1.Effect Analysis of the Implementation of National Essential Medicine System on the Drug Utilization of Outpa-tients in a Shanghai Community Health Service Center

OBJECTIVE:To provide reference for promoting the further implementation of national essential medicine system and rational use of drugs in community health service center. METHODS:Outpatients’information in Changqiao Street Communi-ty Health Service Center from Sept. 2,2009 to Nov. 25,2014 was collected to statistically analyze the drug variety,the number of prescriptions,the consumption sum and the DDDs before and after the implementation of essential medicine system and after the supplement of non essential medicines. RESULTS:Essential medicines occupied the main position of drug utilization in each stage, with substantial growth in respects of consumption sum and number of prescriptions;the sum of Chinese patent drugs and essential cardiovascular drugs occupied more than 50% of the total consumption sum and number of prescriptions,with higher growth rate;essential medicines occupied the top 10 in respects of consumption sum and DDDs,among which most were the drug varieties in“Shanghai supplement essential drugs in community health service center”(2010 Edition),with some overlap the varieties in“Na-tional Essential Medicine System”(2012 Edition);cardio-cerebrovascular drugs occupied the dominant position;the growth rates of cefotiam and atorvastatin took up the top 2 ranking in the list of consumption sum,while the growth rates of calcium carbonate D3 and irbesartan and hydrochlorothiazide took up the top 2 ranking in the list of DDDs. CONCLUSIONS:Essential medicine cata-log covers the common drugs in the community health service center,in which the rational use of Chinese patent drugs and antibac-terial deserves needs to be noticed. The clinical rational use of the essential medicines should be strengthened,the essential medi-cine catalog should be adjusted and non essential medicines should be supplied when necessary in order to truly meet the needs of patients and optimize the allocation of medical resources.

BACKGROUND:Efficacy of stem cel therapy is considerably influenced by oxidative stress. Sirtuin (SIRT) family of mammals is an important deacetylation and antioxidant enzyme that can regulate endogenous antioxidant activities in stem cel s and cel cycle related signaling pathways to reduce the damage and enhance the viability of stem cel s.
OBJECTIVE:To review the regulating function and mechanism of SIRT family.
METHODS:A computer-based search of Web of Science, PubMed and CNKI from 1990 to 2015 was performed for relevant articles about SIRT and stem cel oxidative stress, using the key words of“SIRT, stem cel , oxidative stress, molecular mechanisms”in English and Chinese, respectively. After eliminating literatures which have poor authority or have similar contents, 55 articles were involved.
RESULTS AND CONCLUSION:NAD+-dependent SIRT family is the key enzyme for deacetylation of histones and other proteins. It plays vital regulation roles in metabolism, genomic stability, DNA damage/repair, and chromatin remodeling/stress reaction. Progress in the SIRT-targeted stem cel research wil definitely provide more clues for clinical stem cel transplantation therapy.
3.Application of three-dimensional CT reconstruction in correction of mandibular angle hypertrophy with milling technique through intraoral incision

Objective To observe the surgical results of milling technique in treating mandibular angle hypertrophy through an intraoral approach and the effect of three-dimensional computer tomography (3D-CT) reconstruction.Methods Milling-technique was used to treat mandibular angle hypertrophy of 43 patients through intraoral approach under local anaesthesia.3D-CT was applied preoperatively and postoperatively,the data of mandibular angle and masseter muscle thickness were compared preoperatively and postoperatively.The facial contour improvement was also observed.Results Of 43 cases,33 cases were followed-up for 4 to 24 months after operation.The majority of patients were satisfied with the aesthetic results,changes of anterior and lateral view of basifacial part were obvious,the curve of the mandibular angle was smooth and the shape was natural; no major complications such as facial nerve injury or malfracture were seen.Conclusions Milling-technique is a safe,effective and simple method for mandibular angle reduction,and 3D-CT reconstruction has a positive effect preoperatively and postoperatively.

Objective To analyze the difference of soft and hard tissues between beautiful face and ordinary face in young Han women for providing data basis for modification of lower facial contour.Methods 38 cases of young Han women were selected with craniofacial spiral CT scanning,three-dimensional reconstruction and histomorphometry of the mandibular angle region (12 cases of beautiful face and 26 cases of ordinary face).Results The difference of bony bigonial breadth,lower face width,angle of mandibular angle,mandibular branch width and height,the distance between the mandibular angle point and pogonion in mandibular angle region had statistical significance (P < 0.05);the difference of mandibular angle thickness had no statistical significance (P>0.05);the difference of masseter muscle length,width and thickness had statistical significance (P < 0.05).Conclusions In clinical lower facial contour modification practice,we can refer to the data of soft and hard tissue of mandibular angle region of beautiful face,combining with the specific circumstances of the pursuit of beautiful people and select the appropriate surgical approach for lower facial contour modification.
6.The clinical value of miR-127-3p expression in plasma of patients with breast cancer

