1.UPA mRNA and MMP-9 mRNA expression and their correlation with invasion and metastasis and prognosis of gastric carcinoma

Objective To investigate the mRNA expression of UPA and MMP-9 in gastric carcinoma and their correlation with histological type, growth-type, differentiation, vessel invasion, metastasis and prognosis. MethodsIn situ hybridization was used to detect the mRNA expression of UPA and MMP-9. ResultsIn situ hybridization revealed that among 105 cases, the positive rates of UPA mRNA and MMP-9 mRNA were both 58.1%. There was no significant relationship between UPA mRNA and histological type( r _ s = 0.123, P =0.210)and differentiation ( r _ s =0.102, P =0.298)of the tumor. However, there was a significant difference between growth-type( r _ s =0.344, P =0.001),tumor invasion depth( r _ s =0.296, P =0.002),vessel invasion( r _ s =0.198, P =0.042),lymph node( r _ s =0.332, P =0.001)and distant metastasis( r _ s =0.530, P =0.001);there was no significant relationship between MMP-9 mRNA staining and histological type( r _ s =0.143, P =0.145)and differentiation( r _ s =0.102, P =0.298)of the tumor. However, there was a statistically significant difference between growth-type( r _ s =0.267, P =0.006),tumor invasion depth ( r _ s =0.335, P =0.001)、vessel invasion( r _ s =0.209, P =0.032),lymph node( r _ s =0.343, P =0.001)and distant metastasis( r _ s =0.468, P =0.001);There was a positive relationship between UPA mRNA and MMP-9 mRNA expression( r _ s =0.237, P =0.015). The mean survival time in cases with positive UPA mRNA and MMP-9 mRNA expression were significantly shorter than that of cases with negative expression. ConclusionThe mRNA expression of UPA and MMP-9 can predict the invasion and metastasis of gastric carcinoma, They can be used as markers of prognosis of gastric carcinoma in clinical practice.
2.Regulative mechanisms of oxidative stress in kidney in diabetic nephropathy and interventional effects of Chinese herbal medicine.

In the development of diabetic nephropathy (DN), reactive oxygen specie (ROS) over much in vivo leads to oxidative stress(OS)-related renal injuries, which are characterized by the structural and functional changes in glomerular and renal tubular cells in morphology. The regulative approaches of OS involve the several signaling pathways, in which, both p38 mitogen-activated protein kinase (MAPK) signaling pathway and adenosine monophosphate-activated protein kinase (AMPK) signaling pathway play the important roles as the target of anti-oxidants. The interventional actions of Chinese herbal compound prescriptions and the extracts of single Chinese herbal medicine (CHM) on OS in the kidney in DN include regulating the balance between ROS and antioxidants, reducing the production of AGEs, inhibiting the expression of growth factors and intervening the activity of signaling pathways.

4.Effect of budesonide aerosol treatment on expression of glucocorticoid receptor and nuclear factor-κB in asthmatic mice.

OBJECTIVETo study the effect of budesonide aerosol inhalation on the expression of glucocorticoid receptor (GR) and nuclear factor (NF)-κB in asthmatic mice.
METHODSTwenty-four healthy male BALB/c mice aged 6 to 8 weeks were randomly divided into three groups (n=8 each): normal saline (control group), asthma model (asthma group) and budesonide-treated asthma (BUD group). Asthma was induced by intraperitoneal injection of ovalbumin (OVA) and aluminium hydroxide suspension and aerosol inhalation of OVA solution. Mice were sacrificed 24 hours after the last challenge. Eosinophil count in the bronchoalveolar lavage fluid (BALF) was determined. Pathological examination of the lung tissues was performed and the expression levels of GR and NF-κB were measured by immunohistochemical analysis.
RESULTSEosinophil count in the BALF was significantly higher in the asthma and BUD groups than in the control group (P<0.05). BUD treatment decreased eosinophil count in the BALF compared with the asthma group (P<0.05). The lung tissues in the BUD group showed a less severe infiltration of eosinophils and lymphocytes compared with the asthma group. The percentage of GR-positive cells in the asthma group decreased significantly compared with the control group (P<0.05), and the percentage of GR-positive cells in the BUD group increased significantly compared with the asthma group (P<0.05). Compared with the control group, the percentage of NF-κB-positive cells increased significantly in the asthma group (P<0.05), and the percentage of NF-κB positive cells in the BUD group was significantly reduced compared with the asthma group (P<0.05).
CONCLUSIONSThe action mechanism of budesonide in treating asthmatic mice may be related to the upregulation of GR expression and the inhibition of NF-κB activity.

