3.Reasearch on mechanism of neurotrophins in discogenic low back pain.

Discogenic low back pain is the common type of chronic low back pain. However,its mechanism has not been completely clarified. Considerable evidence shows that neurotrophins play an important role in discogenic low back pain. The paper summarizes the mechanism of neurotrophins on discogenic low back pain according to the pain transfer pathway of neurotrophins in intervertebral disc, dorsal horn ganglia and spinal trigeminal nucleus. Changing the pain transmission by regulating neurotrophins and its receptor will provide a new way for the treatment of discogenic low back pain.

4.Application of low voltage with CARE dose 4D in the computed tomography pulmonary angiography(CTPA)

Objective To assess the feasibility of low voltage with CARE dose 4D computed tomography pul-monary angiography(CTPA)by using image quality and radiation dosage.Methods 92 patients with clinically sus-pected pulmonary embolism(BMI:20 ~30kg/m2)were randomly divided into two groups(group A:n =46,80kV;groups B:n =46,100kV),the male and female of group A were equal to the number.Image quality score(given blind-ly by two senior radiologists ranged from 1 to 5 points).The CT value and standard deviation(SD)were measured and recorded in common pulmonary artery trunk,the main right pulmonary arteries,right low lobar arteries,and erector muscle of spine,calculated the SNR of ROI.Average CT value,signal noise ratio(SNR),CTDIvol,DLP were com-pared between the two groups.Used the CT dose index(CTDIvol)to compare radiation dosage between group A female and male.Results The agent average CT value and SD in arteries in group A [(510.27 ±115.45)HU,(50.06 ± 11.67)HU respectively]were higher than those in group B[(413.32 ±100.38)HU,(35.12 ±11.94)HU respective-ly],there were significant differences between the two groups(t =10.367,8.892,all P =0.000),but the SNR of ROI was (12.36 ±3.90),which was lower than (14.03 ±4.46)in group B,there was significant difference between the two groups(t =-5.238,P =0.001 ).Image quality score of transverse ection(1mm)in group A was (4.20 ± 0.29),which was lower than (4.91 ±0.36)in group B,there was significant difference(t =-20.23,P =0.000), but there was no significant difference in coronal section(5mm),coronal maximum intensity project(MIP)image and the capability of displaying the pulmonary artery branches(all P >0.05 ).The CTDIvol in group A was (2.03 ± 0.39)mGy,which was greatly lower than (5.04 ±1.02)mGy in group B,there was significant difference (P <0.05);but there was no significant difference between female and male in group A[male:(2.05 ±0.39)mGy;female:(2.01 ±0.38)mGy,t =-0.300,P >0.05].Conclusion Radiation dose can be decreased greatly(above 50%)in 80kV CTPA compared 100kV without compressing obviously the image quality for patients BMI 20 -30kg/m2 ,and no significant difference between the group A(80kV)different gender radiation dosage.
5.Pathogenetie features of spontaneous hemopneumothorax in aged people and therapeutic investigation of VATS

Objective To investigated the clinical value at pathogenetie features of spontaneous hemopneu-mothorax in aged people and therapeutic investigation of VATS.Methods 32 aged patients with spontaneous hemo-pneumothorax who underwent VATS.all used small chest wound asssociated with video-assisted thoraeoscopy.Results All procedures were carried OUt salty,recruitments were satisfactary,there were no operative death,all patients cured and had no sepuetae.There were 2 cases had gas leak last for a long time after operation,there Were 7 eBses had reexpansion pulmonary edema,arrhythmias,pulmonary infection.Conclusion VATS is a safe and effeetlve technique with smaller wound and shorter hospital stay for the treatments of spontaneous hemopneumothorax in aged people.
6.Hepatoprotective Effect of Alcohol-Extract Propolis Against Acetaminophen-induced Acute Hepatic Injury in Mice

Objective To investigate the hepatoprotective effect of Propolis alcohol-extract (PAE) against acetaminophen (APAP)-induced acute hepatic damage in mice and to explore the possible mechanism.Methods Sixty-three C57BL/6 MT(-/-)mice were equally randomized into 9 groups.The normal control group and the model group received gastric gavage of normal saline (20 mL?kg-1),four PAE groups were given PAE in the dose of 12.5,25,50 and 100 mg?kg-1 respectively,alcohol +APAP group and alcohol control group received 20 %alcohol 20 mL?kg-1 and PAE control group was given PAE in the dose of 100 mg?kg-1 qd,for 4 consective days.Thirty miniutes after last administration,the mice in the normal control group,PAE control group and alcohol control group received saline 10 mL?kg-1,and the mice in other groups received APAP 380 mg?kg-1 to induce acute hepatic injury.The activities of serum alanine aminotransferase (ALT),aspartate aminotransferase (AST) and lactate dehydrogenase (LDH) were determined,and the liver tissues were collected for histopathological assessment by HE staining under light microscope.The ratio of glutathione (GSH) and oxidized glutathione (GSSG),and the content of GSH,GSSG and malondialdehyde (MDA) in liver homogenate were also measured.Results Compared with the model group,PAE could markedly decrease serum ALT,AST and LDH activity,reduce the MDA level in liver homogenate,and increase hepatic GSH content and the ratio of GSH/GSSG in the liver homogenate.The hepatic histopathological changes in liver were also significantly ameliorated.In PAE control group,GSH content and the ratio of GSH/GSSG were also increased.However,the above indexes remained unchanged in alcohol control group.Conclusion The propolis alcohol-extract can prevent the liver from PAP-induced acute hepatic injury.
7.Cardiac protection of prostacyclin secreted from endothelial progenitor cells against oxidative stress-induced apoptosis:verified by cardiac electrophysiological tests

