1.Relationship between postoperative intra-abdominal hypertension and fluid management during liver transplantation

Objective To study the correlation between fluid management during liver transplantation and postoperative intra-abdorninal hypertension (IAH),and the correlation between intra-abdominal pressure and hemodynamics during liver transplantation.Method From Sep.2008 to Sep.2014,95 cases were admitted to ICU following liver transplantation were enrolled.All recipients were given abdominal color ultrasound examinations,and the IAH was measured.The preoperative Child-Pugh score,total operating time,anhepatic phase time,intraoperative blood loss,crystal solution input,colloidal fluid input,red blood cell infusion,plasma infusion,intraoperative total fluid input,intraoperative fluid input per h,and urinary volume per h and their correlation with postoperative IAH were analyzed by the Logistic regression method.The relationship between hemodynamics data and IAH intra-abdominal pressure was analyzed by the correlation analysis of two variables.Result IAH occurred in 18.94% (18/95) recipients at 72nd h after operation.Fluid input per h (>2000 ml/h) was risk factor (B=1.62;P<0.05;OR=5.07,95% CI:1.41-18.23) of IAH.Urinary volume per h (<200 ml/h) is risk factor (B=-3.21 ;P<0.01:OR =0.04,95% CI:0.01-0.18) of IAH.There was correlation between hepatic artery peak flow velocity (r =0.83,P<0.01),portal vein peak flow velocity (r =-0.182,P<0.05),and retrohepatic inferior vena cava peak flow velocity (r=-0.184,P<0.05) with IAH.Conclusion Fluid input per h should be controlled in a low level and urinary volume per h should be increased in order to prevent IAH.There is correlation between hepatic artery peak flow velocity,portal vein peak flow velocity and retrohepatic inferior vena cava peak flow velocity with IAH.
2.Correlation study on angiotensin Ⅱ type 2 receptor gene single nucleotide polymorphisms in essential and renal hypertension

Objective To study the relationship between angiotensin Ⅱ type 2 receptor (AGTR2) gene single nucleotide polymorphisms and hypertension in patients with essential hypertension and renal hypertension.Methods Direct DNA sequencing was performed to detect the single nucleotide polymorphisms (SNP) in eighty patients with essential hypertension,eighty patients with renal hypertension and forty normal blood pressure controls.Results There was significant difference on A allele frequencies between essential hypertension group and control group ( 56.88% [ 91/160 ] vs 30.00% [ 24/80 ],x2 =15.44,P < 0.001 ).A allele frequency had no correlation with renal hypertension (42.50% [ 68/160 ] vs 30.00% [ 24/80 ],x2 =3.52,P >0.05).There was no significant difference on SNP between essential hypertension group and renal hypertension group ( P > 0.05 ).Conclusion A1675G signal nucleotide polymorphisms of AGTR2 may be associated with the development of essential hypertension,but has nothing to do with renal hypertension.
4.Effects of nano porous beta-tricalcium phosphate/collagen scaffold modified with human bone morphogenetic protein 2 gene on differentiation of MC3T3-E1 cell lines

BACKGROUND:Bone tissue transplantation or osteogenic material fil ing is after used for bone defect repair. To remove autologous bone tissues can lead to additional damage and secondary deformity, therefore, it is extremely urgent to search for a new osteogenic material. OBJECTIVE:To construct the porousβ-tricalcium phosphate (β-TCP)/col agen scaffold modified with human bone morphogenetic protein 2 (hBMP2) gene, and to observe its effects on differentiation of MC3T3-E1 cel lines. METHODS:The porousβ-TCP/col agen scaffold modified with hBMP2 gene was prepared. Then in vitro culture system of MC3T3-E1 cel lines with composite scaffold was established. There were scaffold and plate groups, and each group was divided into two subgroups according to the different concentrations of plasmid. Samples were col ected and observed morphological y by scanning electron microscope and light microscope after complex culture. After 1, 3, 7 and 14 days of induction, calcium nodules were observed through alizarin red staining, the cel cycle was detected by real-time PCR, and expressions ofαI-chain col agen type I gene, Osterix and bone sialoprotein were observed. RESULTS AND CONCLUSION:The number of cel s adhered, differentated and distributed on the composite scaffold was significantly higher than that of the single scaffold (P<0.05). Alizarin red staining and real-time PCR detection showed that the osteogenesis ability of MC3T3-E1 cel lines in the scaffold group was stronger than that in the plate group. To conclude, the porousβ-TCP/col agen scaffold modified with hBMP2 gene is an appropriate candidate for bone defect repair.
5.Relationship between Activity of Thorax and Spinal Motor Ability in Patients with Ankylosing Spondylitis

