Background High myopia can be corrected using laser therapy and intraocular lens(IOL)implantation.IOL implantation is more suitable than laser therapy for high myopia if the patients suffer from thinner cornea. Objective This clinical trail was to assess and compare the visual quality following phakic iris-claw intraocular lens (IOL) and implantable contact lens(ICL) implantation for high myopia.MethodsIris-claw phakic intraocular lens was implanted in 17 eyes of 10 patients with high myopia, and age-,refractive-matched 17 eyes of 9 patients were in implantable contact lens (ICL) implantation group.Uncorrected visual acuity, best correct visual acuity, contrast sensitivity under the 3,6,12 and 18 c/d spatial frequencies,wave-front aberrations, including RMS3,RMS4 and RMSh,were examined in 6 months after operation.Written informed consent was obtained from each patient before surgery. Results There were no significant difference in uncorrected visual acuity, best correct visual acuity between two groups(t=0.489,P>0.05;t=0.853,P>0.05).Non-glare CS under the 3 c/d,6 c/d,12 c/d and 18 c/d spatial frequencies were insignificantly different between two groups ( t = 0. 906, P > 0.05 ; t = 0. 103, P > 0. 05 ; t =0. 694 ,P>0. 05 ; t = 1. 583, P>0. 05), and the similar outcomes were found in glare CS ( t = 0. 323, P>0.05, t =0.041 ,P>0. 05, t = 0.024, P>0. 05; t = 0. 363, P>0.05) in postoperative 6 months. RMS3 and RMSh were significantly elevated in PIOL group compared with ICL group ( t = 11.40, P< 0. 05 ; t = 11.35, P < 0.05), but no evident difference was found in RMS4 between two groups( t = 0. 24,P>0. 05). Conclusion Both PIOL and ICL implantation for high myopia can achieve an equal visual acuity. But clinical outcomes of ICL implantation is much better than PIOL group. A long-term effect should be observed for the further evaluation.

Background Implantation of phakic posterior chamber intraocular corrective lens(ICL)is a new choice for correction of high myopia.Different from laser assisted in situ keratomileusis, implantation of phakic posterior chamber ICL will allow the good imaging quality because it remaines the matched relationship between cornea and lens. But its visual quality after operation is concerned by patient and ophthalmologists.ObjectiveThis study was to observe the effectiveness of implantation of posterior chamber phakic ICL on visual quality in patients with high myopia. Methods Eighty-four high myopia eyes of 42 patients accepted implantation of posterior chamber phakic ICL and follow-up of 6-month duration. The visual acuity, refractive status, wavefront, contrast sensitivity and accommodation were examined and compared before and after surgery. This clinical study complied with Declaration of Helsinki. The written informed consent was obtained from each patient before operation. Results A prospective observational trial design was used. The uncorrected visual acuity and best corrected visual acuity after operation were better than preoperative ones in all of the patients. The eye numbers of > 0.3 were increased after operation in comparison with before operation with a stable result among 1 day,1 month and 6 months after surgery (x2 =10.70,P>0.05).Spherical equivalent refraction was(-15.38 ± 1.03)D before surgery and(+0.55 ±0.06)at 1 day,(-1.22±0.09)D at 1 month and(-0.68 ± 0.06)D after 6 months, showing a significant difference among them(F=16 559.90,P<0.05).Total aberrations and higher-order aberrations were 11.00±0.25 and 0.43 ±0.05 before surgery, the wavefront aberrations were 2.21 ± 0.56 and 0.47±0.04 at 6 months after surgery with significant difference(t =1.65,P =0.10).Each spatial frequency contrast sensitivity and glare sensitivity on photopic and seotopie conditions in postoperation were higher compared with the preoperative(P<0.05).The accommodation in 1 month and 6 months after surgery was greater than that of preoperative in the patients <-16 D(preoperation:2.75 ± 1.20 ; postoperation 1 month:5.75 ± 1.44,postoperation 6 months:6.00 ± 1.52)(P< 0.05),however,in the >-16 D group,no considerably change in accommodation was seen after surgery(F=1597.70,P<0.05).No significant difference was found in accommodation between before and after surgery in >-16 D group(F=2.67,P>0.05).Conclusions Visual quality of high myopia is obviously improved after implantation of posterior chamber phakic ICL,but its long-term outcome is need to further study.