5.Effect of tagalsin on p53 and Bcl-2 expression in hepatoma H(22) tumor-bearing mice.

OBJECTIVETo explore the effect and mechanism of tagalsin on hepatoma cells.
METHODSThe animal models were established by transplanting H(22) mouse hepatoma cells to mouse liver, and ten days later the mice were randomly divided into five groups: blank group, carmofur positive group and tagalsin groups, including low-dose, middle-dose and high-dose groups. Then medicine or oil was given to the mice by gastric gavage in consecutive 5 days with a 2-days interval as a course of treatment, two courses in all. All mice were killed at 24 hours after medication, and the survival period, ascites conditions, aggressive conditions intra- or extra-liver, weight changes, tumor volume and spleen index of the tumor-bearing mice were observed. Pathological changes of the tumors were examined. Apoptotic factors p53 and Bcl-2 protien and mRNA were detected by immunohistochemistry and reverse transcription polymerase chain reaction (RT-PCR).
RESULTStagalsin inhibited the hepatoma growth effectively without influencing spleen index to some extent. The tumor inhibition rate of tagalsin low, middle and high dose groups were 17.9%, 63.1% and 71.8%, respectively. Immunohistochemical results showed that the p53 and Bcl-2 protein positive cell counts of the positive control and experimental groups were significantly lower than those of the blank group (P < 0.01). RT-PCR results showed that the p53 mRNA expression was significantly enhanced and Bcl-2 mRNA expression was decreased in the positive control groups and tagalsin treatment groups, especially in the high dose group, compared with those of the blank group (P < 0.05).
CONCLUSIONStagalsin can inhibit the growth of mouse hepatoma cells significantly. The mechanism of its anti-tumor effect may work via up-regulating the wild type p53 gene expression and down-regulating Bcl-2 gene expression and thus regulating tumor cell apoptosis.

6.A meta-analysis on therapeutic efficiency of self-ligating versus conventional brackets for orthodontic correction

BACKGROUND: The type of brackets is closely related to the efficiency of orthodontic treatment. A self-ligating bracket is characterized by low friction, simple operation, mild pain and discomfort, and it has been an issue of concern in orthodontics. In recent years, studies on the therapeutic efficiency of self-ligating brackets versus conventional brackets for orthodontic correction have been reported, but there is no conclusion. OBJECTIVE: To systematically compare the conventional and self-ligating brackets concerning treatment efficiency. METHODS: A computer-based search of PubMed, EMBASE, MEDLINE, WanFang, VIP and CNKI was performed for randomized controlled trials concerning self-ligating brackets versus conventional brackets for orthodontic correction. The keywords were "orthodontic brackets/ self-ligating, random" in English and "appliance/self-ligating/ conventional/orthodontic correction efficiency" in Chinese. The retrieve time was set from inception to December 2017. Stata12.0 software was used for meta-analysis following literature screening, data extraction and risk assessment of bias. RESULTS AND CONCLUSION: A total of 9 randomized controlled trials involving 603 patients were included. The results of meta-analysis on the efficiency of the two groups showed that there was no significant difference in overall treatment time, number of visits, time of alignment and time of space closure (P > 0.05). Only the time of chairside operation showed significant difference between the two groups (P < 0.05). These findings indicate that compared with the conventional bracket, the self-ligating bracket has no obvious advantage in the therapeutic efficiency.
7.Relationship between bFGF mRNA and MMP-9 mRNA expression in gastric carcinoma and their clinicopathological features as well as patients survival.

