2.Research and Development on Immunotherapy of Alzheimer's Disease
Journal of Shanghai Jiaotong University(Medical Science) 2006;0(07):-
Alzheimer's disease(AD),the most common form of dementia,it is lack of effective cure or preventive treatment.Dementias in the elder are an increasing medical, social and economic problems and current treatments are only mildly effective.Recently,amyloid-beta protein(A?) has become a major therapeutic target.A? vaccine treatment can improve cognition in the patients with AD,but adverse events, such as meningencephalitis were observed in clinical study. The passive A? immunotherapy in humans is effective with possible safety. However, patients need to be monitored carefully.

Doctor's degree is the highest degree for medical education.Nowadays, medical doctor candidates in more and more medical universities are requested to publish research papers in intemational journals embodied in Science Citation Index(SCI),or they can not graduate with doctor's degree. So,it is significant for medical doctor candidates to learn the related knowledge about SCI and Impact Factor(IF).

Objective In this study,we elucidated the role of high mobility group box chromosomal protein 1 (HMGB1) in collagen-induced arthritis (CIA) rat and the antagonist role of ethyl pyruvate by using a rat model of CIA as the research object by comparing the expression of HMGB1 in normal control group,CIA model group and ethyl pyruvate group. Methods Thirty-six rats were randomly divided into 3 groups (n=12):normal control group,CIA group and ethyl pyruvate group. Then the 6 rats were dissected at the 6th,9th week respectively.Thc expression of HMGB1 was analyzed by immunohistochemistry and Pathology-image analysis software in the cytoplasma. The expression of HMGB1 mRNA with real time-polymerse chain reaction (PCR) was evaluate, and the HMGB1 expression of each group were compared with t-test.Results The immunohistochemical results of HMGB1 showed that the expression intensity in the normal control group,CIA model group and ethyl pyruvate group was 2.1±0.6,7.3±1.2,6.0±1.2 respectively at the 6th week; and 2.2±0.7,12.4±4.5,5.5±1.0 at the 9th week respectively. The HMGB1 mRNA real time-PCR results had shown that the relative quantification of the normal control group and CIA model group were 1,2.865,2.602respectively at the 6th week and 1.005,4.694,1.729 at the 9th week.At those two points, the HMGB1 expressions of HMGB1 antagonist group were significantly higher than those of the normal controls (P<0.05).In addition, there was statistical significant difference(P<0.05) in the HMGB1 expression when compared with the placebo group.Furthermore, when the degree of HMGB1 expression among the three groups was compared, the HMGB1 antagonist group was decreased significantly (P<0.05).Conclusion The results has demonstrated that HMGB1 could induce inflammation in the synovial tissue of CIA rats, and has provided the rationale that HMBG 1 could be the target of treating rheumatoid arthritis (RA).The results of this study have shown that ethyl pyruvate could antagonize the effect of HMGB1.This finding may provide a new therapeutic target for the treatment of RA.

OBJECTIVE To survey the antibiotic susceptibility of Pseudomonas aeruginosa and medical treatment among the aged patients suffering from underlying disease.METHODS A total of 326 strains of P.aeruginosa were isolated among the aged patients with underlying disease by sputum culture.Drug susceptibility was performed and effective antibiotic treatment was given.RESULTS Amikacin(8.0%),imipenem(12.0%) and piperacillin/tazobactam(16.0%) could be chosen. The symptom of 326 patients was alleviated after clinical treatment, while some sputum samples were found still positive.CONCLUSIONS Combined use of two kinds of medicines may produce better effects in the patients with underlying disease, who infected with P.aeruginosa, but sputum should not be emphasized to converse negative in this case.

To analyze and summarize the clinicopathological features of renal inflammatory lesions which were easily misdiagnosed as renal tumors. Methods The cases from March 2006 to August 2015 in the Department of Pathology at Xijing Hospital were analyzed, retrospectively.Due to the diagnosis made by clinicians that these were renal (including renal pelvic) tumors, all cases had undergone radical nephrectomy or tumor enucleation.Among them, several inflammatory lesions were confirmed, and were combined with the patients' clinicopathological data and literature review to investigate the diagnosis and differential diagnosis. Results Of the 1 195 cases, only 6 cases (less than 1%) were renal inflammatory lesions, named as xanthogranulomatous pyelonephritis (2 cases),renal malakoplakia (1 case),renal tuberculosis (1 case),IgG4-related tubulointerstitial nephritis (1 case),and renal Wegener's granulomatosis (1 case). These diseases occurred in two males and four females (aged 14 to 55 years). All patients underwent radical nephrectomy due to the detected unilateral solitary renal mass in the imaging examination. Conclusion Renal inflammatory lesions are very rare, which show masses that are easily misdiagnosed as neoplastic lesions. Some of them have similar histological features. As the differences of treatment,a sufficient understanding of the clinicopathological features of these diseases is important to make a correct diagnosis.

Objective To explore the effect of minimally invasive clinical pathway in patients after car-diac interventional therapy. Methods One hundred and twenty patients who received cardiac interventional therapy from June 2005 to October 2006 were divided into the control group and the observation group. The control group received routine nursing while the observation group adopted minimally invasive clinical pathway of nursing. The nursing effect in the two groups was compared. Results The mean hospitalized duration, sat-isfaction degree and health knowledge level in the observation group were superior to those of the control group (P < 0.05). Conclusions The adoption of minimally invasive clinical pathway in patients after cardiac inter-ventional therapy could increase working efficiency nd ensure the nusing quality.

Insulin resistance model of 3T3-L1 adipocytes were prepared with plamotic acid.Adipocytes with generated insulin resistance were cultured with different concentrations of globular domain of adiponectin(gAd:250,500,1 000 ng/ml).The cell culture medium glucose content was detected with the glucose oxidase method, the mRNA expressions of insulin receptor substrate-1 (IRS-1),phosphatidylinositol-3 kinase(PI3K),and protein kinase B(PKB) were detected with real-time quantitative PCR method. The phosphorylation of IRS-1 was detected by Western blot.Compared with the control group, the experimental group showed significantly increased glucose consumption (P < 0.01),and with the increasing gAd concentration, glucose consumption was gradually increasing.IRS-1 phosphorylation was increased gradually with the increasing concentration of gAd.These results suggest that gAd can promote glucose uptake by 3T3-L1 adipocyte model with generated insulin resistance. This may be correlated with promoting insulin signal transduction and improving insulin resistance in adipocytes.
