1.Adherent-invasive Escherichia coli isolated from the patient with inflammatory bowel disease in China

Objective To study adhesion and invasion of adherent-invasive Escherichia coli strains and its ability of survival and proliferation in macrophage cells.Methods Bacteria from the mucosa tissues of ileum,colon and rectum of inflammatory bowel disease (IBD)patients were isolated,cultured and identified.The toxic genes were confirmed by polymerase chain reaction.The adhesion and invasion ability of bacteria and its survival and proliferation ability in macrophage cells were observed after co-culture with cells.Results Five adherent-invasive Escherichia coli strains were isolated from the intestinal mucosa specimens of two patients with Crohn′s disease and one patient with ulcerative colitis.The adhesion rates of five bacteria stains (2A,2B,15A,15B,19B)to Caco-2 cells were (1 .4 ± 1 .3 )%,(1 .5 ± 0.6 )%, (0.6±0.1)%,(1 .4±0.4)% and (1 .1 ±0.8)%,respectively.The adhesion rates to Int-407 cells were (1 .0±0.8)%,(1 .5 ±0.8)%,(1 .0 ±0.8 )%,(1 .0 ±0.8 )% and (0.3 ±0.0 )%,respectively.The invasion rates to Hep-2 cells were (10.1 ±7.0)%,(0.7 ±0.4)%,(0.4 ±0.3 )%,(2.2 ±1 .0)% and (2.1 ±1 .8)%,respectively.The invasion rates to Int-407 cells were (0.7 ±0.5 )%,(0.5 ±0.3 )%, (2.8±1 .2 )%,(0.7 ±0.5 )% and (0.5 ±0.4)%,respectively.All five adherent-invasive Escherichia coli strains could survive and proliferate in macrophage cells.Conclusions Five adherent-invasive Escherichia coli strains are isolated for the first time in Chinese patients with IBD,and all of them could survive and proliferate in macrophage cells.
2.The therapeutic strategy of humeral supracondylar fracture with never injury in children.

OBJECTIVETo determine the clinical features and therapeutic strategy of humeral supracondylar fractures with nerve injuries in children.
METHODSFrom June 2000 to December 2006,28 patients of humeral supracondylar fracture (Gartlang III) with never injuries were analyzed. Twenty-four patients were close injuries and the others were open. Sixteen cases (group A)were treated with close reduction and K-wires fixation. Twelve cases (group B)were treated with open reduction and nerve exploration, among them, contusive nerve injuries in 8 cases, incarceration in 3, complete neurotmesis in 1.
RESULTSThe symptoms of nerve injuries of 28 cases recovered completely within 4 to 6 weeks in 13 cases, 8 to 10 weeks in 2 cases and 12 weeks in 1 case in group A, meanwhile, within 4 to 6 weeks in 6 cases, 8 to 10 weeks in 3 cases and 12 to 16 weeks in 3 cases in group B.
CONCLUSIONMost nerve injuries in humeral supracondylar fractures in children can be cured after close reduction of the fracture. Sometimes, open reduction and nerve exploration are necessary, which has important practical significance.

3.The use of antiparkinsonism drugs in 92 cases in Xi' an city

Objective To investigate the treatment status of antiparkinsonism in Xi'an. Methods Six general hospitals were randomly chosen in Xi' an and all Parkinson' s disease (PD) patients were interviewed by questionnaire from Jan. 2007 to Apr. 2007. Results 92 PD outpatients were enrolled in, including 48 males and 44 females, from 43 to 86 years old (mean 65.6±17.1) with duration of the disease from 0.2 to 27.8 years (mean 4.4 ±9.4). The preference of the drug use from the patients were: 40 (43.5%) preferred taking levodopa, 25 (27.2%) with amantadine and/or trihexyphenidyl, 14(15.2%) with levodopa and others, 4(4.4%) with dopamine agonist and others, 2 (2.2%) with other drugs, 7 (7.6%) with no treatment. There were 69 (75.0%) patients onset with resting tremor, 15 (16.3%) with bradykinesia, 6 (6.5%) with rigidity, and 2 (2.2%) with unknown symptoms. There was no startically significant difference in anti-PD drugs among the patients onset with different symptoms (P>0.05). 45 patients appeared the onset of disease before 65 years old and with no dementia, 47 onset after 65 with or without dementia. There was no significant difference of anti-PD drugs between the two groups (P>0.05). Most patients initiated anti-PD treatment with levodopa but few of them chose dopamine agonist. According to the classification of Hoehn & Yahr, 25(27.2%) belonged to Grade Ⅰ, 53(57.6%) to Grade Ⅱ ,8(8.7%) to Grade Ⅲ ,3(3.3%) to Grade Ⅳ and 3 (3.3%) to Grade Ⅴ. There was no significant differences of anti-PD drugs between different grades of the disease (P>0.05). 55.3% of the patients changed their anti-PD drugs randomly during the therapy, but with no relation to their gender, age, educational level, dementia, the number of family members, course of diseases, or the degree of Hoehn & Yahr, frequency and categories of medicine. Conclusion Anti-PD treatment in Xi' an did not strictly follow the standardized protocol, with few patients using dopamine agonist and over 50% of the patients changed their drugs randomly.
4.An evaluation of self -efficacy application on the rehabilitation of hemiplegic patients after ischemic stroke

