1.Inhibiting effects of oxymatrine on hemorrhagic fever with renal syndrome virus infection in vivo and in vitro

Objective To study the effects of oxymatrine as inhibitor of hemorrhagic fever with renal syndrome virus (HFRSV) infection in vitro and in vivo.Methods In vitro studies,a dose of oxymatrine without cytotoxicity and 76-118 strain of HFRSV was taken to treat Vero cells in three ways:①After treated with oxymatrine for 48 h,Vero cells were attacked by HFRSV at dilution of 10-1 ~ 10-6,respectively for 24 h before changing to maintenance medium; ②Vero cells were first attacked by HFRSV of 10-1 ~ 10-6 dilution respectively,then oxymatrine was used for 48 h before changing to maintenance medium; ③Vero cells were attacked by HFRSV at dilution of 10-1 ~ 10-6 respectively,and meanwhile treated with oxymatrine for 48 h before changing to maintenance mcdium.Each dilution handled four porocytes,and four positive controls were set up at the same time.Indirect immunofluorescence assay (IFA) was performed to determine the inhibitory effect of oxymatrine in experimental group and positive control.In vivo studies,thirty 2-week-old hamsters,weighing about 30-40 g,were divided into experimental and control groups according to body weight,n =15.These aninals were inoculated intraperitoneally with HFRSV in 100TCID50(0.1 ml each); on days 4-13,0.1 ml of oxymatrine 1:100 were given to each hamster in experimental group daily by intraperitoneal injection,while the same amount of saline was given to the control ones.Lung tissue of hamsters was then dissected out to slice to be identified by immunofluorcscence stain.Results It was demonstrated that oxymatrine with the diluted fractions of 1:8 was safe in vitro.When the virus dilution of HFRSV was l0-4,compared with control groups,the differences were statistically significant in method 2 and 3 (z =-2.53,-2.53,all P < 0.05),while no statistical significance in method 1 (z=5.36,P> 0.05).When the virus dilution of HFRSV was 10-1 ~ 10-3,10-5,10-6,the differences were not statistically significant (z--0.00,-0.32,-0.19,4.21,4.21,all P > 0.05).In vivo studies,compared with control group,the differences were statistically significant in experimental group (z =-3.85,P < 0.05).Conclusion Oxymatrine significantly inhibites HFRSV.
2.First-trimester ultrasonography in the diagnosis of fetal holoprosencephaly

Objective To determine the image feature and clinical value of first-trimester ultrasonographic diagnosis of fetal holoprosencephaly.Methods A total 35 580 fetuses at gestational age between 11 + 6 and 13 + 6 weeks were scanned in our hospital by color Doppler ultrasound according to the prenatal ultrasound from Jan 2009 to Jan 2017.Fetal craniocerebral and faceprestige were checked carefully.Cases of fetal holoprosencephaly diagnosed in first-trimester were followed sonographically,and the clinical outcomes were recorded.Results Totally five cases of fetal holoprosencephaly were detected.Four fetuses with other associated malformations were detected by first trimester ultrasound,including one case with beak nosetril,eyes too close,one case with beak nosetril,fetal cleft lip and palate,fetal hydrops,congenital heart disease and mid gut herniation,one case with thickened nucha.One translucency (NT),one case with mandibular micrognathia,one case with trisomy 13 syndrome,the other four cases were not checked.Terminal of pregnancy was performed in four cases during early pregnancy,one case was loss to follow-up.Autopsy was refused in these cases.Fetal appearance revealed one case of cleft lip,single nostril,mandibular micrognathia,one case of hydrops,polyphalangia,microtia,one case of beak nosetril,fetal cleft lip and palate,mid gut herniation,hydrops,one case of beak nosetril,eyes too close.Conclusion Fetal holoprosencephaly can be effectively detected and diagnosed during early pregnancy with standardized prenatal ultrasound.
4.Inhibitory effects of Zengshengping fractions on DMBA-induced buccal pouch carcinogenesis in hamsters.