Objective To study the relationship between activity of thorax and each spinal intervertebral angle in patients with ankylosing spondylitis. Methods Each spinal intervertebral angle of 41 patients with ankylosing spondylitis were measured by Spinalmouse in different postures. And the activity of thorax was measured. Correlation between activity of thorax and shape of spinal were analyzed. Results The activity of thorax positively correlated with the entire lumbar spinal column in flexion (P<0.01), as well as the intervertebral angle of L1/2, L2/3 and L4/5 in flexion (P<0.05), but negatively correlated with the intervertebral angle of L1/2 and L2/3, curvature of the entire lumbar spinal column in upright and the intervertebral angle of L1/2, L3/4, curvature of the entire lumbar spinal column in extension. Conclusion There was a significant relation between activity of thorax and lumbar vertebra motor ability.
6.Clinical examples of professor LI Zhi-dao's "tonifying three qi" acupuncture method.

Professor LI Zhi-dao, according to acupoint selection of syndrome differentiation in TCM basic theory, concluded a new therapy, namely "tonifying three qi" that is mainly based on three acupoints in the Conception Vessel. This method is consisted of Danzhong (CV 17), Zhongwan (CV 12) and Qihai (CV 6) in the Conception Vessel, which could successively nourish clear qi, stomach qi and original qi. In clinic, according to the severity of symptoms of three qi, the acupoints are selected flexibly, which could respectively treat deficiency of heart-lung qi, deficiency of stomach-spleen qi and deficiency of original qi. Some examples are also given in the article.

7.Postoperative two hemodynamics management strategies on the early impact of liver transplantation recipients

Objective To investigate early postoperative impact on recipients after liver transplantation with the two hemodynamic management strategies by using the "gold standard" Swan-Ganz catheter and central venous catheter (CVC) to monitor hemodynamics and guide liquid and vasoactive drug therapy.Methods From May 2006 to January 2009,102 from 107 cases of liver transplantations in our hospital were randomly divided into two groups:PAC group (pulmonary artery Swan-Ganz catheter group,n = 52) and CVC group (n = 50 cases).Hemodynamic parameters were monitored.Results There was no significant difference in the general characteristics between two groups In PAC group and CVC group,the mortality during ICU stay was 7.7 % and 4 %; postoperative 28-day mortality was 11.5 % and 8 % ; the median mechanical ventilation time was 26.5 and 24 h; the median ICU stay was 2.5 and 2 days,respectively.There was no significant difference in the aforementioned parameters (P>0.05).There was no significant difference in renal function and liver function before and 1,5 days after operation,and mean blood lactic acid level 1,2,and 3 days after operation between two groups (P>0.05).In PAC group and CVC group,transient ventricular arrhythmias occurred in 26 cases and 2 cases Conclusion It is not a good strategy using "gold standard" Swan-ganz catheter to monitor hemynamic parameters and guide therapy after liver transplantation in ICU,and on the contrary,the application of "gold standard" Swan-ganz catheter increases incidence of transient ventricular arrhythmias.
8.Relative bioavalability of hydrochloride eperisone granule in healthy volunteers