Objective To evaluate the effect of self -efficacy training on hemiplegic patients after ischemic stroke. Methods Sixty cases were randomly divided into control group (n =30 )and self -efficacy training group (n =30).The self -efficacy group received rehabilitation training according to the self -efficacy theory (5 times /week, 40 min /per time,1 0 weeks of intervention).The movement and balance function were evaluated using Fugl -Meyer score (FMA)and the activities of daily living were evaluated by the task analysis scale and Barthel index.The self -efficacy level and quality of life were evaluated by the general self -efficacy scale (GSES)and medical outcomes study (SF -36).Results The scores of FMA,Barthel,SF -36 and GSES had no significant differences between the two groups before training (P >0.05 )and all indexes improved after training.Six scores of Barthel and all scores of FMA,SF -36 and GSES of self -efficacy group were significantly higher than those of control group after training (all P <0.05 ).Conclusion Self -efficacy application could improve the motor function on the rehabilitation of hemiplegic patients after ischemic stroke.
5.Study on disinfection methods of laminar flow chamber for hematopoietic stem cell transplantation

Objective To explore the effects and feasibility of simplify disinfection method for 100 laminar flow hematopoietie stem cell transplantation rooms in order to improve the care and reduce costs.Methods There were two groups in this study.One group used routine disinfection methods while another used simplify disinfection method.Bacterial culture of air and articles were compared between two groups.Results The qualified rates of bacterial culture were 100 % in both methods.In addition,there were no significant differences in positive rates of air bacteria and article bacteria between two methods (P > 0.05).Conclusions The effects of the two disinfection methods are same.Simplify disinfection method can meet disinfection requirements of the Ministry of Health,reduce the nursing care work intensity,improve efficiency,reduce costs.Overall,simplify disinfection is better than routine method.
6.Construction of eukaryotic expression vector of LYRM1, a novel gene related with human obesity, and establishment of its stable transfected 3T3-L1 cell line.

OBJECTIVETo construct the eukaryotic expression vector of LYRM1 and to transfect it to preadipocytes cell line 3T3-L1.
METHODSThe complete coding sequence of LYRM1 gene was amplified by RT-PCR from human omental adipose tissue and was subcloned into eukaryotic expression vector pcDNA(TM)3.1/myc-His B. The recombinant plasmid was then transfected into 3T3-L1 preadipocytes by liposome. After screening culture by G418, stable transfected 3T3-L1 cell line was established. The expression of LYRM1 protein was identified by Western blot.
RESULTThe recombinant plasmid was confirmed by PCR, restriction enzyme digestion and sequencing. The stable transfected 3T3-L1 cell line was established and the LYRM1 protein was expressed successfully.
CONCLUSIONThe eukaryotic expression vector of LYRM1 has been successfully established and the stably transfected 3T3-L1 cell line also established.

7.Prenatal diagnosis of a case with X-linked spondyloepiphyseal dysplasia tarda.

OBJECTIVETo analyze TRAPPC2 gene mutation in a family with X-linked spondyloepiphyseal dysplasia tarda and to provide genetic counseling and prenatal diagnosis.
METHODSAll of 4 exons of the TRAPPC2 gene and their flanking sequences in the proband and her father were analyzed with polymerase chain reaction and direct DNA sequencing. Genomic DNA of the probands' fetus was extracted from amniotic fluid sampled at 18th gestational week. Gender of the fetus was determined by the presence of SRY gene. The sequence of fetal TRAPPC2 gene was also analyzed.
RESULTSA c.209G>A mutation was identified in exon 4 of the TRAPPC2 gene in the proband and her father. The fetus of was determined to be a male and also have carried the c.209G>A mutation.
CONCLUSIONA c.209G>A mutation of TRAPPC2 exon 4 probably underlies the clinical manifestations in this family. The proband is a carrier, and her fetus is a male carrying the same mutation. Prenatal diagnosis is an effective method for the prevention of the disease.

