2.Screening of probiotic properties of twelve strains of lactobacilli by in vitro techniques

Objective: To study the probiotic properties of 12 selected strains of lactobacilli.Methods: The probiotic properties of 12 selected strains of lactobacilli were investigated for their resistance to pH 2.5 and 0.3% oxgall,their adhesion to Caco-2 cells and antimicrobial activity against enteric pathogenic bacteria.Results: The 12 strains all showed relatively high resistance to acid and bile salt and varied antimicrobial activities against pathogenic bacteria of intestinal infection.Lactobacillus johnsonii JCM1022 exhibited a broad inhibitory effect on enteric pathogenic bacteria,especially on enteropathgenic E.coli and Salmonella enteritidis,while Lactobacillus reuteri JCM1081 possessed a high activity of adhesion to Caco-2 cells(495.07?80.03).Conclusion: Among the 12 strains,Lactobacillus johnsonii JCM1022 and reuteri JCM1081 are qualified candidates for clinical application.
3.Ovarian carcinoma presents as distant metastases without detectable tumors of the origin disease at the first presentation

Objective To study the characteristics, therapies and prognosis of the patients with epithelial ovarian cancer (EOC) that are initially categorized as extra abdominal adenocarcinoma of unknown primary Methods Twenty five patients with EOC, who were treated in the Cancer Hospital of Fudan University from Jan 1986 to Dec 1997, and manifesting as extra peritoneal or liver parenchyma metastases at the time of presentation, without detectable ovarian tumors, were retrospectively studied Results Supraclavicular and inguinal lymph node metastases were common in this group of patients, with 6 and 5 cases respectively, and 6 patients with more than two sites metastases simultaneously 16 patients (64%) were optimally surgical debulked 20 patients with stage Ⅳ EOC initially presenting as extra abdominal metastases experienced a better prognosis, with an estimated median survival of 24 months. Of whom the median survival was 30 months in patients presenting with pleural effusion or supraclavicular lymph node metastases Vs 19 months in those with other sites spread ( P =0 026 4) Conclusions The prognosis of such cases, particularly for those with supraclavicular lymphadenopathy or malignant pleural effusion, is a lot better than other stage Ⅳ EOC patients, probably because of most of the patients initially presenting with distant metastases being generally in a good condition competent for aggressive surgery or multi cycle chemotherapy

6.Study on the optimal choice of therapeutic approaches for elderly women with advanced epithelial ovarian cancer

1cm, there was a significant statistical difference in median survival of 61 and 12 months, respectively (? 2 =16.60, P =0.0001). The median survival for patients with and without peritoneal chemotherapy were 27 and 12 months, respectively (? 2 =3.45, P =0.0633). Residual disease, FIGO stage, recurrent ascites, uterus muscle involvement were independent prognostic determinants of survival identified by Cox's stepwise regression analysis. Conclusions Aggressive surgical cytoreduction should be performed in elderly AEOC patients as well as in younger patients, but multi-course platinum-based chemotherapy should be used in accordance with the performance status of elder women.
7.Cause of Death Related to Medical Disputes in Yancheng Area:A Study of 60 Autopsy Cases

Objective To summarize the pattern and main characteristics of fatal cases related to medical disputes in Yancheng area. Methods Sixty fatal cases of medical disputes were retrospectively analyzed to elucidate the annual incidence, characters of distribution of hospitals, gender and age of the decedents, types of diseases, and cause of death. Results Among 60 fatal cases, most cases happened in health clinics of county, township and village. There were more males than females. The major medical specialties in-volved included internal medicine, surgery, gynecology and pediatrics, with the internal medicine specialty having the highest incidence. Conclusion Police institutions have advantages in investigation of these cas-es in their jurisdictions, which could enhance the ability of local medicolegal examination.

Objective To investigate the timing and role of systemic retroperitoneal lymphadenectomy in patients with ovarian cancer. Methods From Jan. 1987 to Feb. 1994, 50 patients with ovarian cancer, who underwent retroperitoneal lymphadenectomy on second-look laparotomy (SLL), were retrospectively studied. Results The median age was 49 years. Overall survival at 3 and 5 years were 72% and 62%, respectively. 9.Evaluation of late course accelerated hyperfractionated radiotherapy plus concurrent chemotherapy for esophageal carcinoma

Objective To evaluate the treatment results and side-reactions of esophageal carcinoma treated with late course accelerated hyperfration radiotherapy plus concurrent chemotherapy(LCAHR+C). Methods A prospective randomized trial was carried out on 173 esophagus squamous carcinoma patients whom were randomized into two groups:1. LCAHR group—89 patients treated by conventional fractionated radiotherapy during the first two-thirds of the whole course with 40Gy in 20-22 fractions, then followed by LCAHR with 20-30Gy in 14-20 fractions, 1.5Gy per fraction, 2 times per day, to a total dose of 60-70Gy in 34-42 fractions over 37-42 days; 2.LCAHR+C group—94 patients were received the same radiotherapy as LCAHR, supplement with concurrent chemotherapy from the first day of radiotherapy. The chemotherapy regimen was LFP: intravenous infusion of cisplatin 20mg/d, calcium folinate 100mg/d and 5-fluorouracil 500mg/d for five consecutive days, every 28d as one cycle to totally 4 cycles. Results The short-term effective rate was 85% and 95% in LCAHR and LCAHR+C group,respectively(?~2=4.45,P=0.035).The 1-,2-and 3-year local control rate was 73%,55%,49% and 83%,73%,65% in LCAHR and LCAHR+C group, respectively(?~2=5.32,P=0.021).The 1-,2- and 3-year survival rate was 74%,53%,41% and 84%,65%,52% in LCAHR and LCAHR+C group,respectively(?~2= 2.85 , P= 0.091 ).The leucocytopenia and gastrointestinal tract side-reactions in LCAHR+C group were more severe than those of LCAHR group(?~2=7.85,15.06;P=0.005,0.000). Conclusions Late course accelerated hyperfractionated radiotherapy plus concurrent chemotherapy can be taken as a effective treatment for esophageal carcinoma. It can improve short-term curative effect and local control rate, in spite of increase in leucocytopenia and gastrointestinal tract side-reactions.
10.Effect of Short-chain Inulin Supplement on the Gut Microbiota in Mice Fed by High Fat Diet

Objective:Detect the gut microbiota change in mice caused by 10 % short-chain inulin addition in high fat diet condition.Methods:8-week-old male C57/B6J mice,5 mice were fed with high fat diets,5 mice were fed with high fat diets with 10 % short-chain inulin addition.Fed 8 weeks and then collected fresh feces.Detected the three main short chain fatty acids in fresh feces.Extracted gut bacteria genome DNA for 16S rRNA V4 region sequencing.Principal Coordinate Analysis (PCoA),Alpha diversity and LEfSe analysis were performed to detect gut microbiota changes induced by short chain inulin.Results:Gut bacterial DNA amount and SCFAs amount per gram feces increased.PCoA analysis demonstrated fecal microbiota from inulin and control group mice had distinctive different features and clustered well.Inulin group owned lower fecal microbiota diversity compared with control group.LEfSe analysis revealed that in family level,S24_7 increased,Deferribacteraceae,Lachnospiraceae and Ruminococcaceae decreased.PICRUSt predicted that 22 level 2 KEGG Orthology groups changed.Conclusions:Inulin addition altered the gut microbiota composition in mice in high fat diet condition and impact the gut microbiota gene function.
