AIM: To elucidate the relationship between the intracellular calcium concentration changes and left ventricular hypertrophy and function in the spontaneously hypertensive rats (SHR). METHODS: Intracellular free calcium concentrations were measured by Fura 2 methodology and left ventricular function quantitated by cardiac catheterization in 20 SHR aged 10, 22, and 34 weeks and 20 age-matched Wistar-kyoto (WKY) rats. RESULTS: (1) The systolic blood pressure(SBP), intracellular calcium concentrations and left ventricular mass / body weight index ( LVM/BW) were significantly higher in all three age groups of SHR than the corresponding groups of WKY; (2) Compared with age-matched WKY groups, the peak left ventricular pressure descending rate(-d p /d t max ) decreased while left ventricular relaxation time constant (?)increased significantly in SHR aged 22 and 34 weeks. Questionnaire-based survey, physical examination, and blood testing were conducted according to cluster random samplings in Kazakh residents in Xinjiang.2 760 samples were collected to analyze the association of different strata of waist circumference and clustering of metabolic syndrome (MS) components.Accoding to International Diabetes Federation standard, the prevalence of ≥1and ≥2 components of MS showed increasing trend with the increase of waist circunference, and odds ratio of clustering of MS components also increased significantly.The distance of receiver operating characteristic curve was the shortest and the prevalence of MS was 22.1% ;22.4% in men, and 21.9% in women;when the waist circumference was ≥91 cm for men, and ≥88 cm for women.
3.Association of lipoprotein lipase gene Hind Ⅲ and S447X polymorphisms with metabolic syndrome in Kazakh of Xinjiang

Objective To investigate the association of lipoprotein lipase gene Hind Ⅲ and S447X polymorphisms with metabolic syndrome. Methods PCR-RFLP was used to detect lipoprotein lipase Hind Ⅲ and S447X genotypes in 401 subjects(including 201 controls, 200 metabolic syndrome patients). Results ( 1 ) The levels of waist circumference ( WC ) , hip circumference ( HC ) , waist-to-hip ratio ( WHR ) , body mass index (BMI), systolic blood pressure (SBP), diastolic blood pressure ( DBP) , total cholesterol ( TC) , triglyceride (TG), high-density lipoprotein-cholesterol (HDL-C), low-density lipoprotein-cholesterol (LDL-C), and fasting plasma glucose (FPG) were significantly different between metabolic syndrome group and control group (all P< 0.05). (2)The frequencies of H+H+ genotype,H+allele,SS genotype, and S allele for metabolic syndrome were all significantly higher than those for controls( H+H+ genotype:66. 5% vs 54.2% ,P=0.012; H+ allele:78.0% vs 71.4%, P=0.031;SS genotype:89.5% vs 77. 1% , P = 0.001; Sallel:94.5% vs 87. 56% , P = 0.001). (3) The levels of WC, HC, WHR, BMI, SBP, DBP, TG, LDL-C, and FPG in H + H-/H-H- genotype were significantly lower than those in H+H+ genotype, HDL-C was significantly higher than that in H+H+ genotype ( all P<0. 05). The levels of WC, HC, WHR, BMI, SBP, DBP, TC, TG, and FPG in SX/XX genotype were significantly lower than those in SS genotype, HDL-C was significantly higher than that in SS genotype ( all P< 0.05). (4)Multi-way logistic regression analysis suggested that risk factors for metabolic syndrome were smoking, drinking, and SS genotype (OR value was 4.289,2.268, and 2. 597, respectively ). (5) Result of interaction analysis among different factors indicated that the risk for metabolic syndrome in smoker with SS genotype was 3. 996 times of non-smokers with SX/XX genotype. Conclusions The lipoprotein lipase gene S447X polymorphism is associated with metabolic syndrome risk in Kazakh, and SS genotype and S allele may serve as genetic risk factors of metabolic syndrome, H + H-/H-H- and SX/XX genotypes yield beneficial effect for lipid and blood pressure. SS genotype and smoking may exist additive effect.
4.An anatomical study and clinical application of free transfer of foot tissues pedicled with plantar metatarsal artery

