1.Determination concentration of duloxetine hydrochloride in human plasma by HPLC-MS/MS and study on its pharmacokinetics

Objective To establish a liquid chromatography-tandem mass spectrometry(LC-MS/MS)method for the determination concentration of duloxetine in human plasma and study on its pharmacokinetics in healthy human.Methods The separation of duloxetine was performed on Phenomenex C_(18) column with fluoxetine as the internal standard.The mobile phase was composed of 5 mmol L~(-1) ammonium acetate with 0.02% formic acid acetonitrile(55:45).Electrospray ionization source was applied and operated in positive ion mode.A single dose of 60 mg duloxetine hydrochloride was given to 5 male and 5 female healthy volunteers and the plasma was separated.The concentration of duloxetine was determined by HPLC-MS/MS and the pharmacokinetic parameters were calculated.Results The linear range of duloxetine was 0.89-106.80 ng·mL~(-1)(γ=0.9977).The methodological recovery and the extraction recovery ranged between 93.19%-107.27% and 72.81%-89.96%,respectively.Both the inter-day RSD and intra-day RSD were less than 11%.The main pharmacokinetic parameters after a single dose of 60mg duloxetine are as follows: C_(max) was(44.40 ±17.78)ng·mL~(-1),t_(max) was(6.10±1.29)h,t_(1/2) was(12.81 ±2.31)h;AUC_(0-60) and AUC_(0-∞) were (696.04±337.82),(733.82±343.40)ng·h·mL~(-1),respectively.Conclusion The method is simple,accurate and reliable,and suitable for the determination of duloxe-tine in therapeutic drug monitor and its pharmacokinetics study.
2.Clinical and laboratory characteristics of 32 patients with Listeria monocytogenes bacteremia

This study was aimed at analyzing the clinical and laboratory characteristics of patients with Listeria monocyto-genes bacteremia,to provide evidence for its diagnosis and treatment.A retrospective analysis was conducted on the clinical data for patients with L.monocytogenes bacteremia at Tangdu Hospital between September 2012 and April 2022.The data included age,sex,underlying diseases,treatments,and prognosis.Changes in indicators such as white blood cell(WBC)count,mono-cyte percentage,neutrophil percentage,monocyte/neutrophil ratio(M/N),C-reactive protein(CRP),and procalcitonin before and after treatment were statistically analyzed.Among the 32 patients with L.monocytogenes bacteremia,the average age was 31.9 years,and three patients were older than 65 years.The incidence rate was highest in summer(11 patients,34.4%),fol-lowed by spring(9 patients,28.1%).A total of 24 patients(75.0%)had underlying diseases.After accurate diagnosis,the treatment plans of 29 patients were adjusted to target antibacterial therapy consisting primarily of penicillins(17 patients,53.1%)and carbapenems(12 patients,37.5%).After treatment,the levels of neutrophils,lymphocytes,and CRP were signifi-cantly lower than those before treatment(P<0.05).A total of 29 patients(90.6%)improved and were discharged,one patient died,and two patients had poor prognosis.The primary risk factors for L.monocytogenes infection were autoimmune diseases,tumors,and pregnancy.Penicillin was the first choice effective empirical treatment for listeriosis.A clear diagnosis of the pathogen and appropriate choice of antibiotics were particularly important for the treatment of L.monocytogenes infection.
3.Changes in the sonographic appearance of the endometrium after different premenopausal tamoxifen therapies.

OBJECTIVETo assess the effect of different schemes of premenopausal tamoxifen therapy on the endometrium.
METHODSTotally 109 normal premenopausal women positive for high-risk factors of breast cancer were divided into two groups, namely periodic and consecutive tamoxifen treatment groups. Endometrial thickness as examined by vaginal sonography was assessed in relation to duration of tamoxifen use and time from discontinuation of the drug.
RESULTSAfter one year of tamoxifen use, the mean endometrial thickness in periodic treatment group was 6.5-/+1.4 mm, and 10.2-/+2.0 mm in consecutive treatment group. Endometrial thickness increased with the duration of tamoxifen use at the rate of 0.51 mm/year in the periodic treatment group, and 0.73 mm/year in consecutive treatment group. After discontinuation of tamoxifen, the endometrial thickness in the former group decreased by 1.29 mm/year, and by 1.33 mm/year in the latter.
CONCLUSIONSEndometrial hyperplasia is obviously milder in premenopausal women receiving periodic tamoxifen treatment who are at risk for breast cancer than that in women with consecutive treatment. After discontinuation of the drug, the endometrial thickness decreases at a roughly equal slow rate in the two groups.

