1.Large vestibular aqueduct syndrome:23 cases report

This paper reports 23 cases of the large vestibular aqueduct syndrome.All cases are sensori-neural hearing loss,23.91% is moderately severe hearing loss,54.35% is severe hearing loss,and 21.74% is profound hearing loss.The contour of audiogram in high frequency dctcrioration is 74%,che flat contour of audiogram is 13%,the islanded contour audiogram is 13%.Some cases are progressive hearing loss,and the others are sudden hearing loss,two cases among whole cases were found vestibular dysfunction after examination.After high resolution CT scan in inner ear,27 ears were found pathological changes only in vestibular aqueduct enlargement,in addition to these changes,12 ears were found malformation of vestibule and semicircular canals,and 7 ears were showed cochlea malformation.It is said that the disease is caused by abnormal development on endolymphatic duct in early embryonic stage,and the principal inducement is infectious factor.There is not an effective treatment,but the better results can be reached in deaf children with residual hearing,after they wearing hearing aids and getting the auditory-verbal training in the early period of time.If the hearing aid can not reach effective hearing compensation for these patients,cochlea implantation is considered and suggested.
2.Construction and MRI of angiotensin Ⅱ transiently induced abdominal aortic dissection in ApoE-/-mouse model

Objective To establish abdominal aortic dissection model in ApoE-/-mice, and to evaluate the ability of 7.0T MR to detect the abdominal aortic artery aneurysms in ApoE-/-mice in vivo. Methods ApoE-/-mice aged 10 months were infused with angiotensin Ⅱ with 14 days Osmotic minipump after 10 weeks of high lipid diet. Two different doses of angiotensin Ⅱ were given to mice, i.e. 1000 ng/(kg·min) and 500 ng/(kg·min), respectively. The contrast group was infused with saline water. The abdominal aortic artery was observed in vivo with MR before and within 14 days infusion. At last, the pathological changes of the abdominal artery were compared with MRI findings. Results After 6 or 7 days higher dose of angiotensin Ⅱ infusion, aortic dissection was seen. MR T2WI showed crescent-shaped high signal in the vessel wall of one side,the pathological study identified the hematoma between media and adventitia. Abdominal aortic dissection aneurysms were also found in the mice 13 or 14 days after lower dose of angiotensin Ⅱ infusion, which were consistent with pathological studies. Besides, the signal of the vessel wall was significantly higher in both T2WI and PDWI sequences. There was excellent agreement between MR and histopathology. 〖WTHZ〗 Conclusion Abdominal aortic dissection aneurysms model can be successfully established with different doses (1000 ng/(kg·min) and 500 ng/(kg·min)) of angiotensin Ⅱ infusion into ApoE-/-mice fed with high lipid diet. High-resolution MR is able to visualize the abdominal aortic dissection aneurysm formation in vivo.
3.The Inhibitory Effect of Baixuan Xiatare Tablet on Model Mouse with Allergic Contact Dermatitis

OBJECTIVE:To investigate the inhibitory effect of Baixuan xiatare tablet on the model mouse with allergic contact dermatitis (ACD). METHODS:60 BALB/c mice were equally randomized into normal control (isometric solvent) group,model (isometric solvent)group,ebastine(positive control,0.003 g/kg)group and the groups of high,middle and low doses of Baixuan xiatare tablet(2.0,1.0 and 0.5 g/kg). The mice were given drugs,ig,once daily for 14 consecutive days. 0.5% 2,4-dinitrofluoro-benzene(DNFB)acetone olive oil solution was applied,for sensitization,on the prepared mouse’s skins one and two days before administration,and 0.2% DNFB acetone olive oil solution on their left ears 16 days thereafter to establish mouse models of ACD. At 48 h after successful establishment of the models,the thickness of the mouse’s left ear margin was measured and the difference value and swelling degree were calculated;flow cytometer was used to determine the levels of T lymphocyte subsets CD4+ and CD8+ in mouse blood and calculate the ratio of CD4+ to CD8+;the levels of interleukin 4(IL-4)and IL-6 in mouse serum were de-termined. RESULTS:Compared with normal control group,those in the model group had higher difference value of ear margin and swelling degree,lower level of CD4+ in blood and ratio of CD4+ to CD8+,and higher content of IL-6 in serum. There was statisti-cally difference (P<0.01). Compared with model group,those in the groups of high,middle and low doses of Baixuan xiatare tablet had lower degree of left ear swelling and higher level of CD4+ in blood;those in the groups of high and middle doses thereof had lower difference value of left ear margin and level of IL-6 in serum;and those in the group of high dose thereof had higher lev-el of CD8+ in blood. There was statistically significance(P<0.01 or P<0.05). CONCLUSIONS:Baixuan xiatare tablet has inhibi-tory effect to some degree on the mouse model with ACD by a mechanism which may be related to the balance of subsets CD4+and CD8+in blood and the reduction of IL-6 in serum.
4.Population pharmacokinetics of norvancomycin

5.Use of lower trapezius musculocutaneous pedicle flap in repairing of maxillofacial region penetrating defect.

