1.Clinical effect analysis of intervention treatment for patients with atherosclerotic renal artery stenosis

Objective To evaluate the clinical outcome and relative factors of intervention treatment for atherosclerotic renal artery stenosis in elderly patients.Methods The clinical data of 79 patients diagnosed as atherosclerotic renal artery stenosis by angiography and treated by revascularization were analyzed.Results There were 55(69.6%)successes and 24(30.4%)failures in decreasing blood pressure and 28(35.4%)successes and 51(64.6%)failures in improving renal function after intervention treatment.Predictors of favorable outcome of intervention treatment in decreasing blood pressure were related to lower urine protein,higher glomerular filtration rate,higher systolic and diastolic blood pressure before treatment,lower resistance index(RI)of renal artery,and no complication of cerebral vascular diseases.Predictors of favorable outcome of intervention treatment in improving renal function were related with percentage of angiographic stenosis,category of antihypertension and lower urine protein.The logistic regression analysis showed that the percentage of angiographic stenosis was the most important predictor of intervention treatment for blood pressure control,age and level of serum creatinine before intervention treatment were the most important predictors of intervention treatment for improving renal faction.Conclusion Percentage of stenosis(≥85%),age(133 ?mol/L)can be used as the predictors of therapeutic success for renovascular stenosis in older patients.
2.Technology Optimization of Supercritical CO2 Extraction from Zingiberis rhizoma

OBJECTIVE:To optimize the technology of supercritical CO2 extraction from Zingiberis rhizoma. METHODS:With the comprehensive score of the contents of 6-ginger phenol,8-ginger phenol and 10-ginger phenol and the extraction rate of the oil from Z. rhizoma as the index,uniform design method was adopted to investigate the effects of extraction pressure,extrac-tion temperature and extraction duration on the extraction result;verification tests were conducted. RESULTS:The optimal condi-tions were as follows as the extraction pressure of 25 MPa,extraction temperature of 30 ℃ and extraction duration of 2 h. In the verification tests,the average extraction rate of the oil from Z. rhizoma was 3.2%(n=3),and the comprehensive score was 1.874 2 (RSD=0.65%,n=3),with the relative deviation of 0.6% between the measured value and the predicted value. CONCLUSIONS:The optimal extraction technology is stable and feasible,with the advantages of low temperature,short duration.
3.Optimization of culture condition of a moderately thermophilic acidophilic Ferroplasma thermophilum.

The extreme microorganisms Ferroplasma spp., play an important role in bioleaching of sulphide ores at low pH value and temperatures around 50 degrees C. Without cell wall, Ferroplasma spp. is sensitive to pulp density, shearing force and heavy metal ions. Thus it is difficult to obtain their high cell density cultures, which limits the large-scale industrial application. In this paper, the optimum culture conditions of Ferroplasma thermophilum were studied by shaking culture. The results showed that the optimum culture conditions are as follows: 50 degrees C, initial pH 0.5, 50 mL working volume in 250 mL shaking-flask, inorganic nitrogen source (NH4)2SO4. The optimum combination of FeSO4.7H2O, yeast extract and peptone was determined by orthogonal experiments, including FeSO4.7H2O 40 g/L, yeast extract 0.3 g/L, peptone 0.2 g/L. Under the optimum culture conditions, the cell density was up to 6.3 x 10(7) cell/mL, and the oxidation of 40 g/L ferrous sulfate heptahydrate was finished in less than 72 hours. The results might provide information for scale-up of archaeon culture as well as its industrial application.

4. Observation on safety and effects of analgesic and sedative treatment in severely burned patients during shock stage

Objective:
To observe the safety and effects of application of analgesic and sedative drugs in severely burned patients during shock stage.
Methods:
One hundred and eighty patients with severe burns, conforming to the study criteria, were admitted to our unit from August 2014 to August 2016. Patients were divided into analgesia and sedation group and control group according to whether receiving analgesic and sedative treatment or not, with 90 cases in each group. Patients in control group received conventional treatment, while those in analgesia and sedation group received analgesic and sedative treatment for 24 hours besides conventional treatment. Before and at drug administration hour 2, 8, 16, and 24, pain degree of patients in two groups was scored by visual analogue scale (VAS). At drug administration hour 2, 8, 16, and 24, sedation degree of patients in two groups was scored by richmond agitation sedation scale, and the success rate of sedation was calculated. Mental state of patients within 24 hours of drug administration was observed, while pulse oxygen saturation (SpO2), respiratory rate, heart rate, and blood pressure were observed and dynamically evaluated every 2 hours. The accidental extubation, tachycardia, hypertension, hypoxia, bradycardia, hypotension, urinary retention, and respiratory depression of patients within 24 hours of drug administration were monitored and recorded. 5.Effects of Puerarin on Myocardial Tissue Injury and Inflammatory Response in Severely Burned Rats

