2.Characteristics of hospitalized patients with anaphylaxis during different eras at Peking Union Medical College Hospital

To analyze the incidence,clinical manifestations and causes of 42 in patients with anaphylaxis from 1990 and follow its change before and after 2005 retrospectively.The mean age was (39 ± 16) years.The ratio of male and female was 1:2.2.The incidence increased obviously after 2005 (30 vs.12).A majority of patients were females after 2005 (24 vs.5).The proportions of patients with impaired nerve system,circulatory system and hypotension decreased after 2005 (P < 0.01,P < 0.05,P < 0.01).Anaphylaxis attacked more often intra-operatively after 2005 (43.3 % vs.1 / 12).The most common cause was drug(35/42),especially antibiotics(14/35).

Data mining, as known as knowledge discovery in databases, is a non-trivial process of revealing the implied, previously unknown and potentially useful information from the massive data. In recently years, the applications of data mining in the field of pharmaceutical research of traditional Chinese medicine have widespread. Especially in the field of the heritage of experiences of na-tional medical masters, data mining plays an important role. In this study, we would expound of the use of methods of data mining in the heritage of experiences of national medical masters, and analyze their advantages and disadvantages, such as association rules, Bayesian networks, neural networks, and decision trees.

4.Changes of cortical thickness and cortical surface area in patients with bipolar depression

Objective To investigate the changes of cortical thickness and surface area in patients with bipolar depression(BD),and to explore the relationship between abnormal changes in gray matter and clinical symptoms.Methods 28 BD patients and 28 healthy controls underwent T1-weighted MRI.The Freesurfer software was used to process the T1 images,which used a set of automated sequences to analyze cortical thickness and surface area on 66 regions (33 regions of each hemisphere),and the correlation with clinical features was also calculated.Results Compared with controls,BD patients showed thinner cortical thickness in left medial orbitofrontal cortex((2.40±0.12) mm vs (2.55 ±0.18) mm,P=1.2× 10-3) and left rostral anterior cingulate((2.66±0.21) mm vs (2.88±0.27) mm,P=3.1 × 10-4),and smaller area of left cuneus((1 443.13± 131.00) mm2vs (6 634.70±600.16) mm2,P=2.7× 10-4) and right superior frontal gyrus ((6 634.70±600.16) mm2vs (7 300.50±653.39) mm2,P=1.3× 10-3).In addition,the negatively correlation was found between the cortical area of left cuneus and effective illness duration (r=-0.471,P=0.018),and the cortical thickness in left rostral anterior cingulate and total score of HAMD-17(17-item Hamilton Rating Scale for Depression) (r=-0.508,P=0.009).Conclusion There are abnormal altertion of cortical thickness and cortical areas of emotional circuit in bipolar depression,but the brain areas are not completely overlapping.Correlation analysis suggests that cortical thickness and area is related to different clinical features.
5.Correlations of pulp chamber height and dentine thickness of mandibular first molar with age in Chinese Han population based on cone beam computed tomography

Objective:To investigate the relationship between pulp chamber height and dentine thickness of mandibular first molar on long axis with age in a Chinese Han population by cone-beam computed tomography(CBCT).Methods:CBCT images of 420 Chinese Han subjects(210 males and 210 females) were divided into 6 age groups as follows:15-24,25-34,35-44,45-54,55-64 and 65-74 years(n=70,35 males and 35 females).The height of pulp chamber and dentine thickness of the chamber root and floor of mandibular first permanent molar on long axis was measured by Galileo software of CBCT in vertical direction and then statistical analysis was carried out.Results:On long axis,the height of pulp chamber was (0.85±0.54) mm in males and (0.79±0.51) mm in females(P>0.05);dentine thickness of the pulp chamber roof was (3.60±0.49) mm in males and (3.50±0.49) mm in females(P>0.05);and dentine thickness of the pulp chamber floor was (2.57±0.52) mm in males and (2.49±0.5) mm in females(P>0.05).With aging,the height of pulp chamber was decreased(P<0.05) and dentine thickness of mandibular first permanent molar on long axis was increased(P<0.05).Conclusion:In Chinese Han population,the of pulp chamber height of mandibular first permanent molar on long axis is related to age,The dentine thickness increased with ages,the increase of dentine thickness of pulp chamber floor is more significant than that of the roof.
6.Quantitative identification of TCM Q-markers based on analytical hierarchy and the entropy weight comprehensive method by taking Shaoyao Gancao decoction as an example

