2.Analysis on the long-term effects of modified double endobutton technique in the treatment of Tossy type III acromioclavicular joint dislocations.

OBJECTIVETo investigate the long-term clinical effects of modified double Endobutton technique for the treatment of acromioclavicular joint dislocations of Tossy type III.
METHODSA retrospective study was done in 42 patients with acromioclavicular joint dislocations of Tossy type III treated with modified double Endobutton technique from December 2008 to December 2010. There were 24 males and 18 females, ranging in age from 21 to 56 years old (averaged, 32.5 years old). All the patients were treated with open reduction, coracoclavicular ligament reconstruction using double Endobutton technique, and repair of acromioclavicular ligament. The Karlsson system was used to evaluate therapeutic effects. The distance from coracoid to clavicle was measured to evaluate reduction loss.
RESULTSAll the patients were followed up, and the duration ranged from 2.0 to 3.2 years (averaged,2.4 years). According to Karlsson system, 32 patients got an A degree and 10 patients got a B degree at three months post-operatively; 26 patients got an A degree and 16 patients got a B degree at the latest follow-up; 6 patients got an A degree at 3 months after operation lowered to B degree at the latest follow-up. The coracoid-clavicle distance increased from (26.91 +/- 0.91) mm at 3 months after operation to (27.41 +/- 1.10) mm at the latest follow-up. Te patients treated with over-reduction during operation or with heavy physical labour work after operation had obvious widened coracoid-clavicle distance. Bone absorption was found around the plate in most cases, mainly in the clavicular side.
CONCLUSIONTreatment for acromioclavicular joint dislocations of Tossy type III with modified double Endobutton technique has satisfactory early clinical results. But with time passing, loss of reduction and bone absorption around the plate could be observed, and clinical outcomes of some cases downgrade during the long-term follow-up.

3.Validation of Microbial Limit Test for Compound Benzocaine Gel

Objective: To establish a microbial limit test method for compound benzocaine gel.Methods: According to the general principles of Chinese Pharmacopoeia (2015 edition), method applicable experiments were performed respectively for the routine method, neutralization method and dilution & neutralization method.Using the recovery ratio of test bacteria as the index, the medium of dilution & neutralization method was adopted in the total aerobic microbial count and total combined yeasts and molds count, and the neutralization method was used for the control bacteria detection.The neutralizing agents were polysorbate 80 and lecithin.Results: The method of dilution & neutralization could eliminate the bacteriostasis of the drug, and the recovery ratio of each test bacteria was within the range of 0.5-2.0, which was in line with the requirements of Chinese Pharmacopoeia.Escherichia coli, Pseudomonas aeruginosa and Staphylococcus aureus could be detected out in the positive control bacterial test,and bacteria were not detected out in the negative test.Conclusion: The method can be used for the microbial limit test for compound benzocaine gel.

5.LYN′s effects on the migration and invasion of gastric cancer cells through Wnt/ β-catenin signaling pathway

Objective:To explore the effects of LYN on the biological behaviors of gastric cancer cells, such as migration and invasion, and analyze the regulatory mechanism of Wnt/ β-catenin signaling. Methods:Gastric and paracancerous tissue samples from gastric cancer patients admitted to the Affiliated Hospital of Chengde Medical University from Oct. 2017 to May 2018 were selected to detect the expression level of LYN in clinically collected gastric cancer and paracancerous tissue samples by qRT-PCR.Gastric cancer AGS cells were used as experimental subjects, and shRNA-LYN was transfected into gastric cancer AGS cells using Lipofectamine 2000 as experimental group (Sh-LYN group), and untransfected cells as control group (NC group). LYN mRNA expression in AGS cells before and after transfection was detected by qRT-PCR.Transwell detected cell migration and invasion. Cell apoptosis was detected by TUNEL.Western Blot analysis was performed to detect proteins related to the Wnt/β-catenin signaling pathway and to study its regulatory mechanism.Measurement data were expressed as ( Mean± SD), and independent sample t test was used for comparison between the two groups. P<0.05 was considered statistically significant. Results:LYN mRNA expression was significantly up-regulated in gastric cancer tissues ( t=11.04, P<0.001).Sh-LYN transfected gastric cancer cells could make LYN protein expression lower than that of the normal control group( F=961.26, P=0.016).After transfection, migration and invasion of cells in the sh-LYN group were inhibited, and the apoptosis rate of cells in the sh-LYN group was significantly higher than that in the normal control group ( t=3.437, 4.450, P=0.011, 0.026) .Western Blot results: sh-LYN reduced the expression of Wnt3 and β-catenin significantly when compared with the normal control group.The detection of downstream proteins showed that after LYN knockdown, the expression of EMT epithelial labeled E-cadherin was increased, and EMT mesenchymal labeled N-cardherin and vimentin were down-regulated. The other related proteins snail 1 and snail 2 were also down-regulated. Conclusion:LYN knockdown inhibits migration and invasion of gastric cancer cells and induces apoptosis in gastric cancer cells, a process associated with the Wnt/ β-catenin signaling pathway.
6.Study on fractional amplitude of low-frequency fluctuation in male patients with unipolar and bipolar depression