Objective To explore a new method of functional reconstruction of hand digits and joints with free transfer of foot tissues so as to increase the success rate of the operation.Methods After micro-anatomic study of the plantar and dorsal metatarsal arteries,retrograde and free grafts of foot tissues pedicled with plantar metatarsal arteries were designed and applied in transplantation to treat 76 cases of hand digital or joint defects.The surgeries included 58 cases of transfer of the second toe,four cases of transfer of composite tissues of the second toe, eight cases of transfer of proximal interphalangeal joint,and six cases of nail flap transfer.Results The mi- cro-anatomic study found that the first plantar metatarsal artery was anatomically constant and the diameter of its branch to the second toe was larger than that of the first dorsal metatarsal artery.Flaps survived in 75 of the 76 patients(98.7%),with fine appearance and significantly improved function.One patient who had received free transfer of the second toe to reconstruct the thumb function had to undergo a second repair with infraclavicula skin tube because of refractory arteriospasm of anastomosed vessels.Conclusion Transfer with free retrograde grafts of foot tissues pedicled with plantar metatarsal artery to reconstruct hand functions can effectively improve the success rate of the operation,because it is free of the shortcomings of great anatomic variation of blood vessels and time-consuming and complex procedures in conventional transfer.

6.Prevalence and associated risk factors of diabetes mellitus and impaired fasting glucose in populations of Hans and Kazaks in Xinjiang

Objective To investigate the prevalence and associated risk factors of diabetes mellitus in populations of Hans and Kazaks in Xinjiang. Methods A total of 7 299 Hans and Kazaks adults from Shihezi, Xinyuan County of Yining city, and Shawan town of Tacheng City were surveyed. The data were collected according to questionnaire and physical examination and laboratory test. Results The prevalence rates of diabetes mellitus for male, female and all subjects were 8. 14% , 7. 77% , 7. 93% (Hans), and 7. 41% , 4. 90% , 5. 89% (Kazaks), respectively in Hans and Kazaks, Standardized rates were 6. 40% , 7. 06% , 6. 66% (Hans), and 5. 87% , 4. 60% , 5. 28% ( Kazaks), respectively . The prevalence rates of impaired fasting glucose( IFG) were 9. 54% , 8. 08% , 8. 70% ( Hans), and 12. 18% , 8. 62% , 10. 03% ( Kazaks), respectively. Standardized rates were 7. 04% , 7. 23% , 8. 54% ( Hans), and 10. 12% , 8. 09% , 9. 31% ( Kazaks), respectively. Old age, hypertension, dyslipidemia, overweight and obesity, and central obesity were risk factors for diabetes mellitus. Conclusion The prevalence rates of diabetes mellitus and IFG in Hans and Kazaks are higher than those in ethnic minorities in other region. Primary or secondary prevention should be implemented in time.
7.Analysis of haplotype and linkage disequilibrium of PPARγgene rs3856806, rs12490265, rs1797912, and rs1175543 in patients with metabolic syndrome in Kazakhs of Xinjiang

Objective To investigate the effect of haplotype and linkage disequilibrium of PPARγgene rs3856806, rs12490265, rs1797912, and rs1175543 in patients with metabolic syndrome ( MS) in Kazakhs of Xinjiang.Methods MALDI-TOF-MS was used to detect PPARγgene rs3856806, rs12490265, rs1797912, and rs1175543 genotypes in 489 subjects ( including 245 MS and 244 controls ) .Results ( 1 ) The frequencies of rs3856806T, rs12490265A, rs1797912C and rs1175543G alleles for MS group in Kazakhs were all significantly lower than those for controls [ rs3856806T allele:12.53% vs 17.01%; rs12490265A allele: 31.84% vs 38.52%;rs1797912C allele:35.31%vs 43.24%;rs1175543G allele:40.61%vs 47.54%(all P<0.05)].(2)Significant linkage disequilibrium were observed between PPARγgene rs1797912 and rs1175543, rs12490265, and rs1175543 polymorphisms.(3)AGCC and GAAT were significantly different between MS and control group in Kazakhs(both P<0.05).(4) Carrying rs3856806T, rs12490265A, rs1797912C, rs1175543G was 0.267 times that of carrying rs3856806C, rs12490265G, rs1797912A, rs1175543A.Conclusions The PPARγgene rs3856806, rs12490265, rs1797912 and rs1175543 polymorphisms were associated with metabolic syndrome in Kazakhs.There were very strong linkage disequilibrium between PPARγgene rs1797912 and rs1175543, rs12490265 and rs1175543 polymorphisms, The AGCC haplotype and GAAT haplotype may serve as protective factors of metabolic syndrome.Carrying rs3856806T, rs12490265A, rs1797912C, and rs1175543G may confer lower risk of MS in Kazakhs.
8.Population pharmacokinetic modeling and evaluation of propofol from multiple centers.

