1.Analysis of the Bacterial Resistance and Use of Vancomycin in 202 Cases of Neonatal Sepsis in Our Hospital

OBJECTIVE:To provide reference for the rational use of vancomycin in neonatal sepsis. METHODS:A retrospec-tive survey was performed for newborns with neonatal sepsis in our hospital from Sept. 1st in 2011 to Sept. 1st in 2012,and the pathogenic examination results,bacterial resistance and use of vancomycin were analyzed and summarized. RESULTS:Totally 317 strains of pathogens were detected in 202 cases,including 147 strains of G+ bacteria(46.4%),88 strains of fungus(27.7%)and 82 strains of G- bacteria (25.9%). Coagulase-negative staphylococcus was the main pathogens. The use rate of vancomycin was 47.0%(95 cases),and the drug selections were all based on susceptibility,with most of two-drug combination(68 cases). No se-vere adverse reactions were found. CONCLUSIONS:Coagulase-negative staphylococcus is severe and has multi-drug resistance in the cases of neonatal sepsis in our hospital. While,the use of vancomycin is basically rational. However,the clinical use of vanco-mycin should be more cautious to prevent the generation of vancomycin-resistant strains.
2.Progress in urinary protein tumor markers

The development of proteomic technique has sparked new focus on novel protein markers for many diseases including cancer. Urine is one of the most useful biofluids for routine testing to provide an ideal resource for the discovery of novel biomarkers, with the advantage over tissue biopsy samples due to the ease and the less invasive nature of urinary collection. This review summarizes urinary protein tumor markers found in prostate cancer, bladder cancer, lung cancer and breast cancer that have shown potential for urogenital and non-urogenital cancer, so as to provide reference on high sensitive and high specific urinary protein tumor markers for early prevention, early diagnosis and treatment of cancer.
3.The association of duration of hypertension and changes in cognitive function in hypertension patients

Objective To investigate the relationship between duration of hypertension and cognitive function in adult hypertension patients.Methods A total of 224 subjects with normal blood pressure were enrolled in group A,and 1 296 patients with poorly controlled hypertension were further divided by the duration of hypertension into group B (≤5 years),group C (6-10 years),group D (11-20years) and group E (> 20 years).Face-to-face surveys were conducted in all the subjects by trained physicians using Mini-Mental-State-Examination (MMSE) and Clock-Drawing-Test (CDT).The incidences of cognitive impairment were compared among the five groups and the relationship between duration of hypertension and cognitive function were analyzed by SPSS 18.0 software.Results (1) Compared with the normotensive group,the hypertensive group performed worse in the scores of MMSE,CDTs,the memory type cognitive function and the non-memory type cognitive function (MMSE:Z =-2.585,P =0.010;C DTs:Z =-3.689,P < 0.001 ; memory type cognitive function:Z =-2.718,P =0.007 ; non-memory type cognitive function:Z =-1.994,P =0.046).(2) The incidences of cognitive impairment in the five groups were 3.6% (8/224),6.6% (26/393),16.6% (72/433),19.7% (55/279) and 33.5%(64/191),respectively.(3) Compared with the group A,the cognitive function was significantly worse in the group E (MMSE:Z =61.314,P < 0.001 ; CDTs:Z =44.642,P < 0.001 ; memory type cognitive function:Z =35.703,P < 0.001 ; non-memory type cognitive function:Z =54.440,P < 0.001).Conclusions Hypertension is a risk factor for the cognitive dysfunction.The incidence of cognitive dysfunction and the severity are positively associated with the duration of hypertension.In those with hypertension over 20 years,the cognitive dysfunction occurs much more obviously.

To establish a method for the determination of astilbin, peoniflorin, rasmarinci acid, isofraxidin and liquiritin contained in Shaolin Xiaoyin tablets, in order to lay a foundation for designing late-stage dosage forms and clinical medication schemes. In this paper, efforts were made to establish a method for the determination of the blood concentration of the five components and study the in vivo pharmacokinetics in rats. The blood concentration was determined by HPLC. Phenomenex C18 column (4.6 mm x 250 mm, 5 microm) was adopted and eluted with methanol-acetonitrile-0.05% formic acid, the flow rate was 0.8 mL x min(-1), and the wavelength was 275 nm. The samples were processed by the solid phase extraction method. After oral administration of Shaoling Xiaoyin tablets, the rat bloods were collected at different time points to determine the blood concentrations. The experimental results showed that the baseline separation could be adopted for the five components, and astilbin, peoniflorin, rasmarinci acid, isofraxidin and liquiritin showed good linear relations within ranges of 2.48-248, 0.213 6-21.36, 0.531-53.1, 0.704-70.4, 0.253-25.3 mg x L(-1). All the five components could be absorbed in blood and excreted quickly. The method established in this paper is rapid and accurate, and could be used for in vivo analysis on preparations containing similar components. The main components in Shaoling Xiaoyin tablets could be absorbed and excreted quickly, and thus suitable to be made into sustained release tablets. Common preparations are required to be taken for 4-6 times a day.