8.Prevention of ulnar nerve injury during fixation of supracondylar fractures in children by a medial-lateral three-pin fixation technique.

OBJECTIVETo expore the prevention of ulnar nerve injury during fixation of supracondylar fracture in children by a medial-lateral three-pin fixation technique.
METHODSEighty-one patients including 54 boys and 27 girls with average age of 6 years old (2.5 to 11 years)were treated by a three-pin fixation technique with insertion of two Kirscher wires from the lateral side and the third wire through the medial side. They were followed-up without any complications related to the ulnar nerve. According to Garland system, there were 25 cases of type II supracondylar fracture and 56 of type III. All patients were treated by a medial-lateral three-pin fixation technique, and external fixation with plaster for 3 weeks.
RESULTSAll cases achieved immediate stability and long-term bony fusion postoperatively and no iatrogenic ulnar nerve injury happen.
CONCLUSIONThe technique is a stable and reliable methods for unstable supracondylar fracture and the iatrogenic injury ulnar nerve can be avoided completely.

9.Study on the status of cognition among Parkinson' s disease patients in Xi'an

Objective To study the characteristics and influencing factors of cognition on patients with Parkinson' s disease (PD) in Xi'an in order to provide evidence for early recognition and treatment of cognitive impairment on PD patients. Methods Clinically defined PD patients from 7 hospitals in Xi'an from Jan. to Apr. 2007 were assessed with mini-mental state examination (MMSE) and Montreal cognitive assessment (MOCA) for whole cognitive function. Furthermore,Fuld object memory test (FOM) was used to assess delayed memory while rapid verbal retrieve (RVR) was used to assess language fluency. Digit span subtest was used to assess attention and building blocks was used to assess visual space function respectively. Results 100 PD patients were recruited,including 52 men and 48 women,from 43 to 86 years old (65.6±17.1). MMSE scores was used as the standard for Recognition,PD with cognitive impairment accounted for 16%. According to MOCA scores,with PD cognitive disturbances accounted for 83%. Ability for calculation,short-term memory,visual space function,abstract capability,attention and language fluency dysfunction were main cognitive disturbances of PD. Analysis from single factor logistic regression showed that education,age of onset and gender were closely related to the occurrence of cognitive impairment on PD patients. Conclusion Cognitive impairment was common in PD. Ability of calculation,short-term memory,visual space function,abstract capability,attention and language fluency dysfunction were main cognitive disturbances of PD. Cognitive impairment of PD was more likely to occur with low degree of education,late onset of PD,and being female.
10.Comparison of two approaches for the treatment of supracondylar fractures in children by K-wires.

OBJECTIVETo explore the clinical effect between two approaches for the treatment of supracondylar fractures of humerus by K-wires in children.
METHODSFrom Jan. 2004 to Dec. 2006,117 children with supracondylar fractures of humerus were divided into two groups according to different methods of the K-wires fixation involving two K-wires group and three K-wires group. In two K-wires groups,there were 45 children including 31 boys and 14 girls with an average age of 5.6 years ranging from 1 to 11 years; according to Garland classification, 19 cases were type II, 26 cases were type III. In three K-wires groups, there were 72 children including 47 boys and 25 girls with an average age of 6.8 years ranging from 2 to 12 years; according to Garland classification, 22 cases were type II, 50 cases were type II. The movement and carrying angle of elbow joint were measured for all these patients. According to Flynn criteria for supracondylar fracture the results of two approaches with K-wires were compared.
RESULTSAll patients were followed-up from 2 to 24 months (means 15.4 months). In two K-wires group, the results of Flynn were excellent in 27 cases, good in 12 cases, fair in 4 cases and poor in 2 cases, 41 cases gained bony healing, however, fail of fixation arosed in 4 cases and cubitus varus arosed in 2 cases. In three K-wires group, the results of Flynn were excellent in 60 cases, good in 11 cases and poor in 1 case, all cases of three K-wires group achieved bony healing after 6 weeks of operation, the function of joints recovered through exercise and cubitus varus arosed in one case after operation.
CONCLUSIONThe technique of fixation with K-wires is a stable and reliable methods for unstable supracondylar fracture but medial-lateral three-pin fixation is better than one with two pins.
