1.Treating the urinary incontinence in stroke patients with acupuncture,moxibustion and physical therapy

Objective To analyze the urodynamic abnormalities of incontinent stroke patients,and evaluate the effects of acupuncture,moxibustion in combination with physical therapy.Methods One hundred and six in- continent stroke patients were divided into a control group and a treatment group.The control group was treated with conventional methods,and the treatment group with acupuncture,moxibustion and physical therapy modalities such as ultrasound and EMG biofeedback.The Urodynamic examination was performed before and after treatment.Re- sults It was found that the uradynamic abnormalities in the incontinent stroke patients include detrusor hyperreflexia and uninhibited sphincter relaxation.Combined use of acupuncture,moxibustion and physical therapy can significant- ly improve the patients' symptoms.Conclusion Common urodynamic abnormalities.are detrusor hyperreflexia and uninhibited external sphincter relaxation in incontinent stroke patients.Combined use of acupuncture,moxibustion and physical therapy is effective for treating this condition.
2.CT and MR Imaging Study of Tongue Cancer

Objective To study the findings of CT and MRI in tongue cancer.Methods 21 cases of tongue cancer were examined by CT or MRI,CT examined 7 cases,MRI examined 14 cases.Results MRI could display the tongue cancer in all the cases,but CT only display 5 of 7 cases of the tumour.The value of CT and MRI was same in displaying soft tissue direct invasion.MRI and contrast enhanced CT could display cervical lymph node metastases.CT was superior to MRI in the diagnosis of mandible invasion.Conclusion MRI is superior to CT in the display soft tissue of tongue cancer and the metastases.CT is optimal in detection of cortical bone invasion.
3.Professional skill examination and standard training of anaesthesiology residents

Objective To evaluate the training quality of anaesthesiology residents in different hospitals. Methods The scores of 125 residents,attending Shanghai resident clinic skill examination from 2005 to 2007,were analyzed. ResultsThe scores of examinees from municipal hospitals were higher than those from district hospitals,municipal specialized hospitals and traditional Chinese medicine hospitals.The scores of practice part were higher than those of theory part in all groups. Conclusion The training quality is related to the comprehensive strength of the hospital in which examinees work,but the training of theory knowledge still need to be strengthened.
4.The effect of scalp acupuncture combined with rehabilitation training on the regulation of neuro-plasticity-associated protein MAP-2 in gerbils modelling cerebral ischemia and reperfusion

Objective To observe the effect of scalp acupuncture and rehabilitation training on the regulation of neuro-plasticity-associated protein MAP-2 in gerbils modelling cerebral ischemia and reperfusion.Methods Cerebral ischemia and reperfusion were modelled into thirty-six male gerbils.They were then randomly divided into a model group (A),a scalp acupuncture and rehabilitation training group (B) and a rehabilitation training group (C),each of 12 cases.The scalp acupuncture and rehabilitation training were administered daily to the gerbils in group B beginning 24 h after modeling,for 14 d.Any changes in Bederson scores were observed after one,7 and 14 days.On the 14th day the expression of MAP-2 around the infarct focus was detected using western blotting in all 3 groups.Results On the 7th day the average Bederson score in group B (1.81 ± 0.52) was not significantly different from that in group A (2.13 ± 0.49) or group C (2.00 ± 0.31) (P > 0.05).At the end of the treatment,however,there were significant differences between the groups,with group B (0.47 ± 0.31) scoring significantly better than groups C (1.04 ± 0.63) and A (1.46 ± 0.72) (P < 0.05).Group C was also significantly better than group A (P <0.05).The expression of MAP-2 as measured as integrated optical intensity (IOD) in group B (0.91 ±0.18) was significantly higher than that in group A (0.43 ± 0.21) and group C (0.67 ± 0.24) (P < 0.05) ; so was group C compared to group A (P <0.05).Conclusion The scalp acupuncture combined with rehabilitation training can improve the recovery of motor function and enhance the expression of neuro-plasticity-associated protein MAP-2 in gerbils with after cerebral ischemia and reperfusion.
5.Preparation of the cDNA microarray on the differential expressed cDNA of senescence-accelerated mouse's hippocampus.