BACKGROUNDZengshengping (ZSP) tablets had inhibitory effects on oral precancerous lesions by reducing the incidence of oral cancer. However, the severe liver toxicity caused by systemic administration of ZSP limits the long-term use of this anti-cancer drug. The purpose of this study was to evaluate the tumor inhibitory effects due to the topical application of extracts from ZSP, a Chinese herbal drug, on 7, 12-dimethlbenz(a)anthracene (DMBA) induced oral tumors in hamsters. The study also investigated the anti-cancer mechanisms of the ZSP extracts on oral carcinogenesis.
METHODSDMBA (0.5%) was applied topically to the buccal pouches of Syrian golden hamsters (6 - 8 weeks old) three times per week for six weeks in order to induce the development of oral tumors. Different fractions of ZSP were either applied topically to the oral tumor lesions or fed orally at varying dosages to animals with oral tumors for 18 weeks. Tumor volume was measured by histopathological examination. Tumor cell proliferation was evaluated by counting BrdU labeled cells and by Western blotting for mitogen-activated protein kinase (MAPK) protein levels. The protein levels of apoptosis marker Caspase-3 and regulator Bcl-2 protein were also measured by Western blotting.
RESULTSTopical application of DMBA to the left pouch of hamsters induced oral tumor formation. Animals treated with DMBA showed a loss in body weight while animals treated with ZSP maintained normal body weights. Both the ZSP n-butanol fraction and water fraction significantly reduced tumor volume by 32.6% (P < 0.01) and 22.9% (P < 0.01) respectively. Topical application of ZSP also markedly decreased the BrdU-positive cell numbers in oral tumor lesions and reduced the expression level of MAPK. In addition, ZSP promoted tumor cell apoptosis by increasing Caspase-3 expression but decreasing Bcl-2 protein production.
CONCLUSIONThe n-butanol and water fractions of ZSP are effective at inhibiting tumor cell proliferation and stimulating apoptosis in oral cancer suggesting that these fractions have chemopreventive effects on DMBA induced oral carcinogenesis.

5.Effect of compound EXP-2528 on angiotensin II-induced E-selectin and VCAM-1 expression in rat brain microvascular endothelial cells in vitro.

The aim of this study is to investigate the effect and mechanism of angiotensin (Ang) II on E-selectin and vascular cell adhesion molecule-1 (VCAM-1) expression in rat brain microvascular endothelial cells (BMEC) and evaluate the effect of compound EXP-2528, a novel Ang II type 1 (AT1) receptor antagonist. The experiment was performed in cultured BMEC of rat. The mRNA and protein expression of E-selectin and VCAM-1 in BMEC was analyzed by RT-PCR and Western blotting, respectively. The results showed that the mRNA and protein expression of E-selectin and VCAM-1 in BMEC were significantly upregulated by 4 h or 18 h exposure to 1 x 10(-7) mol x L(-1) Ang II. These effects were abolished by pretreatment with the selective AT1 receptor antagonists losartan and compound EXP-2528, but not with the AT2 selective antagonist PD123319. Combining losartan with PD123319 also significantly inhibited Ang II-induced E-selectin and VCAM-1 expression in BMEC, but there was no significant difference compared with losartan group. These findings indicated that Ang II upregulated E-selectin and VCAM-1 in BMEC by activating AT1 receptor and then involved in the development of cerebrovascular disease.

6.Thoughts of TCM Syndrome Differentiation and Treatment of Thyroid-Associated Ophthalmopathy

Thyroid-associated ophthalmopathy (TAO) belongs to autoimmun diseases, which is particularly closely related to the liver, spleen and kidney. Using these three organs as the theoretical foundation, the root of the disease can be grasped. Using syndrome differentiation, deficiency is the basic pattern and excess is the syndrome. The main reason for TAO is liver, spleen and kidney deficiency, while dampness, phlegm-blood stagnation in the eyes are pathogen. The rules of treatments are nourishing yin and clearing liver, supplementing and nourishing liver and kidney, removing dampness and phlegm, eliminating blood stasis and promoting circulation. By combining overall symptoms with ocular region changes, together with dynamic differentiation and flexible treatment, satisfactory results have been obtained in clinical practice.
7.Clinical value of prenatal ultrasonography in diagnosis of multiple fetal megacystis during early trimester

