1.Clinical research on the relationship between breast cancer and thyroid disorder

Objective To investigate the incidence of autoimmune and nonautoimmune thyroid diseases in patients with breast cancer.Methods Clinical and ultrasound evaluation of thyroid gland,detection of serum thyroid hormone and related antibodies,and fine-needle aspiration of thyroid gland were performed in 100 breast cancer patients and 100 control individuals during the period from 2004 to 2008.Results The mean values of anti-thyroid peroxidase antibodies were significantly higher in breast cancer patients than that in control individuals [(104.56±21.54) U/ml vs (22.16±4.65) U/ml,(P=0.030)].The incidence rates of autoimmune and nonautoimmune thyroid diseases were higher in breast cancer patients than that in control individuals[38 % (38/100) vs 17 % (17/100),P=0.0009,26 % (26/100) vs 9 % (9/100),P =0.0016,respectively].Conclusion The results indicate an increased incidence of autoimmune and nonautoimmune thyroid diseases in breast cancer patients.
2.Gait and Surface Electromyogram Characteristics in Underwater Treadmill Training

Objective To explore the characteristics of gait and surface electromyography (sEMG) of underwater treadmill training (UWTT). Methods From January to September, 2015, gait and sEMG parameters of ten healthy adults were synchronously collected using three-dimensional gait analysis system and sEMG systems on land and UWTT. Results When walking in self comfortable speed, compared with walk on land, there was significant increase in gait cycle (t=-11.411, P<0.001) and percentage of the swing phase (t=-5.252, P<0.05), significant reduction in walking speed (Z=-2.803, P<0.01) and step frequency (t=7.421, P<0.001). There was no significant difference in step strength (Z=-0.357, P>0.05). The integral electromyography of biceps femoris, gastrocnemius, tibialis anterior muscle were significant less than iEMG on land (t>3.984, P<0.01), but no significant difference of rectus femoris (t=0.054, P>0.05). Conclusion There are signifi-cant differences in gait and muscle contraction intensity between land walking and UWTT. The dynamic physiology and neural control of UWTT need further study.
3.Development of a wrist wearing and remote heart rate alarm apparatus.

We have developed a new wrist wearing heart rate monitoring alarm apparatus, which can detect the patients' real-time pulse waves. When the abnormal heart rate appears or pulse disappears, the monitoring alarm will sound and dial the remote telephone for help simultaneously. This apparatus uses the switch circuit to control the keyboard of mobile phone, and dials remote telephone in the help of mature technology and communication platform of mobile phones. The intelligent program can distinguish digital pulse signal, pick out the correct cycle of heartbeat intelligently. The new wrist wearing heart rate monitoring alarm apparatus will calculate an average heart rate when it captures consecutively five correct electrocardiograph waveforms. It really provides a simple, inexpensive and effective way for the patients with heart disease.

4.Inhibitory effect of metformin on inflammatory response of Kupffer cells of liver in diabetic mice and its improvement on phagocytic function

Objective:To observe the inhibitory effect of metformin on the inflammatory response of Kupffer cells (KCs) of liver in the diabetic mice, and to explore its improvement on the phagocytic function of KCs, and to elucidate the protective mechanism of metformin on the KCs in the diabetic mice.Methods:The C57BLKS/J db/db mice were divided into diabetes group and diabetes+metformin group, and the C57BLKS/J db/m mice were divided into non-diabetes group and non-diabetes+metformin group, 8mice in each group.The KCs were cultured in vitro and the changes of ultrastrustures of KCs were observed by transmission electron microscope.The levels of interleukin 6 (IL-6) , tumor necrosis factorα (TNF-α) andγ-interferon (INF-γ) in the KCs were detected by ELISA.The expression levels of intercellular adhesion molecule 1 (ICAM-1) protein in the KCs was detected by Western blotting method.Transwell chamber assay was used to detect the neutrophil chemotaxis ability of the KCs, and the phagocytic ability of KCs was observed under inverted microscope.Results:The number of mitochondria and rough endoplasmic reticulum (RER) in the KCs of the diabetic mice was reduced, the RER expanded, the mitochondria swelled, and the lipid droplets were increased.Compared with non-diabetes group, the levels of IL-6, TNF-α, INF-γand the expression level of ICAM-1in the KCs of the mice in diabetes group were significantly increased (P<0.05) ;the neutrophil chemotaxis ability of the KCs was enhanced (P<0.05) , and the phagocytic ability was decreased (P<0.05) .Compared with diabetes group, the levels of IL-6, TNF-α, INF-γand the expression level of ICAM-1in the KCs in diabetes+metformin group were significantly decreased (P<0.05) ;the neutrophil chemotaxis ability was decreased (P<0.05) , and the phagocytic ability was enhanced (P<0.05) .Conclusion:Metformin can inhibit the inflammatory response of KCs in the diabetic mice and improve its phagocytic function and protect the KCs.
5.The clinical effect of holmium laser lithotripsy for urinary calculi via ureteroscope

