1.Effect of xinmailong on metabolism of oxygen free radicals and content of lipofuscin in brain and hepatic tissues

BACKGROUND: Chinese medicine xinmailong (XML) injection is a kind of new nucleoside compound which comes from American giant cockroach of insects. A lot of studies show that XML has obvious effect on cardiovascular activity which can treat acute or chronic congestive heart failure and shock and improve microcirculation.OBJECTIVE: To observe the effect of XML on anti-aging of mice.DESIGN: Controlled observational study.SETTING: Department of Physiology and Pathology, Basic Medical College of Dali College.MATERIALS: The experiment was carried out at the Functional Laboratory of Basic Medical College between July 2002 and September 2004. A total of 40 ICR mice of both sexes were provided by Kunming Medical College,Yunnan Naturally Occurring Drugs Key Laboratory. XML injection was provided by Yunnan Tengchong Pharmacy Factory [batch number: 94 (ZL) 03].METHODS: A total of 40 ICR mice were randomly divided into 4 groups:young control group, aging control group, XML group at high dosage and XML group at low dosage with 10 in each group. All mice were fed routinely with standard feed. Mice in young control group aged 3 months and weighed (20±2) g; meanwhile, mice in other groups aged 20 months and weighed (40±3) g. From the 20th month, mice in XML groups were intraperitoneally injected with 6 mg/kg and 8 mg/kg, respectively, for 30 days with once every day. Mice in young and aging control groups were intraperitoneally injected with the same volume of saline. On the 31st day,heads of mice were cut off to obtain blood which was maintained at hepalean anti-coagulated tube. Brains and livers were obtained rapidly for homogenate with 10% saline. Activities of catalase (CAT), superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px) in serum, content of malondialdehyde (MDA) in brain tissue and content of lipofuscin in cerebral and hepatic tissue were measured according to introduction of kits.MAIN OUTCOME MEASURES: ① Activities of CAT, SOD and GSHPx in serum; ② content of MDA in brain tissue and contents of lipofuscin in cerebral and hepatic tissues.RFSULTS: A total of 40 animals were involved in the final analysis without any loss. ① Activities of CAT, SOD and GSH-Px in serum of mice were lower in aging control group than those in young control group; however,activities of CAT, SOD and GSH-Px in serum of mice were increased in the two XML dosage groups, and there was significant difference from those in aging control group (P < 0.05 or P < 0.01). ② Content of MDA in brain tissue and contents of lipofuscin in cerebral and hepatic tissues of mice in aging control group were higher than those of mice in young control group, but those in the two dosage groups were decreased, which was significant difference from those of mice in aging control group (P < 0.01). However, there was no significant difference between the two dosage groups.CONCLUSION: XML has an effect on anti-aging through improving metabolism of free radicals.
2.Effect of xinmailong on ischemic cardiac muscle during hemorrhagic shock

Xinmailong injection solution was invented at 1988 by prof. Li Shunan in Dali medical college. It was made from the material which has high biological activeness to the cardiac and vascular system. During the experimental shock caused by excessive loss of blood in monkey and dog, it was found by ECG that the T-wave of anterion lead on left chest elevated and became high and sharp after acute blood loss. Arterial blood pressure dropped to 8-5.3 kPa for dog and 8-5.3-2.7 kPa for monkey, changes of T-wave all recovered to near normal level after xinmailong solution was injected intravenously (0.05-0.2 mL/kg). These Results implied that xinmailong might improve the ischemia of myocardium induced by hemorrhagic shock.
3.Evidence mapping of exercise rehabilitation in patients undergoing surgery for valvular heart disease

Objective The existing evidence of exercise rehabilitation in patients undergoing surgery for valvular heart disease was analyzed,described and evaluated by using evidence mapping methods.Methods Systematic reviews and randomized controlled trials on exercise rehabilitation of patients undergoing valvular heart disease surgery were searched from CNKI,Wanfang Data,VIP,CBM,Embase,PubMed,Cochrane Library,Web of Science,Ovid,CINAHL and other databases,and the search period was established until April 23,2024.The quality of the included literature was evaluated,and the feature information of the included literature was visualized in the form of bubble map and table.Results Fifty-one literatures(including 44 randomized controlled trials and 7 systematic evaluations)were finally included.Among the 44 randomized controlled trials,the types of exercise were mainly related to anaerobic exercise(3 literatures),aerobic exercise(11 literatures),flexibility training(8 literatures),and multisport exercise(22 literatures);postoperative inpatient rehabilitation accounted for a relatively high proportion(16 literatures),followed by overall postoperative rehabilitation(13 literatures)and pre-operative prehabilitation(7 literatures).Twenty-seven conclusions were categorized as"beneficial",15 as"possibly beneficial,"and one ach as"unclear"and"no effect".Seven systematic evaluations were subjected to Meta-analysis,and in terms of the design of the included studies,there were only 3 systematic reviews that included randomized controlled trials.Of the study conclusions,three were"beneficial",three were"possibly beneficial"and one was"unclear".Conclusion The evidence mapping shows that exercise rehabilitation is generally beneficial or potentially beneficial in the rehabilitation of valvular heart disease surgery,mainly in the postoperative inpatient rehabilitation phase.High-quality studies are needed to further investigate the effectiveness of exercise programs with different intensities,frequencies,and durations,as well as different valve types,adverse effects of exercise,and other outcome indicators.
4.A Meta-analysis of the effect of mindfulness intervention on patients after PCI