4.Clinical effect and mechanism of hemoperfusion in treatment of children with severe abdominal Henoch-Schönlein purpura.

OBJECTIVETo study the clinical effect and mechanism of hemoperfusion (HP) in the treatment of children with severe abdominal Henoch-Schönlein purpura (HSP).
METHODSA total of 24 children with severe abdominal HSP were divided into two groups: conventional treatment and HP (n=12 each). Ten healthy children who underwent physical examination were enrolled as the control group. Before and after treatment, chemiluminescence was used to measure the serum levels of interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α); thiobarbituric acid colorimetry was used to measure the plasma level of malondialdehyde (MDA); the hydroxylamine method was used to measure the plasma level of superoxide dismutase (SOD); chemical colorimetry was used to measure the plasma level of total anti-oxidant capability (T-AOC).
RESULTSCompared with the control group, the conventional treatment and HP groups had significantly higher IL-6, TNF-α, and MDA levels and significantly lower SOD and T-AOC levels before treatment (P<0.05), but there were no significant differences between the conventional treatment and HP groups (P>0.05). After treatment, the conventional treatment and HP groups had significant reductions in IL-6, TNF-α, and MDA levels and significant increases in SOD and T-AOC levels (P<0.05). The HP group had significantly greater changes than the conventional treatment group; however, there were still significant differences in these indices between the HP and control groups (P<0.05). Compared with the HP group, the conventional treatment group had a significantly lower percentage of children with disappearance of digestive tract symptoms at 4 days after treatment and significantly longer time to disappearance of rash and digestive tract symptoms (P<0.05). Compared with the conventional treatment group, the HP group had a significantly lower amount of glucocorticoid used during treatment and a significantly lower percentage of children who experienced hematuria and/or proteinuria within 6 months of the disease course (P<0.05). There were no significant differences between the two groups in length of hospital stay and recurrence rates of rash and abdominal pain within 6 months of the disease course.
CONCLUSIONSHP can reduce the amount of glucocorticoid used during treatment and the incidence rate of kidney injury in children with severe abdominal HSP, possibly by eliminating IL-6, TNF-α, and MDA.

5.Clinical observation on effect of danzhi xiaoyao powder in treating depression.

OBJECTIVETo observe the effect and side effect of Danzhi Xiaoyao powder (DXP) in treating depression.
METHODSA randomized controlled and double-blinded study was conducted in 63 cases of depression by divided them into the western medicine group (WMG, 31 cases) treated with maprotiline, and the Chinese medicine group (CMG, 32 cases) treated with DXP. The effect of therapy was evaluated before and at the 2nd, 4th and 6th week of the treatment with Hamilton's depressive scale (HAMD), self-rating depression scale (SDS), self-rating anxiety scale (SAS) and the scale for TCM syndrome and symptom differentiation (TCM-SSD), and the side-effect of therapy was assessed with Asberg side-effect scale as well.
RESULTSThere was no significant difference between the two groups in scores of HAMD, SDS, SAS, and TCM-SSD. The markedly effective rate in CMG was 84% and in WMG 87%, showed no significance between them (P > 0.05). The scores of HAMD, SDS and SAS of both groups were remarkably lowered after therapy (P < 0.05). However, the score of Asberg in CMG was lower than that in WMG (P < 0.05).
CONCLUSIONDXP shows the effect equivalent to that of maprotiline, but with obviously less side-effect.

