0.05).The mRNA and protein expressions of survivin in cells transfected by 100-200nmol/L siRNA were significantly lower than those of untransfected cells(P0.05).Conclusion Silencing survivin gene by the RNAi technique can decrease effectively the expressions of survivin gene and protein,suppress significantly the growth and proliferation of A549 cells,and induce apoptosis of A549 cells.The inhibitory effect of RNAi is characterized by its specificity,high efficiency and durability.This may lay an experimental foundation for further study of gene therapy in lung cancer.

A simple, rapid sensitive and accurate ultra performance liquid chromatography tandem mass spectrometric ( UPLC-MS/MS ) method was developed for the simultaneous determination of 2 , 4 , 6-trinitrophenol, trinitroresorcinate, trinitrophloroglucinol residues in soil. The sample was pretreated by using the modified QuEChERS ( quick, easy, cheap, effective, rigged, and safe ) method that completed the extraction and clean-up steps in one procedure. In this none buffer QuEChERS method, samples were extracted with 1% formic acid ﹢ acetonitrile, cleaned up with primary graphitized carbon black ( GCB) and C18 sorbent, then centrifuged and filtrated before detection. The pretreatment method was simple, rapid and effective and can meet the detection requirements. The UPLC-MS/MS method was performed on Waters Accucore PFP (150 mm× 2. 1 mm, 2. 6 μm) and the column temperature was 30 ℃, the gradient elution with acetonitrile and ammonium acetate as the mobile phase and the flow rate was 0. 3 mL/min. The negative electrospray ionization ( ESI-) source under the multiple reaction monitoring ( MRM ) mode and external standard method were used for quantification. The results showed that the correlation coefficients up to 0. 9942 were obtained across a concentration range of 0. 005-5. 0 mg/L. The limits of detection ( LOD) ranged from 0 . 002 to 0 . 005 mg/kg ( S/N=3 ) . The method was validated with soil samples spiked at three fortification levels (0. 01, 0. 1 and 1 mg/kg) and recoveries were in the range of 79. 3%-94. 8% with relative standard deviations (RSD) of 3. 1%-6. 6%.

AIM: To explore the effect of lacrimal duct laser with lacrimal drainage tubes and stents implantation for complexity dacryagogatresia.
METHODS: There were 65 patients ( 82 eyes ) with compound tears nasolacrimal duct obstruction who received lacrimal drainage tubes and stents implantation after laser. The lacrimal duct stents were removed through nasal cavity after 1mo. Lacrimal drainage tubes were removed after 3-6mo. Follow-up periods were 6mo to 1a.
RESULTS: In the 65 patients (82 eyes), 71 eyes were cured, 5 eyes improved, the efficient rate was 93%; there were 6 eyes (7%) with epiphora.
CONCLUSION: Lacrimal duct laser with lacrimal drainage tubes and stents implantation was efficient for complexity dacryagogatresia.

Objective To explore the value of ectopic gastic mucosa imaging in diagnosis of Meckel′s diverticulum.Methods Twenty-five cases suspected of gastrointestinal bleeding underwent conventional ectopic gastic mucosa imaging with 99mT_cO_4-.The diagnostic criteria Meckel′s diverticulum was that abnormal radio active accumulation in the abdomen observed.Results Three cases of ectopic gastic mucosa imaging was positive,and the diagnosis was all confirmed by surgery and pathology examination.The rest 22 cases were caused by other reasons confirmed by affirmative examinations.Conclusions Ectopic gastic mucosa imaging is easy to conduct,safe with no injury to the patients.It can be the first choice of examination in the diagnosis of gastrointestinal bleeding in children.