Objective To study the analysis of ureteroscopy for the treatment of urinary calculi holmium laser combined with the clinical efficacy and complications. Methods Clinical data of 162 cases with urinary calculi from January 2011 to December 2012 were studied,they were divided into the observation group 81cases and control group of 81 cases,the observation group of holmium laser combined with ureteroscopy,the control group used conventional shock wave lithotripsy in the treatment of urinary calculi,compared the clinical efficacy of the two groups. Results About 93.8%stones were fragmented in one treatment session;The time of surgery was 25~125 min(average 57 min);After the opera-tion, length of hospital was 2~10 d (average 6.0 d); Row of net stones in time was 2~6 w(average 3 w). The experimen-tal group were 81 cases,the total effective rate was 100.0%;The control group were 70 cases,the total effective rate was 86.4%,had significant difference(P<0.05). Conclusion Holmium laser combined with ureteroscopy clinical curative effect of treatment of urinary calculi is safe and effective,with less complications,is worth the clinical promotion.
6.experimental study on impacts of miR-214 on the proliferation of human hepatocellular

Objective To investigate the impact of micro ribonucleic acid -214 (miR-214) on the proliferation of hepatocellular carcinoma (HCC) and its mechanism. Methods MHCC97L cells were respectively transfected using miR-214 mimics and negative control mimics to establish M214 group, negative control (NC214) group. And untransfected control (Ctrl) group was established. The contents of miR-214 of MHCC97L cells in three groups were detected by fluorescence quantitative polymerase chain reaction (PCR). Cell counting kit (CCK)-8 assay was used to define the cell proliferation inhibition rate. Plate cloning formation assay was used to define the cell clonality. Cell apoptosis rate was detected by flow cytometery. The expressions of protein c-myc, Bax and B-cell lymphoma (Bcl)-2 were detected by Western blot assay. The comparison of three groups was conducted using one way analysis of variance and pairwise comparison using LSD-t test. Results The miR-214 content of MHCC97L cells in M214 group was (2 536±7) times of that in Ctrl group, where significant difference was observed (LSD-t=58.75, P<0.05). The cell proliferation inhibition rates were (15.33±0.62) %, (24.07±0.75) %, (41.02±0.91) % at the 24, 48, 72 h in M214 group, and significant difference was observed compared with that in Ctrl group (LSD-t =14.64, 19.87, 31.86; P<0.05). The clone formation quantity was (5.3±0.7) /well in M214 group, which was significantly lower compared with that in Ctrl group [(37.1±1.0)/well] (LSD-t =-12.51, P<0.05). The cell apoptosis rate was (42.1±3.2)% in M214 group, which was significantly higher compared with that in Ctrl group [(7.0±0.7) % ] (LSD-t=35.66, P<0.05). In M214 group, the expressions of protein c-myc, Bcl-2 decreased and Bax increased. Conclusion The miR-214 can inhibit the proliferation of human HCC cells through down-regulating the protein c-myc expression and Bcl-2/Bax ratio.
7.Research and implementation of ideological and political elements into the medical physics curriculum

