Objective To understand serological detection results of syphilis among hospitalized patients in a gen-eral hospital,and provide new ideas for further prevention and treatment of syphilis. Methods Clinical data of inpa-tients with abnormal serological detection results for syphilis in a hospital from January 2012 to December 2013 were analyzed. Results A total of 164442 patients were admitted to a hospital from January 2012 to December 2013, 112576 of whom were performed syphilis serological screening,2048 cases were with abnormal results of serologi-cal detection for syphilis. The abnormal serological detection results were mainly in patients of 31~ and 41~ years (39.06% ),followed by patients of 51~,61~ years (34.42% ),and≥71 years (15.63% );unemployed people (34.08% ),farmers(23.05% ),and retirees(19.19% )were the main population with abnormal results. The abnor-mal serological detection results distributed in all 36 departments,the main departments were departments of respir-atory medicine(9.86% ),gynaecology(7.13% ),and cardiovascular internal medicine(6.88% ). Non-marital sex is the main route of transmission(56.79% );the main syphilis serological detection results were both positive for Treponemapallidum particle agglutination (TPPA)assay and rapid plasma reagin (RPR)(46.14% ),as well as TPPA positive and RPR negative (43.31% ). 860 (41.99% )patients were with latent syphilis. Conclusion The current status of patients with abnormal serological detection results of syphilis is not optimistic, mainly concentrated in patients of 31-50 years. It is necessary to strengthen publicity and education on prevention and treatment of syphilis,implement syphilis prevention and treatment policy,intensity syphilis screening,so as to pre-vent the epidemic and spread of syphilis.

ObjectiveTo evaluate the risk of colorectal neoplasia among patients with gastric or duodenal adenoma.MethodsWe retrospectively reviewed 39 patients with gastric or duodenal adenomas between 2007 and 2011 as the study group.A total of 78 patients without upper gastrointestinal adenomas who underwent colonoscopy were included as a control group.Colonoscopic findings were compared between the two groups.ResultsPositive finding rate of colorectal neoplasia in study group was 51.3％ (20/39),with adenomas in 38.5％ (15/39) and cancer in 12.8％ (5/39),which were all significantly higher than those of the control group,14.1％ (11/78) ( P ＜ 0.001 ),12.8％ ( 10/78 ) ( P ＜ 0.05 ) and 1.3％ ( 1/78 ) ( P ＜ 0.05),respectively.The results were similar when upper digestive tract was further divided into stomach and duodenum(50.0％ vs.12.5％ and 52.2％ vs.15.2％ ).ConclusionPatients with gastric or duodenal adenomas are at a significantly higher risk for colorectal neoplasia.Colonoscopy should be recommended for these patients.

OBJECTIVE:To optimize the extraction technology of Tibetan medicine Pedicularis kansuensis and compare con-tent of verbascoside and isoverbascoside differences in P. kansuensis from various habitats. METHODS:Using verbascoside and iso-verbascoside and dry paste yield as comprehensive evaluation indexes,single factor test and orthogonal test were used to investigate the extraction solvent,solvent dosage,extraction time and times to optimize extraction technology,and the verification test was conducted. Contents of the 2 constituents verbascoside and isoverbascoside in P. kansuensis from Gansu,Qinghai and Sichuan were compared. RESULTS:The optimal extraction technology was as follows as 8-fold 50% ethanol,extraction for 3 times,90 min each time. The verification results showed that the average contents of verbascoside and isoverbascoside were 3.49%(RSD=1.28%,n=3),1.26%(RSD=1.32%,n=3),and average dry paste yields were 37.99%(RSD=1.97%,n=3). The contents of verbascoside and isoverbascoside in P. kansuensis from Qinghai were relatively higher. CONCLUSIONS:Optimized extraction tech-nology is reasonable,stable,feasible;the contents of index constituents in P. kansuensis from different habitats have certain differ-ences. The study can provide scientific evidence for the development and utilization of extraction,and the in-depth study of quality evaluation for medicinal material.

AIM: To construct the replicative defecient adenovirus Ad-Runx3 expressing Runx3, and to express it in U251 malignant glioblastoma cells. METHODS: The runx3 gene with a flag tag was amplified by PCR using pCMV5-AML2 as a template, and was confirmed by DNA sequencing. The adenovirus shuttle vector pShuttle-CMVRunx3 was constructed by introducing runx3 DNA fragment into the sites of Kpn Ⅰ and Xho Ⅰ of pShuttle-CMV vector. This recombinant plasmid was linearized by Pmel and electronically transfected into BJ5183 cells to get the recombinant adenovirus vector Ad-Runx3. The recombinant adenovirus expressing Runx3 was infected into U251 malignant glioblastoma cells. The expression of exogenous Runx3 was observed by immonoblotting and its localization was detected by immunostaining using anti-Flag tag antibody. RESULTS: The recombinant adenovirus expressing Runx3 with a Flag tag was constructed and infected into U251 glioblastoma cells. The exogenous Runx3 protein was detected only in the nuclei. CONCLUSION: The recombinant adenovirus expressing Runx3 with a Flag tag is constructed successfully, and the Runx3 protein expressed in the nuclei of infected cells. The study laid a foundation for further research of the function of Runx3 in gliocarcinogenesis.

