1.Correlation research on the MRI quantity of lumbar modic changes and low back pain.

OBJECTIVETo analyzed the relationship between lumbar endplate Modic area changes rate and low back pain by measuring MRI T2 sagittal image of lumbar endplate Modic area changes rate.
METHODSFrom December 2011 to June 2012,70 patients with low back pain in operation were evaluated on pain by VAS and function by JOA,and examined by MRI including 39 males and 31 females with an average age of (51.00 +/- 11.89) years ranging from 29 to 72 years old. Among them, 54 cases had lumbar endplate Modic changes involving 15 cases in types Modic I ,21 cases in type Modic II, 11 cases in type Modic III ,mixed type Modic in 7 cases (eliminated for too few cases). Modic area changes and corresponding vertebral area were measured on MRI T2 median sagittal. The areas of two ways were compared to yield the rate of changes for Modic, for multisegmental Modic changes to calculate the total ratios. A correlation was observed among JOA, VAS and the rate of Modic changes.
RESULTSThe correlation coefficient of change rate of Modic I with JOA score was r = -0.308, P = 0.048 < 0.05, there was a negative correlation;the correlation coefficient of change rate of Modic I with VAS scores was r = 0.428,P = 0.021 < 0.05, there was a positive correlation. The correlation coefficient of change rate of Modic II with JOA score was r = -0.375, P = 0.043 < 0.05, there was a negative correlation;the correlation coefficient of change rate of Modic II with VAS score was r = 0.352, P = 0.041 < 0.05, there was a positive correlation. The area change rate of Modic III had no significant correlation with low back pain degree (P > 0.05).
CONCLUSIONModic I and II area changes rate of of patients with low back pain is closely related to the degree of pain low back pain, Modic III area changes rate is not significant correlated to the degree of lower back pain.

2.Changes of T-helper1/T-helper2 Special Cytokine mRNA Expression during 5 Weeks Incremental Load Training

Objective To explore the exercise induced immunity changes during incremental load training from the view of the expression of helper T cells (Th) mRNA. Methods Using the technique of real-time quantified PCR to observe T-helper-special cytokine mRNA of peripheral blood mononuclear cell dynamically in healthy young male who have been trained with incremental exercise for five weeks. Results There was no obvious change in IL-2 mRNA during five weeks training. Meanwhile IL-4 mRNA increased greatly in the second, third and fourth week. IFN-? mRNA as well as IFN-? mRNA/IL-4 mRNA increased remarkably in the third, fourth and fifth week . Conclusion The expression of T-helper1/ T-helper2 cytokine mRNA were up-regulated after five weeks and the subjects were gradually adapt to the increased load which indicated the enhancement of immunity function .
3.Efficacy and safety of rivaroxaban versus enoxaparin in prevention of deep vein thrombosis after major orthopedic surgery

Objective To evaluate the efficacy and safety of rivaroxaban versus enoxaparin in prevention of deep vein thrombosis after major orthopedic surgery.Methods A total of 212 patients who accepted major orthopedic surgery and received rivaroxaban or enoxaparin in prevention of deep vein thrombosis were recruited in this study.Of the included 212 subjects, 102 patients( experiment group) received rivaro-xaban 10 mg· d-1 orally 6 h after surgery for 28 days and 110 patients ( control group ) received enoxaparin 0.4 mL 12 h before surgery and after surgery for 10 days.The risk of developing deep vein thrombosis within 6 months, perioperative blood transfusion, secondary bleeding and hematoma were compared between the two groups.Results In the period of 6 moths follow-up, 3 (2.9%) and 2 cases (1.8%) of deep vein thrombosis were found in the experiment and control group, respec-tively, but without statistical difference ( P>0.05 ).And no significant difference in coagulation index were recorded between the two groups ( P>0.05).There were 8 cases of blood transfusion, 3 cases of seconda-ry bleeding, 2 cases of wound bleeding, 4 cases of hematoma and 1 case of wound infection in experiment group.And 11 cases of blood transfusion, 5 cases of secondary bleeding, 3 cases of wound bleeding, 5 cases of hematoma and 1 case of wound infection in control group were found.The risk of developing anticoagulation between the two groups was not statistically different ( P>0.05 ).Conclusion Rivaroxaban and enoxaparin in prevention of deep vein thrombosis after major orthopedic surgery has similar clinical efficacy and safety.
4.Analysis of concurrent intravertebral vacuum sign in thoracolumbar fractures after posterior internal fixation.

