1.Construction of Modernized Dispensary in Our Hospital

OBJECTIVE:To recommend modern work model for hospital pharmacy. METHODS: Based on the practice of the construction of modernized dispensary in our hospital, we analyzed the constructions of automated outpatient dispensary and integrated tidy and clear inpatient dispensary as well as informationized traditional Chinese medicine dispensary, the practice of clinical pharmacy and the production of hospital preparations etc. RESULTS & CONCLUSIONS: The modernized pharmacy work pattern carried out in our hospital embodies patients-oriented and rational drug use-based idea, and due to which, informationization and digitalization of pharmacy management has been achieved basically; therefore, this model deserves to be followed by other hospitals.
2.Experience of Clinical Pharmacy in Contagious Hospital

OBJECTIVE:To establish a sound clinical pharmaceutical care system in contagious hospital.METHODS:The working model for clinical pharmacy in our hospital was introduced.RESULTS & CONCLUSION:The rational drug use level was enhanced by the pharmaceutical care provided for both outpatient and inpatients including establishing information system,setting consulting room,establishing medication histories for outpatients,pharmacists' going to clinic to make up for any possible deficiencies or vacancies,collect drug information,give medication lectures among physicians and patients,carry out monitoring of ADR,and make individualized medication scheme etc.
3.Application of ARIMA Model to Drug Storeroom in Drug Purchasing Prediction

OBJECTIVE:To explore the new drug purchasing mode using autoregressive integrated moving-average (ARIMA) prediction model for improvement of the working quality and efficiency in hospital drug storeroom.METHODS:Drug consumption data from week 1 to week 47 in 2008 were collected.According to ABC method,category A drugs were defined among which 10 kinds of drugs were sampled randomly.Based on the data of from week 1 to week 44 in 2008,software SPSS13 was applied for the modeling and fitting of ARIMA model.The established model was applied to predict the data of from week 45 to 47,with the predicated data compared with the actual consumption data.RESULTS:The predicted purchasing amount using ARIMA model were consistent with the actual consumption data,with prediction accuracy for quantity at 89.19% and prediction accuracy for whole unit of purchased drugs at 97.56%,respectively.CONCLUSIONS:Good fitting and high short-medium term predication accuracy were obtained in the prediction using ARIMA model,and which could provide scientific support for drug purchasing and help manage the drug stock reasonably without appearance of out of stock or overstock.
4.Behavior change of adriamycin resistant human breast cancer cell line MCF7

Objective To investigate mechanisms of resistance to adriamycin(ADR)by human breast cancer cell line MCF-7 and to find the alteration of features and celluar behavior of MCF-7 after exposure to ADR.MethodsProliferation speed,population doubling time of MCF(wild type),MCF-7/ADR(exposure to adriamycin)and withdrawl group were respectively tested.Cell phenotype alteration was detected using SP immunohiatochemistry methods.Results No significant difference of proliferation speed was found between MCF-7/ADR and MCF cells.As exposure time prolonged,withdrawl group cells grew faster,thus population doubling time shortened.Differentiation of MCF-7/ADR and wthdrawl group was lower than wild group.The expression of drug resistance associated marker of MCF-7/ADR such as Pgp,LRP,GST-pi,TOPOⅡwas higher than that of MCF-7,ER turned to express negatively,and expression of PR gradually decreased as exposure continued.Conclusion MCF-7 cells exposed to ADR got drug resistant,their proliferation was not suppressed by withdrawl of ADR and even grew faster.Drug resistant cells gained dedifferentiation ability.Their heredity and biochemistry features changed,expression of target enzyme also altered and was reversible by drug withdrawl.
5.Study on Association with Expression in Patients with Chronic Kidney Disease

Objective To investigate the expression of human epididymal protein 4 (HE4) in chronic kidney disease (CKD) and its clinical value.Methods From April 2014 to December 2015,Serum samples of 92 non-cancer patients diagnosed as CKD and 84 healthy controls were collected in Nantong University Hospital.HE4,BUN,Scr,Cys C and β2MG were detected.The difference of HE4 expression in different stages of CKD and the correlation of HE4 with Urea,Scr,Cys C and β2MG were analysized,respectively.ROC curve was used to evaluate the auxiliary diagnostic value of HE4,BUN,Scr,Cys C and β2MG.Results The serum HE4 expression in patients with impaired renal function was 388.2 (130.1~1 659.5)mIu/ L,(F=16.237,P=0.001).Which was significantly higher than that in the control group [38.1 (32.77 ~ 48.17)mIu/L].The serum HE4 expression was increased by the stage of renal damage and the difference was existed among different groups (P<0.05).HE4 expression was positive related with Cys C and β2MG.The AUC of HE4,BUN,Scr,Cys C and β2MG were 0.878,0.785,0.816,0.874 and 0.819,respectively.Conlusion It needs to consider the exsits of impaired renal function when the HE4 was detected.The HE4 might be a novel early diagnostic indication for CKD.
6.Establishment and characteristics of a paclitaxel resistant human mammaryadenocarcinoma cell subline(MCF-7/Taxol)