In order to successfully develop the effective population pharmacokinetic model to predict the concentration of propofol administrated intravenously, the data including the concentrations across both distribution and elimination phases from five hospitals were analyzed using nonlinear mixed effect model (NONMEM). Three-compartment pharmacokinetic model was applied while the exponential model was used to describe the inter-individual variability and constant coefficient model to the intra-individual variability, accordingly. Covariate effect including the body weight on the parameter CL, V1, Q2, V2, Q3 and V3 were investigated. The performance of final model was assessed by Bootstrapping, goodness-of-fit and visual predictive checking (VPC). The context-sensitive half-times and the infusion rates necessary to maintain the concentration of 1 microg x mL(-1) were simulated to six subpopulations. The results were as follows: the typical value of CL, V1, Q2, V2, Q3 and V3 were 0.965 x (1 + 0.401 x VESS) x (BW/59)(0.578) L x min(-1), 13.4 x (AGE/45)(-0.317) L, 0.659 x (1 + GENDER x 0.385) L x min(-1), 28.8 L, 0.575 x (1 + GENDER x 0.367) x (1 - 0.369 x VESS) L x min(-1) and 196 L respectively. Coefficients of the inter-individual variability of CL, V1, Q2, V2, Q3 and V3 were 29.2%, 46.9%, 35.2%, 40.4%, 67.0% and 49.9% respectively, and the coefficients of residual variability were 24.7%, 16.1% and 22.5%, the final model indicated a positive influence of a body weight on CL, and also that a negative correlation of age with V1. Q2 and Q3 in males were higher than those in females at 38.5% and 36.7%. The CL and Q3 were 40.1% increased and 36.9% decreased in arterial samples compared to those in venous samples. The determination coefficient of observations (DV)-individual predicted value (IPRED) by the final model was 0.91 which could predict the propofol concentration fairly well. The stability and the predictive performance were accepted by Bootstrapping, the goodness-of-fit and VPC. The context-sensitive half-times and infusion rates necessary to maintain the concentration of 1 microg x mL(-1) were different obviously among the 6 sub-populations obviously. The three-compartment model with first-order elimination could describe the pharmacokinetics of propofol fairly well. The involved fixed effects are age, body weight, gender and sampling site. The simulations in 6 subpopulations were available in clinical anesthesia. The propofol anesthesia monitor care could be improved by individualization of pharmacokinetic parameter estimated from the final model.

9.Epidemiological study on severe acute respiratory syndrome in Guangdong province.

OBJECTIVESTo understand the epidemiological characteristics of severe acute respiratory syndrome (SARS) outbreaks in some areas of Guangdong province and to provide scientific basis for prevention and control measures against it.
METHODSStandardized questionnaire was used on individual cases. Data on the epidemiological characteristics as time, place, persons and aggregation status of SARS cases, development of the epidemics, were analyzed with software EPI 6.0.
RESULTSThe incidence of SARS in Guangdong province was 1.72/100,000 with case fatality rate as 3.64%. Most cases of SARS occurred between the last ten days of January and the first ten days of February with the peak (61.88% of the patients) occurred in the first ten days of February. As to the distribution of place, Pearl river delta region-economically developed with great number of mobile population-was heavily affected areas (account for 96.66% of the total patients). The majority of patients were young adults and medical staff seemed to be the most affected subgroup (account for 24.9% of the patients in total). Family and hospital aggregation of patients comprised the another two important characteristics of SARS (account for 37.1% of the total patients).
CONCLUSIONCurrent knowledge on SARS suggested that it was an air-borne infectious disease with human beings served as the source of infection. The incubation period of the disease was from 1 to 12 days with a median of 4 days. Respiratory secretions and close contact contributed to person-to-person transmission. Most cases were distributed in Pearl river delta region, an area famous for its economic development and heavy flow of mobile population.

The effects of cephradinum and ceftazidime on the metabolism of Escherichia coli (E. coli) DH5alpha was determined by microcalorimetry. The microbial activity was recorded as power-time curves through an ampoule method with a TAM Air Isothermal Microcalorimeter at 37 degrees C. The parameters such as the growth rate constant (k), inhibitory ratio (I), the maximum power output (Pm) and the time (tm) corresponding to the maximum power output were calculated. The results show that the ceftazidime has a better inhibitory effect on E. coli DH5alpha than cephradinum.
