3.Guide and prediction value of partial pressure of end-tidal carbon dioxide during cardiopulmonary resuscitation

Objective To study the clinical significance of end-tidal carbon dioxide partial pressure (PetCO2 ) during cardiopulmonary resuscitation (CPR) and seek the fixed value according which to decide if we should and when to give up.Methods This was a prospective,observational study.A total of 124 patients with cardiac arrest in or out-of-hospital from may 2003 to March 2009 in emergency department of our hospital were selected.All of them had definite etiological factors.Changes of PetCO2 in 124 cardiac arrest patients during CPR were tracked.Results The gender,age,rescue time in seventy-one patients with the return of spontaneous circulation (ROSC) after endotracheal intubation have a significant difference with that in fifty-three patients without ROSC (P <0.01 ).The PetCO2 of the survival were higher than that of patients without ROSC or with ROSC,but finally died (P <0.01 ).A fixed point 14.4 mmHg of PetCO2 after 20 minutes' CPR can be used as a reference value to guide CPR or predict prognosis.Conclusions Monitoring PetCO2 during CPR has a predictive value on the success of resuscitation.

6.Correlation between polymorphism of regulated upon activation normal T-cell expressed and secreted gene promoter and diabetic nephropathy in type 2 diabetes mellitus

BACKGROUND: Hereditary susceptibility is an important role in episode and development of diabetic nephropathy (DN). Aggregation of monocyte in kidney tissue, differentiation to histocyte and reinforcement of signal are possibly closed to episode and development of DN in type 2 diabetes mellitus (DM). Regulated upon activation normal T-cell expressed and secreted (RANTES) is a chemokine of C-C subtribe. It can mediate infiltration and activation of inflammatory cells and cause injuries of renal corpuscle and renal mesenchymal. This may be a key mechanism of episode and development of DM. Thus, RANTES gene may be one of the susceptible gene of DN. OBJECTIVE :To investigate correlation between RANTES gene promoter -28C/G polymorphism and DN in type 2 DM in Chinese. DESIGN: Case-contrast study. SETTING: Department of Endocrine, Affiliated Hospital of Guiyang Medical College; Center of Diabetes Mellitus of Guizhou Province. PARTICIPANTS: A total of 143 patients with type 2 DM were recruited from the Department of Endocrine, Affiliated Hospital of Guiyang Medical College and Department of Endocrine,Guizhou People's Hospital from September 2003 to September 2005. Among them, 53 cases had normoalbuminuria, 54 microalbuminuria and 36 macroalbuminuria. Another 55 healthy subjects who were selected from Center of Health Examination, Affiliated Hospital of Guiyang Medical College were regarded as control group. METHODS: 3-5 mL peripheral venous blood was collected from all subjects. RANTES gene promoter -28C/G genotype was detected with polymerase chain reaction restriction fragment length polymorphism technique. MAIN OUTCOME MEASURES: ① Allele frequency and genotypic frequency of RANTES gene promoter -28C/G; ② risk factor of DN was analyzed with Logistic regression analysis. RESULTS: All 198 subjects were involved in the final analysis. ① Comparison of CG genotypic frequency of -28C/G: There was no significant difference between normal control group and DM group (P> 0.05); however, that in microalbuminuria group and macroalbuminuria group was higher than that in normoalbuminuria group (24.1%, 47.2%, 9.4%, P < 0.05), and that in macroalbuminuria group was also higher than that in microalbuminuria group (P < 0.05). ② Frequency of G allele of -28C/G: There was no significant difference between normal control group and DM group (P > 0.05); however, that in micro albuminuria group and macrealbuminuria group was higher than that in microalbuminuria group (12.0%, 23.6%, 4.7%, P < 0.05), and that in macroalbuminuria group was also higher than that in microalbuminuria group (P< 0.05). ③ Logistic regression analysis showed that positive genotype of -28G was related to DN (95% Cl:1.402-15.032, P =0.012). CONCLUSION: Positive genotype of RANTES gene promoter-28G may be related to DN.
7.A study of the related factors about acute stress responses of the military freshmen

