1.Effects of mindfulness - based stress reduction on compliance in primary open angle glaucoma patients with topical medication

AIM: To explore the effects of mindfulness- based stress reduction ( MBSR ) on anxiety, compliance and visual function in primary open angle glaucoma( POAG) patients. ·METHODS:Totally 64 patients, who were treated with topical medication and were diagnosed with POAG in the department of ophthalmology at Shaanxi Provincial People's Hospital, were randomly divided into the intervention group ( n=32 ) and the control group ( n=32 ) . Both groups received conventional mental health education while the intervention group was complemented with MBSR intervention. The patient's anxiety status, compliance rates and MD before and after the intervention were evaluated. ·RESULTS:After the MBSR intervention, the scores of HAMA of the intervention group were significantly lower than that of the control group (P<0. 01). The compliance of patients in intervention group was higher than that of the control group (P<0. 01). ·CONCLUSION:The MBSR intervention can effectively relieve the anxiety of patients and improve the compliance in primary open angle glaucoma patients. It can be promoted and put into clinical application.

4.Study on preparation process and formulation optimization of herpetin liposomes.

Herpetin (HPT) is an active monomer constituent isolated from lignanoid in seeds of Herpetospermum caudigerum. HPT shows inhibitory effects in hepatic injury and HBV-DNA and the replication. In the study, we successfully prepare herpetin liposomes by film dispersion method for the first time. The prescription process was optimized, with the entrapment efficiency as the index. According to the optimized prescription, the mass ratio of HPT: phospholipids: cholesterol was 2.44:78.05: 19.51, the hydration and de-molding process was performed with 0.5% F68 solution at 50 degrees C, and the water-bath ultrasonic time was 20 min. The HPT liposomes prepared by this method showed an average entrapment efficiency of (94.50 +/- 2.15)% and a particle size of (119.2 +/- 10.7) nm, which was consistent with the trial expectations and will lay a solid foundation for the hepatic targeting delivery system in future.

5.Assessment of the quality of life of oral cancer patients after reconstruction with free anterolateral thigh perforator flaps.

OBJECTIVEThis study aims to evaluate the quality of life (QOL) of patients who underwent resection of oral cancer and reconstruction by free anierolateral thigh perforator flaps (ALTF).
METHODSA total of 32 patients with oral and maxillofacial malignancies who had undergone the resection of oral cancer and reconstruction by ALTF were retrospectively analyzed. At 12 months postoperatively, the QOL of these patients was assessed by using the 14-item oral health impact profile (OHIP-14) and the medical outcome study short form-36 (SF-36) questionnaires.
RESULTSA total of 32 questionnaires were collected. In SF-36, the highest scoring domains were bodily pain (78.58 ± 14.82), physical functioning (72.08 ± 27.86), and the role of physical (60.00 ± 42.63), whereas the lowest scoring domains were role-emotional (41.67 ± 39.62), followed by mental health (50.75 ± 13.07) and health transition (54.17 ± 21.75). In OHIP-14, the lowest scoring domains were social disability (34.50 ± 11.32) and handicap (36.04 ± 12.05), indicating the functional recovery was better; and the highest scoring domains were physical pain (73.50 ± 18.96) and psychological discomfort (60.17 ± 20.66), indicating the functional recovery was worse.
CONCLUSIONThe ALTF is an ideal selection for the reconstruction of oral defects after cancer resection. In using this flap, the basic social need of patients after surgery can be satisfied. Moreover, the appearance and the functions of chewing, deglutition, and speech can be restored in varying degrees. Thus, ALTF can improve the patients' QOL.

6.Effect of data management quality on clinical research evidence evaluation.

Current clinical evaluation of literature quality has various ways. Most of them lay special emphasis on the evaluation of the design quality, but the evaluation of the implementation process quality is not perfect. Especially data management is not fully emphasized during the enforcement of clinical trials. Data from clinical research were bases for evaluating clinical findings. Although strict specifications and requirements for data management might be strictly written clearly in research protocols, they were not embodied in current clinical research evidence evaluation system. Data management is an important part of implementing the whole clinical trial process, which is a comprehensive reflection of data collecting, logging, sorting, and managing. Its objective is to obtain high quality research data for statistical analysis, thereby coming to a true and reliable conclusion. In order to overall evaluating clinical design and implement, we suggest that present quality evaluation indicators of clinical trails should be completed, and add data management quality evaluation during the whole implement process. Data management plans, standards and requirements for data checking, and management regulations for disobeying data and exception data should be added in quality evaluation indicators for clinical research evidence. The effect of data management quality on clinical research evidence evaluation should be emphasized.