OBJECTIVETo determine the validity of the pulmonary function equipment.
METHODS12 young students (including six males and six females) were enrolled as our research subjects. And the values of oxygen consumption (VO(2)), carbon dioxide production (VCO(2)) and energy expenditures (EE) of the subjects under three typical activity intensities: resting, moderate intensity (on a treadmill with grade 10% and speed 2.7 km/h) and hard intensity (on a treadmill with grade 10% and speed 5.8 km/h) were measured using the pulmonary function equipment (K4b(2)) and Douglas-bag respectively. And the Douglas-bag method was used as reference and the results were compared with the other method.
RESULTSThe measured VO(2) values by using the Douglas-bag and the pulmonary function equipment under three typical activity intensities were: at rest (0.22 ± 0.03), (0.22 ± 0.05) L/min (t = 0.120, P > 0.05); moderate intensity condition (0.95 ± 0.12), (0.96 ± 0.14) L/min (t = 0.240, P > 0.05); hard intensity condition (1.63 ± 0.28), (1.54 ± 0.35) L/min (t = 1.487, P > 0.05). For VCO(2) values: at rest (0.18 ± 0.02), (0.18 ± 0.04) L/min (t = 0.425, P > 0.05); moderate intensity (0.82 ± 0.11), (0.83 ± 0.13) L/min (t = 0.579, P > 0.05); hard intensity (1.64 ± 0.27), (1.52 ± 0.39) L/min (t = 2.330, P < 0.05). And for EE values, at rest (269.40 ± 35.70), (267.02 ± 55.39) kJ/h (t = 0.200, P > 0.05); moderate intensity (1165.76 ± 148.06), (1185.91 ± 161.89) kJ/h (t = 0.326, P > 0.05); hard intensity (2062.91 ± 341.97), (1912.27 ± 483.88) kJ/h (t = 1.718, P > 0.05) respectively. The results showed that there were no significant differences between the two methods except the VCO(2) values under high intensity condition was underestimated by the pulmonary function equipment. Bland-Altman test showed that the difference of the two methods was evenly distributed by the mean and standard error of the system was 24.7 kJ/h. Our data showed the results from the Douglas-bag and the pulmonary function equipment were consistent.
CONCLUSIONPulmonary function equipment had good validity in assessing the energy expenditure in Chinese adults.

Chinese Medical Journal 2011;124(10):1443-1448
BACKGROUNDPrevious studies have demonstrated that serum uric acid (UA) is an independent predictor of incident type 2 diabetes mellitus (T2DM) in general populations. This study aimed to investigate specific characteristics of UA and its relationship between UA and blood glucose and other risk factors in the Chinese population.
METHODSA total of 946 subjects were included in this study. UA, glucose, insulin, fractional excretion of UA (FEua), creatinine clearance rate (Ccr), hemoglobin A1c (HbA1c), fructosamine (FA), blood pressure and lipids were studied and also reexamined after the patients underwent two weeks of combined therapeutics.
RESULTSUA levels were the highest in subjects with impaired glucose regulation (IGR), followed by subjects with normoglycemia (NGT) and finally by subjects with T2DM. The level of the 2-hour postprandial insulin and the area under the curve for insulin (AUCins) showed a similar tendency. The UA levels initially increased with increasing fasting blood glucose (FBG) and postprandial blood glucose (PPBG) levels, up to 7 mmol/L and 10 mmol/L, respectively, and thereafter decreased at higher FBG and PPBG levels. Compared with subjects in the lower serum UA quartile, subjects in the upper quartile of serum UA levels had higher weights, triglyceride levels, and creatinine levels as well as lower Ccr and FEua levels. Compared with women's group, UA levels were higher, and FEua levels were lower in men's group. Sex, body mass index (BMI), mean arterial blood pressure (MAP), serum triglycerides (TG), FA and Ccr were independent correlation factors of UA. UA decreased and FEua increased after the patients underwent a combined treatment.
CONCLUSIONSUA increased initially and then decreased as glucose levels increased from NGT to IGR and T2DM. Compared with NGT and T2DM, IGR subjects had higher SUA levels, which related to its high levels of insulin. Under T2DM, male gender, BMI, MAP, Ccr, TG and FA are independent correlation factors of UA. Glucose-lowering, antihypertensive, lipemia-regulating combined treatments were of advantage to decline of SUA of T2DM.

