1.The clinicopathological study of infantile cytomegalovirus hepatitis.

OBJECTIVETo investigate the clinicopathological features of infantile cytomegalovirus hepatitis.
METHODLiver biopsies from 30 cases of infantile cytomegalovirus hepatitis were observed under optical microscope and electronic microscope.
RESULTThe main clinical manifestations were jaundice, splenohepatomegaly and hypohepatia. Laboratory test showed dysfunction of liver, high level of CMV DNA, and high titer of anti-CMV antibody. Imaging examination demonstrated hepatomegaly. The histological changes were hepatocellular degeneration, necrosis, apoptosis, and fibrosis. The histological characteristics of cytomegalovirus hepatitis, including intranuclear inclusions in multinucleated giant cells and pseudo-lumens, were also observed under optical microscope. In addition, virion was observed in the nuclei and cytoplasm of hepatocytes under electronic microscope.
CONCLUSIONThe viral DNA and serological tests have limited utility for the diagnosis of infantile cytomegalovirus hepatitis, and the final diagnosis depends on histopathology.

2.Sevoflurane influenced inflammatory response on surgery patients with traumatic brain injury

Objective:To investigate the effects of Sevoflurane on inflammatory response,neuro damage and the prognosis on surgery patients with traumatic brain injury ( TBI ) . Methods:From July 2015 to December 2016, 80 TBI patients undergoing neurosurgery were randomly divided into experimental group ( Sevoflurane treatment, n=40 ) and control group ( n=40 ) . The serum levels of TNF-α,IL-6,C-reactive protein ( CRP) and neuron specific enolase ( NSE) were tested with enzyme linked immunosorbent Assay (ELISA). GCS score,APACHEⅡ score and prognosises of the two groups were also compared. Results:Compared with control group,the serum levels of inflammatory related factors (CRP,TNF-α,IL-6) in the experimental group were significantly decreased in 1 hour of intraoperative,at the end of surgery and on 1st day after surgery respectively ( P<0. 05 or 0. 01 ) . Meanwhile, NSE protein expressions were significantly decreased(P<0. 05 or 0. 01). The experimental group received better prognosis compared to the control group,GCS scores[(10. 82±1. 40) vs (8. 29±1. 02),P=0. 03],APACHEⅡ scores[(14. 42±1. 97) vs (18. 03±2. 15),P=0. 02]on the 14th day. Conclusion:Sevoflurane treatment can effectively regulate the inflammatory response, reduce the neurons damage in surgery patients with TBI,possibly playing brain protection effect.
3.Application of next-generation sequencing in clinical oncology to advance personalized treatment of cancer.

With the development and improvement of new sequencing technology, next-generation sequencing (NGS) has been applied increasingly in cancer genomics research over the past decade. More recently, NGS has been adopted in clinical oncology to advance personalized treatment of cancer. NGS is used to identify novel and rare cancer mutations, detect familial cancer mutation carriers, and provide molecular rationale for appropriate targeted therapy. Compared to traditional sequencing, NGS holds many advantages, such as the ability to fully sequence all types of mutations for a large number of genes (hundreds to thousands) in a single test at a relatively low cost. However, significant challenges, particularly with respect to the requirement for simpler assays, more flexible throughput, shorter turnaround time, and most importantly, easier data analysis and interpretation, will have to be overcome to translate NGS to the bedside of cancer patients. Overall, continuous dedication to apply NGS in clinical oncology practice will enable us to be one step closer to personalized medicine.

4.Atypical memory B cells in children with frequently relapsing nephrotic syndrome