OBJECTIVETo investigate the importance of TLICS classification to surgical options of thoracolumbar fractures by analyzing the cause of intravertebral vacuum sign, vertebral collapse and vertebral pseudarthrosis.
METHODSFrom January 2006 to December 2010, the clinical data about 15 patients with postoperative complications by thoracolumbar fracture after posterior internal fixation were retrospectively analyzed. There were 9 males and 6 females, ranging in age from 18 to 75 years, with an average of 54.6 years. Of them, fracture site in T12 was 7 cases, L1 was 5 cases, L2 was 3 cases; compression fractures was in 12 cases and burst fracture was in 3 cases; according to classification of TLICS, 12 cases were of type I ,3 cases of type III. And the causes of complications after posterior fixation were analyzed according clinical manifestation and imaging finding combined with review literatures.
RESULTSAfter 10 to 20 months following-up (with average of 15 months), loss of vertebral height found in 9 cases (4 cases existed vertebral collapse, as well as 3 cases occurred screw loosening) and Intravertebral Vacuum Sign appeared in 6 cases.
CONCLUSIONIn order to avoid the vertebral vacuum and fixation failure, the clinical data of patients should be roundly and carefully evaluated, surgical indications should be strictly controlled and the surgical approach should be selected according to correct classification. Particularly, the reconstruction of the stability of former spinal column shoud be paid more attention.

5.Clinical experience on postoperative balance of hemostasis and antithrombus for patients with hemophilic arthritis after arthroplasty.

OBJECTIVETo observe the clinical significance of postoperative personalized antithrombotic therapy for patients with hemophilic arthritis (HA) patients after arthroplasty.
METHODSFrom September 2005 to October 2013, 11 cases of arthroplasty for hemophilic arthritis in hip and knee total operation 14 times,including 1 case of double knees (calculated as one operation), operation in left knees 6 times, operation in right knees 5 times, 2 in hip. All the patients were male and the age ranged from 23 to 57 years old,with an average of (36.1 ± 11.0) years old; the average weight was (64.1 ± 8.9) kg. All the patients were preoperatively diagnosed and classified as hemophilic arthritis with the radiological images and laboratory tests. According to the function of joints, the risk of postoperative venous thromboembolism (VTE), and dynamic observation of Factor VIII:C (FVIII:C) activity, patients were treated with personalized antithrombus by adjusting the dosage of recombinant human coagulation factor VIII (Kogenate FS). All the patients were orderly divided into postoperatively distal joints moving group and none-moving group to observe the coagulation function.
RESULTSThe enrolled patients had no postoperative complication of VTE and pulmonary embolism (PE). The APTT and D-2 were different between two groups in the postoperative early stage. Length of hospital day was shorter in the moving group than none-moving group.
CONCLUSIONBecause of the self-coagulation disorder, patients with HA tended to bleed. However it doesn't mean that there is no risk of postoperative thrombosis. Therefore,it's important to determine how to control the balance between postoperative antithrombus, hemostasis,and coagulation factor replacement therapy after arthroplasty for HA. Postoperative moving has proved helpful for HA, especially in reducing the risk of hemostasis and shortening the time in hospital.

6.Sensory Preference to Balance Maintained

Objective To explore the influence of cognitive-dual task on balance function and analyze the sensory strategy of balance maintained.Methods Form June,2015 to June,2016,34 healthy young people were randomly divided into control group(n=17)and obser-vation group(n=17).They were tested with the modified Clinical Test of Sensory Interaction on Balance(mCTSIB)on Neurocom Balance Manager,and retested two weeks later,while the observation group completed mental arithmetic cognitive task simultaneously.The center of gravity(COG)sway velocity and the comprehensive sway velocity were measured in both eyes open(EO)and eyes closed(EC)standing on firm or foam support.Results For the second test,The COG sway velocity decreased in the control group as EC standing on foam sup-port(t=2.535,P<0.05),while it increased in the observation group as EO standing on foam support(t=-2.220,P<0.05),and it was more than that in the control group(t=-3.128,P<0.01).The comprehensive sway velocity decreased in the control group(t=3.007,P<0.01),but increased in the observation group(t=-2.170,P<0.05),which was significantly different between two groups(t=-2.143,P<0.05).Conclu-sion Cognitive tasks may interfere balance function in healthy youth,especially in the condition of EO standing on foam support.Sensory re-source may preferer to meet the balance maintained during cognitive-dual tasks.
7.Efficacy and safety of rivaroxaban in the prevention of deep vein thrombosis in patients with hip arthroplasty

