1.Pharmacokinetic Studies of Sinomenine by Blood Microdialysis Technique

[Objective] To explore the feasibility and advantages of blood microdialysis technique for the pharmacokinetic study of sinomenine. [Methods] In this study, rats served as the experimental subjects and sinomenine was administered to the rats by intravenous injection. With microdialysis technique for blood sampling, the blood concentration of sinomenine was determined by high performance liquid chromatography (HPLC) and the associated pharmacokinetic parameters were analyzed by 3p97 program. [Results] The pharmacokinetics process of sinomenine in rats presented as the two-compartment model. The half-life of distribution phase was 10.98min and that of elimination phase was 44.71 min. [Conclusion] It is feasible to study pharmacokinetic parameters of sinomenine by using blood microdialysis technique. In comparison with the traditional technique, blood microdialysis technique is superior to the examination of drug blood concentration .

3.Analysis on Thoracic Endovascular Aortic Repair and Endoleak of“Chimney”Technique for Treating the Patients With Thoracic Aortic Lesions

Objective: To investigate thoracic endovascular aortic repair (TEVAR) and “Chimney” technique for treating the involved left common carotid artery (LCCA) or left subclavian artery (LSA) in Standford B patients with aortic lesion and in-sufficient proximal anchoring area. Meanwhile, to explore the relationship between endoleaking condition and the location of lesion with the prognosis. Methods: A total of 32 relevant patients treated by TEVAR + “Chimney” technique in our hospital from 2011-09 to 2015-07 were retrospectively analyzed. Immediate post-operative image development of LCCA or LSA was observed; cerebral complications, severe upper limb ischemic symptoms and endoleaking conditions were recorded. The patients were followed-up for (3-46) months. Results: Thoracic aortic stent-graft placement was successfully carried out in all 32 patients. Immediate post-operative image development of LCCAor LSAwas favorable, no cerebral complications and no severe upper limb ischemic symptoms were observed. There were 7 patients suffered from endoleak at aortic arch including 6 with the lesion located at the greater curvature side and 1 at the small curvature side. During follow-up period, aortic stent-graft remained in a stable condition and the blood flow in “Chimney” stent was unobstructed. Endoleking condition was gradually reduced and disappeared in 5 patients, it was persisted in 2 patients. Conclusion: “Chimney” technique may prolong anchoring area and keep LCCA or LSA unobstructed, therefore expand the indication of TEVAR in a mini-invasive, safe and effective way. When aortic lesion located at the greater curvature side, the endoleaking probability could be increased.
4.Effect and Safety of Pre-operative Loading Ticagrelor on Myocardium Reperfusion in Patients With Acute ST-elevation Myocardial Infarction During Primary Percutaneous Coronary Intervention

Objective: To explore the effect and safety of pre-operative loading ticagrelor on myocardium reperfusion in patients with acute ST-elevation myocardial infarction (STEMI) during primary percutaneous coronary intervention (PCI). Methods: A total of 105 acute STEMI patients received PCI within 12-hour of onset were studied and they were divided into 2 groups: Ticagrelor group, the patients received pre-operative oral chewing ticagrelor 180 mg,n=58 and Clopidogrel group, the patients received pre-operative oral chewing clopidogrel 600 mg,n=47. The baseline feathers, operative TIMI and corrected TIMI frame count (CTFC), TIMI myocardial perfusion grade (TMPG), no-relfow/slow lfow conditions were compared between 2 groups. Results: The baseline feathers and pre-operative TIMI were similar between 2 groups, bothP>0.05. Compared with Clopidogrel group, Ticagrelor group showed increased ratios of TIMI 3 lfow (94.8% vs 80.9%) and TMPG (89.7% vs 72.3%), bothP<0.05, improved CTFC (20.0 ± 4.9) vs (31.8 ± 3.9),P<0.001; decreased rates of no-relfow/slow lfow,P=0.016 and less MACE occurrence,P<0.05; while the post-operative bleeding events were similar between 2 groups,P>0.05. Conclusion: Prior PCI loading ticagrelor may reduce no-relfow/slow lfow incidence, improve myocardium reperfusion safely and therefore, decrease MACE occurrence in acute STEMI patients.
5.Detection of levels of CD19+ and CD20+ of B-lymphocytes and expression of natural killer cells in the peripheral blood of epileptic children and their significances