Objective To explore the related factors about acute stress responses of the military freshmen,then built a path model among self-efficacy,social support,coping style,state-trait anxiety and acute stress responses.Methods 584 students were tested by General Self-efficacy Scale (GSES),Perceived Social Support Scale (PSSS),Coping Styles Questionnaire,State-Trait Anxiety Inventory (STAI) and Acute Stress Response Scale ( ASP S).Results ①The military freshmen in each factor of acute stress response got a lower score,especially the mean score of the changes of cognition was higher (0.21 ±0.21 ),compared with the other factors,and the changes of pathology (0.053 ±0.114) was lower.②There were significant correlations between self-efficacy,social support,positive coping,negative coping,state anxiety,trait anxiety and acute stress responses (P < 0.01 ).③ Negative coping( r=0.130),trait anxiety(r=0.005) and state anxiety(r=0.002) influenced ASR directly,meanwhile,self-efficacy( r =-1.292) influenced ASR through the state anxiety indirectly,self-efficacy ( r=-7.465 ) and social support ( r =-0.294) influenced ASR indirectly through trait anxiety.Conclusion Acute stress responses of the military freshmen were directly influenced by negative coping and state-trait anxiety,and negative coping style was the main reason.
9.Contrastive Study of Ranunculaceae Aconitum Pendulum Busch and Aconitum Kongboense Lauener

Objective: To identify Aconitum pendulum Busch. and Aconitum kongboense Lauener. Methods: Aconitum pendulum Busch. and Aconitum kongboense Lauener were identified by microscopical identification and TLC. And the contents of aconitine in the two herbs were determined by HPLC. Results:Microscopical identification showed they were different from each other, and aconitine was used as the reference component. Using the solution consisting of hexane ∶ethyl acetate ∶diethylamine (10 ∶6 ∶0. 8) as the developing solvent, the compositions in the two herbs were different in TLC. The content of aconitine in Aconitum pendulum Busch was 0. 71 mg· g-1 , while that in Aconitum kongboense Lauener was only 0. 03 mg·g-1 . Conclusion:Aconitum pendulum Busch. and Aconitum kong-boense Lauener Identified by microscopical identification, TLC and HPLC,SHOW NOTABLE differences between them, and Aconitum kongboense Lauener should not be used as Aconitum pendulum Busch.
10.Bladder cancer in patients under 40 years of age:analysis of 92 cases

Objective To evaluate the clinical features,pathological grades,treatment and prognosis in bladder cancer patients under 40 years.Methods A retrospective review of transitional cell carcinoma of the bladder in patients under 40 years who had been treated from January 1994 to April 2005 were conduc- ted.The patients were divided into 2 groups(group A,20-30 years;group B,31-40 years)based on their age.The differences in pathologic grading,recurrence rate and positive rate of urine cytology were compared between the 2 groups.The statistical analyses were performed using x~2 test.Results The incidence of bladder cancer in patients under 40 years was 4.2%(92/2200).The male/female ratio was 2.7:1.0.At initial visit,86%(68.7% in group A and 91.1% in group B)of the patients presented with gross hematuri- a;and 25.0% in group A and 33.9% in group B concomitantly had frequency and dysuria.The mean disease course in the 2 groups was 3.8 months for male and 6.9 months for female.Solitary tumor occurred in 19 ca- ses(100.0%)in group A and 63(86.3%)in group B;and multiple carcinomas,in 10 cases(13.7%)in group B.All were superficial bladder cancers in group A,while 6(8.2%)were invasive carcinomas in group B.According to WHO pathological grading of bladder cancer,in group A,10 cases(52.6%)had G_1,8 (42.1%)had G_2 and 1(5.3%)had G_3 tumors;in group B,8 cases(11.0%)had G_1,49(67.1%)had G_2 and 16(21.9%)had G_3 tumors(P<0.01).The positive rate of urine cytology was 53.3% in all 92 ca- ses(25.0% in group A and 60.7% in group B,P<0.05).The diagnostic rates by B-ultrasound and cysto- scopy were 98.6% and 100.0%,respectively.Of the 92 patients,11(12.0%)were treated by partial cys- tectomy,73(79.3%)by TUR-Bt and 8(8.7%)by cystectomy.The follow-up was 3-115 months(mean, 39 months).The overall recurrence rate was 12.0%,with 5.3%(1/19)in group A and 13.7%(10/73)in group B.Of 10 patients with multiple carcinomas,6(60.0%)developed recurrence;and of 82 with solitary tumors,5(6.1%)developed recurrence,with significant difference between them(P<0.01).Two of the multiple carcinoma patients developed invasive carcinoma.Conclusions In bladder cancer patients under 40 years,the positive rate of urine cytology,pathological grading and recurrence rate increase with age.Multi- ple tumors,invasive carcinoma and long-term smoking are high risk factors for tumor recurrence.TURBt is the main surgical method for treating bladder cancer patients under 40 years.
