2.Effect of full-responsibility nursing model in the rehabilitation exercise on patients with chronic heart failure

Objective To observe the effect of full-responsibility nursing model in rehabilitation exercise on patients with chronic heart failure (HF).Methods Eighty-six patients with HF were randomly divided into observation group and control group,with 43 cases in each group.sixteen nurses were randomly divided into A and B group ; A group carried out full - responsibility nursing model and was in charge of the rehabilitation exercise of observation group,B group carried out routine holistic nursing model and was in charge of the rehabilitation exercise of control group.After 4 weeks,the effects including EF,6MWT,the knowing of rehabilitation exercise,satisfaction to the hospital were evaluated.Results Before rehabilitation exercise,there were no significant differences in EF and 6MWT( P > 0.05 ).After 4 weeks,EF was( 54.13 ± 9.12 ) and 6MWT was (402.63 ± 74.81 ) in the observation group,while EF was (43.26 ± 8.57 ) and 6MWT was ( 336.16 ±75.82) in the control group.There were significant differences between the two groups ( t =2.093,2.145 ; P <0.05).there were significant differences before and after rehabilitation exercise in two groups ( P < D.05 ).After 4 weeks,EF and 6MWT of the two groups were both increased,but the effect of the observation group was better (P < 0.05 ). After 4 weeks,there were significant differences in the knowing of rehabilitation exercise and the satisfaction to the hospital in the two groups ( P < 0.05 ).Conclusions Full-responsibility nursing model in rehabilitation exercise can make an effective effect.
3.Expression of heme oxygenase 2 in the corpus cavernosum of castrated rats.

OBJECTIVETo determine the expressions of HO-2 and eNOS in the corpus cavernosum of castrated rats and investigate the correlation of HO-2 and eNOS with erectile dysfunction.
METHODSForty SD rats of 10 weeks were equally divided into 3 castrated groups (4, 8 and 12 weeks respectively after removal of bilateral testes) and a sham operation control group. All the rats were detected for serum testosterone (T), intracavernosal pressure (ICP) and mean arterial blood pressure (MAP) 2, 8 and 12 weeks after the surgery, the content of HO-2 analyzed by Western blot and the expressions of eNOS and HO-2 determined by immunohistochemistry.
RESULTSTestosterone level and ICP/MAP ratio were markedly decreased after the castration (P < 0.05). The HO-2 expression was detected in the corpus cavernosum of all but the 12-week castrated rats, and the area under the curve was (341.50 +/- 99.70) in the 4-week castrated group, significantly less than in the 8-week (705.00 +/- 152. 74) and the control group (876 +/- 443. 36) (P < 0. 05), with no obvious difference between the latter two. eNOS was found mainly in the vascular endothelial cells of the corpus cavernosum, significantly less in the castrated (123.94 +/- 30. 23) than in the control group (421.21 +/- 125. 12).
CONCLUSIONAndrogens might mediate the penile erectile function through HO-2 and eNOS in the corpus cavernosum of rats.

4.Expression of Rho-kinase and heme oxygenase in the corpus cavernosum of spontaneous hypertensive rats.

OBJECTIVETo investigate the expression of Rho-kinase and heme oxygenase and the interaction among NOS/NO, HO/CO and RhoA/Rho-kinase in the corpus cavernosum of spontaneous hypertensive rats (SHRs).
METHODSA series of electric stimuli were applied to the corpus cavernosum nerves of 7 SHRs, the changes of ICP/MAP observed continuously and the expressions of ROCK2, HO-2 and eNOS analyzed by Western blot and immunohistochemistry. Another 7 WKY rats were taken as controls.
RESULTSCompared with the controls, ICP/MAP did not rise obviously (P < 0.05), HO-2 dropped sharply (P = 0.006) and the expression of the ROCK2 protein was elevated markedly (P = 0.017) in the SHRs. Immunohistochemistry showed HO-2 to be distributed in the smooth muscle cells and nervous cells of the corpus cavernosum, and eNOS mainly in the vascular endothelial cells. The expressions of HO-2 and eNOS were obviously reduced in the SHRs.
CONCLUSIONNOS/NO, HO/CO and RhoA/Rho-kinase are related with erectile dysfunction in SHRs and may interact on one another.