Objective To investigate the effects of Puerarin on myocardial tissue injury and inflammatory response in severely burned rats and its mechanism. Methods The model rats with 30% TBSA (three degree) were made by bathing in 92℃ water for 18s; Sham burn group, model control group, puerarin 50, 100, 200, 400mg /kg treated groups were set. The histopathological changes of myocardial tissue were observed. The content of myocardial enzymes and cTnT in serum was determined. The activity of antioxidase and the content of MDA in myocardial tissue was measured. The apoptosis of cardiomyocytes was observed. The expression of apoptosis-related proteins was determined. The level of inflammatory cytokines in serum was determined. Results Compared with model control group, the histopathological changes of puerarin treated groups were significantly improved, especially the 200mg /kg group. The content of AST, CPK, LDH, cTnT in serum of Puerarin(100, 200, 400mg /kg) treated groups were significantly decreased. The activity of SOD, CAT, MPO were significantly increased and the content of MDA was significantly decreased. The apoptosis of cardiomyocytes was significantly improved, and the AI was significantly decreased. The expression of cleaved caspase-3, Bax were significantly down-regulated and the Bcl-2 was significantly up-regulated, the Bcl-2 /Bax was significantly increased. The levels of TNF-α, IL-6, IL-8 in serum were significantly decreased, the level of IL-1 in serum of puerarin 200mg /kg treated group was significantly decreased and the IL-10 was significantly increased. All of the differences above were significant(P < 0. 05). Conclusion Puerarin has inhibitive effects on myocardial tissue injury and inflammatory responses in severely burned rats, which was perhaps related to its effects of improving antioxidant enzyme activity, altering the expression of apoptosis-related proteins and affecting the content of inflammatory cytokines.
6.Effect of Wuwei Fuzheng Yijing Decoction on Sperm DNA Integrity in Male Infertility Patients

ObjectiveTo observe the effect of Wuwei Fuzheng Yijing decoction on sperm DNA integrity in male infertility patients. MethodOne hundred male infertility patients with sperm DNA damage (spleen and kidney Qi deficiency and blood stasis) that met the inclusion criteria were randomly divided into an observation group (Wuwei Fuzheng Yijing decoction) and a control group (L-carnitine oral solution), with 50 cases in each group. After 12 weeks of treatment, the DNA fragmentation index (DFI) and traditional Chinese medicine (TCM) syndrome score, progressive sperm motility (PR), sperm survival rate, and sperm concentration of the two groups before and after treatment were observed and compared. ResultThe total effective rate of the observation group was 89.80% (44/49) higher than 62.50% (30/48) of the control group (P<0.05). Compared with the conditions before treatment, the DFI and TCM syndrome score of the two groups were reduced after treatment, while the PR, sperm survival rate, and sperm concentration were increased (P<0.05), and the observation group was superior to the control group in terms of the above indexes (P<0.05). ConclusionWuwei Fuzheng Yijing decoction obviously repairs sperm DNA of male infertility patients and improves sperm quality, with high safety.
7.Correlation between inspiratory muscle strength and pulmonary ventilation function in patients with chronic obstructive pulmonary disease

Objective To analyze the correlation between inspiratory muscle strength and pulmonary ventilation function in chronic obstructive pulmonary disease (COPD) patients.Methods A total of 41 COPD patients admitted in the Second Xiangya Hospital from January to September 2018 were screened.The inspiratory muscle strength (MIP) and peak inspiratory flow (PIF) of patients were measured by using POWER breathe K-5 tester.The MIP was also calculated as a percentage of the predicted value.A pulmonary function detector was used to measure the forced vital capacity (FVC),forced expiratory volume in 1 second (FEV1 %),FEV1 as a percentage of FVC (FEV1/FVC%),peak expiratory flow (PEF),forced expiratory flow as a percentage of the predicted value (FEF 50%) at 50% of vital capacity and forced expiratory flow rate as a percentage of the predicted value (FEF 75%) at 75% of vital capacity.According to the Gobal Inistiative for chronic Obstructive Lung Disease (GOLD) classification method of COPD patients,the corresponding MIP value and PIF value of the patients were divided into 4 groups and conducted the comparison between groups.Pearson correlation was used to analyze the correlation between the above-mentioned inspiratory muscle strength test values and lung ventilation function test values,and a scatter diagram was drawn for the ones that had a correlation.Results Among the 41 patients,39 (95.1%) had a decrease in inspiratory muscle strength.According to the GOLD classification,the mean MIP value has a difference among the 4 groups.The mean MIP of grade Ⅲ was [(47.09 ± 29.42) cmH2O],which was higher than that of grade Ⅳ of [(24.72 ± 7.66) cmH2 O] (P < 0.05).There was no difference between PIF.The MIP was positively correlated with FVC (P <0.05).There was no correlation between MIP and FEV1 %,FEV1/FVC%,PEF,FEF50% and FEF75% (P >0.05).The MIP as a percentage of the predicted value had a weak positive correlation with FVC and FEV1% (P <0.05).There was no significant correlation between MIP as a percentage of the predicted value and FEV1/FVC%,PEF,FEF50% and FEF75% (P >0.05).PIF had a weak positive correlation with FVC,PEF,FEF50% and FEF75%,with statistically significant difference (P < 0.05).There was no significant correlation between PIF and FEV1 % and FEV1/FVC% (P >0.05).Conclusions Inspiratory muscle dysfunction was common in patients with COPD.There was a difference in MIP among different GOLD lung function classification in COPD patients.Some values of the inspiratory muscle strength and lung ventilation function had a weak-moderate correlation,while some values had no correlation.Therefore,inspiratory muscle strength test cannot replace the lung ventilation function test in COPD patients.
8.Study on the Hemostatic Effect of Porcine Fibrin Sealant Patch on Bleeding Wound of Liver in Rats and Gluteus Maximus in Heparinized Rabbits