Aim To establish a new paclitaxel resistant human mammary adenocarcinoma cell subline(MCF-7/Taxol) and investigate its characteristics.Methods A paclitaxel resistant human mammary adenocarcinoma cell subline(MCF-7/Taxol) was developed by gradually increasing the concentration of Taxol from the parent cell line MCF-7 in vitro.The multidrug resistance of MCF-7/Taxol to anticancer agents was evaluated by SRB assay;the distribution of their cell cycles was detected by flow cytometry;the positive expression rate of P-gP、LRP、ToPoII、GST?、ER and PR was measured by S-P immunohistochemistry;the intracellular accumulation of Taxol was assessed by HPLC;the morphological features and the celluar ultrastructure characteristics were observed respectively by light microscopy and scanning electron microscopy.Results MCF-7/Taxol was resistant to several chemotherapy agents,such as Hydroxycamptothecine,Epirubicin,Doxorubicin,Mitoxantone and so on.The IC50 of MCF-7/Taxol to Taxol was 525 times higher than that of MCF-7,and the IC50 of MCF-7/Taxol stopped administrating Taxol for three months was 150 times higher than that of MCF-7.The multiplication time of MCF-7/Taxol was longer than that of the natural cell and the proportion of cells in S-phase increased while that in G1-phase decreased.The expression levels of P-gp、LRP and GST? increased,and ER and PR were not observed.The morphology of MCF-7/Taxol became larger and irregular,and the surface of natural cell was in the shape of floss,but MCF-7/Taxol was in the shape of bead.The intracellular of Taxol was observed in both MCF-7/Taxol and stoppage administrating.Conclusions MCF-7/Taxol cell subline was a typical multidrug resistant cell line which had basic characteristics of drug resistance cells.It was supposed that there was a cell subline which was tumor stem cell of MCF-7 included in this multidrug resistant cell line.
7.18F-FDG PET/CT monitoring for early tumor response to cisplatin in VX2 tumor-bearing rabbits

Objective To evaluate the value of 18F-FDG PET/CT in early in vivo monitoring of tumor response to cisplatin,and analyze the relationship between 18F-FDG uptake in tumor and the corresponding pathological changes.Methods Thirty VX2 rabbits were divided into 5 groups by random number table with 6 in each group,including 4 treatment groups and 1 control group.18F-FDG PET/CT were performed before and after (6,12,24 and 36 h post-injection respectively) intravenous administration of cisplatin (7 mg/kg) in the treatment groups,respectively.The control group was injected with physiological saline followed by 18F-FDG PET/CT.The ROI was drawn and the SUVmax and T/NT ratio were calculated.The tumor necrosis rate and apoptosis index were observed by histopathologic examination.Paired t test,GamesHowell test and arcuation correlation analysis were used to analyze the data.Results Significant differences were found in SUVmax and T/NT of the control group before and after injection of physiological saline (6.58±1.67 vs 9.77±2.45,52.93±3.90 vs 29.34±3.31;t=-5.480,17.593,both P<0.05).18F-FDG uptake decreased after 6 h post-injection of cisplatin,with the mean SUVmax decrease rate of (11.83±8.89) % and the mean T/NT decrease rate of (59.00±8.22)%.In the 24 h treatment group,18F-FDG uptake decreased most,and the mean SUVmax decrease rate was (42.33±33.80)%,the mean T/NT decrease rate was (83.50± 7.69) %.The SUVmax and T/NT of those 2 groups were significantly different from those of the control group,and no difference was found between the 2 treatment groups(all P<0.05).The changes of SUVmax and T/NT were positively correlated with apoptosis index and tumor necrosis rate (r=0.750,0.794,0.804,0.874,all P<0.05).Conclusion 18F-FDG PET/CT is a sensitive method for monitoring early response to tumor chemotherapy in VX2 tumor-bearing rabbits at 24 h after treatment.
8.Induction of UGT1A1 expression by praeruptorin A and praeruptorin C through hCAR pathway.