Objective To determine the imaging features and clinical value of first-trimester ultrasonographic screening of fetal megacystis in multifetal pregnancy.Methods Retrospective analysis was undertaken in 2159 cases of multifetal pregnancy screened in our hospital at gestational age from 11th to 13 + 6th weeks by color Doppler ultrasound between Jan 2011 and April 2016.Fetal bladder was defined by showing two umbilical arteries and the longitudinal length of the bladder was measured in mid-sagittalplane.Clinical progression of fetal megacystis diagnosed in first-trimester were followed and the outcomes were recorded.Results Totally six cases of fetal megacystis in multifetal pregnancy were detected,in which five cases were detected in one fetus of twin pregnancy and the other case was detected in one fetus of triplet pregnancy.In all cases of fetal megacystis,longitudinal length of the fetal bladder was more than 7 mm and double umbilical arteries were detected.In two cases of naturally-occurring twin pregnancy,abortion was performed and postnatal autopsy revealed prune belly syndrome.In the rest four cases of multifetal pregnancy through in vitro fertilization-embryo transfer (IVF-ET),two cases of twin pregnancies underwent selective feticide.The other two cases of twin and triplet pregnancies refused intervention,among which still birth happened in the megacystis fetus and spontaneous fetal death at gestational age of 13 + 2 weeks happened in the monochorionic monoamniotic twins of the triplet pregnancy.Totally,four live babies were born,among which three were healthy.The other baby was found to have 1.5 Mb deletion in chromosome 17q12 by microarray at gestational age of 32 weeks due to increased renal echogenicity.Conclusions First-trimester ultrasonography can effectively detect and diagnose megacystis in multifetal pregnancy.Early diagnosis and timely intervention are helpful for improving the outcome of fetal megacystis in multifetal pregnancy.

Objective To study the expression levels ofinterlenkin-6 (IL-6), IL-6 receptor (IL-6R),its soluble IL-6 receptor (sIL-6R) and soluble glycoprotein 130 (sgp130) in brain tissues of refractory temporal lobe epilepsy (TLE) and explore their role in TLE. Methods Forty patients with TLE and other 40 patients with trauma/hypertensive cerebral hemorrhage (controls), admitted to and performed surgery in our hospital from January 2010 to December 2010,were chosen in our study.Brain tissues from temporal lobe were collected during the surgery.By using real time-PCR and ELISA,the mRNA or protein expression levels of IL-6,IL-6R,sIL-6R and sgp130 were analyzed. Results The mRNA expressions of IL-6 and IL-6R in the TLE brain tissues were significantly higher as compared with those in controls (P<0.05).ELISA indicated that sIL-6R level in the TLE brain tissues was significantly higher than those in controls (P<0.05); however,sgp130 level in the TLE brain tissues was slightly higher than those in controls without significant difference (P>0.05). Conclusion The high level of IL-6 may influence the function of varies types of cells within the epileptic foci of patients with TLE through classical signaling pathway or trans-signaling pathway, which is potentially involved in the epileptogenesis of TLE.
10.Treatment of advanced neuroendocrine tumors with ~(90)Y-DOTATATE

Objective To evaluate the effcacy and side effects of [~(90)Y-1,4,7,10-tetraazacyclododecane-N,N',N",N'"-tetraacetic acid (DOTA)~0,Tyr3]octreotate (~(90)Y-DOTATATE) combined with lysine as a renal protective agent.Methods Twenty-five patients with advanced neuroendocrine tumors were confirmed as somatostatin receptor subtype-2 (SSTR2)-positive by somatostatin receptor scintigraphy (SRS).Each patient received 1-5 cycles of treatment and the interval between two cycles of treatment was 6-9 weeks.~(90)Y-DOTATATE was administered intravenously within 30 min.Lysine was injected before and after the administration of ~(90)Y-DOTATATE.After each treatment cycle.the side effects were assessed according to National Cancer Institute Grading Criteria(NCIGC).The etticacy was evaluated by the WHO criteria 8 weeks after the last treatment.Results Pattial remission was found in 1 patient (4%).minor response in 3 patients(12%),stable disease in 16 patients (64%)and tumor progression in 5 patients (20%).Two patients suffered from renal functional injuries and 3 patients developed leukocytopenia.Three patients showed nausea while another 3 patients presented vomiting.Conclusions ~(90)Y-DOTATATE in association with lysine may be a promising treatment method for the patients with metastatic and inoperable neuroendocrine tumors.More research work may be directed to reduce renal injury.