Objective To investigate the efficacy and safety of rivaroxa-ban in the prevention of deep vein thrombosis in patients with hip arthro-plasty.Methods Ninety-eight patients with hip arthroplasty were re-cruited in our hospital.The included 98 cases were randomized divided into rivaroxaban group ( n=46 ) with rivaroxaban 10 mg · d -1 orally with 5-week and low molecular heparin ( LMH) group with LMH 4100 U with 2-week.The risk of deep vein thrombosis between the two groups was recorded within the flowing 6 months.Results With a follow-up peri-od of 6 months, the incidence of deep vein thrombosis were 2.3%(1/44) and 14.6%(7/58) in the rivaroxaban group and LMH group re-spectively , which indicated the incidence was much lower in rivaroxaban group ( P<0.05 ).But the platelet count , coagulation function , drainage volume and subcutaneous ecchymosis were not different between the two groups ( P>0.05 ).Conclusion Rivaroxaban can significant decrease the risk of deep vein thrombosis in patients with hip arthroplasty without increasing the side effects.

9.Study on the source of human infection with H9N2 influenza virus from live poultry markets in Changsha

Objective To explore the source of human infection H9N2 avian influenza virus (AIV). Methods Environmental AIV nucleic acid monitoring was conducted for live poultry markets in Changsha city from 2014 to 2015, and data of human infection H9N2 subtype AIV cases worldwide were collected. Phylogenetic trees of hemagglutinin(HA), neuraminidase(NA)and non-structural protein(NS)genes from human infection H9N2 subtype AIV, the live poultry markets environmental H9N2 subtype AIV and partial avian H9N2 subtype AIV were constructed using the MEGA 6.06 software, respectively. Results In 2014-2015, H9 subtype AIV had the highest nucleic acid positive rate (44.76%) in the live poultry markets environment of Changsha city, and the pollution was serious. A total of 27 cases of human infection with H9N2 subtype AIV had been reported worldwide, and most of these patients recovered after treatments.Epidemiological survey showed that 59.26% (16/27) of cases had a clear history of exposure to poultry or live poultry markets. The phylogenetic trees of HA, NA and NS genes showed that the human infection H9N2 subtype AIV isolates isolated from Hunan and Guangdong were closely related to the H9N2 subtype AIV isolated from the live poultry markets environment in Hunan and Guangdong provinces from 2013 to 2016. The nucleotide similarity was as high as 97%-99%. Conclusion Live poultry market is one of the sources of human infection with H9N2 influenza virus.
10.Improvement Effect and Its Mechanism of Marmesin on Cognitive Impairment in Mice with Alzheimer's Disease

Objective To investigate the improvement effect and mechanism of marmesin on cognitive impairment in Alzheimer's disease(AD)mice.Methods Fifty mice were randomly divided into five groups:blank group,model group,low-and high-dose marmesin groups and donepezil(positive drug)group,with 10 mice in each group.After 21 days of continuous administration,except for the blank group,the mice in other groups were given intraperitoneal injection of scopolamine to establish the AD model.Network pharmacology was used to construct the protein-protein interaction(PPI)network of common targets of marmesin in the treatment of AD,and gene ontology(GO)and Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment analysis were performed to provide further research direction.The cognitive function of AD model mice was evaluated by Morris water maze,open field test and new object recognition test.Nissl staining was used to observe the damage of hippocampal neurons.The levels of acetylcholine(Ach),acetylcholine transferase(ChAT),acetylcholinesterase(AChE),reactive oxygen species(ROS),malondialdehyde(MDA)and catalase(CAT)in hippocampus of mice were detected by kit.The protein expression levels of interleukin 6(IL-6),interleukin 1β(IL-1 β),tumor necrosis factor α(TNF-α),nuclear factor E2-related factor 2(NRF2),silent information regulator homologous protein 3(SIRT3),Kelch-like ECH-associated protein 1(KEAP1),quinone oxidoreductase 1(NQO1)and heme oxygenase 1(HO-1)in hippocampus were detected by Western Blot.Results Compared with the model group,the latency of Morris water maze test was significantly shortened in the high-dose marmesin group,the time of entering the target area in the open field new object test and the movement distance in the central area of the open field were prolonged,the number of neurons in the hippocampal CA1 and CA3 regions was significantly increased,the levels of ChAT and Ach in the hippocampus were significantly increased,AChE level was significantly decreased,CAT level was significantly increased,ROS and MDA levels were significantly decreased,TNF-α expression level was decreased,SIRT3 and HO-1 expression levels were increased,and KE AP1 protein expression level was decreased,the differences being statistically significant(P<0.05 or P<0.01 or P<0.001).Conclusion Marmesin can effectively improve the learning and memory impairment of AD mice,and its mechanism may be related to the activation of NRF2/SIRT3 signaling pathway,thereby alleviating oxidative stress level and neuroinflammation,and repairing cholinergic neuron function.