This study is purposed to investigate the effects of praeruptorin A (PA) and praeruptorin C (PC) on UGT1A1 in HepG2 cells through hCAR pathway. PA and PC were incubated with HepG2 cells for 24 h and 48 h, mRNA and protein expressions of UGT1A1 were determined by real-time PCR and Western blotting assays. Additionally, effects of PA and PC on UGT1A1 mRNA and protein expressions were also measured after transient transfection of a specific CAR siRNA for 72 h in HepG2 cells. UGT1A1 mRNA and protein expression levels were significantly increased by PA and PC after incubation for 48 h. Moreover, the mRNA and protein up-regulations of UGT1A1 were attenuated by transient transfection of a specific CAR siRNA, suggesting the induction was mediated by CAR. The results suggest that PA and PC can significantly up-regulate UGT1A1 expression partially via the CAR-mediated pathway.
9.Survey and analysis of cognition and demand of the residency standardized training among clinical medical students

Objective To analyze the cognition and demand of the residency training program among clinical medical students to provide scientific basis for the promotion of this policy.Methods From May 24 to 25,2015,a self-designed questionnaire survey was conducted on 645 undergraduate clinical medical students,who came from Zhengzhou University,Xinxiang Medicine College,Henan University.Data of cognition and demand of the residency standardized training were statistically analyzed using SPSS 22.0 software.Enumeration data were analyzed by chi-square test and abnormal distribution data based on Wilcoxon rank sum test.Results 616 valid questionnaires were recovered.259 undergraduate clinical medical students (42.00%) did not quite understand the resident standardized training policy and 225 (36.50%) did not known the policy at all.Before and after clinical practice,the cognition of the residency standardized training had statistically significant difference (x2=87.596,P=0.000),the knowledge of the policy (x2=10.939,P=0.012) and the degree ascending alleviate employment pressure (x2=29.349,P=0.000) were the main influencing factors of the clinical medical students' choices after graduation.Conclusion Improving medical students' understanding of the significance of medical education after graduation,and the integration of professional degree training and residency standardization training effectively helps to promote implementation of the policy.
10.A comparative study of 18F-FDG PET/CT and CT in the early evaluation of response to chemotherapy in patients with non-small cell lung cancer

Objective:The relationship between the effect of early metabolism in 18F-FDG PET/CT and conventional CT based on the RE-CIST standard to evaluate the best objective response after chemotherapy in patients with non-small cell lung cancer (NSCLC). Meth-ods:We studied 40 patients with unresectable locally advanced or advanced NSCLC that were confirmed pathologically. The patients were 35 years old to 78 years old and included 31 males and 9 females. Three patients have unresectable stageⅢA, 8 patients have stageⅢB, 29 patients have stageⅣ, 12 patients have squamous cell carcinoma, and 28 patients have adenocarcinoma. The PET/CT for the effect of chemotherapy was evaluated in NSCLC according to the SUV standard (SUVmax reduction>30%of primary lung can-cer after one cycle of chemotherapy), and the CT for the effect of chemotherapy was evaluated on the basis of NSCLC according to the RECIST standard. The objectives of the study are as follows:compare the differences and consistency between 18F-FDG PET/CT metabol-ic response after the first cycle of chemotherapy and the RECIST best objective response after the first or second cycle of chemothera-py with the paired chi-square test and kappa test;calculate the 18F-FDG PET/CT to predict the best objective response of two cycles of chemotherapy according to RECIST on the basis of NSCLC in terms of sensitivity, specificity, accuracy, positive predictive value, and neg-ative predictive value;compare the differences in SUVmax reduction between the metabolic remission group and metabolic no relief group with the two-sample t-test. All statistical methods were 0.05 for the inspection level, and P<0.05 was considered statistically sig-nificant difference (SPSS19.0). Results:Differences were found between the first cycle of chemotherapy for the RECIST best objective response and 18F-FDG PET/CT metabolic response (χ2=5.063, P=0.021), and the results had bad consistency (Kappa=0.240, P=0.085). No differences were observed between the second cycle of chemotherapy for the RECIST best objective response and 18F-FDG PET/CT metabolic response (χ2=2.083, P=0.146);the results had good consistency (Kappa=0.413, P=0.006). The sensitivity, specificity, accura-cy, positive predictive value, and negative predictive value were 82%, 61%, 70%, 61%, and 82%, respectively. The differences in SUV-max reduction between the metabolic remission group and metabolic no relief group with the two-sample t-test were statistically sig-nificant (P<0.001). Conclusion: 18F-FDG PET/CT may predict the best objective response to chemotherapy for NSCLC patients. Com-pared with conventional CT, 18F-FDG PET/CT can be an early and accurate way to evaluate the chemotherapy effect in NSCLC.