6.Research on the electromyographic signals of orbicularis oculi muscle of rabbit

Objective To study the features of electromyogaphic signals of orbicularis oculi muscle (OOM) of normal rabbits in various movement states, and to clarify relationships between functional actions of OOM and their electromyographic signals, hoping to obtain information concerning the electromyographic signals controlling OOM as reference for restoring the eye-closing function by artificial facial nerve prosthesis in patients with unilateral peripheral facial palsy. Methods The electromyographic signals were extracted from OOM of normal rabbits by implanted microelectrodes through upper and lower eyelids. Then the features of these electromyographic signals were analyzed in the time domain and the frequency domain. Results The peak values of the absolute electromyographic amplitude for natural continuous eye-opening event,natural continuous eye-closing state, natural eye-blinking movement and evoked eye-closing state were (28. 8 ± 4. 8) μV, (36. 0 ± 4. 7 ) μV, ( 398. 8 ± 195.7 ) μV, and ( 715.4 ± 249. 7 ) μV, respectively. The peak frequency values of the power spectrum density (PSD) of electromyographic signals for the four modes were(98 ± 17 ) Hz, ( 142 ±22) Hz, (203 ±58) Hz, and( 349 ± 81 ) Hz, respectively. The electrical activities during the natural continuous eye-opening event and the natural continuous eye-closing state were stable and displayed low amplitudes. During the spontaneous blink state and the evoked eye-closing state, the electromyographic amplitudes markedly increased, and the increased level in the latter state was stronger than that in the former state. When rabbits continuously closed eyes or opened eyes, all of the peak values of thc absolute voltage amplitudes were less than 50 μV. The absolute amplitude values of the starting site were between 50 μV and 60 μV during the spontaneous blink and the evoked eye-closing movements. The whole frequency band of the energy of PSD about OOM was between 0 Hz and 500 Hz, and the focus frequency range was between 20 Hz and 350 Hz. In the time domain, the difference was not significant for the electromyographic signals of OOM between the continuous eye-opening state and the continuous eye-closing movement ( P > 0. 05 ), but there were statistically significant differences in the other states for their pairwise comparisons( P <0. 05). In the frequency domain, there was no statistically significant difference for the peak frequency of PSD about the electromyographic signals when comparing the continuous eye-opening state with the continuous eye-closing event( P > 0. 05 ). When comparing this item in the other movements with each other, however, the differences were statistically significant ( P < 0. 05 ). Conclusions OOM relaxes when eyelid keeps continuously opening. The action of eyelid-closing is due to contraction of this muscle.Each state has its own features of the electromyographic signals for OOM, these features can be used as criteria for computers to judge and identify various movement states of OOM. However, it is difficult to distinguish the natural continuous eye-opening event from the natural continuous eye-closing state, based on the features of electromyographic signals in the time and frequency domain.

Objective To explore the effect of clinic professional nursing guidelines in nursing admini stration.Methods Clinic professional nursing guidelines were formulated according the specific condition in different apartment,the content includes two parts: routine disease care and symptom nursing guideline.Five departments of respiratory medicine,neurosurgery,oncology,orthopedic surgery and oncology radiation were randomly selected as studying subjects and the data was compared before and after guidelines.Results Neurosurgery was selected as a representation,basic nursing quality as(2.223 ±0.315),satisfaction degree was(41.26 ±3.25)before guideline and(3.122 ±0.214)and(46.12 ±5.26)after guideline respectively.The differences were statistically significant (t =21.833,12.740; P<0.05 or <0.01).Conclusions Application of clinic professional nursing guidelines could standardize the nursing behavior and provide scientific,accurate,professional and practical nursing guidelines for first-line nurses,especially for those on further training,new graduate and new to switch department nurses,and raise the work effect and guarantee the work quality,reduce the nursing risk and ensure the nursing safe for them.

9.Correlation between the expression of thrombospondin-1 and neovascularization in mucoepidermoid carcinoma.

