1.Feasibility of Applying Extrusion-puffing Technique to the Preparation of Qingyin Teabag

Objective To study the feasibility of applying extrusion-puffing technique to the preparation of Qingyin Teabag.Methods Extrusion-puffing techique and water decoction were used to prepare the Qingyin Teabag,and thecorresponding products were compared by pharmacodynamic tests .Meanwhile,a dissolution test was carried out for the monarch drug Radix Glycyrrhizae in the teabag before and after pressurizing-expansion,thus to explore the influence of extrusion-puffing technique on the dissolution speed of ammonium glycyrrhetate from Radix Glycyrrhizae.Results With ammonium glycyrrhetate as the index,T50 and Td of Radix Glycyrrhizae before extrusion puffing extraction were 20.25 min and 56.20 min,while were 7.975 min and 18.975 min respectively after extrusion puffing extraction,the difference being very significant.Pharmacodynamic tests showed effects of the products processed by extrusion-puffing technique were slightly superior to those by water decoction.Conclusion It is feasible to adopt extrusion-puffing technique to the preparation of Qingyin Teabag.
2.Advance in Assessment of Upper Limb and Hand Motor Function in Patients after Stroke (review)

Patients often suffered limb dysfunction after stroke. Upper limb and hand function recovered more slowly than lower limb, and became one of the research focuses in rehabilitation medicine. Timely and effective assessments were important to guide the rehabilitation treatment, evaluate the treatment efficacy and predict functional recovery of upper extremity and hand function. The assessment methods of upper limb and hand motor function included subjective evaluations and objective evaluations. The former included different kinds of scales, which involved assessment focusing on muscle, motor pattern, change of upper limb and hand function. While the latter contained biomechanics, neuroelectrophysiology, functional magnetic resonance imaging, upper limb robotic evaluation system and so on. Scales were proved to have good reliability and validity. But they could not show patients' function accurately because of the subjectivity, and the data could not be kept. Evaluation system with computer was applied in clinical practice more and more widely. In this review, we summarized the assessments on upper limb and hand motor function in patients after stroke.
3.Study on the Preparation Technology of Tangzu Granula

Objective To optimize the preparation process of Tangzu Granula. Methods With the paste-forming rate and total content of paeoniflorin as the indexes, the optimal water-extraction condition was screened by orthogonal design. With hydroscopicity, fluidity and formability as the indexes, the appropriate ratio of the excipient and the formula were selected. Results The optimum water-extraction condition was as follows: 10-fold water, decoction for 2 times and 1.5 hour for each time. The optimal ratio of the excipient and the formula for preparation of Tangzu granules were two proportions of the extract and three proportions of the excipient which consisted of lactose and dextrin(2 : 1, w/ w). Conclusion The technology is effec-tive, scientific, reasonable, and practical.
4.Correlation research of delighted thinking on improving negative emotion of depression patients

Objective To investigate the effect of delighted thinking on improving negative emotion of depression patients. Methods 100 depression patients were randomly divided into the observation group and the control group with 50 in each according to admission sequence. Both groups was executed antidepressionant drugs treatment and routine psychiatric care simultaneously. The observation group was given delighted thinking training on the basis of above treatment. The emotional recovery of two groups was observed. Results There was significant difference on facial expression, communication and limbs language after executing delighted thinking training in the observation group. And there was significant difference on scores of Hamilton depression rating scale (HAMD) at discharge. Scores of Nurses' Observation Scale during early, middle and late stage of delighted thinking training greatly improved compared with those before training. Conclusions Delighted thinking contributes to throw off negative thinking pattern of self-denial, stimulate positive passion threshold, improve depressed mood and raise treatment effect for depression patients.
5.Investigation of Toxoplasma gondii infection in pregnant women in Qingdao area