Objective To detect the expressions of CD19 and CD20 of B-lymphocytes (CD19+B, CD20+B) and natural killer (NK) cells in the peripheral blood of epileptic children and explore their significances. Methods Four hundred and fifty-eight epileptic children, admitted to our hospital from January 2008 to December 2010, were chosen as patient group; another 52 healthy subjects were chosen as controls. The expressions of CD19+B and CD20+B and NK cells were detected by flow cytometry; their results were compared. Ninety-two epileptic children were treated with intravenous immunoglobulin (IVIG) and the effect of IVIG therapy was studied. Results The CD19+B level ([22.35±6.54]%) and CD20+B level ([21.50±8.41]%) in the epileptic children were obviously higher than those in the healthy controls ([16.86±4.02)%,[16.13±4.19]%, P<0.05); and the level of NK cells ([9.1 1±4.90]%) in the epileptic children was significantly decreased as compared with that in the healthy controls ([14.72±4.15]%, P<0.05). The CD19+B level ([18.26±5.03]%) and CD20+B level ([16.74±5.12]%) 6months after MG treatment were decreased significantly as compared with those before treatment ([22.74±6.25]%,[21.61±8.03]%, P<0.05]; while the level of NK cells ([14.65±4.58]%) 6 months after IVIG treatment was increased significantly as compared with that before treatment ([9.07±4.76]%,P<0.05). Among the 92 patients treated with IVIG, 70 enjoyed good results and 22 had non-effective resutls; The changesofCD19+Blevel ([7.99±5.34]%) and CD20+B ([8.21±5.21]%) before and after treatment in effectively treated patients by IVIG were significantly different as compared with those in ineffectively treated patients by MG ([3.78±2.76]%,[3.66±2.48]%, P<0.05); however, the changes of level of NK cells before and after treatment showed no significant difference between effectively treated patients and ineffectively treated patients ([5.28±4.55]%,[4.53%±4.43]%, P>0.05). Conclusion Dysfunctions of B-lymphocytes and NK cells exist in epileptic children, and MG treatment shows good effect on immune dysfunction of them. The levels of CD19-B and CD20-B can be used as monitoring indicators in the IVIG treatment of epileptic children.
6.Induction of transforming growth facor-β2 to the expression of lysyl oxidases in human Tenon capsule fibroblasts in vitro

Background Researches showed that transforming growth factor-β2 (TGF-β2) promotes the activity of human Tenon capsular fibroblasts (TFs),which plays a role in the scarring of filtering blebs after antiglaucoma surgery.However,its mechanism is not fully clear.Lysyl oxidases (LOXs) are important extracellular matrix proteases which can catalyze the cross-linking of collagen and elastin.Investigating the impact of TGF-β2 on the expression of LOXs has a great significance for the understanding of the pathogenesis of filtering bleb scarring and its prevention.Objective This study was to investigate the effect of TGF-β2 on the expression of LOXs in cultured human TFs.Methods The TFs at 4-8 generations were divided into normal control group and different concentrations of TGF-β2 treated-groups,and 100,200,400,800 μ1 of TGF-β2 with the final concentration of 2,4,8 and 16 ng/ml was added into the medium to treat human TFs respectively for 24 hours.The LOXs in the cells were detected by Western blot to determine the optimal dose of TGF-β2.The 4 ng/ml TGF-β2(200 μ1) was used to treat human TFs for 6,12,24 and 48 hours respectively,and the change of LOXs expression in the cells over time was assayed by Western blot.The expression and distribution of LOX protein in the normal cells and TGF-β2-treated cells was detected by using immunofluorescence technique.This study was approved by Daping Hospital of Third Military Medical University Ethic Commission.The guardians of the patients who offered the specimen knew the purpose of the study and signed informed consent.Results Western blot assay showed that the expressions of LOX,LOXL1,LOXL2,LOXL3 and LOXL4 in the cells were gradually elevated from the normal control group and 2,4,8,16 ng/ml TGF-β2-treated groups,showing significant differences among the groups (F =37.338,13.438,31.067,11.767,15.167,all at P<0.01).The expression of LOXL2 protein in the cells was 0.68±0.07,1.09±0.10,1.32±0.07,1.50± 0.06 and 1.89±0.12 in the normal control group and 6-,12-,24-and 48-hour groups respectively after 4 ng/ml TGF-β2 treatment,with a significant increase over time (F =82.832,P=0.000).The expression of LOX was weak in the normal cultured TFs,while the fluorescence intensity of LOX expression was evidently enhanced in the cytoplasm of the cells in the TGF-β2-treated group.Conclusions TGF-β2 upregulates the expressions of LOXs in human TFs in a dose-and time-dependent manner,which probably offers a basis for the further study on the prevention of filtering bleb scarring after glaucoma surgery.
7.Research on Construction Methods of Traditional Chinese Medicine Health Maintenance