Objective To investigate the etilolgy of failed pyeloplasty in children and to study the feasibility of redo laparoscopic surgery for recurrent nreterupelvic junction obstruction.Methods The clinical data of 39 patients with recurrent ureteropelvic junction obstruction after the primary pyeloplasty underwent redo laparoscopic surgery were analyzed retrospectively between September 2009 and June 2016 in our institution.There were 31 males and 8 females with a mean age of 66 months,ranged from 4 to 204 months.28 patients had left obstructions and 11 had fight obstructions,who were diagnosed by ultrasonography,MRU,and MAG3 renal scan et a1.Under general anesthesia,we identified that two patients (5.1％) had renal calices strictures,four patients (10.3％) had unsolved disease ureter and anastomotic strictures,two patients (5.1％) had adhesion band and anastomotic strictures,one patient had adhesion band,high ureteropelvic anastomosis and ureter torsion,and 30 (76.9％) patients had anastomotic strictures.Thirty-three patients underwent cdismembered pyeloplasty,five patients underwent onlay appendiculoureteroplasty and one patient underwent ureterocalicostomy.Results All surgeries were successfully completed without conversion.No intraoperative complication was encountered.One patient had persistent,frequent and intolerable flank pain with severe hydronephrosis after surgery,he subsequently underwent nephrectomy.One patient had persistent severe hydronephrosis which was repaired with on-lay appendiculoureteroplasty.One patient had anastomotic adhesion and balloon dilatation was performed with no further obstruction in follow up imaging.The rest of the patients got complete clinical or radiologic resolution.The successive rate was 36 of 39 (92.3％) at a mean follow up of 25 months (ranging 3-60 months).Conclusions Unsolved ureteric pathologies,anastomotic stricture,renal calices stricture,adhesion band,torsion of ureter and high ureteropelvic anastomosis all contribute to recurrent obstruction after the primary pyeloplasty.Laparoscopic redo pyeloplasty is safe and feasible for skilled surgeons.

Objective:To explore the clinical efficacy and surgical indications of Y type osteotomy in the treatment of post-tuberculous thoracolumbar severe angular kyphosis.Methods:From March 2012 to June 2018, 36 patients with post-tuberculous thoracolumbar severe angular kyphosis were treated with Y type osteotomy, including 22 males and 14 females, aged 23.6±5.7 years (range, 7-57 years). The parietal vertebrae of kyphosis were located in the upper thoracic vertebra in 3 cases, the thoracic vertebra in 11 cases, the thoracolumbar segment in 17 cases, and the lumbar vertebra in 5 cases. The Cobb angle of kyphosis before the operation was 92.8°±23.3° (range, 60°-147°). The visual analogue scale (VAS), American Spinal Injury Association (ASIA) neurological function grade, and Kirkaldy-Willis function score were used to evaluate the clinical effect. The imaging evaluation indexes were interbody kyphosis angle and spinal bone fusion.Results:The operation was successful in all the 36 patients. The operation time was 210 ±25.9 min (range, 180-270 min), the intraoperative blood loss was 520 ±110 ml (range, 400-800 ml), and the postoperative follow-up time was 26.38±1.75 months (range, 22-30 months). The postoperative kyphosis Cobb angle was corrected to 16.5°±7.7° (range, 5°-35°), which was significantly improved compared with that before operation( t=25.438, P<0.01), and the correction rate was 82.2%. At the last follow-up, the kyphosis angle was 16.5°±7.1° (range, 6°-32°), which was not significantly different from that after the operation. The preoperative VAS score was 7.3±1.8 (range, 3-9), and the postoperative VAS score was 2.4±0.8 (range, 1-3), while the improvement rate was 67.1%. At the last follow-up, it was 1.1±0.6 (range, 0-2), and the improvement rate was 85.0%. According to the Kirkaldy-Willis functional score, the results were excellent in 25 cases, good in 8 cases, and fair in 3 cases at the last follow-up, with an excellent and good rate of 91.7%. Before the operation, 9 cases were accompanied by neurological dysfunction (ASIA grade: grade C in 2 cases, grade D in 7 cases). At the last follow-up, all the 9 patients recovered to grade E. During the operation, the electrophysiological nerve monitoring was abnormal in 2 patients, and the awakening test was negative in 1 case. In another patient, neuroelectrophysiological monitoring after posterior column osteotomy showed a decrease in bilateral sensory and motor function. There was no compression around the spinal cord in the osteotomy area, so the operating bed was gradually folded and partially restored to kyphosis and temporarily fixed with double rods. Neuroelectrophysiological monitoring suggested the recovery of nerve function. The awakening test showed that the nerve function of both lower limbs recovered close to the preoperative state, and further osteotomy and internal fixation was performed 2 weeks later. The nerve function of both lower limbs returned to normal after 3 months. After the operation, one patient's muscle strength of the lower limbs decreased from grade 5 to grade 3, and the sensory function was normal. After symptomatic support treatment such as neurotrophic drugs, it returned to normal 2 weeks later. 1 case developed delayed neurological dysfunction 1 year after the operation. Neurotrophic drugs and rehabilitation treatment improved it. The sinus of the incision was formed in one case 3 months after the operation and healed after debridement and suture. Conclusion:Y typeosteotomyis a safe and effective method for patients with post-tuberculous thoracolumbar severe angular kyphosis. Compared with traditional osteotomy, anterior support bone grafting can be avoided, and spinal shortening can be reduced.

