1.Analysis on Repeated Offline of Files in Database in No.1 Military Medical Project

Objective To explore the cause of repeated offline of files in the database of No.1 Military Project and find the method to solve the problem. Methods The abnormal offline of files in database is not by chance,and its document in oracle is not very clear. Using the knowledge from internet,the fault was repeatedly simulated on test machine and the source of the fault was found. Results Solving plans and countermeasures were put forward. Conclusion The database of the No.1 Military Medical Project is the core of military hospital work,so its importance is self-evident. The technical support of No. 1 Military Project is strengthened,the right maintenance direction of the database is pointed out as well as the right solution to deal with the problem.

5.The design and optimization of a microfluidic device for sperm motility screening

Objectiye To optimize the depth of the microchannel and the time point for sperm collection,and improve the efficiency of sperm screening on a microfluidic device. Methods Microchannels with four different depths of 25, 50, 100 and 200 μm were tested. Mice sperm were added to the inlet of the microchannel. The relative quantity and motility of sperm in the outlet were recorded at different collection times, i.e. ,5, 15, 30 and 60 min. Statistical method one-way ANOVA and appropriate post-hoc testing were applied to analyze differences between different groups, and further to select the best-fit depth of the microchannel and the time point for collection. Results In microchannels with depths of 25, 50, 100 and 200 μm, the sperm motilities measured in each outlet were (85.4 ± 2.3)%, (85.8 ± 5.8)%,( 87. 2 ± 2. 8 ) %, (76. 5 ± 2. 8 ) % respectively with statistical significance ( F = 5.8, P < 0. 05 ). No obvious differences were found among 25-100 μm channels, however the motility dramatically decreased in the 200 μm group. The relative sperm quantities were (5.2 ±2.0)%, (7.2 ±2.5)%,(12.3 ±2.0)%,(7. 7 ± 1.1 ) % respectively with statistical significance ( F = 6. 9, P < 0. 05), which increased with channel depth from 25 to 100 μm,while it decreased in the 200 μm channel Taking 2 indexes into account, 100 μmwas the most fit channel depth for sperm motility screening. The sperm motility in the outlet gradually decreased with time. At the time points of 5, 15, 30 and 60 min after adding sperm, the sperm motilities were (99. 6 ±0. 7)%, (87.2 ±2. 8)%, (79. 3 ±2. 2)% and (62. 6 ±8.0)% respectively with statistical significance ( F = 37. 3, P < 0. 01 ). Yet the relative quantities of sperm in the outlet increased almost three times in this process. At the time points mentioned above, the relative quantities of sperm were (5.8±1.1)%, (10.6 ± 0.9)%, (12.1 ± 1.7)%, (17.9 ± 3.4)% respectively with statistical significance ( F = 17.8, P < 0. 01 ). Thus 15-30 min was the ideal screening time. Conclusion An effective microdevice for sperm screening with optimized depth and collection time period is developed,which may contribute significantly for the screening of healthy sperm on microfluidic chips.
6.Effects of Different Chinese Medicine Materials on Dissolution of Pills and Their in Vitro - in vivo Evaluation

Objective To study the effects of five different kinds of Chinese medicine materials on dissolution of pills; To evaluate their in vitro - in vivo correlation.Methods Five different types of traditional Chinese medicine materials, such as starch, fiber, protein, grease and polysaccharide materials, were selected by uniform design to the proposed 8 model formulation and preparation of pills, and the cumulative release rate and in vivo plasma concentration of berberine hydrochloride were determined by HPLC. The effects of Chinese medicine materials on the drug release behavior of pills in vitro - in vivo were investigated. And the in vitro - in vivo correlation of the pills was evaluated.Results Starch and fiber materials could promote the release of the pills, and protein and grease materials has a blocking effect on the pills dissolution. Polysaccharide materials have no significant effect on the dissolution of the pills. Pills in vitro - in vivo correlation was significant.Conclusion Chinese medicine materials have the characteristics of medicine-assisted unity, which can control the dissolution and bioavailability of pills by adjusting the proportion of powder in the prescription. And the pills have good correlation in vitro - in vivo.
7.Effects of Bushen Wenyang Huayu Recipe on TRPV1 and Sensitization Factor NGF in Experimental Endometriosis.

OBJECTIVETo study the effect of Bushen Wenyang Huayu Recipe (BSWYHYR) on nerve growth factor (NGF) and transient receptor potential vanilloid receptor I (TRPV1) in experimental endometriosis (EMT), and to explore its mechanism for treating EMT-induced pain.
METHODSTotally in-bred line BALB/c 75 female mice were divided into five groups, i.e., the sham-operation group, the model group, the high dose BSWYHYR group, the low dose BSWYHYR group, the gestrinone group, 15 in each group. Writhing response was observed in each group. Serum contents of NGF were detected using ELISA. Expression levels of NGF and TRPV1 in uterus and ectopic foci were detected using immunohistochemical staining SP and Western blot. mRNA expression levels of NGF and TRPV1 in uterus and ectopic foci were detected by Real-time PCR.
RESULTSThe serum NGF content in the model group was higher than that in the sham-operation group (P < 0.01), and there was positive correlation between NGF and the writhing frequency (r = 0.574, P < 0.01). Compared with the model group, serum levels of NGF significantly decreased in the 3 treatment groups (P < 0.05). Compared with the sham-operation group, mRNA and protein expression levels of NGF and TRPV1 increased significantly in the model group (P < 0.01). Protein expression levels of NGF and TRPV1 decreased significantly in the 3 treatment groups, when compared with the model group (P < 0.01). mRNA expression levels of NGF and TRPV1 decreased most in the high dose BSWYHYR group (P < 0.01). NGF in uterus and ectopic foci was positively correlated with protein and mRNA expression levels of TRPV1 (P < 0.01).
CONCLUSIONSNGF and TRPV1 participated in the occurrence of pain in EMS. BSWYHYR played an important role in inhibiting EMT-induced pain through reducing the up-regulation of NGF on TRPV1.