Alzheimer' s disease (AD) is the most common form of dementia in the elderly. AD is an invariably fatal neurodegenerative disorder with no effective treatment. Senescence-accelerated mouse prone 8 (SAMP8) is a model for studying age-related cognitive impairments and also is a good model to study brain aging and one of mouse model of AD. The technique of cDNA microarray can monitor the expression levels of thousands of genes simultaneously and can be used to study AD with the character of multi-mechanism, multi-targets and multi-pathway. In order to disclose the mechanism of AD and find the drug targets of AD, cDNA microarray containing 3136 cDNAs amplified from the suppression subtracted cDNA library of hippocampus of SAMP8 and SAMR1 was prepared with 16 blocks and 14 x 14 pins, the housekeeping gene beta-actin and G3PDH as inner conference. The background of this microarray was low and unanimous, and dots divided evenly. The conditions of hybridization and washing were optimized during the hybridization of probe and target molecule. After the data of hybridization analysis, the differential expressed cDNAs were sequenced and analyzed by the bioinformatics, and some of genes were quantified by the real time RT-PCR and the reliability of this cDNA microarray were validated. This cDNA microarray may be the good means to select the differential expressed genes and disclose the molecular mechanism of SAMP8's brain aging and AD.

6.Preparation of human meniscus acellular matrix.

OBJECTIVETo investigate development of a cell extraction process for preparing human meniscus acellular matrix, and morphology and biomechanical properties.
METHODSHuman meniscus were subjected to modified eight-step detergent, then, the specimens were assessed by staining with haematoxylin-eosin, toluidine blue, sirius red, saffron O, alcain blue and hoechst-33258, et al. The ultrastructure of the specimens was observed with scanning electron microscope. Transient recovery rate of deformation, maximal recovery rate of deformation and maximal compressive strength were tested to determine the biomechanical properties of the scaffold.
RESULTSEvery stain confirmed that the celluar constituents of the specimens were removed. The specimens stained positively by staining with sirius red. Lacuna were found irregularly not only on the surface of the meniscus,but also in the meniscus with scanning electron microscope. Pores in the specinmens were large, the diameter of pores was 80 to 760 microm, porosity was over 67%. The transient recovery rate of deformation was (89.62 +/- 1.04)%, the maximal recovery rate of deformation was 100% and the maximal compressive strength was (3.04 +/- 0.13)N, when the specimens were compressed 30%.
CONCLUSIONThe modified eight-step detergent can remove the immunogenic cell components from human meniscus, in addition, 3D extracellular matrix can be retained. The scaffold has good biomechanical properties. This scaffold stands a good chance to be an implant for future tissue engineering of the human meniscus.

7.The role of galactomannan detection in the diagnosis of invasive pulmonary aspergillosis in critically ill patients

Objective To evaluate the usefulness of serum galactemannan(GM) for the diagnosis of invasive pulmonary aspergillosis (IPA) in critically ill patients. Methods Study was conducted between February 2007 and July 2008. Included patients on admission ICU who suffer from suspected IPA. GM test and culture were collected 2 weekly. Chnical feature, mycological evidence and optical density index (ODI) were noted. Clinically invasive fungal infection(IFI) were defined proven, probable and possible. The patients were classified into neutropenia, non-neutropenia and treated with immunosuppressive agents, non-neutropenia and non-immunosuppressive agents. To compared of the sensitivity and specificity of GM in different patients. Results 94 patients were included, 4 patients were proven, 29 patients were probable, 34 patients were possible IFI, 27 patients were non-IPA. The positive rate of the GM was 31.9% (30/94). The sensitivity and specificity of GM in proven cases and probable cases are 66.7% and 92.6%. GM assay tended to become positive earlier than the culture 2-10(5.33±2.17)d. We found that differences in patient diagnosis and selection might account for the disparities seen for positive rate for the GM test. There was positive in three of the four patients with proven, the positive rate of GM was 65.5% for probable cases, for possible cases was 17.6%, for non-IPA cases was 7.4% (P=0.001). For patient with neutropenia , treated with immunosuppressive agents and without immunosuppressive agents, the positive rate of GM was 52.9%vs 41.7% vs 34. 6% (P=0.015) ;the sensitivity was 80.0% vs 70. 0% vs 53.8% (P=0.011), the ODI was 1.365 (0.582-6.736) vs 1. 123 (0. 623-6.868) vs 0.554 (0.522-0.823), P=0. 005, respectively. Conclusion These results show that GM test is useful for early diagnosis IPA in critically ill patients. Differences in patient selection and diagnosis might account for the disparities seen for positive rate and sensitivity for the GM test. It has been higher sensitivity and ODI in the patient treated by immunosuppressive agents.
8.The clinical value of multi-slice spiral CT in assessing the risk of esophageal bleeding

Objective To investigate the clinical value of multi-slice spiral CT in the evaluation of esophageal variceal bleeding .Methods 50 cirrhosis patients with esophageal varices received multi-slice spiral CT and gastroscopy detection .The application value of multi-slice CT in the assessment of esophageal bleeding was evaluated according to the results of gastroscopy detection .Results CT angiography score had significantly positive correlation with the severity of endoscopic varices and endoscopic red color sign (r=0.762,0.687,all P<0.01).The sensitivity and specificity of CT angiography score in diagnosis of endoscopic red signs RC 3 were 76.92% and 92.50%. Conclusion The results of multi-slice CT and gastroscopy are positively correlated with the severity of esophageal varices,which can be used to predict the risk of esophageal bleeding .
9.Expression of Adaptin-2 in Different Stages of Mouse Cochlea