Objective To observe and compare the efficacy and safety between propofol-fentanyl and propo- fol-ketamine for anesthesia of artificial abortion.Methods 450 ASA physical statusⅠandⅡpatients scheduled for elective artificial abortion were randomized into three groups:P group were given intravenous normal saline 2ml be- fore propofol(n=150) ;PF group were given intravenous fentanyl 1?g/kg then propofol(n=150);PK group were given intravenous ketamine 0.4mg/kg then propofol(n=150).Total dose of propofol,extinction time of lash reflex, heart rate(HR),mean arterial pressure(MAP),pulse oxygen saturation(SpO_2),time to open the eye when called, time of full recovery(answering correctly and walking unaided),discomfort,awareness during operation,self-feeling were recorded.Analysis of date was with SPSS 12.0.P＜0.05 was considered significant.Results No episodes of nausea,vomiting,cardio-cerebral syndrome or awareness were noted in all patients;Total dose of propofol,and extinc- tion time of lash reflex in P group were significantly more than those in PK or PF group(P＜0.05),but there was no difference between PK and PF group;MAP,minimum HR and SpO_2 during operation were obviously lower than preoperation or postoperation in those in P or PF group,minimum HR and SpO_2 during operation in P group were significantly lower than in PF or PK group;Minimum HR and SpO_2 during operation in PK group were lower than preoperation,but its decrease extent was less than that in P or PF group(P＜0.05);Time to eye opening in PF or PK group was significantly less than that in P group(P＜0.05);Time to full recovery were not different in three groups;Incidence of pain on injection in PF group(10%)or PK group(8%)was obviously lower than P group (80%);Incidence of coughing and moving during operation in PF group(8%)or PK group(6%)was obviously less than P group(30%);No difference were noted with respect to degree of comfort,time of full recovery,bleeding vol- ume during operation,time to operation.Conclusion Painless artificial abortion can be preformed effectively and safely with propofol-fentanyl or propofol-ketamine,propofol-ketamine especially fits for painless artificial abortion compared with propofol-fantanyl.

Objective To evaluate the effects of vitamin C combined with low dose of gabapentin on neuropathic pain (NP) in rats.Methods Fifty male Sprague-Dawley rats,weighing 225-275 g,aged 6-7 weeks,were randomly assigned into 5 groups (n =10 each):sham operation group (group Sham),NP group,vitamin C group (group VitC),gabapentin group (group Gap),and vitamin C combined with gabapentin group (VitC + Gap group).NP was induced by chronic constrictive injury in the rats anesthetized with pentobarbital sodium.The sciatic nerve was exposed and 4 silk ligatures were placed on the sciatic nerve at 1 mm intervals.In VitC,Gap and VitC+ Gap groups,vitamin C 500 mg/kg,gabapentin 30 mg/kg,and vitamin C 500 mg/kg + gabapentin 30 mg/kg diluted in normal saline 1 ml were injected intraperitoneally,respectively,twice a day for 7 consecutive days starting from 7 days after NP.Normal saline 1 ml was given in Sham and NP groups.Before NP and at 3,7,and 14 days after NP,the paw withdrawal threshold to mechanical stimuli (PWMT) and paw withdrawal latency to thermal stimuli (PWTL) were measured.At 14 days after NP,the blood samples were drawn from the carotid arteries.The rats were then sacrificed and the spinal cord was removed for determination of concentrations of malondialdehyde (MDA) in serum (by using Thiobarbituric acid method) and Slc23a2 mRNA expression (by RTPCR).Results Compared with Sham group,PWMT and PWTL in left paws were significantly decreased in NP,VitC,Gap ard VitC + Gap groups,the concentrations of MDA in serum were increased and Slc23a2 mRNA expression was down-regulated in NP group,Slc23a2 mRNA expression was down-regulated in VitC and Gap groups,and no significant change in Slc23a2 mRNA expression was found in VitC + Gap group.Compared with NP group,PWMT and PWTL in left paws were significantly increased,the concentrations of MDA in serum were decreased,and Slc23a2 mRNA expression was up-regulated in VitC + Gap group,the concentrations of MDA in serum were decreased in VitC group,and no significant changes were found in PWMT and PWTL in left paws in VitC and Gap groups.There were no significant differences in PWMT and PWTL in fight paws at each time point between the five groups.Conclusion Vitamin C combined with low dose of gabapentin can relieve NP effectively,and up-regulation of Slc23a2 mRNA expression,increased trafficking of vitamin C to neurons and inhibition of oxidative stress may be involved in the mechanism.