5.Detecting caries susceptible population in children with gtfB oligonucleotide probe.

OBJECTIVETo establish a quick, exact and inexpensive method to detect caries susceptibility in children.
METHODS125 caries free children, aged 3-4 years, were randomly sampled. A combination of polymerase chain reaction (PCR) and gtfB oligonucleotide probe hybridization method was used to detect Streptococcus mutans in saliva. The participants were followed up for a year and the clinical examination results were compared with the laboratory results. The perspective study was used to evaluate the detecting approach.
RESULTSWhen the combination of PCR and hybridization method was used to detect caries susceptibility of the sample, the predictive sensitivity, predictive specificity and predictive reliability were all increased to 69.2%, 46.8% and 54.3%, respectively as compared to only PCR, which were 56.4%, 44.2% and 48.3%, respectively. The samples with both the positive and negative results of hybridization detection had caries clinically, but the dmft index and prevalence were higher in the positive (dmft was 2.15 +/- 0.86, and the prevalence was 23.28%) than in the negative(dmft was 1.58 +/- 0.51, and the prevalence was 10.34%) which was statistically significant (P < 0.05).
CONCLUSIONThe probe was found to be quite potential in detecting caries susceptibility, but the predictive specificity and predictive reliability values were not significant.

6. Relation of plasma galectin-3 level with parameters of chronic inflammation and insulin resistance in the patients with polycystic ovary syndrome

A total of 108 subjects were enrolled in this study, including 21 healthy subjects(control group), 34 non-obese patients with polycystic ovary syndrome [PCOS1 group, body mass index(BMI)<25 kg/m2], 32 obese patients with PCOS(PCOS2 group, BMI≥25 kg/m2), and 21 simple obese patients whose age and BMI matched with PCOS2(OB group). BMI, waist-hip ratio(WHR), fasting plasma glucose(FPG), postprandial 2h plasma glucose(2hPG), HbA1C, fasting insulin(FINS), postprandial 2h insulin(2hINS), sex hormones, and lipid parameters were determined. 7.An analysis on epidemiological characteristics of Japanese encephalitis in Zhejiang Province

Objective To analyze the epidemiological characteristics of Japanese encephalitis (JE) in Zhejiang Province from 2007 to 2016 for developing the measures of control and prevention. Methods The data were collected from national notifiable diseases registry system (NNDRS) and case-based JE surveillance system (JESS) from 2007 to 2016, and they were analyzed through descriptive epidemiological method and Microsoft Office Excel 2010. Results From 2007 to 2016, a total of 595 JE cases were reported in Zhejiang Province, with an average annual incidence of 0.114 per 100000 population, among which, 559 (93.95%) were laboratory confirmed. During the ten years, 20 cases died and the average annual case fatality rate was 3.36% . Cases were distributed mainly in Wenzhou, Ningbo and Taizhou Cities, which accounting for 54.79% of the total.The peak months were July, accounting for 86.89% of all the cases.And 85.71% of all the cases were in the age of 0-14 years and 47.73% were scattered children.And 22.77% of the JE cases were vaccinated but 14.01% of them did not complete the whole course.And 77.23% of the cases did not have the experience of vaccination or unknown.Conclusion The incidence of JE is decreasing. The occurrence of JE is sporadic with distinct seasonal peak and mainly concentrating in young-age children.
8.Economic evaluation on new strategy for preventing poliomyelitis in Zhejiang Province

Objective To evaluate the economic effect of new strategy for preventing poliomyelitis in Zhejiang Province. Methods Based on the population of Zhejiang Province in 2016, cost-effectiveness ratio (CER) , benefit-cost ratio (BCR), and net benefit (NB) were applied to calculate the health economic difference for the new strategy and the original strategy as compared to no vaccination strategy. Univariate sensitivity analysis was used to assess the robustness of results with main parameters; including burdens of poliomyelitis, cost of vaccines, and the vaccination program itself, and the discount rate. Results CERdisease, CERdeath, BCR and NB for the new strategy were 1:52700 RMB Yuan per case, 1:1813700 RMB Yuan per case, 12.26 and 796.6622 million and CERdisease, CERdeath, BCR and NB for the original strategy were 1 :32900 RMB Yuan per case, 1 :1133900 RMB Yuan per case, 19.58 and 823.1753 million, respectively. The results of the univariate sensitivity analysis with main parameters were robust. Conclusion The new strategy for preventing poliomyelitis is necessary for this period of the global polio eradication. Though the new strategy appears not as good as the original strategy on economic evaluation, its health economic benefit is significant.
9.Serum leptin level and polymorphism of leptin receptor gene in patients with stroke

