After a description of the combined teaching-oriented and learning-oriented teaching model of medical information retrieval course, the results it achieved were investigated with questionnaires and its advantages were summarized from the aspects of national conditions, information literacy training and medical education.

Objective To design the meta-data scheme for medical records database of traditional Chinese medi-cine.Methods The meta-data scheme for medical records database of traditional Chinese medicine was designed u-sing the MARC meta-data according to the analysis of resources, investigation of literature, networks and users. Results The meta-data scheme for medical records database of traditional Chinese medicine is consisted of 17 fields and 9 parts.Conclusion MARC, as the mata-data criteria, can organize and describe the medical records of tradi-tional Chinese medicine.However, the standards for selection and description of meta-data criteria need to be fur-ther studied.

Objective To explore the effect of intracoronary injection of epinephrine and Vera Pammy in patients with myocardial infarction with slow blood flowduring percutaneous coronary intervention (PCI).MethodsFrom January 2013 to October 2016, 68 patients with acute myocardial infarction with solw blood flow in Shanghai Jiading Hospital of Traditional Chinese Medicine during PCI were randomly divided into the epinephrine group(37 cases) and the verapamil group(31 cases).The two groups were treated with conventional drugs before surgery.The epinephrine group were injected into the coronary artery of 50 micrograms of epinephrine, the Vera Pammy group were injected into 200 micrograms of Vera Pammy.The coronary angiography, blood flow dynamics, as well as biochemical indexes in the two groups were observed.ResultsAfter the initial medicine, the ratio of TIMI flow grade 3 in the epinephrine group 81.80% was significantly higher than that in the Vera Pammy group 64.52%(P<0.05).The ratio of TIMI flow grade 3 after three times treatment, CTFC value after initial and 3 times treatment between the two groups have no significance.The blood pressure in the epinephrine group was (82.67±10.87) mmHg, which was significantly higher than that in the Vera Pammy group (70.46±9.54) mmHg(P<0.05).NT-proBNP, cTnT, LVEF in the two groups have no significance.hs-CRP in the epinephrine group was (7.68± 0.76) mg/L, which was significantly lower than that in the Vera Pammy group (9.25±1.05) mg/L.ConclusionThe effect is different which epinephrine and Vera Pammy were used in the patients with myocardial infarction with slow blood flowduring percutaneous coronary intervention.The use of epinephrine can significantly increase blood pressure, and the effect of initial medication is better.

Studies show that a high incidence of ED is associated with chronic diseases of various systems, and its pathogenesis is not fully understood. Objective To evaluate the clinical features,pathological grades,treatment and prognosis in bladder cancer patients under 40 years.Methods A retrospective review of transitional cell carcinoma of the bladder in patients under 40 years who had been treated from January 1994 to April 2005 were conduc- ted.The patients were divided into 2 groups(group A,20-30 years;group B,31-40 years)based on their age.The differences in pathologic grading,recurrence rate and positive rate of urine cytology were compared between the 2 groups.The statistical analyses were performed using x~2 test.Results The incidence of bladder cancer in patients under 40 years was 4.2%(92/2200).The male/female ratio was 2.7:1.0.At initial visit,86%(68.7% in group A and 91.1% in group B)of the patients presented with gross hematuri- a;and 25.0% in group A and 33.9% in group B concomitantly had frequency and dysuria.The mean disease course in the 2 groups was 3.8 months for male and 6.9 months for female.Solitary tumor occurred in 19 ca- ses(100.0%)in group A and 63(86.3%)in group B;and multiple carcinomas,in 10 cases(13.7%)in group B.All were superficial bladder cancers in group A,while 6(8.2%)were invasive carcinomas in group B.According to WHO pathological grading of bladder cancer,in group A,10 cases(52.6%)had G_1,8 (42.1%)had G_2 and 1(5.3%)had G_3 tumors;in group B,8 cases(11.0%)had G_1,49(67.1%)had G_2 and 16(21.9%)had G_3 tumors(P＜0.01).The positive rate of urine cytology was 53.3% in all 92 ca- ses(25.0% in group A and 60.7% in group B,P＜0.05).The diagnostic rates by B-ultrasound and cysto- scopy were 98.6% and 100.0%,respectively.Of the 92 patients,11(12.0%)were treated by partial cys- tectomy,73(79.3%)by TUR-Bt and 8(8.7%)by cystectomy.The follow-up was 3-115 months(mean, 39 months).The overall recurrence rate was 12.0%,with 5.3%(1/19)in group A and 13.7%(10/73)in group B.Of 10 patients with multiple carcinomas,6(60.0%)developed recurrence;and of 82 with solitary tumors,5(6.1%)developed recurrence,with significant difference between them(P＜0.01).Two of the multiple carcinoma patients developed invasive carcinoma.Conclusions In bladder cancer patients under 40 years,the positive rate of urine cytology,pathological grading and recurrence rate increase with age.Multi- ple tumors,invasive carcinoma and long-term smoking are high risk factors for tumor recurrence.TURBt is the main surgical method for treating bladder cancer patients under 40 years.