OBJECTIVETo investigate an safe and effective new technology (treatment) to repair maxillofacial region penetrating defect.
METHODSThe lower trapezius musculocutaneous flap is parallel just like as two leaves which is connected to each other, and was folded to provide the liner of oral cavity and external cover.
RESULTSTotally twelve folding lower trapezius musculocutaneous pedicle flap survived. Postoperative follow-up for 1 approximately 3 years, the patients restored the function as well as the shape of maxillofacial region.
CONCLUSIONSThe lower trapezius musculocutaneous pedicle flap is a suitable material for maxillofacial region reconstruction, further more, the successful rate is perfect.

OBJECTIVEto explore the value of in vivo dynamic monitoring of abdominal aortic atherosclerosis (AS) by high field magnetic resonance (MR) imaging (MRI) in apoE-/- mice fed a high fat diet or infused with angiotensin.
METHODShigh fat diet or angiotensin II infusion was applied to apoE-/- mice for establishment of abdominal aortic atherosclerosis model. Abdominal aorta MRI was performed at 3 time points (baseline, 3 and 6 months) in 13 high fat diet fed apoE-/- mice aged 10-12 months and 3 wild-type control mice; 10 apoE-/- mice aged 6 months were infused with angiotensin II (1000 or 500 ng × kg(-1)× min(-1), n = 5 each) or saline for 14 d through Osmotic minipump. The abdominal aortic artery MRI was performed at baseline and 14 d after infusion. Black blood sequences of FLASH T1 weighted images and Proton density weighted-T2 weighted dual echo images were obtained. At each observation time post MRI, mice (n = 3, 5 and 5 for high fat diet group and n = 5 and 5 for angiotensin II infusion group) were sacrificed for pathological examination of the abdominal artery.
RESULTS(1) the abdominal aorta atherosclerosis was identified in both high fat diet and angiotensin II treated apoE-/- mice but in WT controls. Lesion progression was documented in high fat diet fed apoE-/- mice characterized by significantly increased vessel wall (a marker of atherosclerotic burden, F = 29.94, P < 0.05) and gradually increased plaque signal in PDW and T2W images. Results derived from MRI corresponded histopathology findings in high fat diet fed apoE-/- mice (correlative coefficient = 0.84, 0.95, 0.90, P < 0.05, respectively). Both MRI and histology showed increased lipid composition and decreased fibrotic composition in these mice. (2) The vessel wall area increased significantly [(1.21 ± 0.21) mm(2) vs. (2.65 ± 0.48) mm(2), P < 0.05] and the abdominal aortic dissection aneurysms was identified in apoE-/- mice infused with high angiotensin II. The vessel wall area also increased [(0.85 ± 0.11) mm(2) vs. (1.01 ± 0.17) mm(2), P < 0.05] in low angiotensin II infused apoE-/- mice and the coefficient between MR and histopathology is 0.934.
CONCLUSIONabdominal aortic unstable plaque model could be established by both high fat diet and angiotensin II infusion in apoE mice, angiotensin II infusion can transiently accelerate the progression of AS and can induce abdominal aortic dissection. Serial MR black blood sequences could demonstrate the development and progression of atherosclerosis in mouse abdominal aorta with excellent agreement to histopathology finding in terms of atherosclerotic burden and plaque composition. Thus, MRI appears to be a useful tool for in vivo AS plaque dynamic monitoring in mice.

OBJECTIVETo study the effect of alternatively activated macrophages /mononuclear phagocytes(MNP) on breast cancer cells and explore the mechanisms for the action of tumor-associated macrophages in breast cancer.
METHODSHuman peripheral blood monocytes were isolated and cultured in vitro and divided into 3 groups, namely classically activated monocytes (CAM) which were induced by lipopolysaccharide, alternatively activated monocytes (AAM) induce by IL-4, and control cells treated with the culture medium only. After cell culture for 48-72 h, the mRNA of tumor necrosis factor-alpha (TNF-alpha), alternative monocytes activation- associated CC-chemokine 1 (AMAC-1), and beta-actin of the 3 groups were extracted for RT-PCR, or the cells were cocultured with breast cancer cell line SKBR3, or seeded in chicken chorioallantoic membrane along with SKBR3.
RESULTSTNF-alpha mRNA was significantly increased in CAM, and AMAC-1 was highly expressed in AAM. The coculture experiments showed that CAM exhibited obvious inhibitory effect on SKBR3 cells after a 3-day culture whereas AAM significantly promoted the growth of SKBR3 cells after a 5-day culture. In chicken on chorioallantoic membrane experiment, the macrophages promoted tumor angiogenesis and AAM showed the most obvious effect.
CONCLUSIONIL-4 induces high expression of AMAC-1, a molecular marker of AAM, in the macrophages, and AAM can promote the growth of SKBR3 cells and tumor angiogenesis.