Objective To understand the status of Toxoplasma gondii(TOX)infection in pregnant women in Qingdao area. Methods Antibody capture ELISA was used to detect the TOX-IgM,IgG and TOX-DNA in 1 341 pregnant women and 201 infer-tility,miscarriage,stillbirth pregnant women from June 2011 to July 2013. Results Among 201 abnormal pregnant women,43 cases were TOX antibody positive,accounting for 22.39%,and among the normal pregnant women,84 cases were TOX antibody positive,accounting for 6.26%,and there was a significant difference(χ2 =10.60,P<0.05). The TOX positive rate of 20-29 years old women was higher than that of the women aged over 30 years(χ2=21.9,P<0.05). The TOX antibody positive rates of families with pets and families without pets were 26.63%and 8.77%respectively(χ2=10.93,P<0.05). The TOX antibody posi-tive rates were 10.94%(22/201)in unemployed women,3.98%(8/201)in farmers,3.98%(8/201)in staff,and 3.48%(7/201)in workers. Conclusions The TOX infection rate in abnormal pregnant women is significantly higher than that in normal pregnancy women;therefore,the health education should be strengthened. In addition,in young women,especially 20-29 years old women, and some special occupation women,the control measures should be enhanced.
6.Self-microemulsifying drug delivery system of Acanthopanax senticosus total saponin

Objective To develop self-microemulsifying drug delivery system(SMEDDS) of Acanthopanax senticosus total saponins(ASTS) and the determination method.Methods The equilibrium solubility of ASTS in different compositions of oils,emulsifier and assistant emulsifier was investigated.The self-microemulsion formula was optimized by constructing the pseudo-ternary phase diagrams of blank SMEDDS and the studying the self-microemulsifying efficiency and the stability of drug-loaded SMEDDS.Taking isofraxidin,an effective component of A.senticosus,as the content determination index,the content was determined by HPLC.Results The optimal self-microemulsion formula was composed of Caf,propanediol,and ethyl linolenate.The ratio of them was 16∶4∶5.The average particle size was 40 nm.The average content of isofraxidin was 92.6 ?g/mL.Conclusion The acquired microemulsion with small particle size is stable.The content determination method of taking isofraxidin as the quality control index is accurate and reliable.
7.Studies on the effect of ultrasound on properties and bonding strength of dentin smear layer

Objective:
To explore the effect of ultrasound on dentin smear layer's surface and bonding strength of the universal resin adhesive under self-etching mode.
Methods:
Forty mandibular third molars without caries were randomly divided into two groups; one was polished with silicon carbide sandpaper; the other was polished with silicon carbide sandpaper followed by ultrasonic treatment. Scanning electron microscope (SEM) was used to observe surface of the dentin. Treated teeth were bonded with two universal resin adhesives, Clearfil Universal Bond (pH=2.3) and All-Bond Universal (pH=3.1), and the penetration of the bonding interface was observed with a confocal laser scanning microscope (CLSM) after Rhodamine B staining. Finally, the micro tensile bond strength test was conducted to test the adhesion.
Results:
The SEM showed that after polishing with silicon carbide sandpaper, the smear layer of the dentin surface was scratched, and dentin tubules were almost completely blocked, with no obvious dentin tubules exposed. After ultrasonic treatment, the scratches were reduced, and a large number of dentin tubules were exposed. CLSM showed that both adhesives could penetrate the dentin along the dentin tubules more deeply after ultrasound treatment. Micro tensile bond strength tests showed that ultrasonic treatment could enhance the bonding strength of two universal resin adhesives. However, there was no statistical difference in bonding strength between these two universal resin adhesives under the same treatment. .
Conclusion
Ultrasound can partially remove the smear layer on dentin's surface, expose dentin tubules, and increase universal resin adhesives' penetration depth and bonding strength under self-etching mode
8.Effect of Intensive Dose Arvastatin on Preventive Contrast-induced Nephropathy in Elder with Coronary Heart Disease after Elective Percutaneous Coronary Intervention

