1.Influence of anti-depression therapy on inflammatory cytokines and quality of life in patients with acute coronary syndrome complicating depression

Objective To investigate the influence of anti-depression therapy on inflammatory cytokines and quality of life(QOL) in patients with acute coronary syndrome(ACS) complicating depression.Methods Ninety-four patients with ACS complicating depression(HAMD total score ≥8) were randomly divided into the treatment group and control group,47 cases in each group.The control group was given the conventional drug therapy,pantoprazole 40 mg /d,on this basis the treatment group was given sertraline 50 mg/d.The treatment course in the two groups was 6 months.The change of HAMD score,inflammatory cytokines,QOL and incidence of adverse cardiovascular events were observed and compared between the two groups.Results The HAMD score after 3-,6-month treatment in the treatment group was significantly lower than that in the control group (P<0.05).The levels of inflammatory cytokines after treatment in the two groups were all reduced,while the IL-6 and TNF-α levels in the treatment group were lower than those in the control group (P<0.05);the scores of all dimensions and total scores of SAQ after 6-month treatment in the treatment group were significantly improved,and the anginal attack situation,treatment satisfaction degree and total score were significantly higher than those in the control group (P<0.05).During 12-month follow-up,the incidence rate of major and minor cardiovascular events,alimentary tract hemorrhage events and total events had no statistical difference between the two groups(P>0.05).Conclusion The antidepressant therapy is safe and effective for treating ACS patients,can effectively improve the depression emotion,inhibits inflammatory reaction,relieves somatic symptoms,and improves QOL.
2.Laboratory investigation of reactive plasmacytosis in patients with severe fever with throbocytopenia syndrome

Objective To investigate the immunological characteristics and clinical significance of reactive plasmacytosis in patients with severe fever with throbocytopenia syndrome (SFTS).Methods Bunyavirus-infected patients who were diagnosed with SFTS were collected from March 2015 to October 2015 in Taizhou Hospital.Morphology analysis of bone marrow and peripheral blood (PB) smear, as well as flow cytometry analysis of plasma cell immune phenotype from peripheral blood were conducted.Serum immunoglobulin levels and helper T hymphocytes (Th)1/Th2 cytokine expressions were detected.Mann-Whitney U test was used.Results PB plasma cells from all of the SFTS patients increased in varying degrees, and the phenotype of the plasma cells was CD19+CD38++CD45+CD138+, which indicated normal mature plasma cells.The ratio of PB plasma cells was >0.030 in 10/16 patients, and >0.300 in 2/16 patients.The ratios of PB plasma cells in the patients with severe and critical groups were significantly higher than that in the mild group (0.052 vs 0.016, P<0.05).Monocytoid histiocytes and hemophagocytes were observed in the BM morphology of 9 patients.Three of them were diagnosed as hemophagocytic lymphohistiocytosis (HLH).The ratio of plasma cells was more than 0.030 in the bone marrow of 8 patients.The serum levels of interlewkin (IL)-6, IL-10 and interferon (IFN)-γ in acute phase were significantly elevated with the median level of 49.75 ng/L, 26.98 ng/L (reference value 2.6 to 4.9 ng/L) and 17.57 ng/L, respectively.While the levels of IL-2, IL-4 and twmor necrosis fautor(TNF)-α were not significantly changed.The serum IL-6 and IL-10 levels in the patients with severe and critical groups were both significantly higher than those in the mild group (IL-6: 132.36 vs 22.81 ng/L;IL-10: 75.28 vs 6.33 ng/L, both P<0.05), but the difference of IFN-γ level was not significant (P>0.05).The serum IgG, IgA and IgM levels did not increase in acute stage, with the median of 11.6 g/L, 2.56 g/L and 1.60 g/L (reference value 0.46 to 3.04 g/L), respectively.Conclusion The patients with SFTS show excessive humoral and cellular immunity, and the severity of disease is positively correlated with the ratio of peripheral plasma cells and the levels of cytokines IL-6 and IL-10.
3.Plasticity of the visual cortex area 17 after form deprivation in tree shrews

Objective To preliminarily explore the feasibility of tree shrew as a new kind of animal model in research of amblyopia,to discuss the primary visual cortex plasticity mechanism of form deprivation in tree shrew,and to provide a theoretical basis for further understanding the mechanism of amblyopia formation and recovery.Methods Sixty 30-days old tree shrews were divided into five groups,12 in each group:the group A had the right eye sutured for 1 month;the group B had the right eye sutured for 2 months;the group C had the left eye sutured for 1 month and then opened and the righ eye was sutured for 1 month,in other words,the group C was performed by alternating suture;the tree shrews of control group 1(D1) were in the same age as the the group A,but fed in normal breedingenvironment;the tree shrews of control group 2(D2) were at the same age of groups B and C,but fed with a normal diet.Samples of the visual cortex were taken after the completion of modeling,and were processed to observe the histology and ultrastructure of the visual cortex,the neuron apoptosis,and the c-fos protein expression in the tree shrews of different groups.Results Damages to different degrees were found by histological and electron microscopic examination of the visual cortex in each experimental group,and they were more obvious in the group sutured for 2 months.TUNEL staining showed that there were no significant differences between the apoptosis in the experimental and control groups.The expression of c-fos mRNA and protein in the experimental groups was decreased,and it was the lowest in the group sutured for 2 months.There was a small increase in the c-fos expression after the alternate suture,and no significant difference of c-fos expression was found in the control groups.Conclusions Different degrees of deprivation amblyopia lead to different histopathological changes.There is a plasticity in the neurons affected by amblyopia.Tree shrew can be used as an ideal animal model for the studies of form deprivation amblyopia.
4.Effect of metal ions on monocyte-macrophage cells viability and RANK expression