Aim The relative bioavalability of hydrochloride eperisone granule in 10 healthy volunteers was studied. Methods The time-plasma concentrations of hydrochloride eperisone granule, as test drug, and myonal, as reference drug, were determined by GC-MS, with tolperisone senuing as internal standard.The pharmacokinetic parameters of both reference and test drug were calculated and analyzed with two-one side test and confidential interval test. Results The results showed that the AUC0-8, AUC0-∞, Cmax, Tpeak, t1/2(?) and t1/2(?) were (17.9?1.3)ng?h?ml-1 and(18.6?1.6)ng?h?ml-1, (19.1?1.2)ng?h?ml-1 and (20.2?1.6)ng?h?ml-1, (5.2?0.5)ng?ml-1 and (5.4?0.5) ng?ml-1, (1.05?0.18)h and (1.08?0.23)h, (0.78? 0.13)h and ( 0.82?0.14)h,( 1.8?0.3)h and (1.8?0.3)h, respectively. The relative bioavalability of test drug was (105? 5)%. Conclusion It can be concluded that the test and reference are bioequivalented between individuals, preparations and periods.
9.Literature study on species of honeysuckle flower.

Honeysuckle flower is a traditional herbal medicine in China Through systemically sorting and studying literature of Chinese medicine, this article pointed out that leech used by the traditional Chinese medicine in ancient time has the features of twist vine, slight purple stem with clothing hair; opposite growing leaves, ovule shape with clothing hair on both side; two flowers growing from one pedicel, labiate corolla with 3.2 cm longth, flower grows from white color to yellow color, each branch axil grows only one pedicel, the involucre is ovoid shape, and the flower season is from mid-March to mid-May. Among all species of caprifoliaceae, only Lonicera japonica Thunb. meets these botanic features. Therefore, L. japonica Thunb. should be used as the orthodox species of herbal honeysuckle flower.

10.Effect of emulsified isoflurane anesthesia on expression of hippocampal amyloid-beta protein and phosphorylation of Tau protein in rats

Objective To investigate the effect of emulsified isoflurane anesthesia on the expression of hippocampal amyloid-beta protein (Aβ) and phosphorylation of Tau protein in rats.Methods Seventy-two healthy Sprague-Dawley rats,aged 8 weeks,weighing 250-300 g,were randomly divided into 3 groups:normal control group (group C,n =12),fat emulsion group (group E,n =12),and 8% emulsified isoflurane group (group EI,n =48).30% fat emulsion and 8% emulsified isoflurane 0.15 ml/100 g were slowly injected via the tail vein in groups E and EI,respectively.Morris water maze test was performed 6 days before anesthesia.Twelve animals in each group were chosen at 2 h after administration (T1) in E group,at the corresponding time points in C group,or at T1 and 1,7 and 14 days after administration (T2-4) in EI group and underwent spatial probe tests,and the escape latency was measured.The rats were anesthetized with intraperitoneal 1% pentobarbital sodium 4 g/100 g after the end of Morris water maze test.Blood samples were taken for determination of the plasma S100 protein concentration.The rats were then sacrificed and brains were removed for determination of the expression of hippocampal Aβ and phosphorylated Tau (p-Tau) protein by immunohistochemistry.Results The escape latency and swimming distance were significantly shorter on 3rd,4th and 5th days than those on 1st day of place navigation test before anethesia (P < 0.01).Compared with group C,the escape latency and swimming distance were significantly prolonged and the time of staying at the original platform quadrant was shortened at T1,and the plasma S100 protein concentration was decreased at T4 in group EI (P < 0.05),and no significant change was found in the each parameter of Morris water maze test,plasma S100 protein concentration,and expression of Aβ and p-Tau protein in group E (P > 0.05).The escape latency and swimming distance were significantly shorter and the time of staying at the original platform quadrant was longer at T2-T4 than at T1 in group EI (P < 0.05).Conclusion Emulsified isoflurane anesthesia exerts no effect on the expression of hippocampal Aβ and phosphorylation of Tau protein,indicating that hippocampal Aβ and Tau protein are not involved in emulsified isoflurane anesthesia-induced cognitive dysfunction in rats.