Anterior approach refers to a method of hepatectomy which is first to resect the hepatic parenchyma and then to free the liver; hanging maneuver refers to placing a tape before the inferior vena cava for hanging the liver during hepatectomy.In October 2011,anatomical trisectionectomy was performed on a 54-year-old male patient with large hepatocellular carcinoma in the left medical lobe and right lobe with anterior approach and hanging maneuver.The diameter of the tumor was 16 cm,and was in the ⅢA/T3NOM0 stage.The indocyanine green retention at fifteen minutes was 5.4％,and the ratio of hepatic left lateral lobe volume over the standard total liver volume was 44％.The left bile duct was slightly dilated because of the compress of the tumor.The operation started with the isolation and dissection of the inflow vessels,including the right hepatic artery,the right portal vein,the middle hepatic artery,the portal vein branches of left internal lobe.The hepatic parenchyma transection was performed along the fight side of the falciform ligament.A tape was passed between the anterior surface of inferior vena cava and liver,and the liver was suspended during the transection.The left bile duct was cut at the right side of round ligament,and then the middle hepatic vein and the right hepatic vein were resected.The ligaments around the liver were dissected and the right hepatic lobe was removed.Finally,the end-toend anastomosis between the left hepatic duct and the common hepatic duct was performed.The operation lasted for 4 hours and the intra-operative blood loss was 350 mL.The patient was recovered well.At the end of 4 months after surgery,magnetic resonance cholangiopancreatography showed that the anastomosis of the bile duct was unobstructed,and there was no recurrence of tumor inside the liver.

BACKGROUND:Systemic lupus erythematosus (SLE) is classified into four types, and the major treatment is to tonify kidney and nourishyin, clear blood stasis and toxin by the traditional Chinese medicine (TCM). Even though, there are stil many patients with poor efficacy. Mesenchymal stem cels have the capacity of multiple differentiation, hematopoietic support and immune regulation, thus having been used for the treatment of refractory, recurrent SLE and achieving good effects. OBJECTIVE:To investigate the therapeutic effect of umbilical cord-derived mesenchymal stem cel transplantation on SLE patients with different patterns of syndromes. METHODS: Twenty-one SLE patients were clustered to four syndrome types of TCM, including heat-toxin,yin deficiency of liver and kidney,yang deficiency of spleen and kidney, andqi stagnation and blood stasis. The changes in clinical and laboratory indicators were analyzed statisticaly before and after cel transplantation. RESULTS AND CONCLUSION:The level of 24-hour proteinuria and SLE disease activity index scores in SLE patients were significantly decreased at 1, 3, 6 months after cel transplantation (P < 0.01). Umbilical cord-derived mesenchymal stem cel transplantation could significantly reduce the 24-hour proteinuria in SLE patients withyin deficiency of liver and kidney at 1, 3 and 6 months (P < 0.01), while slightly reduce the 24-hour proteinuria in SLE patients with heat-toxin andqi stagnation and blood stasis at 1, 3 months (P < 0.05) as wel as in SLE patients withyang deficiency of spleen and kidney at 1 month (P < 0.05). Additionaly, umbilical cord-derived mesenchymal stem cel transplantation could increase the serum albumin levels in al the SLE patients (P < 0.01), although the changes in patients with heat-toxin were moderate (P < 0.05). Al the SLE patients of four types had an increasing trend of their platelet counting after cel transplantation, but there was no statistical difference before and after cel transplantation. Taken together, umbilical cord-derived mesenchymal stem cel transplantation is effective for treatment of SLE, but has different therapeutic efficacy on SLE patients with different syndrome types of TCM.

Objective To evaluate the value and the significance of 16-slice spiral computed tomography angiography(MSCTA) in identifying the vertebral artery course variation.Methods MSCTA data in 22 patients with anomalous path of vertebral artery were retrospectively analyzed,13 of them had undergone DSA examinations,and 5 of them had also undergone MRI examinations.Multi-planar reconstruction(MPR),curved MPR,thin-slab maximum intensity projection(MIP),volume rendering(VR) or slab VR(cut-plane VR) were used to assess the anomalous path of the vertebral artery and its relationship with the adjacent large vessels.Virtual endoscopy(VE) was used to evaluate the orifice of the vertebral arteries and its relationship with the aortic arch and the ostia of the adjacent large vessels.Different views of vertebral artery were analyzed on DSA.MSCTA and DSA images were analyzed in blinded fashion for the accuracy of anomalous vertebral artery path and origin detection.Results were compared in a secondary consensus evaluation.Results The anomalous path of the vertebral artery in all 22 patients were correctly displayed on MSCTA,7 of them had anomalous origin of vertebral artery,the anomalous of ostia showed clearly.The vertebral arteriae with course variation were all going up at the front of the foramen transversarium of C6,then bended into the C5,C4 or C3 foramen transversarium.DSA could only be detected the anomalous origin in 13 patients undergone DAS.5 patients indirectly showed having course variation of vertebral artery by MRI.Conclusion MSCTA is superior to DSA in showing the anomalous path and orifice of vertebral artery,it should be considered as a non-invasive imaging tool for these diseases.