OBJECTIVE:To study the hemostatic effect of porcine fibrin sealant patch (DBT) on bleeding wound of liver in rats and gluteus maximus in heparinized rabbits. METHODS:48 rats and 24 rabbits were randomly divided into sham operation group,operation control group (gauze hemostasis),medical collagen sponge group and DBT group. Except for sham operation group,animals in other groups were reduced for rat model with liver bleeding or heparinized rabbit model with gluteus maximus bleeding. The hemostatic time was recorded,bleeding amount was calculated;DBT degradation and wound adhesion in liver after 3,13 weeks were observed;re-bleeding rate of heparinized rabbits in medical collagen sponge group and DBT group were investi-gated. RESULTS:Compared with sham operation group,the hemostatic time and bleeding amount of animals in operation control group were significantly increased(P<0.01). Compared with operation control group,the hemostatic time and bleeding amount of animals in DBT group and medical collagen sponge group were significantly reduced (P<0.01). After 3,13 weeks,different de-gree of adhesion appeared in the wound of rats in each group,while the adhesion scores had no statistical significances(P>0.05). After 13 weeks,liver margin of rats in administration groups was slightly blunt,but it basically had been restored to preoperative state,with good healing. DBT can be degraded and absorbed completely. The re-bleeding rate of rabbits in DBT group were33.3%,66.7% in medical collagen sponge group. CONCLU-SIONS:DBT has good hemostatic effect on fragile organs and the body with blood coagulation dysfunction,and can be de-graded and absorbed. Its effect is equivalent to medical colla-gen,while the adhesive strength is slightly better than the latter.
9.Clinical Efficacy of Fuzheng Huaji Longbi Decoction in Treating Benign Prostatic Hyperplasia with Syndrome of Healthy Qi Deficiency and Blood Stasis

ObjectiveTo explore the clinical efficacy and safety of Fuzheng Huaji Longbi decoction in treating benign prostatic hyperplasia (BPH) in the patients with the syndrome of healthy Qi deficiency and blood stasis. MethodA total of 94 BPH patients were randomized into control and observation groups, with 47 patients in each group. The control group was treated with doxazosin mesylate sustained-release tablets, and the observation group with Fuzheng Huaji Longbi decoction on the basis of the therapy in the control group. After eight weeks, the international prostate symptom score (IPSS), quality of life (QOL) score, residual urine volume (RUV), maximum urinary flow rate (Qmax), TCM syndrome score, TCM symptom score, electrocardiogram, and liver and kidney function were determined to evaluate the clinical efficacy and safety of the two groups. ResultAfter 8 weeks of treatment, the total response rate in the control group was 63.64% (28/44), which was lower than that (84.44%, 38/45) in the observation group (χ2=5.026, P<0.05). The clinical efficacy in the observation group was higher than that in the control group (Z=-2.17, P=0.030). The treatment in both groups decreased the IPSS, QOL score, RUV, and TCM syndrome scores and increased the Qmax (P<0.05). Moreover, the observation group had lower IPSS, QOL score, RUV, and TCM syndrome score (P<0.05) and higher Qmax than the control group after treatment (P<0.05). The treatment in the observation group decreased all the TCM symptom scores (P<0.05), while that in the control group only decreased the frequency of urination at night and the scores of dysuria, weak urine stream, and post-urinary drainage (P<0.05). After treatment, the observation group had lower frequency of urination at night and lower scores of mental fatigue, cold limbs, lower abdominal discomfort, and loose stool than the control group (P<0.05). No adverse events associated with the administration of Fuzheng Huaji Longbi decoction were observed during the treatment period. ConclusionFuzheng Huaji Longbi decoction is effective in treating BPH in the patients with the syndrome of healthy qi deficiency and blood stasis. It can relieve the clinical symptoms and improve the quality of life, being a safe and reliable choice for clinical application.