Objective To observe the distribution of atypical memory B cells in peripheral blood of children with frequently relapsing nephrotic syndrome(FRNS).Methods A total of 60 children with primary ne-phrotic syndrome(PNS)admitted to the hospital from October 2020 to March 2023 were selected as the re-search objects.According to the response to glucocorticoid(GC),they were divided into non-frequently relap-sing nephrotic syndrome(NFRNS)group(25 cases)and FRNS group(35 cases).A total of 20 age-and gen-der-matched healthy children were enrolled as the control group.The changes of atypical memory B cells in each group before and after GC treatment were compared,and the correlation between the changes and clinical data was analyzed.Results Before GC treatment,The percentages of total B cells(CD19+CD20+),total memory B cells(CD19+CD20+CD27+),resting memory B cells(CD19+CD20+CD21+CD27+)and atypical memory B cells(CD19+CD20+CD21-CD27-)in FRNS group and NFRNS group were significantly higher than those in control group.And the FRNS group was significantly higher than the NFRNS group(P<0.05).After GC treatment,the percentages of total B cells,total memory B cells,resting memory B cells,acti-vated memory B cells(CD19+CD20+CD21-CD27+)and atypical memory B cells in FRNS group and NFRNS group were lower than those before GC treatment(P<0.05).The FRNS group had a significantly higher pro-portion of atypical memory B cells than the NFRNS group and the control group(P<0.05).Before GC treat-ment,the 24 h urinary protein in FRNS group and NFRNS group were higher than those in control group,and the levels of immunoglobulin G and albumin were lower than those in control group.The 24 h urinary protein in FRNS group was significantly higher than that in NFRNS group(P<0.05).Before GC treatment,there was a positive correlation between 24 h urinary protein and the proportion of atypical memory B cells in FRNS group(P<0.05).Conclusion There is abnormal distribution of atypical memory B cells in peripheral blood of FRNS children.The increase of atypical memory B cells can be used as a marker of recurrence of FRNS af-ter GC treatment.

6.Amount Change of Peripheral Blood NK Cells in Patients with Hematologic Malignancies and Its Clinical Significance.

OBJECTIVETo study the amount change of peripheral blood NK cells in patients with hematologic malignancies and its significance.
METHODSA total of 200 patients with hematologic malignancies in our hospital from June 2013 to March 2015 were chosen as study objects, out of them 105 patients were in aute stage and 95 patients were in remisson stage. At same time 100 people from healthy medical examination in our hospital were chosen as control group. The mumber change and subgroups of their peripheral blood NK cells were analyzed and compared.
RESULTSIn control group the absolute number of NK cells was (412.91 ± 167.35)/µl, the relative number of NK cells was (13.31 ± 2.56) %; in group at acute stage of leukemia the absolute number of NK cells was (97.84 ± 23.18)/µl, the relative number of NK cells was (6.79 ± 0.78) %; in group at acute stage of lymphoma, the absolute number of NK cells was (101.79 ± 25.63)/µl, and the relative number of NK cells was (7.12 ± 1.03) %; in group at remission stage of leukemia, the absolute number was (297.17 ± 87.56)/µl, and the relative number was (10.15 ± 1.64) %; In group at remission of lymphoma, the absolute number of NK cells was (288.52 ± 118.52)/µl, and the relative number of NK cells was (10.82 ± 1.97) %. The number of NK cells between different groups showed statistical difference (P < 0.05). In remission group, the number of NK cells before and after treatment had statistical difference (P < 0.05). In control group, the number of CD56(bright) subgroup was (25.28 ± 4.72) %, the number of CD56(bright) subgroup at the acute stage of leukemia was (65.46 ± 11.21) %, and the number of CD56(bright) subgroup at the acute stage of lymphoma was (70.71 ± 12.14) %, the number of CD56(bright) subgroup at remission stage of leukemia was (23.35 ± 4.67) %, the number of CD56(bright) subgroup at remission stage of lymphoma was (24.89 ± 4.58) %. The number of CD56(bright) subgroup between different groups showed statistical significance (P < 0.05).
CONCLUSIONThe number and function of peripheral blood NK cells in patients with hematologic malignancies have been confirmed to be obvious decrement, but after treatment the number of NK cells in those patients showed increment.

7.Research Progress on Estimation of Postmortem Submersion Interval.

Postmortem interval (PMI) estimation is one of the most important and difficult academic tasks in forensic sciences. Due to the influence of the corpse itself and the water environment, corpses in water have unique corruption phenomenon and laws. Based on the experience of traditional PMI studies of corpses on land, forensic practitioners across the world have proposed a variety of practical methods for estimating postmortem submersion interval (PMSI). This paper summarizes the literatures related to PMSI in recent years, and introduces methods to infer PMSI according to the phenomenon of corpses, the development of insects, the succession pattern of aquatic organisms, and the changes of other physical and chemical indexes of corpses, in order to provide some reference for the study of PMSI of corpses in water.

Based on the theory of consolidating the root and cultivating the primary, the paper collates and reviews the theoretical evidences and the characteristics of Xin'an medical masters in treatment of bi syndrome with acupuncture and moxibustion so as to provide the ideas for further research. Xin'an medical masters thoroughly acquainted with the theory of consolidating the root and cultivating the primary in treatment of bi syndrome with acupuncture and moxibustion, emphasizing the regulation of qi and blood, yin and yang, the nutrient qi and the defensive qi; and replenishing the middle jiao (spleen and stomach) and the lower jiao (kidney). The acupoint selection was distinctive, in which, the acupoints were selected and stimulated in terms of the etiology and the pathogenesis of diseases, as well as the properties of special points. The remarkable therapeutic effect on bi syndrome was ensured through specially selecting he-sea points, ashi points and "yin-yang opposite" points; effectively using the penetrating needling technique and moxibustion method and combining acupuncture with herbal medication.