OBJECTIVETo investigate the mRNA expression of bFGF and MMP-9 in gastric carcinomas and to find their correlation with tumor microvascular density (MVD), invasion, metastasis and patients survival.
METHODSIn situ hybridization and immunohistochemistry technique were used to test the expression of bFGF mRNA and MMP-9 mRNA and protein of CD34 in 105 specimens of gastric carcinoma.
RESULTSIn situ hybridization revealed that the positive rates of bFGF mRNA and MMP-9 mRNA were 60.95 and 58.1%, respectively; The mean MVD (46.09 +/- 11.52, 43.75 +/- 13.41 piece/0.72 mm(2)) in tumors with bFGF mRNA and MMP-9 mRNA positive expression was significantly higher than that (29.41 +/- 12.47; 33.45 +/- 13.92 piece/0.72 mm(2)) in tumors with their negative expression, respectively; The positive expression rates of bFGF and MMP-9 mRNA were correlated to invasion depth (r(s) = 0.211, P = 0.031; r(s) = 0.335, P = 0.001, respectively), growing pattern (r(s) = 0.324, P = 0.001; r(s) = 0.267, P = 0.006, respectively), vessel invasion (r(s) = 0.579, P = 0.001; r(s) = 0.209, P = 0.032, respectively), lymph node metastasis (r(s) = 0.405, P = 0.001; r(s) = 0.343, P = 0.001, respectively) and distant metastasis (r(s) = 0.474, P = 0.001; r(s) = 0.468, P = 0.001, respectively), but not correlated to tumor type (r(s) = 0.134, P = 0.173; r(s) = 0.103, P = 0.145, respectively) and differentiation (r(s) = 0.096, P = 0.332; r(s) = 0.102, P = 0.298, respectively); And then, the mean MVD in tumors with infiltrating type, stage T(3)-T(4), vessel invasion, lymph node metastasis and distant metastasis was significantly higher than that in tumors with expanding type (t = 10.105, P = 0.001), stage T(1)-T(2) (t = 5.961, P = 0.001), non-vessel invasion (t = 7.394, P = 0.001), non-lymph node metastasis (t = 3.819, P = 0.01) and non-distant metastasis (r = 10.578, P = 0.001); There was a positive relationship between MVD and bFGF mRNA and MMP-9 mRNA (t = 3.207, P = 0.002; t = 7.035, P = 0.001), respectively; the mean survival time in cases with positive bFGF mRNA and MMP-9 mRNA and MVD value >/= 39.5 was significantly shorter than that in cases with their negative expression and MVD value < 39.5.
CONCLUSIONSbFGF and MMP-9 promote angiogenesis in gastric cancer. Test of the expression of bFGF and MMP-9 may act as an useful index to determine angiogenesis, invasion, metastasis and patients survival.

8.Devine's technique with free skin grafting for concealed penis with prepuce deficit.

OBJECTIVETo investigate the clinical effect of Devine's technique with free skin grafting in the treatment of concealed penis with prepuce deficit.
METHODSThis study included 7 children with concealed penis, aged 6 - 15 (mean 8.6) years, 6 of them treated by circumcision previously. All the patients underwent Devine's operation to resect the inelasticity sarcolemma and lengthen the penis. The length of prepuce deficit ranged from 2 to 4 cm. Intermediate split thickness skin grafts of the corresponding length were taken from the femoribus internus to wrap up the tunica albuginea penis, followed by the procedures of saturation, encapsulation and fixation.
RESULTSSurgery time ranged from 70 to 120 minutes, averaging 90.5 minutes. The penis was prolonged about 2 - 4 cm after surgery. A 6-month follow-up revealed desirable penile appearance and normal penile erection.
CONCLUSIONDevine's technique with free skin grafting from the femoribus internus is an ideal treatment for concealed penis with prepuce deficit.