The quality markers (Q-markers) of traditional Chinese medicine (TCM) have become a topic of interest in TCM research in recent years. Nonetheless, there is still no consensus on how to scientifically characterize TCM Q-markers. Our study establishes an identification method for TCM Q-markers based on the analytical hierarchy process (AHP) and the entropy weight comprehensive method. By constructing an evaluation system encompassing the target layer, the factor layer and the control layer, AHP can be used to analyze the weight of three core TCM quality attributes, including effectiveness, testability and specificity. Following that, the entropy weight method is employed to analyze the specific indicators for each attribute based on the literature and experimental data. Finally, the comprehensive weight of each index is obtained by combining the two weights, and the comprehensive weight and the specific value of each component is multiplied and summed to obtain the integrated score ranking, and thereby identify the TCM Q-markers. Taking Shaoyao Gancao decoction as an example, the analysis revealed that the top 8 components are as follows: paeoniflorin > quercetin > albiflorin > glycyrrhizic acid > naringenin > liquiritin > oxypaeoniflorin > benzoylpaeoniflorin, and can be identified as Q-markers of Shaoyao Gancao decoction. This study not only provides support for the establishment of quality standards and process quality control of TCM formulae, but also provides innovative ideas and methods for quantitative evaluation and accurate identification of TCM Q-markers.
7.Fresh frozen plasma for the treatment of hereditary angioedema acute attacks.

OBJECTIVETo determine the safety and efficacy of fresh frozen plasma (FFP) infusion for the treatment of hereditary angioedema (HAE).
METHODSThe medical records of patients with HAE admitted to Peking Union Medical College Hospital who had received FFP infusion during 2004 and 2010 were reviewed and PubMed database from 1966 to the present were searched using the following hereditary angioedema and fresh frozen plasma. The patient's age, sex, body location of HAE attacks, the dose of FFP infusion, time of beginning to improvement, time to complete remission, complication, C1 inhibitor activity, and outcome were analyzed.
RESULTSA total of 13 enrolled patients (7 male and 6 female) received 16 times of FFP infusion, including 2 patients undergoing FFP infusion in Peking Union Medical College Hospital and 11 patients reported in the literature. The mean dosage of FFP infusion was 586∓337 mL. Two cases suffered from worsening abdominal pain and one case experienced skin rash. Only 1 patient had no improvement in symptom owing to transfusion related reaction. There was a definite improvement in symptom 49∓19 minutes after beginning FFP infusion. The remission time decreased from 61.7∓27.0 hours to 3.3 (2.0, 12.0) hours after FFP infusion. FFP infusion was effective for both type I and type II HAE.
CONCLUSIONFFP seems to be safe and effective for acute attacks of HAE.

9.Study on vascular morphology of integument tissues of the channel area in the leg.

OBJECTIVETo observe the distribution of the blood vessels in the integument tissue of the channel area of legs.
METHODSThe integument tissue of the lower limbs in the 12 cadavers were dissected with macro-and micro-dissection, radiographical technique of systemic artery and technique of image pattern analysis to observe and analyze the origins, branches and anastomoses in the integument tissues along the channels of legs.
RESULTSThe distributional density of the blood vessels in the integument tissues of legs along the channel area of the three-yin meridians of the foot, the Gallbladder Meridian, and the Urinary Bladder Meridian was higher than that in the other parts. They formed an obvious nutrient vascular chain on the arteriogram. The distributional density in the channel area of the Stomach Meridian was not obviously increased and the obvious nutrient vascular chain could not be seen.
CONCLUSIONAn obvious nutrient vascular chain is formed in the integument tissue along the channel area of legs except the Stomach Meridian.

10.Clinical and pathological analysis of oral manifestations of 40 patients with secondary syphilis.

OBJECTIVETo analyze the clinical and pathological features of 40 patients with secondary syphilis.
METHODSA total of 40 cases of secondary syphilis confirmed by serology were collected during 1994-2004 and were first diagnosed on presentation with oral lesions.
RESULTSThe white patch in oral mucosa was found in 32 cases with painless or slight pain in most cases. The most common site of the lesion was the tongue. The histological examination on eight cases was initially misdiagnosed as oral candidosis or lichen planus, but confirmed as syphilis after serology revealed nonspecific inflammation with intraepithelial microabscess and dense perivascular infiltration of lymphocytes and plasma cells in connective tissue. The symptoms showed dramatic improvement in 16 cases after benzathine penicillin treatment.
CONCLUSIONSThe oral manifestations of syphilis have specific clinical and pathological feature and attention should be paid to the suspicious oral lesions when patients are first presented in a dental office.