BACKGROUND: Similar to other prosthesis, metal-metal prosthesis would produce plenty of wear particles and metal ions, mainly presented as cobalt (Co~(2+)) and chromium (Cr~(3+)), which can lead to osteolysis, eventually, result in aseptic loosening. OBJECTIVE: To observe the effect of Co~(2+) and Cr~(3+) ions on the cells viability and expression of RANK in rats m0nocyte-macrophage cells (RAW264.7) in vitro. METHODS: Monocyte-macrophage cells (RAW264.7) were cultured in vitro, and then the cells were exposed to Co~(2+) and Cr~(3+) ions. The cell viability was assured by MTT test and the level of RANK Mrna was detected by semi-quantitative RT-PCR at different times. RESULTS AND CONCLUSION: Compared to the control group, MTT test demonstrated that Co~(2+) and Cr~(3+)ions could decrease the cell activity of monocyte-macrophage cells obviously. When the cells were exposed to Co~(2+) Cr~(3+) ions, compared to the control group, the Mrna expression of RANK of the metal ions group was increased at 12 hours (P < 0.05), reached its peak level at 24 hours (P < 0.05), and decreased at 48 hours than that of 24 hours (P < 0.05). The results revealed that metal ions have a cytotoxic effect on monocyte-macrophage cells, stimulate the expression of RANK, and have the potential of facilitating monocyte-macrophages cells transform into osteoclast-like cells.
5.Twin pregnancy outcomes of maternal hepatitis B virus carrier state

To investigate the impact of maternal hepatitis B virus (HBV) carrier state on twin pregnancy outcomes. Methods From January 2004 to December 2012, 569 women with twin pregnancy were hospitalized in Nanjing Drum Tower Hospital. Thirty-two women positive for hepatitis B surface antigen (HBsAg)(negative for hepatitis B e antigen, with normal liver function before conception)were included in the HBsAg-positive group and the other 537 women were included in the HBsAg-negative group. The pregnancy outcomes of the two groups were compared by t test and Chi-square test. The risk factors for neonatal birth weight were analyzed by multivariate regression analysis. Results Compared with the HBsAg-negative group, the HBsAg-positive women had a higher incidence of abnormal liver function (alanine aminotransferase≥50 U/L) [18.8% (6/32) vs 5.8% (31/537), χ2=6.367, P=0.012]. The incidence of gestational diabetes mellitus was 21.9% (7/32) in the HBsAg-positive group, which was higher than in the HBsAg-negative group [11.6% (62/537)], although the difference was not significant (χ2=2.132, P=0.144). The incidences of intrahepatic cholestasis of pregnancy,hypertensive disorders complicating pregnancy, premature rupture of membranes, placenta previa, fetal distress, postpartum hemorrhage, preterm birth, caesarean section, umbilical cord around the neck, meconium-staining amniotic fluid and neonatal asphyxia were no statistical difference between two groups (all P<0.05, respectively). Multivariate regression analysis showed that gestational diabetes mellitus (β=67.869, 95%CI: 0.494-135.244, P=0.048), maternal age (β=6.592, 95%CI: 2.020-11.880, P=0.006) and gestational age (β=164.069, 95%CI:154.426-173.712, P<0.01) were risk factors for neonatal birth weight, but not the maternal HBsAg-positive status (β=78.864, 95%CI: -16.950-174.678, P=0.107). Conclusion Twin pregnancy and HBV carrier state increase the risk of abnormal liver function, but not other adverse pregnancy outcomes and newborn diseases.
6.A clinical control study of error monitoring in the patients with schizophrenia