Objective To investigate the expression and clinical value of miR-127-3p in plasma of patients with breast cancer .Methods 80 cases of breast patients , 70 cases of benign breast tumor patients and 70 cases of normal control group were recruited .A real-time fluorescent quantitative reverse transcription polymerase chain reaction ( RT-PCR ) method for detecting miR-127-3p was established; Liner, reproducibility, and specificity were evaluated.In addition, correlations between the relative expression of plasma miR-127-3p and the concentrations of CEA and CA 153 were assessed.the relationship of miR-127-3p expression and clinicopathological features was further determined by Mann-Whitney test.Results The method for detection of plasma miR-127-3p was established.The relative plasma expression of miR-127-3p in breast patients [ 10.561 ( 5.424 -16.465 ) ] was significantly higher than that in benign breast tumor patients [3.015 (1.987-5.035)] (P=0.000 6) and healthy controls [2.375 (1.173-4.370)] (P=0.000 2).However, there was no significant difference between benign tumor patients and healthy control group (P=0.143).Positive relationship was found between the relative expression of miR-127-3p and the concentration of CA153 (R2 =0.457, P=0.003).The area under the ROC curve (AUC) of miR-127-3pwas 0.763, which was higher than that of CEA and CA 153.No significant difference was found between plasma miR-127-3p expression and clinicalpathological features including tumor size , differentiation and tumor node metastasis stage (P>0.05).Conclusions miR-127-3p was increased in breast cancer patients and may be an important diagnostic index for breast cancer .

8.Expression of lncRNA RN7SK in tissue samples from gastric cancer patients and its screening diagnostic value

Objective To explore the expression of long noncoding RNA ( lncRNA) RN7SK and its screening value in clinical diagno-sis of gastric cancer. Methods The expression of lncRNA RN7SK in gastric cancer cells and paired gastric cancer tissues were meas-ured by qRT-PCR. The screening efficacy was detected by the receiver operating characteristic ( ROC) curve. Results The expression levels of lncRNA RN7SK in gastric cancer lines SGC-7901(3.91±0.53),MGC-803(3.44±0.29),HGC-27(4.04±0.87)and BGC-823 (4.30±1.13) were markedly higher than that in human gastric mucosal epithelial cell line GES-1, and the difference had statistical sig-nificance(1.02±0.27, t=12.33, 7.48, 7.20 and 4.90, P<0.05). The expression of RN7SK was up-regulated in 85.5% (47/55) of gastric cancer tissues while down-regulated in the the other 8 tissues ( 14. 5%) . The area under ROC curve ( AUCROC ) of lncRNA RN7SK expression in gastric cancer patients was 0.827 and the 95% confidence interval (CI) was from 0.746 to 0.907. When the cut-off value was 0.618, the sensitivity and specificity were 0.836 and 0.782 respectively. Conclusion The level of lncRNA RN7SK should be overexpressed in gastric cancer tissues so that it may have high screening efficacy and could be used as a potential molecular marker for the diagnosis of gastric cancer.

10.Alterations in Spontaneous Brain Activity in Drug-Naïve First-Episode Schizophrenia: An Anatomical/Activation Likelihood Estimation Meta-Analysis

Objective:
The etiology of schizophrenia is unknown and is associated with abnormal spontaneous brain activity. There are no consistent results regarding the change in spontaneous brain activity of people with schizophrenia. In this study, we determined the specific changes in the amplitude of low-frequency fluctuation/fractional amplitude of low-frequency fluctuation (ALFF/fALFF) and regional homogeneity (ReHo) in patients with drug-naïve first-episode schizophrenia (Dn-FES).
Methods:
A comprehensive search of databases such as PubMed, Web of Science, and Embase was conducted to find articles on resting-state functional magnetic resonance imaging using ALFF/fALFF and ReHo in schizophrenia patients compared to healthy controls (HCs) and then, anatomical/activation likelihood estimation was performed.
Results:
Eighteen eligible studies were included in this meta-analysis. Compared to the spontaneous brain activity of HCs, we found changes in spontaneous brain activity in Dn-FES based on these two methods, mainly including the frontal lobe, putamen, lateral globus pallidus, insula, cerebellum, and posterior cingulate cortex.
Conclusion
We found that widespread abnormalities of spontaneous brain activity occur in the early stages of the onset of schizophrenia and may provide a reference for the early intervention of schizophrenia.