9.Clinical Application Value of Chitinase-3-like Protein 1 in Patients with Different Liver Diseases

Objctive To explore the clinical value of Chitinase-3-like protein 1(CHI3L1)in patients with chronic hepatitis B (CHB),liver cirrhosis and liver cancer.Methods 96 clinically diagnosed patients in Department of Infectious Diseases Hos-pital of Anhui Provincial Hospital from Jan 2016 to Feb 2017(28CHB,44livercirrhosis,24liver cancer,15healthy controls) were analyzed.The serum level of CHI3L1 was measured by enzyme-linked immunosorbentassay(ELISA).The Golgi pro-tein(GP73)was tested by double-antibody sandwich immunochromatographic assay.The Alpha-fetoprotein(AFP)was tested by means of chemical luminescence.Results There were significant differences between the groups of chronic hepati-tis B(CHB),liver cirrhosis,liver cancer and healthy controls on the CHI3L1 level(χ2=70.249,P<0.001).The CHI3L1 level of the liver cirrhosis group and the liver cancer group increased significantly compared with that of the healthy controls (P<0.001).The GP73 levels of these groups were significantly different(χ2=44.963,P<0.001).The GP73 levels of the CHB group,the liver cirrhosis group and the liver cancer group all increased significantly compared with that of the healthy controls(P<0.05).The AFP levels of these groups were significantly different(χ2=57.606,P<0.001).The AFP level of the liver cancer group increased significantly compared with that of the CHB group,the liver cirrhosis group and the healthy controls(P<0.001).Based on the receiver operating characteristic(ROC)curve of CHI3L1,GP73 and AFP in the CHB group and the liver cirrhosisgroup,the Area Under roc Curve(AUC)of CHI3L1 was 0.953(95%CI:0.902~1.000),the sensitivity was 88.6%,and the specificity was 92.9%,which was higher than GP73 and AFP.Based on the ROC curve of CHI3L1,GP73 and AFP in the liver cirrhosis group and the liver cancer group,the AUC of AFP was 0.930(95% CI:0.871~0.989),the sensitivity was 75.0%,and the specificity was 97.7%,which was higher than CHI3L1 and GP73.The corre-lation between the CHI3L1,GP73 and AFP in CHB,liver cirrhosis and liver cancer groups were analyzed.There was a posi-tive correlation between AFP and GP73(rs=0.491,P<0.001),a positive correlation between AFP and CHI3L1(rs=0.452,P<0.001),a positive correlation between GP73 and CHI3L1(rs=0.554,P<0.001).Conclusion CHI3L1 is good at diagnosis of liver cirrhosism,better than GP73 and AFP.And AFP could be more beneficial in patients with liver cancer, better than CHI3L1 and GP73.
10.Relationship Between Cardiac Troponin Ⅰ Autoantibody and Left Ventricular Remodeling in Patients With Acute Myocardial Infarction

Objectives: To analyze the relationship between cardiac troponin I autoantibody (cTnIAAb) and left ventricular remodeling in patients with acute myocardial infarction (AMI). Methods: A total of 131 AMI patients were enrolled. Serum levels of cTnIAAb were measured by ELISA. Echocardiography was examined at the onset of AMI and 1 year follow-up evaluation. Taking left ventricular end systolic volume (LVESV) increasing>15% as left ventricular remodeling, 2-classified logistic stepwise regression analysis was conducted to screen 12 risk factors related to left ventricular remodeling. Results: 23/131(17.6%) patients were with positive cTnIAAb and 82.4% with negative cTnIAAb. 49 patients lost contact and in the rest 91 patients, 21.1% were with positive cTnIAAb. Clinical information was similar between cTnIAAb positive and negative patients upon admission, P>0.05; echocardiography showed that 28 (42.2%) patients had LVESV increasing>15% by 1 year follow-up study whom including 10 (52.6%) patients with cTnIAAb positive and 18 (25.4%) negative. 2-classified logistic stepwise regression analysis indicated that BNP peak and positive cTnIAAb were the risk factors for left ventricular remodeling (OR=1.001, 95% CI 1.001-1.002) and (OR=3.552, 95% CI 1.148-10.989), both P=0.028. Conclusions: Serum cTnIAAb was positive in part of AMI patients which was related to increased risk of left ventricular remodeling; cTnIAAb might be involved in pathophysiological process of left ventricular remodeling in AMI patients.