In order to improve the students' initiative and values in knowledge seeking, this study integrates ideological and political elements into the medical physics curriculum. On the basis of the standard curriculum, 11 ideological and political elements corresponding to 6 knowledge points are added to the courseware and teaching plan and implemented in the classroom. The changes of students' ideological and moral level and values before and after teaching were statistically analyzed by questionnaire. The results show that through the integration of ideological and political elements, 98.15% (106/108) think it is necessary to integrate the ideological and political elements into the curriculum. The proportion of students who study for the purpose of working skills, national medical care and world medical care has increased, while the proportion of students who earn credits has decreased, which shows that students' learning initiative and values have been improved.
8.Orientation and digital innovation construction of Medical Physics curriculum

Medical Physics is an interdiscipline which is formed by applying the basic principles, methods, and techniques of physics to clinical medical research such as prevention, diagnosis, and treatment of human diseases, and it is a compulsory professional basic course for medical students. However, there are many medical students reflect that the content of this course is obscure and difficult to understand. Teaching effect is not ideal. The main reasons are that the teaching method is single and the reference materials are few. Based on the actual situation of the course, Hebei Medical University, China promotes formative evaluation from the content of teaching materials, teaching means, and other aspects. At the same time, by combining with the Internet, the digital construction has been realized, deepening the reform of the Medical Physics curriculum, and remarkable results have been achieved.
9.Clinical characteristics and management strategies of late bleeding after laparoscopic pancreaticoduodenectomy

Objective:To study the clinical characteristics and management strategies of late bleeding after laparoscopic pancreaticoduodenectomy (LPD).Methods:The clinical data of 58 patients with post-pancreaticoduodenectomy hemorrhage (PPH) admitted to the Department of Hepatobiliary Surgery of the Second Hospital of Hebei Medical University from March 2018 to March 2022 were retrospectively analyzed, including 42 males and 16 females, aged (61.88±11.02) years old. According to the occurrence of intra-abdominal erosion factors (e.g., pancreatic fistula, biliary fistula, gastrointestinal anastomotic fistula, intra-abdominal abscess), patients were divided into the erosion group ( n=42) and non-erosion group ( n=16). All patients underwent standard lymphadenectomy. Clinical data including the PPH time-point, occurrence of rebleeding, and treatment outcomes were accessed. The management strategies of PPH in the two groups of patients were analyzed. Results:The PPH time-point in the erosion group and non-erosion patients was 8.00 (5.00, 19.25) d and 21.50 (12.75, 26.75) d, respectively ( P=0.001). PPH can occurred within one month after surgery in both erosion and non-erosion groups. In the erosion group, 31 cases (73.81%, 31/42) were treated by re-operation, two (4.76%, 2/42) by interventional radiology and nine (21.43%, 9/42) with conservative protocol, respectively. In the non-erosion group, five cases (31.25%, 5/16) were treated by re-operation, seven (43.75%, 7/16) by interventional radiology and four (25.00%, 4/16) with conservative protocol, respectively. The incidence of re-bleeding is higher in the erosion group [47.6% (20/42) vs 12.5% (2/16), P<0.05]. Clinical manifestations, sites and severity of bleeding, and treatment outcomes were also different in the erosion and non-erosion groups (all P<0.05). Conclusions:The occurrence of intra-abdominal erosion factors can affect the clinical characteristics and treatment strategy of late bleeding after laparoscopic pancreaticoduodenectomy. Surgery remains the treatment of choice for post-pancreaticoduodenectomy hemorrhage either as an urgent or last resort.
10.Research on the construction and application of digital question databaseof medical physics

Objective:Through constructing an online question database of medical physics, teachers and students can make use of the Internet to realize interaction in or after class, so students' ability of independent learning and the quality of teaching can be improved.Methods:Based on teaching outlines, the type, quantity and difficulty of the test questions were discussed and determined by teachers in the teaching and research office, and the online question database of medical physics was constructed with the help of Internet platform to realize self-testing and teacher-student interaction.Results:Through the construction and application of online question database, students' learning initiative was mobilized, the effectiveness of teacher-student interaction was improved, and empirical materials for teaching formation evaluation were obtained. Self-examination and teacher-student interaction help improve students independent learning and problem solving ability. And data generated on the platform enable teachers to know students' learning situation and their teaching quality.Conclusion:The construction and application of Internet + digital online question database of medical physics can greatly promote the teaching effect.