This article was aimed to explore the method of traditional Chinese medicine (TCM) health maintenance knowledge base building. TCM health maintenance knowledge classification method was studied under the guidance of TCM health maintenance classification system to construct a special literature database to store TCM health maintenance knowledge. Based on TCM literature resources, TCM health maintenance knowledge service platform was designed and developed for the static display and dynamic query support of TCM health maintenance knowledge. The content of knowledge base will be accessed to the whole society. The results showed that the construction method can be a system arrangement and modern interpretation of TCM health maintenance knowledge, in order to support TCM health maintenance knowledge base and its update and service system. It was concluded that under the guidance of TCM theories, the construction of TCM health maintenance knowledge base will realize the health maintenance knowledge-sharing and service requirement of TCM.
8.Optimization of Inclusion Compound for Osthol with Hydroxypropyl-β-Cyclodextrin by Orthogonal and Rotational Combination Design

Objective To study the process of hydroxypropyl-β-cyclodextrin (HP-β-CD) inclusion techniques for osthol. Methods The inclusion complex of osthol and HP-β-CD was prepared by unsaturated water solution and freeze-drying technique. Inclusion techniques were selected by screening on quadratic orthogonal rotation combination design method, and the entrapment efficiency was identified by HPLC. Results The optimal technical conditions were as follows:the ratio of HP-β-CD and drug was 4.5∶1;temperature for electric mixer was 35 ℃;the stirring time for thermostatic waterbath was 210 min. Conclusion This method is reasonable and it may have a prosperous future of development and application.
9.Simultaneous determination of five constituents in Baishao Formula Granules by HPLC

AIM To establish an HPLC method for the simultaneous content determination of five constituents in Baishao Formula Granules (Paeoniae Radix Alba).METHODS The analysis of aqueous extract of this drug was performed on a 30 ℃ thermostatic Phenomenex C18 column (4.6 mm × 250 mm,5 μm),with the mobile phase comprising of acetonitrile-0.05% phosphoric acid flowing at 1.0 mL/min in a gradient elution manner,and the detection wavelength was set at 230 nm.RESULTS Paeoniflorin,albiflorin,paeoniflorin sulfonate,gallic acid and benzoic acid showed good linear relationships within the ranges of 0.020-0.639 mg/mL (r =0.999 8),0.005-0.172 mg/mL (r =0.999 9),0.020-0.652 mg/mL (r =1.000 0),0.003-0.097 mg/mL (r =0.999 8),0.002-0.058 mg/mL (r =0.999 7),whose average recoveries were 99.1%,98.3%,98.6%,98.1% and 99.5% with the RSDs of 1.86%,1.37%,1.69%,1.46% and 2.26%,respectively.The contents of various constituents in twenty-seven batches of samples demonstrated obvious differences.CONCLUSION We should pay attention to Paeoniae Radix Alba in Baishao Formula Granules due to its unstable quality.
10.Neural stem cell transplantation for central nervous system diseases via the cerebrospinal fluid

BACKGROUND:Currently, neural stem celltransplantation can be performed through three main approaches:local lesions, blood circulation, and cerebrospinal fluid.
OBJECTIVE:To review the transplantation of neural stem cells or neural precursor cells via the cerebrospinal fluid in the treatment of central nervous system diseases.
METHODS:A computer-based search of PubMed and CHKD databases was performed to retrieve articles concerning transplantation of neural stem cells via the cerebrospinal fluid, and its application and therapeutic mechanism in the treatment of central nervous system diseases in both animal experiment and clinic study published from 2000 to 2009.
RESULTS AND CONCLUSION:It is suitable for neural stem cellsurvival, proliferation, and differentiation in the cerebrospinal fluid. Transplantation of neural stem cells via the cerebrospinal fluid is effective and feasible to treat central nervous system diseases. However, some problems have not been solved, such as the source of neural stem cells, the optimal time window and celldose, the safety and the long-term effect. Further studies are needed to pave the way for the intrathecal injection of neural stem cells in the treatment of central nervous system diseases.