7.Analysis of clinical and molecular pathology in 8 patients of Chinese dysferlinopathy

Objective To investigate the clinical and molecular pathological features of dysferlinopathy in China.Methods Four patients with limb-girdle muscular dystrophy2B(LGMD2B)and 4 patients with Miyoshi-type distal muscular dystrophy(MMD)were clinically analyzed, their skeletal muscle were biopsied and immunohistochemical stained.Four cases of each Duchenne-Aran muscular atrophy and myotis were served as controls.Results The clinical situation of dysferlinopathy was characterized by progressive muscle weakness and atrophy, consistent with progressive muscular dystrophy.Histochemical staining revealed muscle fibers degenerating, regenerating and necrosis in a varying degree.Connective tissue was seen proliferated and inflammatory cells infiltrated in the majority of cases.Immunohistochemical staining with anti-dysferlin monoclonal antibody identified the deficiency of dysferlin on the sarcolemma and in the sarcoplasm of 8 cases with dysferlinopathy.Conclusions(1)The clinical and pathological characters of dysferlinopathy are consistent with progressive muscular dystrophy;(2)Anti-dysferlin monoclonal antibody immunohistochemical staining is a reliable method to diagnose dysferlinopathy, which is worth of wide application in clinic.
8.Drug-resistant Gene and Disinfectant-resistant Gene for MRSA

OBJECTIVE To study the drug resistance,drug-resistant genes and(disinfectant)-resistant genes of(MRS)A.METHODS The drug resistance and mecA gene of the ?-lactamase and aac(6′)/aph(2″),aph(3′)-Ⅲ,ant(4′,4″) genes of aminoglycoside and qac(A/B) disinfectant-resistant genes were(detected) in 47 strains of MRSA.(RESULTS) In all 47 strains of MRSA,46 MRSA isolates were mecA positive,39 MRSA isolates were aac(6′)/aph(2″) positive,30 MRSA isolates were aph(3′)-Ⅲ positive,6 MRSA isolates were ant(4′,4″) positive,and 19 MRSA isolates were qac(A/B) positive.CONCLUSIONS MRSA is multiple-drug resistant.The main resistant mechanisms of MRSA to aminoglycosides and disinfectant are related to the drug-resistant genes of aminoglycoside and disinfectant-resistant genes.Clinic physician must pay attention to the diagnosis and(therapy) of MRSA,and control the hospital infection.
9.In vitro induction,proliferation and function characterization of dendritic cells from rat bone lnRITOW cells

Objective To establish a method of inducing dendritic cells(DC)from rat bone marrow cells in vitro,and identify the phenotype and function characteristics.Methods The rat bone malToW cells were collected and cultured in vitro under the condition of recombinant rat GM-CSF(rrGM-CSF)and recombinant rat IL-4(rrIL-4).After 2 weeks,the morphological character of DCs was observed under light microscope and scanning electron microscope.Expression of MHC-Ⅱ,CD80 and CD86 were detected by flow cytometry.The ability to stimulate allogenic T cells of the cultured DCs was detected by mixed lymphocyte reaction.Results DCs showed typical morphology with elongated dendritic processes under inversion microscope and scanning electron microscope.DCs at day 6 revealed immature phenotype,including MHC-Ⅱ(29.03 ±4.39)%,CD80(21.98±7.08)%and CD86(25.94±6.80)%.DCs at day 12 showed higher expression of MHC-Ⅱ(74.05±5.97)%,CD80(79.85±6.53)%and CD86(81.00±7.47)%,and stimulatory capacity of allogenic T cells,compared with that in DCs at day 6.Conclusion Matured DCs could be generated from rat bone marrow cells and attendance with rrGM-CSF and rrIL-4,which present the feasibility for further research on its application to allograft immunorejection.
10.The impact of different infarction sites on left ventricular systolic vortex-a pilot study

Objective To compare the impact of anterior and inferior-posterior regional wall motion abnormalities on the vortex in systolic phase by vector flow map(VFM) technique.Methods Sixty patients with myocardial infarction were divided into 2 groups:the left anterior descending artery single branch lesion group (LAD group) and the left circumflex artery and/or right coronary artery lesion group (LCX/RCA group).By VFM technique,left ventricular systolic blood flow field was observed.Parameters such as the vortex duration were calculated and compared between two patient groups and also with healthy control group.Results LAD group did not show significant difference from the control group in isovolumic systolic vortex,this group was characterized with large size of vortex and usually forming local vortex at the apex in early ejection.On the contrary,LCX/RCA group had small isovolumic systolic vortex,the early ejection vortex of this group was significantly smaller than that of LAD group and larger than that of control group.The phenomenon was usually observed in the center of the lumen.Both LAD and LCX/RCA group had longer total vortex duration and vortex collapse time than the healthy control group,these differences were not significant between LAD and LCX/RCA group.There were no significant differences in isovolumic vortex duration among the three groups.Conclusions Anterior wall infarction had more impact on systolic blood flow field than inferior-posterior wall infarction.The VFM technique could be applied for observation and evaluation of the changes in vortex characteristics.