Objective To evaluate the effect of mindfulness-based intervention in patients after percutaneous coronary intervention(PCI)through Meta-analysis.Methods Nine databases,including CNKI and PubMed,were searched for randomized controlled trials on the effect of mindfulness intervention in patients after PCI,and the search period was from the establishment of the database to May 31,2024.Two researchers independently screened the literature,extracted data and evaluated the quality,and used Revman 5.4 software for Meta-analysis.Results A total of 11 randomized controlled trials involving 998 patients were included.The results of Meta-analysis showed that compared with conventional care,the use of mindfulness-based intervention therapy could positively improve patients'postoperative anxiety,perceived stress level,mindfulness awareness and other indicators.In terms of safety measures,the incidence of cardiac adverse events was not statistically significant compared with usual care(P>0.05).Conclusion Mindfulness intervention has a significant effect in patients after PCI.
5.Risk factors for complications of the retromandibular approach in patients with parotid gland posterior lower pole tumors

Objective:To analyze the risk factors for complications of the retromandibular approach in patients with parotid gland posterior and lower pole tumors.Methods:A retrospective analysis was conducted on the clinical data of 140 patients with parotid posterior lower pole tumors admitted to the Xingtai Third Hospital from October 2019 to October 2021. They were divided into two groups based on whether complications occurred: the occurrence group and the non occurrence group. General data of the two groups of patients were collected, including age, gender, course of disease, previous surgical history, number of tumors, tumor length, resection range, facial nerve dissociation, tumor site resection frequency, and fascia preservation; Single factor and logistic multivariate analysis were conducted to determine the risk factors for complications of the posterior retromandibular approach in patients with parotid gland posterior and lower pole tumors.Results:A total of 140 patients with parotid gland posterior lower pole tumors underwent retromandibular approach treatment, with complications occurring in 38 cases (27.14%), including 7 cases of temporary facial paralysis, 10 cases of facial depression, 11 cases of Frey syndrome, 2 cases of fistula, and 8 cases of sensory abnormalities of the greater auricular nerve. Through logistic multivariate analysis, it was found that the number of tumors ≥ 2 ( OR=2.856), the resection range (total resection) ( OR=2.477), the number of surgeries ≥3 ( OR=5.637), facial nerve dissociation ( OR=3.526), and lack of fascia preservation ( OR=2.551) were all risk factors for postoperative complications in patients with parotid posterior pole tumors (all P<0.05). Conclusions:In clinical practice, relevant prevention and treatment measures should be formulated for these high-risk factors to reduce the incidence of postoperative complications.
6.Effect of enhanced recovery after surgery nursing on dissection of aorta patients:A Meta-analysis

Objective To systematically review the efficacy of enhanced recovery after surgery(ERAS)nursing on patients with dissection of aorta.Methods Nine databases,including PubMed,CNKI,etc were searched to identify randomized controlled trials involving dissection of aorta patients undergoing ERAS nursing.The retrieval time was from setting up of the databases until March 2024.Literature quality assessment followed by"Cochrane handbook for systematic reviews of interventions.Version 5.1.0",and Meta-analysis was conducted utilizing software of RevMan 5.4.Results Nine literatures with a total of 723 patients were included.The results of Meta-analysis showed that compared with the conventional nursing model,the application of ERAS nursing model could effectively promote the process of patients'postoperative recovery,and there were significant differences in indicators such as the time of postoperative mechanical ventilation,the time of the first postoperative out-of-bed activity,the time of tubing removal,the incidence of complications,the length of hospital stay,and the degree of perceived pain(P<0.05).Conclusion Implementing nursing model based on ERAS on dissection of aorta patients can effectively enhance patients'postoperative recovery outcomes,thus worth clinical promotion and application.