6.The effects of modified expander in the orthodontic treatment of the cases with Angel Ⅰ malocclusion

Objective:To investigate efficacy of modified expander in the treatment of non-extraction patients with Angel Ⅰ maloccul-sion.Methods:1 4 Angel Ⅰcases aged 1 0 -1 4 years treated by non-extraction and fixed rapid expander.Jaw bones of the cases were laser scanned before(T1 )and after treatment(T2).The differences of arch width and length between T1 and T2 were analyzed.The In-dex of treatment complexity,outcome and need(ICON)was used to assess the pre-treatment and post-treatment study models.Data were statistically analysed with SPSS1 3.0 software.Results:The arch width increased significantly from T1 to T2.Good occlusal rela-tionship and soft tissue profiles were achieved in all cases.All cases were evaluated as greatly improved by ICON.Conclusion:AngelⅠ malocclusion can be treated successfully by increasing arch width with fixed rapid expander technique without tooth extraction.
7.Femoral-deep femoral crossover bypass for unilateral iliofemoral arteriosclerosis obliterans

Objective To evaluate the clinical results of femoral-deep femoral crossover bypass in the treatment of long-segment unilateral iliac artery occlusive disease.Methods From July 1995 to December 2010,40 patients (28 males,12 females,aged from 66 to 90,with mean age of 73) with comprehensive unilateral iliac-superficial femoral arteriosclerosis obliterans were enrolled in this procedure.All patients suffered from unilateral common iliac,external iliac,common femoral,and superficial femoral arteriosclerosis obliterans.These patients were treated with femoral-deep femoral crossover bypass.Postoperative ankle-brachial index,blood flow velocity and patency rates in 5,7 and 10 years and limb salvage rates in 5,7 and 10 years were evaluated.Results There was no perioperative mortality nor extremity amputation.35 (87.5% ) patients were followed-up from 1 to 13 years (mean 5.7 y).Anklebrachial index rose from preoperative 0.23 ± 0.10 to postoperative 0.55 ± 0.11 (t =15.91,P =0.000 ).Popliteal arterial velocity rose from preoperative ( 14 ±6) cm/s to postoperative (34 ± 10) cm/s (t =15.63,P =0.000) ; Tibial arterial velocity rose from ( 10 ±4) cm/s to (22 ±7) cm/s (t =15.71,P =0.000).The primary and secondary patency rates were 60.1%,44.3%,25.3%,and 93.5%,86.8%,57.9% at 5,7 and 10 years,respectively.Limb salvage rates were 97.5%,95%,and 90%,at 5,7 and 10 years,respectively.Conclusions Femoral-deep femoral crossover bypass is safe and reliable in treating certain unilateral iliofemoral occlusive disease,especially for high-risk old patients or those who are not indicated for endovascular therapies or direct aortic approaches.

9.Effect of 1,25-dihydroxyvitamin D3 influence on expressions of Ki67 and mTOR in Thy-1 nephritis model of rat

Objective To study the expressions of Ki67 and mTOR in Thy-1 nephritis model of rat who were given 1,25-dihydroxyvitamin D3[1,25(OH)2D3] and to explore its mechanism. Methods Healthy male SD rats (n=90) were random?ly divided into three groups: control group, model group, 1,25(OH)2D3 treatment group (n=30 in each group). Model group and 1,25(OH)2D3 treatment group were intravenously injected with anti-Thy1 monoclonal antibody once via tail vein while the control group were administrated with same volume of normal saline through the same route. 1,25(OH)2D3 were adminis?trated at 0.5μg per day intra-gastrically for consecutive 21 days in 1,25(OH)2D3 treatment group while equal volume of pea?nut oil were given in control group and model group. Six rats were randomly selected from each group and sacrificed at the 1st , 3rd , 7th , 14th and 21st after drug intervention. Twenty four hour urine sample were collected in each rat just before it was culled to detect 24-hour urinary protein excretion. Renal tissue samples were harvested and stained with hematoxylin&eo?sin (H&E) and PAS to determine the renal pathological variation and the expressions of mTOR and Ki67 were assessed by immunohistochemistry. Results Urine protein begin to be detected at the first day after model was established, peaked at the 3rd days then started dropping until the 14th day when urine sample turned to normal. Urine protein levels were lower in 1, 25(OH)2D3 treatment group at the 1st,3rd,7th day after model establishment than those in model group(P<0.05). Compared with model group, the pathological damage of renal tissue in 1,25(OH)2D3 treatment group were alleviated at the 3rd and 7th day after model establishment (P < 0.05). Expressions of Ki67 and mTOR in 1, 25(OH)2D3 treatment group were reduced compared with those in model group (P<0.05). Twenty four hour urinary protein and expressions of Ki67 and mTOR as well as renal pathological damage were all positively correlated with each other. Conclusion 1,25(OH)2D3 can inhibit the proliferation of glomerular mesangial cells in Thy-1 nephritis model of rat. And its therapeutic mechanism may be associated with down reg? ulating expressions of Ki67 and mTOR.
10.Modification of pendulum appliance

Among several intraoral distalizing devices,the pendulum appliance is one of the most commonly used noncompliance appli-ances.It is widely used to correct Class Ⅱ molar relationship.More healthy teeth can be preserved with the using of pendulum appliance in the tooth extraction of borderline cases.The pendulum appliance was modified and used in clinic.Long-term observasion showed that the modified pendulum appliace is effective in orthodontic treatment.
