1.Clinical observation of poractant alfa injection combined with nasal continuous positive airway pressure in the treatment of premature infants with respiratory distress syndrome

Objective To observe the clinical effect of poractant alfa injection(curosurf)combined with nasal continuous positive airway pressure(CPAP)in the treatment of neonatal respiratory distress syndrome(NRDS). Methods Fifty patients of infants with NRDS were divided into 25 cases of observation group(curosurf combined with CPAP)and 25 cases of control group(CPAP)according to the methods of the treatment.The differences in treatment efficacy rate and arterial blood gas index were compared.Results The efficacy rate of the observation group (88%)was higher than that of the control group(56%),the difference between the two groups was statistically significant(χ2 =4.861,P <0.05 ).After the treatment,the arterial blood gas index of the observation group were improved obviously compared with that before the treatment,the differences were statistically significant(all P <0.05).After the treatment,the arterial blood gas index of the control group were improved obviously compared with that before the treatment,the differences were statistically significant(all P <0.05).After treatment,the differences in pH value,PaO2 and PaCO2 of the observation group were statistically significant compared with the control group(t =6.00,8.49,4.46,all P <0.05).Conclusion Poractant alfa injection combined with nasal CPAP in the treatment of NRDS is better than using CPAP alone.
2.Effects of axial stress on healing of lengthened callus

AIM: To explore the effects of axial stress regulation on healing of lengthened ca llus. METHODS: Tibiofibulars of 30 rabbits were lengthened 2 cm with t wice plane sreminobicular crib external fixator. Experimental group continues to lengthen 3 mm, and then were drawn and shortened 0.5mm once every 24 h separate ly for 10 days. Ten days later, tibiofibulars were axial short-stressed 1 mm wee kly with three times. Animals were killed 58 days after operation and X-ray, bon e density and histology tests were performed with lengthened bone. RESULTS: X-ray examination had no significant difference between two group animal when le ngthen ended, but X-ray showed lengthened callus was different on day 58. In con trol group, lengthened center had a serial callus and cortex had no formed, in experimental group, lengthened center had serial cortex and transfixial medullar y cavity expect serial callus. 3.Investigation of the Effect of Naloxone injection in the treatment of Primary Apnea of Preterm Infant.

Objective To asses the clinical efficiency of Naloxone injection in the treatment of Primary Apnea of preterm infant. Methods There were 116 cases randomly divided into experimental group including 72 cases, whch was treated with Naloxone injection and control group including 44 cases,which was treated with Aminophylline injection. Results The total effective rates of the two groups are 95.8% and 68.2%.The rates of excellence of the two groups are 63.9% and 40.9%,respectively,there is a significant difference between them(P<0.01). Conclusion Naloxone injection is effective to treat Primary Apnea of preterm infant.
4.Research progress of targeting PD-1/PD-L1 in immunotherapy for non-small-cell lung cancer

Lung cancer is the leading cause of cancer-related death in the world.PD-1 is a member of the CD28 family expressed on the surface of a variety of immune cells.PD-L1, one of PD-1 ligands, plays a pivotal role in the ability of tumor cells to evade the host immune system.A recent study showed that signaling via mutant EGFR in lung tumor cells upregulates directly tumor PD-L1 expression.In this review, the clinical significance of the PD-1/PD-L1 pathway in immunotherapy for non-small-cell lung cancer (NSCLC) and the relevance to targeted therapy were summarized.

In recent years, Africa has achieved remarkable results in promoting the development of traditional medicine. Many other countries will benefit from studying their examples and adopting similar measures.
6.Development of medical science and technology and doctor-patient relationship