Objective To study the relation between stroke and both plasma leptin level and polymorphism of leptin receptor gene. Methods Ninty-nine patients with stroke and 97 patients with other diseases (controls), admitted to our hospital from January 2007 to December 2008, were chosen in the experiment. Polymerase chain reaction-restriction fragment length polymorphism (LEPR) examination was performed to detect the leptin receptor gene (Gln223A rg gene) polymorphism in these patients. The level of serum leptin was analyzed by enzyme linked immunosorbent assay. Results The genotype frequencies of GG, GA and AA in Gln223Arg gene in patients with stroke were 66.67%, 20.20% and 13.13%, respectively, while those in controls were 78.35%, 15.46% and 6.19%, respectively; those in these 2 groups were not significantly different (P>0.05). The allele frequencies of A and G in patients with stroke were 76.77% and 25.23%, while those in controls were 86.08% and 13.92%; those in the 2 groups were significantly different (P<0.05). The level ofleptin in patients with stroke was markedly higher that that in controls (P<0.05). Binary regression analyses showed that history of hypertension, elevated fasting blood glucose and cholesterol, increased plasma leptin level were associated with stroke. Conclusion The allele frequencies of A in Gln223A rg gene, hypertension, elevated fasting blood glucose and cholesterol and increased plasma leptin level might be correlated with the increased risk of stroke.
10.Arsenic trioxide restores ERα expression in ERα-negative human breast cancer cells and its treatment efficacy in combination with tamoxifen in xenografts in nude mice.

OBJECTIVETo study the demethylation effect of arsenic trioxide (As2O3) on ERα-negative human breast cancer MDA-MB-435s cells and its possible mechanisms, and to observe its treatment efficacy in combination with tamoxifen (TAM) after ERα re-expression.
METHODSMTT assay was used to examine the inhibitory effect of As2O3 treatment alone or in combination with TAM on cell proliferation. A nude mouse xenograft model was used to further examine the treatment efficacy in vivo. MSP was used to detect the methylation status of ERα gene after treated with As2O3 in MDA-MB-435s cells and the transplanted tumor tissues. RT-PCR was used to detect the mRNA expression of DNMT1 and Erα. Western bolt was used to detect the DNMT1 and ERα protein expression. The diameter of xenograft tumors was measured weekly, and the tumor growth curve was drawn.
RESULTSThe level of proliferation of the MDA-MB-435s cells was significantly suppressed after treatment with different concentration of As2O3 alone or As2O3 combined with TAM, and the 4 µmol/L As2O3 + TAM treatment for 72 h showed the highest inhibition rate (62.6%). 1, 2, 4 µmol/L As2O3 had demethylation effect on MDA-MB-435s cells, and the DNMT1 mRNA and protein expression was inhibited and accompanied by ERα mRNA and protein re-expression. The unmethylation specific bands of ERα gene were enhanced after treated by As2O3 alone or As2O3 combined with TAM in the xenograft tumors. The expression of DNMT1 mRNA and protein was inhibited, and accompanied by ERα mRNA and protein re-expression. An significant decrease of volume and weight of the xenograft tumors in the As2O3 treated alone or combined with TAM groups was observed compared with those of the normal saline group or TAM alone group (P < 0.05), and the 4 mg/kg As2O3 + TAM group had the highest inhibition rate of tumor weight (79.5%) and volume (76.4%).
CONCLUSIONSERα can be re-expressed in ERα-negative breast cancer MDA-MB-435s cells after treated with As2O3 by inhibiting the DNMT1 activity. MDA-MB-435s cells are re-sensitized to endocrine therapy after ERα re-expression. As2O3 combined with TAM may provide a new therapeutic approach for patients with ERα-negative breast cancer in the clinic.