In type 2 diabetes mellitus,the levels of leptin were higher in female and obese subjects compared with male and non-obese subjects.There was negative relationship in female between leptin and HbA_1c,and negative relationship between leptin and HDL-C in male.Gender and obesity had no effect on TNF-? and NP Y.

Human IL-6,IL-10,IL-13 and SCF cDNAs encoding extra - cellular domain were isolated by reverse transcription polymerase reaction (RT-PCR). These cDNA fragments were inserted into expression vector PBV220 which contains PRP_(L), promoters and SD sequence. Then the E. coli DH5? were tronsformed with the recombinant plasmids. After screening, the clones carrying those recombinant plasmids were identified. SDS-PAEG and biological assay indicated that human IL-6, IL-10 and IL-13 were expressed in E.coli. The expression level of rhIL-6 was as high as 30% of the total bacterial protein.

ObjectiveTo evaluate the clinical results of intracoronary implantation of the phosphorylcholine dexamethasone coated biodivysio small coronary stents. Methods Prospective analyses of long-term results of the phosphorylcholine dexamethasone coated biodivysio coronary stents, and their implantation performed on 82 coronary artery disease patients were conducted. We implanted 106 stents of which 54 were the phosphorylcholine dexamethasone coated biodivysio coronary stents, and 52 the phosphorylcholine coated biodivysio coronary stents. Results The success rate of the implantation of both groups was 100%. The major adverse cardiac events (MACE) of the 3-month follow-up in the phosphorylcholine dexamethasone coated biodivysio coronary stents group were 9.8%; and in the phosphorylcholine coated biodivysio coronary stents group were 22.0%. The restenosis rate of the 8-month follow-up in the phosphorylcholine dexamethasone coated biodivysio coronary stents group was 4.9%; and in the phosphorylcholine coated biodivysio coronary stents group was 23.0%. Conclusion The implantation of the phosphorylcholine dexamethasone coated biodivysio coronary stents is a safe and efficient interventional procedure with a high success rate and few MACE. It may reduce restenosis rate.

Optimization of Traditional Chinese Medicine (TCM) clinical trials as philosophy, process and method, is extremely necessary and can be applied in different levels. Optimization of TCM clinical trials should follow some principles and procedures, and be executed in two ways. Optimization of treatment plan should be supported by the research basis and literature experience, and be perfected by research group and experts argumentation. Optimization of design plan is modified and accomplished by expert discussion and repeated consulting multi-disciplinary experts. Specific research objective, exact target population definition, proper design plan selection, valid calculation of sample size, precise and appropriate main outcome measures, data management system, medical ethics and statistical analysis are the focuses.

The principle of TCM diagnosis standard is communicational convenience, and the international universal standard recommended by WHO should also be paid attention to. When there’s no corresponding standard, inferior standard is available, but self made standard is generally not accepted. It’s suggested to apply the same standard for one program. The standards include international standard, regional standard, domestic standard, industrial standard, provincial standard and enterprise standard, the hierarchies and levels of these standards reduce successively. It’s thought that medical standard consists of international standard (issued by WHO or set by international academic conference), domestic standard (set by government authorities, national academic organization and conference), diagnosis standard set by provincial academic organization and textbook standard by universities and colleges. A lot of literature network database, as well as ICD-10, therapy guide, professional periodicals, professional society website, and books could be used for searching. TCM clinical trials also cover syndrome diagnosis standard. The conclusion standard and exclusion standard setting and influence of improper standard setting is also discussed in this article.