The rapid development of medical science and technology, which has greatly promoted the development of medicine and the improvement of diagnosis and treatment, has also brought about various negative impacts in clinical practice on the doctor-patient relationship. The paper attempts to analyze these impacts from such perspectives as medical treatment, society, ethics, and law and proposes some corresponding countermeasures in perspective of the present reality: (1)rationally controlling medical expenses; (2) effectively stepping up communication; (3)appropriately handling difficult ethical problems; (4) amplifying necessary health rules and regulations. BACKGROUND:No particularly effective medicine could treat human immunodeficiency virus 1(HIV 1) associated dementia(HAD) at present,which mainly because the mechanism of HIV 1 infection induced neural damage and necrosis is still not completely clarified. OBJECTIVE:To investigate the effects of human immunodeficiency virus I type(HIV 1) enveloped protein(EP) gp120 on the synaptic transmission and plasticity in hippocampal CA1 area of rat to clarify the mechanism of HAD generation. DESIGN:A paired design. SETTING:Department of Pathophysiology of Medical College of Jinan University. MATERIALS:The study was conducted in the Department of Pathophysiology(a tertiary national key laboratory of National Administration for Traditional Chinese Medicine,registration number: TCM 03 131) of the Medical College of Jinan University from January to October of 2003.Male SD rats aged between 2 and 5 weeks were used in the study. INTERVENTIONS:Brain slice perfusion and recording technique was employed. MAIN OUTCOME MEASURES:To record the excitatory postsynaptic potential(EPSP) in hippocampal CA1 area in rat;to investigate the effects of gp120 on long term potentiation(LTP) induced by high frequency electric stimulation in Schaffer lateral branch. RESULTS:gp120 had inhibitive effect on LTP in hippocampal CA1 area in rat [the average amplitude of LTP decreased from normal (216.1± 14.0) % to(90.8± 6.0) % ,n=12,P< 0.01],but had no effect on basic EPSP.PKA/PKC protein kinase(PK) inhibitor H7 could reverse this inhibitive effect [the average amplitude of LTP was(198.8± 16.2) % ,n=8,P< 0.01]. CONCLUSION:gp120 might participate the generation of HAD through inhibiting LTP in hippocampal CA1 area.

Programmed death-ligand 1 (PD-L1 ),which is highly expressed in NSCLC,can be divided into two categories:membrane PD-L1 and soluble PD-L1 .PD-L1 participates in tumor immune escape through combining with its receptor.PD-L1 immune checkpoint inhibitors have entered the phase Ⅰ studies and showed a good application prospect.It is also found that the sensitivity of PD-L1 immune checkpoint inhibitors is strongly associated with the expression of PD-L1 in tumor.Therefore,PD-L1 can be used as a biomarker to pre-dict its curative effect.

Objective To discuss the pathogenesis of cerebral hemorrhage in maintenance hemodialysis patients.Methods From Jan.2002 to Dec.2004all the 261 hemodialysis patients in changai Hospital were divided into two groups on the basis of with or without cerebral hemorrhage :(1)the control group,(2)the group of cerebral hemorrhage.Clinical data of 261 hemodialysis patients were retrospectively analyzed.Results In the group of cerebral hemorrhage,the incidence of high blood pressure was 81.1%:the blood pressure after medical therapy not up to standard was 83.8%;the usage of ordinary heparin was 78.4%;the average RRF was(3.8?1.9)and those of the control group were 62.5%,52.7%,52.7% and(7.1?3.3).There were significant differences between the two groups.Conclusion The maintenance hemodialysis patients with cerebral hemorrhage result from multiple factors.Positive control of the blood pressure,selection of appropriate anticoagulant and protection of RRF have important clinical significance.
10.Review of E2F Transcription Factor 2 Expression in Rheumatoid Arthritis Fibroblast-like Synovial Cells (RA FLs)

Objective To study the relationship between E2F2 and the pathological features of RA and the mechanism of E2F2 expression.Methods The interference efficiency was detected by RT-PCR after RA FLs transfected with E2F2 siRNA.The cell proliferation was analyzed by MTS and the expression of numerous genes was tested by ELISA.To analyze the influence of E2F2 in the immune response by RT-PCR,RA FLs were separately treated with TNF-t and PDTC.The way of regulating E2F2 experssion was tested by Chip.Results Compared with negative control groups,the level of E2F2 mRNA were decreased in the cells transfected with E2F2 siRNA (P < 0.01).Silencing E2F2 suppresses the proliferation of RA FLs (P < 0.05).TNF-α regulates the expression of E2F2 in RA FLs via the NF-κB pathway.Conclusion Up-regulation of E2F2 is associated with the process of RA and TNF-α regulates the expression of E2F2 in RA FLs via the NF-κB pathway.Thus E2F2 might be a potential target for use in the development of new therapeutic approches of RA.