8.SNP rs16917496 within SET8 3'UTR is associated with the age of onset of breast cancer.

OBJECTIVEWe have identified a SNP within the seed-binding region for miR-502 in the 3'-UTR of the SET8 gene that codes for a methyltransferase for histone H4. SET8 methylates TP53 and thus regulates cell proliferation and genome stability. This study is to investigate the role for this SNP and its interaction with the TP53 codon 72 SNP in the age of onset of breast cancer.
METHODSWe conducted a case-only study of 1, 110 breast cancer cases. PCR-RFLP was used for SNP genotyping. Ages of onset of breast cancer among different genotypes were analyzed using SAS software.
RESULTSOur analysis revealed that the SET8 CC and TP53 GG genotypes were independently associated with earlier age of onset of breast cancer in an allele-dose dependent manner. Moreover, individuals with both SET8 CC and p53 GG genotypes developed cancer at age of 47.74 years, compared with 54.55 years for individuals with both SET8 TT and TP53 CC genotypes.
CONCLUSIONSmiR-502-binding SNP in SET8 may modulate SET8 expression and contribute to early development of breast cancer either independently or together with the TP53 codon 72 SNP.

9.Long-term home non-invasive ventilation therapy in elderly patients with chronic hypercapnic respiratory failure

Objective To evaluate the effects,safety and economic cost of long-term home noninvasive ventilation (NIV) therapy in elderly patients with chronic hypercapnic respiratory failure.Methods A total of 128 elderly patients with chronic hypercapnic respiratory failure were randomly assigned to two groups:the NIV group (n=66) with conventional therapy in addition to long-term home NIV therapy,and the control group (n=62) with conventional therapy alone.Compared were parameters before and after two year follow up,which included dyspnea grade,scale for accessory muscle use,scoring for emotional disorders,mean pulmonary pressure (mPAP) by electrocardiography,arterial blood gas,the times of pulmonary infection and hospitalization rates,the duration of hospitalization invasive ventilation,the duration of in RICU and in hospital stay,tracheal intubation rates and mortality.The medical cost was calculated.Results After two years,the differences in the dyspnea grade,scale for accessory muscle use,anxiety scores,depression scores,mPAP,arterial PaCO2 and PaO2,hospitalization rates,the times of pulmonary infection,the days of hospitalization for exacerbation in the home NIV group [2.2± 0.2,2.4 ± 0.3,4 ± 1,5.3 ± 1.2,(36.6±5.2)mm Hg,(50.2±4.5)mm Hg,(63.5±4.2)mm Hg,(1.3±0.2) times/year,(2.4±0.2) times/year,(15.8 ± 4.4) days/times] were statistically significant compared to the control group [4.1±0.2,4.9±0.5,12±3,11.3±1.6,(45.2±5.2)mm Hg,(67.3±4.5) mm Hg,(48.3±4.3)mm Hg,(5.4±0.4)times/year,(8.9 ±0.3) times/year,(38.5± 6.3) days/times] (all P<0.01).The duration of invasive ventilation,the days in RICU and in hospital stay,tracheal intubation rates on admission to the hospital were significantly decreased in the home NIV group [(8.2 ± 2.2)days,(9.6±3.1) days,(15.8±4.4) days,(2±0.2) times/two years],as compared with the control group [(15.8±3.4) days,(18.6±4.4)days,(38.5±6.3)days,(8.0±0.8) times/two years].The mortality was decreased significantly in the home NIV group (3.0 %)compared with the control group (29.0%) (P<0.05).The medical cost in two years was significant lower in the home NIV group [(6.4 ± 0.5) thousand yuan],compared with the control group (18.4 ±0.6) thousand yuan (P<0.01).Conclusions Long-term home NIV therapy in patients with chronic hypercapinc respiratory failure is effective,safe and can decrease the mortality and medical cost.
10.Early mechanical ventilation improves the clinical outcome of the patients with severe brain injury

Objective To evaluate the effect of early mechanical ventilation on the clinical outcome of the patients with severe brain injury. Methods In 251 patients sustaining severe brain injuries with Glasgow Coma Scores(GCS)of 5 to 8,early mechanical ventilation was administered in 128 patients,with the other 123 patients serving as the control group.The oxygen saturation(SaO2),oxygen pressure(PaO2),carbon dioxide pressure(PaCO2),GCS score,heart rate(HR),andblood pressure (BP)were measured before and after the ventilation in the two groups,and the prognosis of the patients were evaluated.Results Before the treatment,the two groups showed comparable SaO2,PaO2,PaCO2,BP,HR and GCS scores(P>0.05).Compared with the control group,early mechanical ventilation significantly improved the blood gas parameters 24 h and 7 and 14 days after the treatment. SaO2 and PaO2 showed significant increases(P<0.05)and PaCO2 decreased significantly(P<0.05)after early ventilation,which resulted in no significant changes in the HR and BP(P>0.05). Conclusion Early mechanical ventilation can significantly improve the blood gas parameters and the clinical outcome of the patients with severe brain injury.