Objective To investigate the expression of adaptin-2(AP-2) in different stages of mouse coch‐lea and its probable role in the auditory generation and formation .Methods Mice were divided into 4 experimental groups by age (7 days old ,15 days old ,35 days old ,16 months old) ,which respectively represented the newborn mice ,developmental mice ,mature mice and old mice .Auditory brainstem response (ABR) ,laser scanning confocal microscopy (LSCM) ,immune-fluroscence histochemistry and qRT - PCR were used in this study .Results For the 15 days old ,35 days old ,16 months old groups ,ABR average threshold was 18 .67 ± 1 .21 dB nHL ,13 .83 ± 1 .47 dB nHL ,37 .83 ± 7 .68 dB nHL ,respectively ,for the 7 days old groups no responses were observed .The AP-2 im‐munoreactivity was found in all the stages of mice cochlea inner hair cell (IHC) cytoplasm ,especially in IHC basal part ,nearby the ribbon synapse .For the 7 days old ,15 days old ,35 days old ,16 months old groups ,immune-flu‐roscence histochemistry IMV(intensity mean value)were 190 .91 ± 17 .27 ,494 .06 ± 27 .63 ,838 .41 ± 38 .23 ,682 .65 ± 72 .22 ,respectively .For the 7 days old ,15 days old ,35 days old ,16 months old groups ,mRNA RQ(relative quantity)were 0 .53 ± 0 .09 ,1 .03 ± 0 .02 ,1 .00 ± 0 .09 ,1 .03 ± 0 .06 ,respectively .Developmental mice expressed significantly higher than those of the newborn in the AP -2 protein expression level which was measured by immuno -fluores‐cence histochemistry and qRT PCR(P<0 .01) .There was no significant difference between old and mature mice in the AP-2 protein expression level measured by qRT PCR (P> 0 .05) ,but mature mice had significant advantages by immuno-fluorescence histochemistry .Conclusion AP-2 protein expression level may closely related to the au‐ditory formation and maintenance because its expression gradually increases with age in mice .
10.The genotype study for the Escherichia coli extended spectrum β-lactamase in Macao and Shenyang, China

Objective To identify the genotypos of extended spectrum β-1actamase (ESBLs)-producing of Escherichia coli ( E. coli) clinical strains isolated from the Macao and compare the results with the genotypes of clinical strains collected in the first Clinical College, China Medical University (CMU) in Shengyang. Methods The clinically isolated E. coli strains including 209 strains from Macao and 150 strains from CMU were collected. Based on the standard of CLSI2006, the ESBLs-producing strains was identified and its isoelectric point(pI) value was detected by isoelectric focusing (IEF) method. The pI values were used to design the primers for PCR amplification. The amplified DNA sequences were then compared with the GenBank and the ESBL genotypes were confirmed. Results ( 1 ) The positive rate of ESBLs-producing strains of E. coli was 30. 1% (62/209) from Macao and 54. 0% (81/150)from CMU. (2)The genotype of 56 (90. 5% ) β-lactamase(ESBLs)-producing E. cull strains from Macao was CTX-M56. Most of them were CTX-M-14 (76. 2% ), other genotypes including CTX-M-9 (4. 8% ), CTX-M-22 (3.2%), CTX-M-24(3.2%), CTX-M-27(1. 6% ), and CTX-M-15( 1.6% ) were found. Six strains were unidentified. (3)The genotype of 74(91.5% )β-lactamase(ESBLs) -producing E. coli strains from Shenyang was CTX-M. Most of them were CTX-M-14 (65.4%), other genotypes including CTX-M-3 ( 13.6% ), CTX-M-24 (4. 9% ),CTX-M-22(2.5%), CTX-M-15(2.5%), CTX-M-9(1.2%) and CTX-M-28(1.2%) were found. Seven strains were unidentified. Conclusions CTX-M genotypo was the mostly identified ESBL-preducing E. Coli strains from Macao and the results were similar with that from CMU. Among them, the CTX-M-14 was the major genotype. Other genotypes included CTX-M-9, CTX-M-15, CTX-M-22, CTX-M-27, and CTX-M-24.However, two genotypes of CTX-M-3 and CTX-M-28 were not found in the clinical isolates in Macao and one genotype of CTX-M-27 was not found from the CMU clinical isolates.