Objective To study the function mechanism of the Sinisanon on hippocampal molecular expression in rats with post-traumatic stress disorder.Methods Fifty SD rats were divided equally into 5 groups:blank control group,model group,negative control group group,positive control group and experimental group.The control blank group did not copy the model,do not receive treatment,the normal feeding.Model group,repetitive post-traumatic stress disorder model was induced by current stimulation,but not treated.Negative control group,equal volume of 0.9％ NaCl.Positive control group (paroxetine hydrochloride 0.42 mg· mL-1) and experimental group (Sinisan,Containing crude drug 0.24 g · mL-1).It given to drugs one hour before the model establishment.The rats were administered with 10 mL · kg-1,once a day,for a total of 7 d.At the end of 5 hours after the last administration,the hippocampus tissues were collected.Fingerprints were used to compare the difference peaks of hippocampus tissue in the rats with post-traumatic stress disorder.High performance liquid chromatography conditions:the gradient elution.The chromatograph condition:Agilent HC-C1s (4.6 mm × 250 mm,5 μm),speed of flow 0.8 mL · min-1;flowing A is methanol,flowing B is acetonitrile,flowing C is 0.05％ phosphorus acid water;wavelength of detection 260 nm;column warm 23 ℃.Results Compared with the blank control group,the model group is increased by 2nd,3rd,4th,6th and 7th chromatographic peak.Compared with the blank control group on peak area of 1 th (220.40 ± 9.25) mAU × min,5 th (86.27 ± 7.39) mAU × min,peak area in model group 1th (82.50 ± 9.60) mAU × min were significantly decreased (P ＜ 0.05),5th peak area (123.07 ± 9.28) mAU × min was significandy higher (P ＜ 0.05).Compared with model group,the peak area at the time of 1th (95.50 ±9.89) mAU ×min,5th (119.30 ±9.93) mAU ×min in negative controlgroup,it did not change significantly from chromatographic peak (P ＞ 0.05).Compared with negative control group,positive control group and experimental group disappear by 2nd,3rd,4th,6th and 7th peak,and 1th (230.42 ± 11.21),(234.80 ± 12.47) mAU ×min peak area was significantly higher (P ＜0.05) while 5th (71.29 ± 1.82),(72.16 ±2.55) mAU × min peak area were significantly decreased (P ＜ 0.05).Compared with positive control group,experimental group increased by 8th and 9th,it hint the two peaks may be chemical constituents or metabolites in the organism of Sinisan.Conclusion The Sinisan have significant positive effect on hippocampal molecular expression.

By using huge primer PCR Cys86 (TGC) of PoIFN-alpha was mutated to Tyr(TAC), and the first code TGT was simultaneously changed to TGC, which is a bias code of E. coli. The expression plasmid pGEX-IFN was constructed successfully. Recombinant porcine IFN alpha, which is expressed as inclusion bodies, was about 20% of the total proteins. The inclusion body was dissolved in 8 mol/L urea and subsequently renatured by dilution in refolding buffer. In order to obtain pure protein, the renatured IFN alpha was purified by FPLC, and the cytokine activity (5200 IU/mg) was verified by inhibiting the cytopathic effect.