Objective To investigate the error monitoring function damages on the patients with schizo?phrenia ( SCH) . Methods A total of 32 patients with schizophrenia were compared with matched 34 health con?trols ( HC) on the error monitoring tasks which were compiled by E?Prime. Results The comparison between SCH group ((713.22±174.52)ms,( 491.14±170.29) ms,( 1060.31±130.84) ms,(8.28±12.55)time,( 8.00± 7.53)time respectively) and HC group ((560.73±156.94) ms,(395.62±188.03) ms,(989.85±104.33) ms, (2.97±4.13) times,(3.12±6.50) times) on the reaction time of choice,assessment,incongruent condition,the numbers of uncertain and the numbers of dropout were significant ( t=-3.737, P=0.000;t=-2.159, P=0.035;t=-2.426, P=0.018;t=-2.282, P=0.022;t=-2.824, P=0.006) . The SCH group and HC group did not signifi?cantly difference in Full Correct((124.72±23.74)/(131.74±21.96)times),Full Error((15.69±17.64)/(13.35± 18.63)times),Part Correct((6.83±10.40)/(4.21±7.03)times),Part Error((2.91±10.91)/(0.62±1.10)times) and Accuracy((0.831±0.161)/(0.874±0.159))(P>0.05).There was no significantly correlation among the course of disease,HAMA,HAMD and the error monitoring. Conclusion These results demonstrate that the error monitoring function damages on the patient with SCH may be involved in the dysfunction of anterior cingulate cortex.
7.Thyroid function of women of reproductive age in iodine sufficiency and iodine deficiency

Objective To study the effects of different level of iodine nutrition on the thyroid function in women of reproductive age. Methods A total of 100 (50 from each) women of reproductive age but not pregnant were collected from iodine deficient and adequate areas. The questionnaire was obtained individually with items concerning personal history of thyroid diseases, goiters and category of edible salt and drinking water based on the project design. The household salt and drinking water were collected for measuring iodine content, and blood samples were obtained for TSH, FT4 and FT3 testing. Results The coverage of iodized salt and the median level of urinary iodine in iodine deficient women(72.0% and 95.5 μg/L) were obviously lower than that in iodine sufficient women(100.0% and 167.4 μg/L, χ2=16.28, U = 632.00, P < 0.01). Median level of serum TSH in iodine deficient women (2.56 m U/L) appeared in an increasing tendency compared to the iodine sufficient women (1.88 mU/L), but there was no significance (U=990.50, P > 0.05). Serum FT4 mean level in iodine deficient women [(14.7±2.0) pmol/L]was lower than that in iodine sufficient women[(17.0±3.8)pmoI/L, t=3.76, P<0.01]. There was no difference in serum FT3 between two group women[(5.1±1.4), (4.8±0.5)pmoI/L, t = 1.59, P > 0.05]; but FT3/FT4 ratio in iodine deficient women(0.33±0.04) was markedly higher than that in the iodine sufficient women(0.30±0.04, t=3.13, P<0.01). The percentage of thyroid dysfunction in iodine deficient women[20.0% (10/50)]was higher compared with the iodine sufficient women[8.0%(4/50)], but without significance(χ2=2.99, P>0.05). Conclusions Iodine deficiency is a primary cause leading to hypothyroid in women of reproductive age. Long term of iodized salt usage is an efficient way to correct iodine deficiency.
8.Meroterpenoids and isoberkedienolactone from endophytic fungus Penicillium sp. associated with Dysosma versipellis.

Seven meroterpenoids and five small-molecular precursors were isolated from Penicillium sp., an endophytic fungus from Dysosma versipellis. The structures of new compounds, 11beta-acetoxyisoaustinone (1) and isoberkedienolactone (2) were elucidated based on analysis of the spectral data, and the absolute configuration of 2 was established by TDDFT ECD calculation with satisfactory match to its experimental ECD data. Meroterpenoids originated tetraketide and pentaketide precursors, resepectively, were found to be simultaneously produced in specific fungus of Penicillium species. These compounds showed weak cytotoxicity in vitro against HCT-116, HepG2, BGC-823, NCI-H1650, and A2780 cell lines with IC 50 > 10 micromol x L(-1).

9.Roles of NRF2 regulating gamma-glutamylcysteine synthetase in lung of rats with chronic obstructive pulmonary disease.

AIMTo investigate the expression and relationship of gamma-glutamylcysteine synthetase (gamma-GCS) and NF-E2-related factor2 (NRR2) in lung of rat with chronic obstructive pulmonary disease (COPD)in order to elucidate the possible important role of gamma-GCS and NRF2 in pathogenesis of COPD.
METHODSThe rat COPD model was established by intratracheal instillation of lipopolysaccharide twice and exposed to cigarette smoke daily. The gamma-GCS activity was measured, the expression of gamma-GCS mRNA in lung was examined by in situ hybridization (ISH) and reverse transcription-polymerase chain reaction (RT-PCR), the protein expressions of NRF2, gamma-GCS in lung were detected by immunohistochemical (IH) and Western blot respectively.
RESULTSThe gamma-GCS activity was higher in COPD group than that in control group. The expressions of gamma-GCS mRNA in COPD group was stronger than those in control group. ISH showed that the gamma-GCS mRNA was expressed in alveolar epithelium and bronchial smooth muscle cell in COPD. The protein expressions of NRF2, gamma-GCS were significantly higher than the control group. IH showed that NRF2, gamma-GCS proteins were expressed in alveolar and bronchial epithelium in the COPD group. There was a positive correlation between NRF2 and gamma-GCS and gamma-GCS mRNA.
CONCLUSIONNRF2 may play an important role in the mechanism of COPD oxidative stress vis up-regulation of gamma-GCS.