Objective: To analyze the change in community first diagnosis and two-way referral rate under the condition of the regional longitudinal medical consortium implementation in the country at large. According to the simulation results, some suggestions for hierarchical medical system were put forward. Methods : The community first diagnosis and two-way referral rates were determined using the literature meta-analysis, and were simulated under different conditions by means of the system dynamics. Results :The analysis results confirmed that simply increasing the financial subsidies can increase the community first diagnosis rate within a range from 0.08% to 0. 17%. For in case increases are made by the government in technical financial input to enhance the technical level of the community health service centers and in compensatory financial input to reduce the higher hospitals dependence on the light disease, results showed that the lower conversion rate increased within the range of 0.33%~0.45% in terms of the two-way referral rating. On the contrary, when technical financial input is increased to improve the technical level of community health service centers, the rate of change was in the range of 7.52% '- 12.61% - Conclusions : Firstly, the diversified strategy should be used to promote the achievement of graded diagnosis and treatment. Secondly, the Health Commission plays a key role in the promotion of themedical consortiumconstruction. Thirdly, medical insurance reimbursement differential strategy should be implemented and the supply-side reform should be strengthened. At last, attention should be paid to the rational development of existing referrals, especially the upper and lower ones.

OBJECTIVETo measure the energy expenditure of reading and watching TV for young women.
METHODSAll 30 girls in the college were selected according to questionnaire, medical examination, blood test, liver function and thyroid gland level. Energy expenditure of reading and watching TV was measured respectively by the Cosmid K4b2 portable gas analysis system from Italy. Basic metabolic rate (BMR) and resting metabolic rate (RMR) were measured as to analyzing results conveniently.
RESULTSThe energy expenditure in reading was (226.35 +/- 56.07) kJ/h and the heart rate was (69 +/- 5) bmp. For watching TV it was (220.79 +/- 65.69) kJ/h and the heart rate was (68 +/- 5) bmp. BMR was (178.53 +/- 53.51) kJ/h and the heart rate was (62 +/- 6) bmp. RMR was (214.76 +/- 44.56) kJ/h and the heart rate was (65 +/- 6) bpm.
CONCLUSIONSThe energy expenditure in reading should be higher than that of watching TV, however, the difference was not obvious. The expenditure rates might get closer to those of RMR. It might be regarded as an expenditure of very light physical active in Chinese.

OBJECTIVETo determine the validity of bioelectrical impedance analysis (BIA) in measuring body composition and to compare with the reference stable isotope dilution method.
METHODSAccording to the data from 150 students of Bethune Military Medical College, 16 female young adults with normal BMI were selected as the participants. The experimental period was 14 days. During the experimental period, the subjects were centralized on management. At the first day of the period, double labeled water was given to the subjects orally. The urine collection time-points were 2h, 4h, 6h, 8h and the 2nd to 14th day after the administration of isotope dose. All isotope measurements were made by using an isotope ratio mass spectrometer. From the isotope curing curve, the indexes of body composition were gained. At the same time, the body compositions were also measured by using BIA method every day (3 hours after dinner). The paired t test and Pearson correlations were used to measure the significance and the degree of correlation between the measurements determined by using the two methods.
RESULTSThere were no significant differences between the measurements determined by using the two methods. TBW, FFM, BF and BF% measured with the BIA method were significantly correlated with those measured by using the isotope method (r = 0.556, 0.556, 0.817 and 0.606 respectively).
CONCLUSIONBIA might provide an accurate and reliable assessment in body composition of the healthy Chinese adults.
