group B (263 ± 78)/μL (t =-2. 759, P = 0. 008 ).ALT level in group A was (57 ±49)U/L, which was significantly higher than in group B (31 ± 14) U/L (t = 2. 027, P = 0.047). CD4 + T lymphocyte count in group A3 was significantly higher than that in A1 (t=-2. 191, P =0.045), while ALT level in A1 was much higher than that in subgroups A2 and A3 ( t = 2.568 and 2.478, P ＜ 0. 05 ). The incurrence of drug-induced hepatitis in HIV/HCV co-infected group was much higher than that in HIV infected group (55.5％ vs. 27.4％, x2 = 10. 182, P = 0.001 ).ConclusionsHCV co-infection in HIV patients shows no impact on virological response to HAART, but the immunological response is poorer.Hepatotoxicity is common among patients receiving HAART, especially those who are receiving NVP containing regimens. LPV/r based regimens are recommend for HIV/HCV coinfected patients.