BACKGROUND:Prostacyclin (PGI2) and its analogs have been reported to prevent pressure overload-induced cardiac hypertrophy, and to reduce cardiac ischemia/reperfusion injury. However, clinical application of PGI2 is chalenging due to its short half-life (< 2 minutes). Thus, we have generated PGI2 expressing rat endothelial progenitor cel strains (PGI2-EPCs) that constitutively secrete prostacyclin.
OBJECTIVE:To investigate the protective effectof PGI2-EPCs against oxidative stress-induced cardiomyocyte injury.
METHODS:Cultured H9c2 celsin vitrowere assigned into four groups: H9c2 cels treated by H2O2for 4 hours. H9c2 cels were pretreated by conditioned medium (colected form EPCs and PGI2-EPCs or colected form EPCs and PGI2-EPCs mixed with native EPCs) before the addition of H2O2. PBS instead of conditioned mediums served as negative control. The paracrine effect of PGI2-EPCs onin vitro angiogenesis of native EPCs was evaluated. MTT and Hoechst 33342 assays were used to examine the protective effect of conditioned medium on H2O2-induced rat embryonic cardiomyocyte apoptosis and cel viability. Finaly, the effect of conditioned medium on the electric activities of adult cardiomyocyteswas measuredby whole-cel patch clamp techniques.
RESULTS AND CONCLUSION:When native EPCs mixed with conditioned medium of PGI2-EPCs, the total length of tubes was significantly longer compared with those mixed with CM of EPC. Rat embryonic cardiomyocytes pretreated with conditioned medium of PGI2-EPCs significantly reduced H2O2-induced apoptosis and preserved cel viability compared with pretreatment with EPC-conditioned medium and without pretreatment (P< 0.01). Pretreatment of rat adult cardiomyocyteswith conditioned medium of PGI2-EPCs abolished H2O2-induced early afterdepolarization and shortened H2O2-induced action potential duration prolongation (P< 0.01) towards baseline.Our findings indicate thatPGI2-EPCs protect against oxidative stress-induced cardiomyocyte injury through paracrine action.ThisStudy providesthe groundwork for an innovative cel therapy approach to treat ischemic heart disease.
8.The relationship between lymphocyte phenotype and clinical presentation in patients receiving immunotherapy combined with chemoradiotherapy

Objective:To observe the relationship between lymphocyte phenotype and clinical presentation in cancer patient receiving immunotherapy combined with chemoradiotherapy.Methods:42 patients were enrolled into the study.Among these patients,22 received stimulated normal peripheral blood lymphocytes(PBLs) as well as chemoradiotherapy,and 20 patients received chemoradiotherapy only as control group.Immunotherapy was administrated biweekly,totally 8 times.Results:22 patients receiving immunotherapy had lymphocyte phenotype change which was also in accordance with symptom improvement,but didn't reach statistical significance.Among 22 patients receiving immunotherapy had 15(68.18%) had lymphocyte phenotype change which was in accordance with symptom improvement,including elevation of CD3 + 3,CD + 4(P
9.An additional augmentative locking compression plate combined with bone graft for treatment of aseptic subtrochanteric fracture nonunion after intramedullary nailing

Objective:To investigate the efficacy of an additional augmentative locking compression plate combined with bone graft for aseptic subtrochanteric fracture nonunion after intramedullary nailing.Methods:From October 2016 to October 2019, 32 patients (25 males and 7 females) were treated at Department of Orthopedics, The Sixth People's Hospital Affiliated to Shanghai Jiaotong University for aseptic subtrochanteric fracture nonunion after intramedullary nailing. Their ages ranged from 27 to 68 years (average, 50.5 years) and their nonunion time from 9 to 24 months (average, 12.2 months). According to the Weber-Cech classification for nonunions, 9 cases belonged to the highly vascular type and 23 to the ischemic type. With the original intramedullary nails retained, all patients were fixated with an additional augmentative locking compression plate before simultaneous autologous iliac bone grafting. All patients were followed up regularly for fracture union, function of the affected hip and complications.Results:The 32 patients were followed up for 6 to 24 months (average, 12.9 months). All nonunions obtained clinical and imaging union. The clinical union time ranged from 3 to 8 months (average, 5.2 months), and the imaging union time from 6 to 10 months (average, 7.4 months). At the last follow-up, the therapeutic efficacy was evaluated as excellent in 28 cases and as good in 4, giving a good to excellent rate of 100%(32/32), according to the Sanders scoring for post-traumatic hip joint; the efficacy was evaluated as excellent in 21 cases, as good in 10 cases and as fair in one, giving a good to excellent rate of 96.9%(31/32), according to the Harris hip scoring. No such complications occurred as infection or loosening or breakage of internal fixation.Conclusion:An additional augmentative locking compression plate combined with bone graft is a reliable and effective treatment for aseptic subtrochanteric fracture nonunion after intramedullary nailing.
10.Real-time Gait Training System with Embedded Functional Electrical Stimulation.

To solve the problem that mostly gait analysis is independent from the treatment, this work proposes a system that integrates the functions of gait training and assessment for foot drop treatment. The system uses a set of sensors to collect gait parameters and designes multi-mode functional electrical stimulators as actuator. Body area network technology is introduced to coordinate the data communication and execution of the sensors and stimulators, synchronize the gait analysis and foot drop treatment. Bluetooth 4.0 is applied to low the power consumption of the system. The system realizes the synchronization of treatment and gait analysis. It is able to acquire and analyze the dynamic parameters of ankle, knee and hip in real-time, and treat patients by guiding functional electrical stimulation delivery to the specific body locations of patients.