BACKGROUNDResearchers have recently demonstrated that thrombospondin-1 (TSP-1) has an important function in regulating neovascularization. Whether it inhibits or accelerates neovascularization, however, is still controversial. We found few reports about the correlation between TSP-1 and vascularization in mucoepidermoid carcinoma. In this research, the distribution and expression of TSP-1 in mucoepidermoid carcinoma were investigated. We also analyzed (1) the correlation between the expression of TSP-1 and microvessel density (MVD), as an indicator of neovascularization activity, and (2) the effect of TSP-1 on neovascularization and tumor growth in the subcutaneous xenotransplanted model of mucoepidermoid carcinoma.
METHOD(1) The sites and intensity of expression of TSP-1 and the MVD were analyzed in 45 cases of mucoepidermoid carcinoma after surgery by the method of streptavidin-peroxidase (SP) immunohistochemistry; and (2) recombinant human thrombospondin-1 (rhTSP-1) was injected twice a week for five consecutive weeks around the tumor in the subcutaneous xenotransplanted tumor model of mucoepidermoid carcinoma in nude mice. Each week, the tumor size was measured, in order to draw the growth curve of the xenotransplanted tumor model of mucoepidermoid carcinoma, and MVD was measured.
RESULTS(1) The positive expression of TSP-1 protein was 57.78% (26/45). Most positive staining for TSP-1 was found in the cytoplasm of the cancer cells, while some staining occurred in the extracellular matrix. The mean MVD in 45 cases of mucoepidermoid carcinoma was 58.17 +/- 19.77 per 100 visual fields. Tumors with a high expression of TSP-1 showed a low MVD value, and the TSP-1 immunocompetence and microvessel density showed a significant negative correlation (r(s) = -0.947, P < 0.001). (2) The xenotransplanted tumors with the injection doses of 1.25, 0.75 and 0.25 microg/ml respectively were 36.97%, 53.36% and 73.61% of the size of the control group ((451 +/- 92), (651 +/- 113), (898 +/- 86) and (1220 +/- 157) mm(3) respectively, F = 53.167, P < 0.001), and their weights were respectively 35.14%, 51.35% and 70.27% of the control group ((1.3 +/- 0.5), (1.9 +/- 0.5), (2.6 +/- 0.3), and (3.7 +/- 0.7) g respectively, F = 62.669, P < 0.001). Their MVDs were 25.00%, 45.93%, and 72.20% respectively of the control group and concentration dependent (15.43 +/- 3.45, 28.35 +/- 4.24, 44.57 +/- 3.35 and 61.73 +/- 5.43 per 100 visual fields respectively, F = 54.582, P < 0.001).
CONCLUSIONSThe TSP-1 has a higher expression in mucoepidermoid carcinoma and the expression has a significant negative correlation with neovascularization. The TSP-1 inhibits neovascularization and tumor growth, and it might be a new biological therapy for treatment of patients with mucoepidermoid carcinoma.

10. The reversal effect of albiflorin on multidrug resistance of human ovarian cancer and its mechanism

Aim To study the reversal effect of albiflorin(AL)on multidrug resistance of human ovarian cancer and the potential mechanism. Methods The drug resistance reversal effect of AL on SKOV3/DDP cells was detected by CCK-8 kit,and the effect of AL on P-glycoprotein(P-gp)function was detected by flow cytometry. The effects of AL on MYC,WWP1 and ABCB1 in SKOV3/DDP cells were detected by RT-qPCR and Western blot. The MYC-knockdown SKOV3/DDP cell line was constructed by RNA interference technology,and its drug resistance,P-gp function and related gene and protein expression changes were investigated. Results AL had a drug resistance reversal effect on SKOV3/DDP cells and a concentration-dependent inhibitory effect on P-gp function. The inhibitory effects of AL 25,50 and 100 μmol·L-1 on ABCB1/P-gp,MYC and WWP1 were gradually enhanced. The inhibitory effect of MYCi975,a MYC inhibitor,on ABCB1/P-gp,MYC and WWP1 was stronger than or equivalent to that of AL 100 μmol·L-1 group. After knockdown of MYC in SKOV3/DDP cells,cell drug resistance,P-gp function,and related gene and protein expression were inhibited. Conclusions The drug resistance reversal effect of AL on SKOV3/DDP cells may be related to the inhibition of P-gp function and the expression of ABCB1/P-gp,MYC and WWP1,which provides an experiment base for the development of AL as a drug resistance reversal agent for the clinical treatment of ovarian cancer.