9.Expression of E2F1 Gene in Acute Leukemia and Its Clinical Significance.

OBJECTIVETo investigate the expression of E2F1 gene in patients with acute leukemia(AL) and its clinical significance.
METHODSSeventy-two AL patients treated in March 2015 -March 2016 in our hospital were selected, and 24 healthy people were selected as controls. RT-PCR and Western blot were applied to determine the level of E2F1 gene transcription and expression, and the statistical analysis was performed to reveal the clinical value of E2F1 gene.
RESULTSThe relative expression levels of E2F1 gene and protein in bone marrow of AL patients was higher than that in control group (P<0.05). The levels of WBC, β-MG and LDH in the patients with high expression of E2F1 gene were higher than those in patients with low expression of E2F1 gene(P<0.05), but the E2F1 gene expression did not correlate with sex, fever, fatigue, bone marrow blast ratio, peripheral blood blasts ratio (P>0.05). The complete remission (CR) of patients with low expression of E2F1 was significantly higher than that of patients with high expression of E2F1(P<0.05). And the drug resistance in the patients with low expression of E2F1 gene was lower than that of patients with high expression of E2F1 gene (P<0.05). The expression level of E2F1 gene decreased significantly in patients with symptomatic remission after treatment (P<0.05). The expression levels of E2F1 gene in M1, M2 and M5 patients decreased significantly after treatment (P<0.05). Kaplan-Meier survival analysis showed that OS and DFS in the patients with low expression of E2F1 gene were higher than those in patients with high expression (P<0.05). Multivariate Cox regression analysis showed that the age and E2F1 gene were the independent influencing factors of OS (P<0.05); the sex and E2F1 gene were the dependent factors of DFS (P<0.05).
CONCLUSIONThe expression level of E2F1 gene in patients with AL has been found to be higher, the higher level of E2F1 gene closely relates with AL patients, E2F1 gene can be used as a biological target for the clinical treatment of AL.

10.Expression and Promoter CpG Island Methylation Status of miR-34b in Leukemia Cell Lines and Their Clinical Significance.

OBJECTIVETo explore the expression and promoter CpG island methylation status of miR-34b in leukemia cell lines and their clinical significance.
METHODSA total of 10 cases of non-hematologic diseases were selected as control group, and the bone marrow cells of control group and HL-60, K562 cells were selected; the relative expression of miR-34b was detected in bone marrow cells, HL-60 and K562 cell lines by fluorescence quantitative PCR, and the MiR-34b methylation status was detected by methylation-specific PCR, the HL-60 and K562 cell lines were treated with decitabine, and the expression levels and methylation status of miR-34b in the 2 cell lines were detected by the same method. Has-miR-34b was transfected into K562 cells, which were divided into non-transfection group, negative control group and Has-miR-34b transfection group; if the transfection was successful, the cell proliferation should be recorded at different time points of culture, and the proliferation inhibition rate should be calculated.
RESULTSThe relative expression level of miR-34b in the control group was (5.23 ± 0.75), in HL-60 was (0.05 ± 0.01) and in K562 was (0.04 ± 0.01). The difference between 3 groups was statistically significant (F = 44.812, P < 0.01). The promoter regions of CpG island in HL-60 and K562 cell lines were methylated, while the bone marrow cells were not methylated in 10 cases of non hematologic diseases children.Through miR-34b expression levels of HL-60 and K562 cell lines significantly increased by decitabine treatment (P < 0.05), and the methylation of leukemia cell line promoter region CpG island was found before and after decitabine treatment, but after administration of decitabine the methylation significantly decreased, suggesting that decitabine has an inhibitory effect on methylation of promoter region CpG island. After being cultured for 48, 72, 96 and 120 hrs, the cell proliferation in Has-miR-34b transfection group reached to 24.8%, 46.7%, 33.6% and 4.7%, repectively, and significantly lower than that in non transfection group (P < 0.05).
CONCLUSIONCpG island methylation of miR-34b promoter region in leukemia cell lines can decrease the expression levels of miR-34b, which is also the reason why miR-34b can reduce the inhibition of cell proliferation, thus miR-34b might be a tumor suppressor gene involved in the regulation of leukemia.