ObjectiveTo investigate the effect of intensive dose atorvastatin on preventive contrast-induced nephropathy (CIN) in elder with coronary heart disease (CHD) after elective percutaneous coronary intervention (PCI).Methods110 subjects older than 60 who received elective PCI,were randomly divided into intensive dose atorvastatin group (the study group,n =50) and conventional treatment group (the control group,n =50).On the ba sis of the hydration therapy,the study group received atorvastatin and the control group received atorvastatin.Scr、β2- MG and liver function were checked for evidence of tubular or glomemlar damage before and after elective PCI were compared between the two groups.Ccr was calculated according to Cockcroft-Gault formula;The incidence of the major adverse cardiovascular events (MACE) and cytotoxicity and hepatotoxicity of rosuvastation were respectively recorded in 30 days follow-up period.ResultsCcr in the study group was significantly higher than that in the control group at day 1 [( 73.12 ± 16.89 ) ml/min vs ( 63.89 ± 18.42 ) ml/min,P =0.036],day 2 [( 65.32 ± 13.46 ) ml/min vs (55.63 ± 15.47 )mL/min,P =0.021] ;Blood β2-M in the study group was significantly lower than that in the control group at day 1 [( 2.44 ± 0.42 ) ml/min vs ( 2.69 ± 0.63 ) mL/min,P =0.009],day 3 ( 2.52 ± 0.46 ) mL/min vs (2.81 ±0.63) ml/min,P =0.011],day 3[(2.37 ±0.43) ml/min vs (2.54 ±0.65 ) ml/min,P =0.021].The incidence of CIN was lower in the study group than that in the control group(6% vs 24%,P =0.012).During 30days clinical follow-up,the incidence of the MACE in the control group was more than the study group ( x2 =5.316,P =0.021).There was no significant difference between the two groups for the cytotoxicity and hepatotoxicity.ConclusionHigh dose atorvastatin may be more efficient in prevention CIN in elder before elective PCI and this higher dose may be safe to the elder.
9.Effects of Robot-assisted Task-oriented Training on Hand Function after Stroke

Objective To study the effects of the rehabilitation robot-assisted task-oriented training on the hand function in patients after stroke. Methods From June, 2015 to September, 2016, 35 inpatients suffering from stroke were randomly allocated to control group (n=17) and trial group (n=18). Based on the routine rehabilitation, the trial group accepted robot-assisted task-oriented training, while the control group accepted therapist-assisted task-oriented training, for two weeks. They were measured the active range of motion (AROM) of fingers, assessed with fingers motor of Fugl-Meyer Assessment (FMA) and modified Barthel Index (MBI) invovled with hands before and after train-ing. Results The inpatients dropped three in the control group, two in the trial group. AROM of extension and flexion of all the fingers, the AROM of extension and total of three fingers of thumb, index and middle, and the total AROM of each finger improved in the trial group af-ter training (t>2.937, P<0.05), while the AROM of extension and flexion of all the fingers, AROM of extension, flexion and total of the fin-gers of thumb, index and middle, total AROM of the fingers of thumb, index and little improved in the control group after training (t>2.528, P<0.05);the AROM of extension and total of the fingers of thumb, index and middle, and the total AROM of fingers of thumb and index im-proved more in the trial group than in the control group (t>2.535, P<0.05). The scores of mass flexion, mass extension, opposition, cylinder grip, spherical grip and total score of FMA improved in the trial group after training (Z>2.000, P<0.05), while the scores of mass extension, opposition and the total score of FMA improved in the control group after training (Z>2.000, P<0.05). There was no significant difference between the two groups on the items and total scores after training (P>0.05). The scores of feeding, dressing, toilet transfers, bathing, groom-ing of MBI and the total score of them improved in the trial group after training (Z>2.041, P<0.05), while the total score of MBI improved in the control group after training (Z=-2.527, P<0.05). There was no significant difference between the two groups in the items and total scores after training (P>0.05). Conclusion The rehabilitation robot-assisted task-oriented training can improve AROM of hemiplegic fingers and grip function.
10. Effect of Zhen′gan Xifeng decoction combined with mirror therapy on the recovery of upper limb function of stroke patients with syndrome of Yin deficiency and wind stirring

Objective:
To observe the effect of Zhen′gan Xifeng decoction combined with mirror therapy on the recovery of upper limb function of stroke patients with syndrome of Yin deficiency and wind stirring.
Methods:
From January 2015 to October 2017, 90 stroke patients with syndrome of Yin deficiency and wind stirring complicated with hemiplegia of upper limbs in Wenzhou Hospital of Traditional Chinese Medicine were randomly divided into the combined treatment group, the mirror treatment group and the control group according to the digital table, with 30 cases in each group.All patients were treated with conventional western medicine, combined with limb function rehabilitation training; on this basis, the combined treatment group was given mirror therapy in conjunction with Zhen′gan Xifeng decoction, the mirror group was given mirror treatment, the control group only received routine treatment.The three groups were treated for 8 weeks.The recovery of upper limb function score(FMA), arm action test score(ARAT), functional independent measurement(FIM) were used to evaluate the curative effect, and the TCM syndrome scores of the patients before and after treatment were compared in the three groups.