8.Investigation on the application status of aerosol inhalation by suburban patients with chronic obstructive pulmonary disease

Objective To investigate the application status of aerosol inhalation by suburban patients with chronic obstructive pulmonary disease ( COPD) .Methods A total of 82 patients with COPD were chosen by convenience sampling , and self-designed questionnaires were used to investigate their application of aerosol inhalation .Results Eighty-two questionnaires were handed out and 82 were recovered , with the effective rate of 100%.Patients hospitalized in respiratory department had statistically higher score in application of aerosol inhalation than those who were hospitalized in non-respiratory department ( t =9.27, P <0.01 ).Linear correlation analysis showed that the score was in positive correlation with the patients ’ educational background , whether hospitalized in respiratory department , and whether examined upon further interview with doctors ( r=0.350, 0.728, 0.511, respectively;P<0.01).Logistic regression analysis indicated that whether hospitalized in respiratory department and patients ’ educational background were the main influencing factors ( R2 =0.58, 0.37, respectively ).Conclusions To improve patients ’ ability of using aerosol inhalation correctly , professional skill trainings are necessary for nurses in non-respiratory department .
9.Present situation of congenital defects in five counties (cities) of Gansu province in 2009-2010

Objective To understand the situation of congenital defects' in five counties/cities in Gansu province so as to provide scientific evidence for the development of effective interventions.Methods General imformaton was collected on all the neonates who were born in Dunhuang city,Jingchuan county,Hui county,Weiyuan county and Yongjing county in Gansu province between Oct.1st,2009 to Sep.304th,2010,with all of their gestational age above 28 weeks.Neonates would include live birth,dead fetus and still birth.Results The overall incidence of congenital defects was 7.49‰ in the five counties/cities in Gansu province in 2009.Ranking order in the top three showed as congenital heart disease,pigmented nevus and limb deformity.Disease with the highest mortality was congenital heart disease (0.79‰).The incidence of congenital defects was 8.35‰ in 2010 with the ranking order of the top three as congenital heart disease,neural tube defects/pigmented nevus and hydrocephalus.Diseases having the highest mortality was congenital heart disease (1.10‰o).Different incidence rates on congenital defects were seen in realted areas,with the highest incidence as 14.65‰ in Dunhuang city.Hui county had the lowest incidence—3.28‰.Conclusion Different incidence of congenital defects were seen in respective areas in Gansu province,with the change of ranking orders.Different strategies should be developed differently depending on the current states of congenital defects in respective areas,according to the three-grade prevention model,to reduce the occurrence of congenital defects.
10.Hypofibrinogenemia is a risk factor of poor prognosis of HELLP syndrome

Objective Observing changes in thehaemostatic system to determinetheseverity and prognosis of HELLP syndrome. Methods 127 cases of HELLP syndrome diagnosed in Peking University Third Hospital were enrolled from August 2010 to August 2018. Maternal and fetaldemographic characters, postpartum complications,length of hospital stay,prothrombin time(PT),activated partial thromboplastin time (APTT), Fibrinogen(Fg) and D-Dimer(D-D) were collected. Results There was no statistical difference inparturient PT, APTT, D-D levels between maternals of HELLP syndrome with and without postpartum hemorrhage,which were [9.6 (9.0, 11.5)s vs 9.4 (8.9, 9.7)s, P=0.243], [30.2 (29.1, 38.3)s vs 29.8 (27.7, 31.8)s, P=0.151], and [0.80 (0.52, 4.52)μg/ml vs 0.91 (0.55, 2.48)μg/ml, P=0.923] respectively. There was a statistically significant difference obvious difference in parturient Fglevels between two groups [(2.94±1.48) g/L vs (3.61±1.00)g/L, P=0.022). The receiver operating characteristic curve(ROC) analysis showed that the AUC of fibrinogen level when estimating postpartum hemorrhage was 0.688(95%CI:0.600-0.767), cut-off value was 3.04 g/L, negative predictive value was 74.3%. There was a negative correlation between parturient Fg and days of hospital stay of HELLP syndrome maternal(r=-0.182, P=0.040). There was no statistical difference in parturient PT, APTT, Fg and D-D levels between the fetal survival group(n=93) and non-survival group (n=34), and between the distressed group (n=23) and he undistressed group(n=70) (P>0.05). Conclusions The low parturient Fg level may be a risk factor of maternal adverse clinical outcomes in HELLP syndrome. Maintaining the Fg at a stable level may reduce the incidence of HELLP syndrome adverse outcomes.