Objective To explore the characteristic differences of the fractional amplitude of low-frequency fluctuation (fALFF) feature of the spoutaneous neural activity between young male unipolar depression and bipolar depression patients,and determine the biological markers to distinguish the two disorders.Methods Twelve male unipolar depression,12 bipolar depression patients and 11 age and educated-matched healthy males underwent resting-state functional magnetic resonance imaging scanning at 3.0 Tesla.The whole brain' s fALFF were calculated and analyzed.Results The differences of the fALFF of the three groups had significant differences (P<0.01,Alphasim) in the right orbital medial frontal gyrus (6,33,-9;K =29),the left medial frontal gyrus (-6,60,3;K =44) and the left paracentral lobule (-3,-27,5 1;K =20).The unipolar depression subjects had significantly higher fALFF compared with heahhy controls in the left anterior cingulate gyrus.The bipolar depression subjects had significantly higher fALFF compared with healthy controls in the bilateral medial frontal gyrus and the left middle cingulate gyrus.And the unipolar depression subjects had significantly lower fALFF compared with bipolar depression ones in the right orbital medial frontal gyrus,the right anterior cingulate gyrus and the bilateral medial frontal ~rus(all P<0.05).Conclusions Abnormalities exist in the brain regions in male with unipolar or bipolar depression patients in the resting state,and the abnormal regions are different.
7.Microvasculature change in the diagnosis of early esophageal carcinoma using magnifying endoscopy

Objective To investigate the effective connectivity of limbic circuit in patients with major depressive disorder when they recognized dynamic positive face expressions,aiming to discuss the possible mechanism of emotion processing in depressed patients.Methods Eighteen depressive patients and eighteen well-matched healthy control volunteers participated in the experiment.All subjects were asked to recognize the emotion face during the magnetoencephalograph (MEG) scanning.The regions of interested (ROI) brain areas included the orbital frontal cortex (OFC),the anterior cingulated cortex (ACC),the amygdala (AMYG),the hippocampus and the insula.The MEG data were preprocessed by the SPM8 software and further analyzed by the Granger casual model (GCM).The non-parameter permutation was used to compare the value of effective connectivity between the healthy controls and the depressed patients.Results Compared with healthy controls,the effective connectivity from the ACC to the AMYG (P=0.0052),from the OFC to the AMYG(P=0.0046),from the Hippocampus to the ACC (P=0.0016),and from the ACC to the Hippocampus (P=0.0042)was significantly reduced in depressed patients.Conclusion The depressed patients display decreased interaction of the limbic circuit during the happy facial emotion processing,indicating that the depressed patients are unable to deal with the positive stimuli,and to certain extent,explaining the abnormal neuropathophysiological mechanism of positive stimuli in MDD.
9.Detection and antibiotic sensitivity test of Klebsiella peneumoniae in Ailurus fulgens

To confirm the etiology of a dead case for a 6 year-old female Ailurus fulgens,one strain of the predominant bacteria from pathologic tissues(heart,liver,spleen,lung and other samples) of the dead Ailurus fulgens were examined and isolated.The isolate was named R1 and no other bacteria were isolated.The bacterial etiological examination(morphological characteristics,biochemical characteristics and 16S rDNA gene detection)of R1 showed that it was identifed as K.peneumoniae.Artificial infection to mice about R1 was also conducted in this study.R1 had strong pathogenicity to mice and the LD50 is 6.5 × 104 CFU/mL.Moreover,the clinical and pathological features of the dead mice were consistent with that of the Ailurus fulgens.To find effective therapeutic drugs of curing other Ailurus fulgens,antibiotic sensitivity test of R1 was conducted,and the results revealed that R1 was highly sensitive to cefotaxime et al,moderately sensitive to amikacin and resistant to penicillin.These data showed that K.peneumoniae was bacterial pathogen leading to death of the Ailurus fulgens and it had strong resistance to penicillins,macrolides and virginiamycin and it had broad drug resistance spectrum.However,R1 is sensitive to cephalosporins and aminoglycoside antibiotics.
10.One-stage compound grafting of antibiotic-impregnated calcium sulfate and autogenous cancellous bone for the treatment of chronic calcaneal osteomyelitis.

OBJECTIVETo explore the treatment of chronic calcaneal osteomyelitis with bone defect after debridement and evaluate its clinical outcomes.
METHODSFrom June 2009 to June 2011, 52 patients with chronic calcaneal osteomyelitis were treated with stage-one compound grafting of antibiotic-impregnated calcium sulfate and autogenous cancellous bone,including 12 females and 40 males with an average age of 43 years old ranging from 18 to 67. According to Cierny-Mader classification, there were 34 cases with stage III and 18 with stage IV. There were 32 cases on right side and 20 on left,with a course of 6 months to 3 years. The area of soft tissue wound ranged from 3.0 cm x l.5 cm to 23.0 cm x l2.0 cm. The clinical effects were evaluated according to infection controlling, calcium sulfate absorption,bone defect repair and heel functional recovery.
RESULTSAll patients were followed up for 2 to 3.8 years (averaged 2.8 years). Primary healing was achieved in 52 patients. Two cases of recurrence were found post-operatively, 1 case in 3 months and another in 5 months,which were cured after a second operation. Bone repair healing was gained in 1.5 to 3.5 months (averaged 2.5 months). Complete radiological absorption of calcium sulfate was found in 1.2 to 3 months(averaged 2.2 months). Local exudation after removal of drainage tube had been persisting in 10 patients for 2 to 3 months, which was consistent with the time when cacium sulfate were totally absorbed. Flap had partial necrosis in 4 cases,and the wounds were closed after appropriate treatment finally. The mean Maryland score was 88.15±7.70. There were excellent results in 32 cases, good in 14, fair in 6.
CONCLUSIONA satisfactory short-term clinical results can be gained by one-stage compound grafting of antibiotic-impregnated calcium sulfate and autogenous cancellous bone in chronic calcaneal osteomyelitis, but the long-term results need further follow-up. And much more study is also demanded to reduce the exudation of calcium sulfate.
